    Notes: Abstract The problems encountered in the diagnosis and treatment of primary hyperparathyroidism were studied in 69 cases. The accuracy of imaging for hyperplasia was less than that for adenoma or carcinoma and the major causes for multiple operations were a failure to locate the four glands and mediastinal adenoma. The intravenous administration of high doses of calcitonin could reduce the serum calcium level of patients in hypercalcemic crisis. Carcinoma required ipsilateral modified radical neck dissection because of lymph node metastases, and non-medullary thyroid carcinoma was often associated with primary hyperparathyroidism. We found removal of the parathyroid adenoma and biopsy or extirpation of only one macroscopically normal gland to be a fully satisfactory procedure after bilateral neck exploration and attempting to identify at least four glands.

    Notes: Abstract We report the unusual case of a 77-year-old man with cervical Castleman's disease associated with benign M-proteinemia. The patient was found to have an anterior cervical tumor during a follow-up examination after surgery for early gastric cancer, at which time blood biochemistry tests revealed M-proteinemia at the position of β-globulin with a high level of IgG and low levels of IgA and IgM. Serum protein immunoelectrophoresis revealed M-protein consisting of the IgG-k chain, and urine protein immunoelectrophoresis demonstrated Bence Jones protein consisting of the chain. No abnormalities were noted in bone marrow aspiration or bone scintigram. The results of imaging and fine-needle aspiration biopsy of the tumor led to a preoperative diagnosis of cervical malignant lymphoma with suspected M-proteinemia derived from this disease, and resection of the anterior cervical tumor with bilateral neck dissection was performed. Histopathological examination subsequently demonstrated four mixed-type tumors of Castleman's disease. The pathogenesis of Castleman's disease remains unknown; however, we speculate that the complication of benign M-proteinemia in this patient was not incidental, but caused by an underlying immunological abnormality of the B cells.

    Notes: Abstract Leiomyomatosis peritonealis disseminata (LPD) is a rare condition, reported to occur chiefly in premenopausal women. In fact only five cases of LPD in postmenopausal women have been documented in the English literature. We report herein a case of LPD occurring in a postmenopausal woman who had undergone a total hysterectomy 15 years earlier and received no subsequent hormonal therapy. At laparotomy, a cystic mass measuring 10.0×12.0 cm was found adhering to the jejunum. There were also innumerable tumor nodules ranging from 0.5 to 5.0 cm in diameter attached to the antimesenteric borders of the intestine and the parietal peritoneum. We resected the cystic main tumor together with 20 cm of jejunum to release the intestinal obstruction. Several daughter tumors were also resected as biopsies for histological examination. Histologically, the lesions were multiple and consisted of proliferating smooth muscle cells with rod-like nuclei showing extremely rare mitotic figures, confirming a diagnosis of LPD.

