    Notes: A high incidence of positive patch tests to merthiolate (thimerosal) has been observed in young adult males without previous or present skin disease. In battery-tested clinical material several positive merthiolate tests occurred, mainly in the younger age groups, while a high-age group did not react even to a strong merthiolate patch test. Although the clinical and histological features of the reaction are mainly of an allergic type, evidence is presented that the merthiolate reaction should be considered a false positive test, peculiar to that substance and to young skin.

    Notes: Because of reports of sensitization and/or elicitation of contact allergy to orthopaedic metallic implants we performed a prospective study in 60 patients operated for extremity fractures. The implants, consisting of plates, screws, wires, pins and staples, were all made of stainless steel. The patients were tested epicutaneously and intracutaneously with nickel, cobalt and chromium within 2 days of surgery and one year later. It was possible to retest and evaluate 48 patients (80%). Of these 48 patients three were tested positive to nickel at the primary testing; they were also positive at the secondary testing. The remaining 45 patients were all negative to the three metals at both test occasions. There were no dermatologic or orthopaedic complications due to the materials used. The present prospective study did not reveal any cases of contact allergy or dermatitis as a consequence of metallic bioimplants in the treatment of extremity fractures.

    Notes: In a controlled double-blind experimental study, patients with contact allergy to nickel or gold were exposed systemically to their contact allergen. Nickel sulfate was given orally, gold sodium thiomalate intramuscularly, and placebo by both routes. Clinical reactions including body temperature were followed and 7 plasma cytokines and acute phase reactants were assayed before and 24 h after provocation. Clinical flare-up, with reactivation of healing patch tests and/or toxicoderma-like reactions, was observed in patients receiving their specific allergen. A transient fever was noted in patients allergic to gold when exposed to gold, not in others. There was a significant increase in plasma tumour necrosis factor-α, soluble tumour necrosis factor receptor 1, inter leukin-1 receptor antagonist, and neutrophil gelatinase associated lipocalin in patients allergic to gold when exposed to gold allergen. Furthermore, in patients allergic to nickel exposed to nickel allergen there was a highly significant increase in plasma soluble tumour necrosis factor receptor 1. Interferon-gamma, interleukin-4, and C-reactive protein did not increase in any of the patient groups. Clinical reactions during flare-up in contact allergy are accompanied by release in the blood of several cytokines. This release is not a consequence of a toxic effect or bound to a particular allergen. Instead, the cytokine release should be considered a general phenomenon characteristic of the flare-up in contact allergy.

    Notes: The cutaneous sensitivity to benzalkonium chloride, sodium lauryl sulphate and potash soap was determined in 54 monozygotic and 46 dizygotic twin pairs. Comparing the intra-pair reaction strength a higher degree of concordance was found among monozygotic than among dizygotic twins, and even more so when monozygotic twins were compared with matched controls.

    Notes: By and large, positive patch tests with thimerosal lack clinical relevance and should cause concern neither to the dermatologist nor to the patient. The substance should be retained in standard test series for scientific purposes, but on clinical grounds its inclusion does not seem to be indicated. Nevertheless, the phenomenon is intriguing in its own right.

    Notes: Guinea pigs were sensitized to dinitrochlorobenzene (DNCB) by the intracutaneous route and challenged epicutaneously tin the flanks. The intensity of the allergic contact dermatitis was evaluated by inspection and palpation as well us by wet weight determination. With the purpose of diminishing tissue neuropeptides, and substance P in particular, the animals were treated with capsaicin either between induction and challenge, or before sensitization, In comparison with controls, the contact dermatitis was Enhanced in both groups of animals treated with capsaicin.
