    Notes: Summary The effects of ipriflavone (7-isopropoxy-3-phenyl-4H-1-benzopyran-4-one) on bone mineral density (BMD) of the 3rd lumbar vertebra and on calcium (Ca)-related factors, including serum calcitonin (CT) levels before and after rapid calcium infusion (4 mg/kg for 5 minutes), were studied in 11 elderly female subjects (80 ± 2 years of age, mean ± SE). Ipriflavone (IP) administration (600 mg/day, 7 months) resulted in inhibition of BMD loss in 7 patients (responders, mean change of BMD value 2.2 ± 2.3%), whereas 4 patients showed a loss of BMD (nonresponders, mean change of BMD value -13.1 ± 2.6%) compared with pretreatment values. The responder group showed a significant increase in mean pretreatment serum CT levels (from 20 ± 2 pg/ml to 42 ± 7 pg/ml,P 〈 0.05) after treatment with IP, and a significant decrease in the mean basal serum level of corrected Ca (from 9.6 ± 0.2 mg/dl to 8.7 ± 0.1 mg/dl,P 〈 0.01) after treatment with IP; nonresponders did not show these changes. For responders, both the percentage of change and the maximal value of serum CT in response to Ca infusion were maintained at rather high levels, both before and after IP treatment; nonresponders showed almost no response to a stimulation test for CT. These findings suggest that IP inhibits bone loss in elderly female subjects possibly through the mechanism of increasing CT secretion.

    Notes: Summary To clarify perinatal vitamin D metabolism, we measured 25-hydroxyvitamin D (25OHD), 24,25-dihydroxyvitamin D [24,25(OH)2D], 1,25-dihydroxyvitamin D [1,25(OH)2D], calcium (Ca), phosphorus (P), parathyroid hormone (PTH), and human calcitonin (CT) in paired maternal, cord, and infant serum. Cord serum 25OHD was significantly lower than the maternal level, and cord serum 24,25(OH)2D was also significantly below the maternal concentrations. Maternal, cord, and infant serum 1,25(OH)2D, on the other hand, was significantly higher than the normal adult level. The serum PTH was low, but the CT concentration was high in the cord. Cord serum Ca and P levels were significantly higher than maternal. The reason for the elevated circulating 1,25(OH)2D level in the perinatal period is uncertain, and we speculate that the possible factors are gonadal steroids, placental lactogen, prolactin, and CT. In addition, serum 24,25(OH)2D and 1,25(OH)2D concentrations are under some control by the fetus.

    Notes: Abstract We evaluated serum levels of calcium-related factors and bone mineral content in 40 healthy elderly female subjects (mean age ± SD, 79 ± 7 years) as possible factors relating to calcification of the abdominal aorta. There were no significant differences in serum levels of total cholesterol, triglycerides, and estradiol between elderly female subjects with and without calcification of the abdominal aorta. Elderly female subjects with calcification of the abdominal arota, when compared to those without calcification, showed significantly reduced values of bone mineral content of the distal radius. Moreover, the elderly female subjects with calcification of the abdominal aorta showed significant increases in serum levels of parathyroid hormone and calcitonin, and significantly decreased levels of 24,25-dihydroxyvitamin D3. There were no significant differences in serum levels of 25-hydroxyvitamin D3 and 1,25-dihydroxyvitamin D3. These data suggest that elevated secretion of parathyroid hormone plays an important role in the development of calcification of the abdominal aorta.

    Notes: Abstract: To compare the efficacy of estriol (E3) for postmenopausal and senile osteoporosis, we administered orally 1 g/day calcium lactate alone (control groups) or with 2 mg/day estriol (E3 groups) to 20 postmenopausal women aged 50–65 years and 29 elderly women aged 70–84 years, and determined their bone mineral density (BMD) of the lumbar vertebrae AP scan by dual-energy X-ray absorptiometory. Of 41 subjects who completed 10 months of treatment, 8 postmenopausal women and 12 elderly women in the E3 groups showed a significant (P 〈 .05) increase in BMD, 5.59% ± 4.79% and 3.83% ± 7.90% of the respective basal values, while 10 postmenopausal women and 11 elderly women in the control groups showed a decrease in BMD, −4.02% ± 7.00% and −3.26% ± 4.60% of the respective basal values, after 10 months. On the other hand, genital bleeding as a side effect of E3 occurred in 6 elderly subjects at this dose. Moreover, decrease in serum level of corrected calcium was seen only in the elderly women receiving E3. Although a lower dosage of E3 may be recommended for elderly subjects, these observations suggest, first, that hormone replacement therapy with E3 has efficacy for involutional osteoporosis, and, second, that the bones in elderly women also maintain responsiveness to E3.
