Zusammenfassung Obwohl wiederholt auf die Wichtigkeit von Veränderungen des urethrovesikalen Winkels für die Belastungsinkontinenz hingewiesen wurde, sind wahrscheinlich auch andere Faktoren, vor allem eine Insuffizienz der Levatorplatte, des Diaphragma urogenitale und der perinealen Stützfunktion von Bedeutung. Durch die einfache Applikation von Bariumpaste in die Vagina während der Perlenketten-Urethrocystographie kann man einen objektiven Befund bezüglich einer Insuffizienz des Diaphragma urogenitale, der Levatorplatte oder des Dammes erheben. Eine exakte Indikationsstellung zur richtigen Operationsmethode und eine objektive postoperative Bewertung des Ergebnisses wird dazu beitragen, die Bedeutung der gleichzeitig durchzuführenden hinteren Kolporrhaphie und Perineorrhapie für die Behandlung zu erfassen.

Summary Although the importance of defects of the urethrovesical angles in urinary stress incontinence has been repeatedly demonstrated, other factors, notably deficiency in the levator plate and the course of the pelvic diaphragm and perineal support may also be of importance. By the simple application of barium paste within the vagina at the time of bead chain urethrocystography one can obtain objective pre-operative information about deficiencies in the pelvic diaphragm, levator plate, and the perineum. Rational selection of the proper operative procedure and objective postoperative evaluation will help to identify the role of coincident posterior colporrhaphy and perineorrhaphy in the therapeutic plan.
