    Notes: Summary The effects of a sulphonylurea, gliquidone, on insulin binding and the insulin induced rate of glycogen synthesis, were studied in rat hepatocytes in primary culture. Hepatocytes were cultured for 48 h. During the second 24 h of this period, the hepatocytes were incubated with or without gliquidone (5 mg/l). The binding of 125I-insulin and the insulin stimulation of glycogen synthesis from 14C-glucose were measured. Gliquidone influenced neither insulin binding nor the basal rate of glycogen synthesis, but it did enhance the effect of insulin on glycogen synthesis. Responsiveness was increased by gliquidone at all insulin concentrations used (10–10,000 mU/l); at 1000 mil/l the drug increased glycogen synthesis from 310 to 430% above the basal rate. Half-maximal stimulation was reached in control cells at an insulin concentration of 95 mU/l and in gliquidone-treated cells at 94 mU/l, which indicates unchanged insulin sensitivity. Based on these experiments with cultured rat hepatocytes it appears that the extrapancreatic action of gliquidone is not mediated by an effect on insulin binding.

    Notes: Summary The effect of the catecholamine isoprenaline (10−5mol/l) and of the tumour promoting phorbolester tetradecanoyl-β-phorbol acetate (10−9mol/l) on insulin stimulated 3-O-methyl-glucose transport was studied in freshly isolated human adipocytes. Both substances reduced the maximal responsiveness of the glucose transport system to insulin by approximately 50%. To test if this is caused by inhibition of the insulin receptor kinase the receptor from phorbolester and isoprenaline treated cells was solubilized, partially purified and its kinase activity studied in vitro. Insulin stimulated 32P-incorporation into the β-subunit of the insulin receptor of phorbolester or isoprenaline treated cells was reduced to 20–60% of the values found with receptor from control cells at insulin concentrations between 10−10mol/l and 10−7mol/l. This inhibition of kinase activity of receptor from phorbolester and isoprenaline treated cells was observed at nonsaturating adenosine triphosphate levels (5 μmol/l), and it could be overcome with higher concentrations of γ-32P-adenosine triphosphate in the phosphorylation assay. A Lineweaver Burk analysis of the insulin stimulated receptor phosphorylation revealed that the Michaelis constant for adenosine triphosphate of the receptor kinase from phorbolester and isoprenaline treated cells was increased to 〉100 μmol/l compared with 〈50 μmol/l for receptor from control cells. We conclude from the data that catecholamine and phorbolester treatment of human adipocytes modulates the kinase activity of the insulin receptor by increasing its Michaelis constant for adenosine-triphosphate, and propose that this modulation of receptor kinase is a mechanism that can contribute to the pathogenesis of insulin resistance in human fat cells.

    Description / Table of Contents: Zusammenfassung An der Klinik und Poliklinik für Allgemein-und Abdominalchirurgie der Johannes-Gutenberg-Universität Mainz wurden zwischen 1984 und Februar 1992 6 Patienten wegen einer aortoduodenalen Fistel operiert. Anamnese, Symptome, Diagnostik und operative Behandlung dieser Patienten werden anhand anderer Studienergebnisse analysiert und dargestellt. Bei allen Kranken wurde zwischen einem und 10 Jahren vor Auftreten der aortoduodenalen Fistel eine Aortenprothese implantiert. Führendes Symptom war die gastrointestinale Blutung. Die Angiographie und die Computertomographie waren in unserem Krankengut die Methoden mit der größten Treffsicherheit. Bei 5 Patienten wurde die infizierte Prothese explantiert, 4 dieser 5 Patienten erhiel ten anschließend einen axillobifemoralen Bypass, em Kranker starb vor der Rekonstruktion. Die Direktnaht der Aorta und Verschluß der Darmläsion mit anschlieBender Netzinterposition wurde bei einem weiteren Patienten durchgeführt. Zwei Kranke, unter Notfallbedingungen im Volumenmangelschock operiert, starben innerhalb der ersten 6 postoperativen Tage. Die anderen Patienten überlebten einen Zeitraum von mindestens einem Viertel bis zu einem Jahr.
    Notes: Abstract Six patients were operated on for an aortoduodenal fistula at the Department of General and Abdominal Surgery, Johannes-Gutenberg-University Mainz. All patients had received an aortic graft implant between 1 and 10 years ago. The etiology, symptoms, and diagnostic and surgical treatment of these six cases are presented, and the results are discussed in comparison with the results of other studies. The main symptom of all of our patients was gastrointestinal bleeding. In our patients arteriography and computed tomography were the best diagnostic procedures. At surgery, five patients underwent graft excision and axillobifemoral bypass (only four patients, as one died before implantation). In the other patient a local repair, with closure of the graft defect and bowel defect, followed by interposition of an omental pedicle, was performed. Two patients died within 6 days of operation due to multiple organ failure.

