    Description: The SO-84 cruise from Las Palmas to Cape Town studied the underwater volcanism associated with the St Helena hotspot. Wide-spaced grid mapping of a region south and west of St Helena island identified three previously unknown hotspot volcanoes (which we named Josephine 16°27'S-9°W, Jade 16°13'S-7°46'W and Benjamin 16°12'S-8°31'W) and several older, probably near-ridge-generated seamounts. The newly discovered hotspot seamounts, and the previously-known seamounts of Bagration, Bonaparte and Kutuzov were mapped and sampled. Onboard geochemical studies confirm that they all show trace-element (Rb, Nb, Y, Zr, Sr) enriched, hotspot magma-type compositions. Morphological studies and the state of the samples collected from the seamounts (all have at least 1 mm-thick Mn crusts) suggest that none of the volcanoes is presently active, and that the St Helena hotspot is probably extinct at the present time. Josephine appears to be the youngest volcano, and one dredge here yielded fossil hydrothermal material (Fe-hydroxides). To test the hypothesis that the extinction of the hotspot is due to its magmas being channelled to the Mid-Atlantic Ridge, several dredges were taken on the ridge opposite St Helena. Furthermore, in the proposed influence zone of the St Helena plume head, Grattan seamount (9°44'S-12°48'W) was also sampled to examine whether its composition has been affected by the St Helena plume. During the transit from Las Palmas to St Helena, several sediment cores were taken in 3000 m water depth close to the ridge axis for palaeoclimatological work.

    Notes: Abstract Patients who underwent surgical procedures usually develop elevated body temperature, changes of plasma levels of some proteins, and leucocytosis. These alterations are summarized as the postoperative acute-phase reaction. Also endotoxin can induce the described phenomena suggesting that endotoxin may play a role concerning the induction of the acute phase reaction. In order to test that hypothesis we determined endotoxin plasma levels preoperatively and daily postoperatively in patients who were operated on because of goiter (n= 20), colonic, pancreatic and gastric diseases (n= 58). A significant increase of endotoxin plasma levels was found at the first and third day after abdominal surgery whereas after goiter surgery the increase revealed to be only very slight. However, the decrease between the first and second postoperative day in the latter group was again statistically significant suggesting postoperative endotoxemia even after minor operations. Furthermore a correlation between the amount of circulating endotoxin and pulmonary or infectious complications could be established in patients after major operations even at the first postoperative day suggesting a pathogenetic relevance of postoperative endotoxemia.

    Description / Table of Contents: Summary Until now reports of factitious disease have not been found in the ENT literature. In contrast, the dermatologic literature estimates an incidence from 0.01 to 0.26%. In order to achieve effective treatment for these patients, knowledge of the characteristic symptoms is fundamental. The present paper describes the variety of disease possible, using three cases treated between 1992 and 1994 in the ENT Department of Cologne. According to the literature, females are involved in a ratio of 9:1, with the majority working in medically related professions. Depression and anorexia are typical symptoms. When a factitious disease is suspected, psychiatric consultation is essential. Confrontation of a suspect patient by the otolaryngologist is not often considered because several reports quote suicidal behavior in up to 25% of self-manipulating patients and tendencies to refuse to follow psychiatric treatment are considerable.
    Notes: Zusammenfassung Insbesondere dermatologische und psychiatrische Veröffentlichungen beschäftigen sich bisher mit artifiziell erzeugten Erkrankungen. In der HNO-ärztlichen Literatur taucht dieses Krankheitsbild so gut wie nicht auf, obwohl nach dem vorliegenden Schrifttum mit einer Inzidenz von 0,06–0,1%, bezogen auf stationäre Behandlungen, gerechnet werden muß [14, 15]. Die vorliegende Arbeit charakterisiert anhand von Fallbeispielen die Erscheinungsformen der Erkrankung und gibt Hinweise zum Umgang mit den Betroffenen. Wir berichten über insgesamt 3 Fälle, bei denen wir in Zusammenarbeit mit der psychiatrischen Universitätsklinik Köln zwischen 1992 und 1994 eine artifizielle Erkrankung diagnostizierten. Bei allen Erkrankten handelte es sich um weibliche Patienten, alle hatten eine medizinische Berufsausbildung absolviert. Zwei Patientinnen riefen durch Manipulation chronisch entzündliche Veränderungen hervor, die zu einer Vielzahl von operativen und konservativen Maßnahmen führten, eine Patientin induzierte Blutungen des äußeren Gehörgangs durch heimliche Heparin-Injektionen. Die Fallbeispiele belegen ausdrücklich, daß auch im HNO-ärztlichen Krankengut das Artefakt in die Differentialdiagnose mit einbezogen werden muß. Die oben genannten Beispiele stellen bereits mehrere Untergruppen der artifiziell manipulierten Krankheit dar. Zur Abklärung des Artefakts bei schlecht heilenden Wunden ist der Okklusionsverband ohne weitere Maßnahmen empfehlenswert. Bei autoagressiven Patienten werden bis zu 25% Suizidversuche beschrieben. Therapie und Konfrontation mit dem Erkrankungsverdacht sollten deshalb ebenso wie die Diagnosesicherung durch einen Psychiater erfolgen [10, 19, 20].