    Description / Table of Contents: Résumé On a étudié les effects de la présence de facteur de croissance des fibroblastes (bFGF) en utilisant les anticorps anti-bFGF chez les patients ayant une hyperplasie des parathyroïdes dans le cadre des néoplasies endocrines multiples de type I (MEN-I) et chez les patients ayant une hyperparathyroïdie primitive non MEN. On a ensuite comparé ces résultats aux données de l'analyse cytométrique de flux de l'ADN. Une coloration immunohistologique avidine-biotine-peroxydase a été obtenue chez 25 glandes parathyroïdes provenant de 11 patients ayant une MEN-I et 38 glandes provenant de 20 patients avec une hyperparathyroïdie primitive non MEN. On a défini comme résultat positif, une coloration uniforme de plus 50% des cellules. On a analysé le contenu d'ADN en cytométrie de flux chez 18 glandes parathyroïdes provenant de patients ayant une MEN-I et chez 24 glandes provenant de patients avec une hyperplasie non MEN. La proportion des glandes ayant une coloration positive a été de 72% chez les patients MEN-I et d e 18% chez les patients non MEN, différence statistiquement significative (p〈0.01). Les nodules des parathyroïdes étaient composés de cellules oxyphiliques chez 7 des 25 glandes parathyroïdes provenant de patients ayant une MEN-1 et chez 10 des 38 glandes provenant de patients avec une hyperparathyroïdie primitive non MEN et toutes les cellules étaient positives pour la bFGF. Il n'y avait aucune corrélation significative entre les cellules positives pour la bFGF et la cartographie ADN. Il est possible que la bFGF joue un rôle dans le développement de l'hyperplasie des glandes parathyroïdes, surtout dans la maladie MEN-I. L'augmentation des cellules oxyphiliques est peut-être corrélée avec l'expression de la bFGF.
    Abstract: Resumen El hiperparatiroidismo primario fue investigado desde el punto de vista inmunohistoquímico mediante la determinación de la presencia del factor básico de crecimiento fibroblástico (bFGF) utilizando un anticuerpo anti-bFGF, en las glándulas paratiroides hiperplásicas de pacientes con neoplasia endocrina múltiple tipo I (NEM-I) y de pacientes sin NEM, observándose que los resultados se correlacionan bien con el análisis de DNA. Veinticinco glándulas hiperplásicas de 11 pacientes con NEM-I y 38 de 20 pacientes con hiperparatiroidismo primario no-NEM, fueron inmuno-histológicamente coloreadas por el procedimiento del complejo avidina-biotina-peroxidasa. Se consideró como positiva la coloración cuando 50% o más de las células aparecieron teñidas on forma uniforme; además, también se hizo el análisis de DNA por citometría de flujo de 18 glándulas hiperplásicas de pacientes con NEM-I y de 24 glándulas hiperplásicas de pacientes no-NEM. La proporción de las glándulas positivas a la coloración fue de 72% en los pacientes con NEM-I y de 18% en los no-NEM, siendo significativa la diferencia entre los dos Grupos (p〈0.01). Los nódulos consistieron en células oxifilicas en 7 de 25 glándulas hiperplásicas en los casos NEM-I y en 10 de 38 glándulas hiperplásicas en los casos no-NEM; todas las células fueron positivas para bFGF. El bFGF posiblemente juegue un papel en el desarrollo de la hiperplasia de las glándulas paratiroides, especialmente en los pacientes con NEM-I. El aumento en el número de células oxifilicas puede estar correlacionado con la expresión de bFGF.
    Notes: Abstract Primary hyperparathyroidism was investigated using the presence of basic fibroblast growth factor (bFGF) from the immunohistochemical viewpoint with an anti-bFGF antibody in hyperplastic parathyroid glands of patients with multiple endocrine neoplasia type I (MEN-I) and of patients with non-MEN. The results corresponded well with the data from the DNA analysis. Twenty-five hyperplastic parathyroid glands from 11 patients with MEN-I and 38 glands from 20 patients with non-MEN primary hyperparathyroidism were stained immunohistochemically according to the avidin-biotin-peroxidase complex procedure. When 50% or more of the cells appeared uniformly stained, it was judged positively stained. In addition, 18 hyperplastic parathyroid glands from patients with MEN-I patients and 24 hyperplastic parathyroid glands from non-MEN patients were also analyzed for DNA using flow cytometry. The ratio of positively stained hyperplastic parathyroid glands was 72% in MEN-I patients and 18% in non-MEN patients. The difference between the two groups was significant (p〈0.01). The nodules consisted of oxyphilic cells in 7 of 25 hyperplastic parathyroid glands from MEN-I patients and in 10 of 38 hyperplastic parathyroid glands from non-MEN patients, and all the cells were positive for bFGF. There was no significant correlation between bFGF staining and the DNA ploidy pattern. bFGF possibly plays a role in the development of parathyroid gland hyperplasia, especially in MEN-I patients. The increase of oxyphilic cells may be correlated with the expression of bFGF.

The diastereomer 26 of 2 yielded the β-diol isomer 49 of (-)-K-76, and 26′ gave another diastereomer 47′ of O-methyl K-76.