    Description / Table of Contents: Zusammenfassung Im Zeitraum von September 1985 bis Februar 1994 wurde bei 183 Patienten ein rekonstruktiver Eingriff im aortoiliakalen Bereich durchgeführt. Die Grunderkrankung war in 108 Fällen eine arterielle Verschlußkrankheit, in 75 Fällen ein abdominales Aortenaneurysma. Bei 120 Patienten wurde eine Bifurkationsprothese, bei 63 Patienten eine Rohrprothese implantiert. Die Früh- und Spätkomplikationen werden beschrieben, differenziert nach chirurgisch und nichtchirurgisch, wobei die chirurgischen Komplikationen im wesentlichen durch die Prothesenart und durch den Leistenanschluß bedingt waren, während die nichtchirurgischen Komplikationen vom Risikoprofil des Krankenguts abhängig waren. Die Unterschiede in der postoperativen Letalität ergaben sich durch die Dringlichkeit des Eingriffs. Der Stellenwert der endovaskulären Rekonstruktion im Vergleich zur rekonstruktiven Gefäßchirurgie wird diskutiert.
    Notes: Abstract From September 1985 to February 1994, 183 pa- tients (arterial occlusive disease n = 108, abdominal aneurysm n = 75) underwent reconstruction of the abdominal aorta. In 120 patients an aorto-bi-iliac or aorto-bi-femoral reconstruction was performed and in 63 patients a tube-type reconstruction. The early and late postoperative complications are described. The complications were dependent on the choice of graft, the distal anastomosis and the preoperative risk factors. The mortality from elective repairs was compared with the mortality in emergency repairs. The role of endovascular reconstructive surgery in comparison to conventional reconstructive procedures is discussed.

    Description / Table of Contents: Zusammenfassung Intensivmedizinische Maßnahmen werden von den Patienten, den Angehörigen und dem Personal unterschiedlich bewertet. Ein mangelndes Verständnis für die Intensivtherapie kann zu einer negativen Einstellung zur Intensivmedizin führen, deren Bild in der Öffentlichkeit prägen und die Identifikation der Mitarbeiter mit der Intensivmedizin stören.¶   Zur Objektivierung führten wir auf unserer Intensivstation eine Befragung zur medizinischen Technik, zur räumlichen Umgebung, zu allgemeinen medizinischen Fragen und zum Ausmaß der intensivmedizinischen Therapie bei 40 Mitarbeitern (11 Ärzte, 23 Pflegekräfte, 6 Beschäftigte des Hilfspersonals) durch. Die Dauer der Beschäftigung auf der Intensivstation reichte von 1–240 Monaten (Median 18 M.), das Alter lag zwischen 21 und 53 Jahren (Median 30 J). 73% der Befragten hielten die Intensivmedizin in der jetzigen Form für sinnvoll, bei den Ärzten waren dies 100%, beim Pflegepersonal jedoch nur 57%. Nur 9% der Ärzte erachteten die Geräte als beängstigend für den Patienten gegenüber 61% beim Pflegepersonal. Die Geräuschkulisse wurde von 65% des Pflegepersonals als stark belästigend empfunden, 45% der Ärzte sahen dies genauso. Nur 22% des Pflegepersonals glaubten, dass sich die Patienten auf der Intensivstation wohl und beschützt fühlten, 48% gingen von unglücklich bis sehr unglücklich aus. 46% der Ärzte waren der Ansicht, dass sich die Patienten wohl fühlten. 100% des Pflegepersonals waren der Meinung, dass bei den verstorbenen Patienten die organersetzende Therapie zu lange fortgeführt würde, wohingegen nur 45% der Assistenzärzte dieser Meinung waren.¶   Vom ärztlichen Personal wurden die intensivmedizinischen Maßnahmen als sinnvoll, für den Patienten weniger belastend und effektiver eingestuft als durch das Pflegepersonal. Die Gruppe des Pflegepersonals vertrat in vielen Fällen eine konträre und negativistische Meinung, die ihre Ursache möglicherweise in Unsicherheit, mangelnder Information und in fehlenden Erfolgserlebnissen hat. Die kompetente Aufklärung und Information über Krankheit und Prognose, die wiederholte Besprechung diagnostischer und therapeutischer Maßnahmen innerhalb des Intensiv-Teams sind daher neben dem Patientenoutcome die entscheidenden Faktoren der Meinungsbildung zur Intensivmedizin.
    Notes: Summary The measures taken in intensive care medicine are rated differently by the patients, their family, and personnel. A lack of understanding for intensive care therapy can lead to a negative attitude towards intensive care medicine, which in turn makes an impression on public opinion, and disturbs the reputation of the co-workers in intensive care medicine.¶   To form an objective view, we carried out a survey of 40 co-workers (11 physicians, 23 from the nursing staff, and 6 ancillary personnel) regarding the medical technology, the environment in the unit, general medical questions, and the extent of intensive care therapy. The length of employment in the intensive care unit ranged from 1–240 months (median 18 months), the ages ranged from 21 to 53 years (median 30 years). Of those questioned 73% found intensive care medicine in its current form to be purposeful: 100% of the physicians and only 57% of the nursing staff were of this opinion. Only 9% of the physicians believed the equipment to be fear-inducing for the patients; in contrast, 61% of the nursing staff believed this to be true. The noise level was rated as very disturbing by 65% of the nursing staff and 45% of the physicians. Only 22% of the nursing staff believed that the patients felt comfortable and protected on the intensive care unit; 48% felt that they were unhappy or even miserable. Of the physicians 46% believed that the patients were comfortable. Of the nursing staff 100% were of the opinion that for the deceased patients, organ substitution therapy had continued too long. On the other hand, only 45% of the residents shared this opinion.¶   The medical staff rated the intensive care measures as purposeful, less disturbing for the patients and more effective than did the nursing staff. In many cases, the nursing staff as a group exhibited a contrary and negative opinion. This could possibly be caused by insecurity, lack of information and lack of successful experiences. Next to patient outcome, the competent explanation and information regarding the disease and prognosis and the recurrent communication within the intensive care team, with respect to diagnostic and therapeutic measures are the deciding factors in the opinion formation regarding intensive care medicine.
