    Notes: Background IgE-mediated cow's milk proteins (CMPs) allergy shows a tendency to disappear with age. The sooner tolerance is detected, the earlier the substitute diets can be suspended and the quicker family emotional hardship is alleviated.Objective To analyse the specific IgE levels to cow's milk and its proteins, which help to separate tolerant from no tolerant children in the follow-up of infants with allergy to cow's milk.Patients and methods Sixty-six infants diagnosed with IgE-mediated allergy to CMPs were included in this prospective follow-up study. Periodic reassessments were carried out every 6 months until they were 2-years old and then, annually, until tolerance arose or until the last reassessment in which tolerance had not been achieved. Non-tolerant infants were followed, at least, for a period of 3 years. In each visit, the same skin tests and determination of specific IgE (CAP System FEIA) for milk and its proteins were carried out. The open challenge test was repeated unless a clear transgression to milk, which came to be positive, had taken place within the previous 3 months in each of the follow-up visits. Specific IgE levels to milk and its proteins, in different moments of the follow-up were analysed by means of the receiver-operating characteristic curve to predict clinical reactivity.Results Throughout the follow-up 45 (68%) infants became tolerant. The follow-up mean for tolerant infants was 21.2 months whereas for non-tolerant infants it was 58 months. The specific IgE levels which were predictors of the clinical reactivity (positive predictive value (PPV)〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:09547894:CEA1976:ges" location="ges.gif"/〉90%), grew as the age of the infants increased: 1.5, 6 and 14 kUA/L for milk in the age range 13–18 and 19–24 months and in the third year, respectively. Specific IgE levels to casein: 0.6, 3 and 5 kUA/L, respectively, predicted clinical reactivity (PPV〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:09547894:CEA1976:ges" location="ges.gif"/〉90%) in the different analysed moments of the follow-up. The cut-off points: 2.7, 9 and 24 kUA/L for milk and 2, 4.2 and 9 kUA/L for casein, respectively, predicted clinical reactivity with an accuracy 〈inlineGraphic alt="geqslant R: gt-or-equal, slanted" extraInfo="nonStandardEntity" href="urn:x-wiley:09547894:CEA1976:ges" location="ges.gif"/〉95% corresponding to a specificity of 90%.Conclusions Monitorization of specific IgE concentration for milk and casein by means of the CAP system in allergic children to CMPs allows us to predict, to a high degree of probability, clinical reactivity. Age factor must be taken into account to evaluate the specific IgE levels which are predictors of tolerance or clinical reactivity.

    Notes: Abstract Internodal and leaf segments from pedunculate oak (Quercus robur L.) seedlings were used as explant source to induce somatic embryogenesis. Auxin treatment influenced embryogenic response, which only occurred in explants initially cultured on media containing 4 mg/l naphthaleneacetic acid (NAA) and different benzyladenine (BA) concentrations. After 6 weeks of culture on induction medium, the explants were transferred to medium supplemented with 0.1 mg/l BA and 0.1 mg/l NAA, and 4 weeks later, they were subcultured in a growth-regulator-free medium, in which somatic embryos arose through indirect regeneration on the surface of a nodular callus. Somatic embryos were induced in explants of two out of four seedling provenances. The induction frequency ranged from 16% in leaf explants to 4% in internodal explants. Somatic embryos developed two cotyledons, which were translucent or opaque-white in appearance, but anomalous morphologies were also observed. Different embryogenic lines were established and maintained by repetitive embryogenesis in multiplication medium containing 0.1 mg/l BA plus 0.05 mg/l NAA. These results indicate that tissues from explants other than Q. robur zygotic embryos are able to produce embryogenic cultures.

    Notes: Abstract We prospectively analysed the normal contrast-enhanced MRI features of the facial nerve and determined criteria for pathological contrast enhancement. We studied 31 patients with clinically normal facial nerves with T1-weighted images before and after contrast medium. The intensity, thickness and right-left symmetry of enhancement were assessed in each segment and correlated with MRI features observed in abnormal facial nerves. Enhancement along at least one segment of the facial nerve was seen in 98 % of cases, but only within the facial canal: labyrinthine segment: 78.2 %; geniculate ganglion: 96.9 %; tympanic: 88.4 %; mastoid: 66.6 %. Marked (++) to intense (+++) enhancement was seen in the labyrinthine segment in 17.4 %, the geniculate ganglion in 36.3 %, and the tympanic (25.6 %) and mastoid (7.1 %) segments, whereas intense enhancement was only seen in the geniculate ganglion (6 %) and the tympanic segment (11.6 %). A right-left asymmetry was noted in 69 % of cases. No correlation was found between enhancement and the thickness of the nerve. No enhancement of the eighth nerve was seen. We suggest three criteria for pathological enhancement: enhancement outside the facial canal; extension of enhancement to the eighth nerve; and intense enhancement in the labyrinthine and/or mastoid segments.