    Notes: Abstract Tests were conducted to determine the effects of cadmium on leg regeneration in larvae of the Northwestern salamander, Ambystoma gracile. Cadmium significantly affected limb regrowth (LOAEL=lowest observed adverse effect level) at 193.1 μg/L in a 24-day test and at 44.6 μg/L in a 10-day test. There were no significant adverse effects (NOAEL=no observed adverse effects level) at 48.9 μg/L in 24-day and 12.8 μg/L in 10-day tests.

    Notes: Abstract Tests were conducted to determine the effects of cadmium (Cd) on survival, growth, and bioaccumulation in larvae and metamorphosed juveniles of the Northwestern salamander, Ambystoma gracile. A 96-h LC50 value of 468.4 μg/L Cd was determined for 3-mo old larvae. Significant adverse effects of Cd (in water) on larval growth were observed at 227.3 μg/L in a 10-day exposure and at 193.1 μg/L Cd (LOAEL) in a 24-day exposure; no significant adverse effects were seen at 106.3 μg/L Cd in the 10-day test and 48.9 μg/L Cd (NOAEL) in the 24-day test. In the juvenile feeding tests, regurgitation of Cd-spiked food occurred at 5,701 and 2,458 μg/g Cd, but not at 982 μg/g. Cadmium tissue bioconcentration values up to 63 times the water concentration were seen in the water/larval tests. No bioaccumulation occurred in the larval and juvenile feeding tests, although similar tissue Cd levels were produced by both water and feeding exposures.

    Notes: Summary In this open, two-period crossover study in 18 healthy male volunteers, a single oral dose of 50 mg diclofenac was administered alone and on day 7 of multiple oral dosing of 5 mg b.i.d. isradipine to assess a possible pharmacokinetic interaction. The effect of these drugs on ex vivo platelet function was also determined. Serial blood samples were obtained over 12-hour periods on three occasions: after the single diclofenac dose; after the morning dose of isradipine on day 6 and after co-administration of both drugs on day 7 of steady-state isradipine administration. Additional samples were taken at 2 h post dose for determination of ex vivo platelet aggregation. Isradipine plasma concentrations were determined by a gas chromatographic method and diclofenac plasma concentrations by an HPLC method. The pharmacokinetic characteristics of diclofenac were unaltered during co-administration. The maximum plasma concentration of isradipine was increased 19.6% during co-administration from 5.06 to 6.05 ng·ml−1. This is not expected to be of clinical importance. Isradipine's apparent total body clearance and steady-state AUC remained unchanged. Ex vivo induced platelet aggregation was not affected by any of the treatments.

Korrelation zwischen derIn vitro-Empfindlichkeit gegenüber Antibiotika und Veränderungen in der Konzentration vonP. aeruginosa pro Gramm Sputum wurde erkennbar. Resistenzentwicklung unter Therapie trat gegenüber allen verwendeten Antibiotika auf. Voraussetzung für optimalen Effekt von Antibiotika ist auch bei Mukoviszidose die Kenntnis des Resistenzverhaltens der Erreger sowohl vor als auch während der Therapie.
    Notes: Summary Strains ofPseudomonas aeruginosa resistant to clinically relevant antibiotics (β-lactams, quinolones, aminoglycosides) were detectable in sputa of cystic fibrosis patients. Correlations betweenin vitro susceptibility and bacteriological resultsin vivo were demonstrated at a quantitative level.P. aeruginosa strains susceptible prior to antibiotic therapy were observed to become resistant towards each of the compounds used for treatment. We conclude that antibiotic therapy in cystic fibrosis has to be optimized by culture specific selection of the drugs and consecutive bacteriological follow-ups.
