Notes: Summary Since the salivary glands are rich of norepinephrine, cyclic AMP as second messenger of β-adrenergic function was determined in the parotid saliva of patients with arterial hypertension. The cAMP was determined in correlation to the flow rate by the radio-isotope dilution test of Gilman. The corresponding plasma renin activity was determined by radioimmunoassay. In patients with normal renin hypertension and with high renin hyper-tension an increase of the cAMP concentration up to six times the normal range could be observed, while in low renin hypertension the values were similar to those of a normal control group. The increased secretion of cAMP could be inhibited by propanolol. The conclusion is, therefore, that the β-adrenergic system is activated in normal and high renin hypertension.

Notes: Summary In the perfused cat submandibular gland (according to Petersen and Poulsen) the effect of various diuretics on saliva secretion and K+ accumulation was investigated. It is known that stimulation with ACh causes a sudden K+ release and subsequent K+ uptake in the acinar cells, which can be studied in the venous efflux of gland. Ouabain (10−5 M), given intraarterially, blocked K+ uptake and saliva secretion. In the unstimulated gland Ouabain caused an increased K+ efflux, probably due to an ACh liberation at the nerve endings (similar results were published by Poulsen, 1974). High dosis of furosemide (10−3 M) reduced saliva secretion and K+ uptake. There was no change of K+ efflux initially. Amiloride even at a high dosage (0.5×10−3 M) did not alter saliva secretion and K+-accumulation. However, in the saliva Na+-concentration increased and K+-concentration decreased slightly, indicating an effect on the duct system. K + -canrenoate (10−3 M) decreased K+-efflux, K+-uptake and saliva secretion. Probably, this effect is due to an anticholinergic action on the ACh receptor protein.

Notes: Summary The influence of different sexual hormone levels during menstrual cycle on the composition of human parotid saliva was investigated. During midcycle there were significantly enhanced concentrations of ionized calcium, total calcium, inorganic phosphate and sodium in parotid saliva. The changes in chloride concentrations were parallel to that of sodium and the excretion of potassium was inverse to that of sodium. The maximal output of total protein andα-amylase was found during midcycle and menstruation.

Notes: Summary About 30% of hemodialyzed patients are suffering from chronic fluid overload despite advice to restrict the oral fluid intake. To investigate the cause of the abnormal drinking behaviour a clinical study was performed in 51 non-diabetic patients with endstage renal disease exhibiting lower interdialytic weight gain (〈3 kg,n=17) and increased interdialytic weight gain (〉3 kg,n=34). Blood pressure, body weight self-estimated thirst intensity before and after hemodialysis were analyzed. Biochemical and behavioral variables were measured including hormonal factors of water and sodium metabolism. Significant differences of dry weight, creatinine, urea nitrogen and thirst intensity were found between the two groups. Catecholamines, renin, angiotensin II, aldosterone, vasopressin and atrial natriuretic peptide exhibited a similar pattern in both groups. Atrial natriuretic peptide decreased during hemodialysis in both groups, angiotensin II, however, and norepinephrine showed an exaggerated response to ultrafiltration rate in polydipsic patients. These results suggest that changes in serum osmolality during hemodialysis did not contribute to thirst and drinking behaviour. It seems that postdialytic hypovolaemia together with higher plasma-angiotensin II-levels is responsible for increased oral intake of fluid and excessive weight gain.

Notes: Summary The present study was performed to investigate as to whether peripheral insulin resistance exists in spontaneously hypertensive rats (SHR). After a 12 h fasting period, SHR had significantly higher serum glucose and higher plasma glucagon values in comparison to normotensive control rats (WKY). There was a tendency for higher serum insulin concentrations as well, but this difference did not reach significance. After oral glucose loading or glucose/insulin administration, serum glucose and insulin levels were also higher in SHR compared to WKY rats. Muscle glycogen and glucose concentrations were identical in fasted SHR and WKY rats. With an oral glucose load or glucose/insulin treatment there was a significant increase in muscle glycogen, whereas glucose values declined in skeletal muscle. Both total (a+b-form) phosphorylase activity as well as the active a-form of the enzyme were similar in skeletal muscle of SHR and WKY rats. Glucose/insulin administration or oral glucose loading induced a considerable reduction of both a+b-form and a-form activities. The decrease in muscle phosphorylase activities was almost identical in both groups of animals. There was also a comparable activity of muscle glycogen synthetase activity in all groups of rats. Despite subtile changes of glucose, glucagon and to a lesser degree insulin levels which would be suggestive of insulin resistance, the data obtained from skeletal muscle argue against peripheral insulin resistance in spontaneously hypertensive rats.
