Notes: Large numbers of milia were seen in areas of skin affected with bullae in a 68-year-old woman who had extensive lichen sclerosus et atrophicus. Although milia are frequently seen in other sub-epidermal bullous disorders, they have not previously been reported in lichen sclerosus et atrophicus.

Notes: A case of chronic benign familial pemphigus (CBFP) is described in which a severe exacerbation was associated with infection with Herpesvirus hominis. This occurrence, which has a bearing on treatment, may not be as uncommon as the absence of previous reports would suggest.

Notes: Four cases of herpes simplex virus infection in exacerbations of pemphigus are described. Three of these patients had severe generalized infection requiring systemic anti-viral therapy. The presenting features, methods for rapid viral diagnosis and appropriate treatment are discussed.

Notes: Summary Stetocystoma multiplex is usually a monor cosmetic problem by rarely can be extremely disfiguring. Unfortunately, the very patients who would benefit most from treatment are usually regarded as having too may cysts for surgery. We report a case of extensive steatocystoma multiplex which was treated by a simple surgical technique under general anaesthetic.

Notes: Two brothers are described who were affected with Gardner's syndrome. Both had epidermoid cysts scattered over their irunk and limbs. Three of four epidermoid cysts removed from one of them and examined histologically showed an unusual change in which part of the wall of each cyst was replaced by granulation tissue and contained groups of shadow cells. Histo-logical and histochemical studies established that these were epidermoid cysts and not pilomatrixomas.Gardner's syndrome is a genetically determined disorder in which multiple adenomatous polyps of the colon and rectum are associated with soft and hard tissue tumours (Gardner, 1951). Epidermoid cysts are the soft tissue tumours that occur mosl commonly in this syndrome, but on two occasions a calcifying epiihelioma of Malherbe (pilomatrixoma) has been reported (Piffaretti & Foroglou, 1965; Braillon, Chapuis & Boulanger, 1972).In the family described in this paper, the proband had three soft tissue tumours on his legs which histologically showed some features suggestive of pilomatrixomas. However, each was a typical epidermoid cyst in which part of the wall was replaced by granulation lissue and there were groups of shadow cells within the cyst. It is suggested that these lesions represent unusual epidermoid cysts rather than pilomatrixomas.

Notes: A 25-year-old male is described who had extensive linear comedo naevi present from birth, involving the face, both sides of the trunk, both arms, one palm, both legs and one sole. Within these lesions multiple trichilemmal cysts were arising giving rise to a zosteriform pattern of skin nodules. Both lesions were confirmed histologically. It is suggested that comedo naevus is a hamartomatous abnormality of the hair follicle in which all layers, including the external root sheath, are involved and from which trichilemmal cysts may arise.

Notes: In a study often families with the basal cell naevus syndrome, epidermoid cysts occurred on the skin in 55%, of affected individuals. The distribution of these cysts, mainly on the limbs, is very similar to that occurring in Gardner's syndrome, but unlike the latter disease the presence of cysts is not helpful in making the diagnosis, nor is it of any prognostic significance.

Notes: Patients diagnosed as suffering from erysipelas or cellulitis were subjected to bacteriological and serological investigations. The serological tests used included the anti-streptolysin O reaction (ASO), the anti-deoxyribonuclease B test (ADB) and the anti-hyaluronidase tests (AHT) that are specific both for the group A streptococcus (Streptococcus pyogenes) and for the human pyogenic streptococci of group C or group G. Antibody tests to the α-lysin and the nuclease of Staphylococcus aureus were also employed. Conventional bacteriological culture methods were used plus needle aspiration of injected saline in most patients with erysipelas, but recognized pathogens were isolated in only 42% of cases. Our results indicate the limitations of these tests for making initial diagnoses and deciding treatment. Serial serological testing was very successful in differentiating cellulitis due to group A, C or G haemolytic streptococci, or occasionally Staphylococcus aureus, but was positive in only 40% of cases of erysipelas.

Notes: Thirty-nine clinically uninflamed cysts of the three most common varieties, epidermoid cysts, tri-chilemmal cysts and steatocystoma multiplex were removed under sterile conditions and the contents cultured under aerobic and anaerobic conditions. Seventy-three percent of epidermoid cysts grew significant numbers of organisms whereas none of the trichilemmal cysts did so. The organisms found were the common skin commensals, Staphylococcus epidermidis biotype I, anaerobic Gram positive cocci of the Gaffya type and Corynebacterium acnes Type I. This indicates that cysts which clinically have a punctum, namely epidermoid cysts, may become colonized with surface bacteria under normal circumstances. If they become colonized with pathogenic bacteria they become inflamed, a not uncommon clinical picture. The results from patients with steatocystoma multiplex were difficult to interpret because of the small numbers of patients involved and the unusual clinical appearance they presented.