    Notes: Abstract This study was designed to examine the hypothesis that some environmental factors increase the risk for insulin-dependent diabetes mellitus. Data on dietary history was collected from 80 diabetic children from the Santiago de Chile Registry and from 85 nondiabetic control subjects who were comparable in terms of age, sex, and ethnic characteristics. Early exposure was defined as the ingestion of food sources other than maternal milk before 3 months of age. To define genetic susceptibility to insulin-dependent diabetes mellitus each subject was typed in terms of HLA DQA1 and DQB1, and the possible conformation of susceptible heterodimers was considered as a risk marker. Fewer children were exclusively breast fed in the diabetic group than in the control group (21.55±15.05 vs. 33.95±20.40 weeks, P〈0.01). In addition, exposure to cow's milk and solid foods occurred earlier in the diabetic group than in the control group (15.90±10.95 vs. 21.15 13.65 and 16.85±10.25 vs. 21.20±12.35 weeks, P〈0.05). Our data show that a short duration of breast-feeding and early exposure to cow's milk and solid foods may be important factors in the development of insulin-dependent diabetes mellitus. The high relative risk observed in individuals genetically predisposed indicates an interaction effect between genetic and environmental components.

    Notes: Summary For the non-operable malignant glioma patients, prognosis remains poor, with a survival of 8 months for the glioblastomas (G), and 15 months for anaplastic astrocytomas (AA). 27 histologic proven malignant gliomas (17 AA and 10 G) were treated between April 1991 and June 1992. Median age was 48 years. The therapeutic protocol consisted of three courses of intra arterial chemotherapy (IAC) with ACNU, at intervals of six weeks, and a localised 60 Gy radiotherapy between the first and the second IAC course. 72 courses of IAC were delivered (2,4 per patient). Response rate was 51,8%. Median survival (MS) was 13 months, with a survival rate of 28% at 24 months. For the AA, MS was 21 months, with a survival rate of 37% at 24 months. For the G, median survival was 10 months. Responders were 65 % for AA, 30% for G. Non responders all died before 24 months had relapsed with a MS of 9 months. 54% of responding patients had a 2 years survival. Toxicity were acceptable with 7% of haematological toxicity and a partial loss of visual acuity in 11% of the cases. No chronic neurological sequellae were noted. We compare theses results with two previous trials, concerning inoperable patients, treated by an association of radiotherapy and systemic chemotherapy. Survival seems to be equivalent with HeCNU and with this treatment, but toxicity decreases with ACNU. Early radiotherapy does not increase complications. This treatment can be used for patients with inoperable malignant gliomas.

    Description: Las especies Yersinia ruckeri y Edwardsiella sp. son patógenos de importancia económica por la alta morbiletalidad que provocan en la acuicultura. En este trabajo se analiza la frecuencia de aparición y el comportamiento epizootiológico de ambos patógenos en los peces de cultivo en Cuba, en diferentes regiones del país y en sistemas de cultivo extensivo en embalses e intensivo en estanques y jaulas flotantes, del 2006 al 2008. Del total de brotes analizados, estas especies se encontraron en 18 eventos epizoóticos, cuatro en el 2006, 12 en el 2007 y dos en el 2008. Edwardsiella sp. se aisló en 16 de ellos, mientras que Yersinia ruckeri se encontró en nueve. Las especies afectadas fueron Oreochromis sp., Clarias sp. y Micropterus salmoides floridanus. Estos resultados indican que ambos patógenos provocan enfermedadades con importantes pérdidas económicas en Cuba y afectan con más frecuencia a las especies en cultivo intensivo.
    Description: Within Enterobactereaceae family only Yersinia and Edwardsiella genus are considered as fishes pathogens. Yersinia ruckeri and Edwardsiella sp. are the most important pathogens for aquaculture because their high morbility. In this study the frequency of apparition and the epizotiologic behavior of these bacterias in different regions of Cuba and in extensive and intensive culture system during the years 2006-2008 were analized. From the total of the epizootiologic events, these pathogens were found in 18: four in 2006, 12 in 2007 and two in 2008. Edwardsiella species were isolated in 16 and Yersinia ruckeri in nine. The affected species were Oreochromis sp., Clarias sp. and Micropterus salmoides floridanus. These results indicate that both pathogens cause illness with important economic looses in Cuba and that they affect species in intensive culture more frequently.
