Zusammenfassung Bei 14 Patienten nach größeren abdominal-chirurgischen Eingriffen bei unkompliziertem postoperativem Verlauf wurden hämodynamische, blutgasanalytische und biochemische Messungen im arteriellen, prähepatischen und posthepatischen Blut durchgeführt. Folgende Parameter sind erfaßt worden: HZV, arterieller Druck; Pfortader- und Lebervenendruck, ZDV; Blutgase und pH; Lactat, Pyruvat und Glucose. Es werden errechnete Mittelwerte aus Messungen am 1. und 2. postoperativen Tag mitgeteilt. Ergebnisse: 1. In der frühen postoperativen Phase ist der Pfortaderdruck nicht erhöht. Es besteht ein niedriger transhepatischer Druckgradient. 2. Die arterio-portale O2-Gehaltdifferenz ist gegenüber der GesamtkörperAVD02 niedrig. 3. Der transhepatische Lactatgradient ist negativ, d. h. die Leber behält in der frühen postoperativen Phase eine Lactat extrahierende Funktion.

Summary After major abdominal operations without postoperative complications in 14 patients haemodynamic, blood gas analytical and biochemical estimations were performed in arterial, prehepatic and posthepatic blood. The following parameters were included: cardiac output, arterial pressure, portal and hepatic vein pressure, central venous pressure, blood gases and pH, lactate, pyruvate, and glucose. Median values were calculated from estimations on the 1st and 2nd postoperative day. Results: 1. In the early postoperative phase the portal pressure is not increased. There is a low transhepatic pressure gradient. 2. The arterio-portal O2 content difference is low compared with the whole-body AVDO2. 3. The transhepatic lactate gradient is negative, i.e. the liver retains a lactate-extracting function in the early postoperative phase.
