    Notes: Thiuram mix is tested in the standard series at a test concentration of 1% pet. The single thiurams (DPTD, TMTD, TMTM, TETD), however, are usually tested at 0.25% pet. in Germany. In other countries, the individual components of thiuram mix are tested at 1% pet. The German Contact Dermatitis Research Group (DKG) compared both patch test concentrations in 530 patients in order to find out if (i) a significant number of positive patch tests are missed by testing at the lower concentration, (ii) problems with irritant test reactions occur by increasing the test concentration to 1%, and (iii) the sensitivity of the thiuram mix rises when the breakdown test is done with the higher concentration. Slightly more positive reactions were seen with the higher concentration, but this increase did not reach statistical significance. The reaction index, as a measure for the relation of positive to irritant and/or questionable reactions, remained unchanged for the individual thiurams. The sensitivity of the mix also did not change when the breakdown test was performed with 1% pet. instead of 0.25% pet. Thus, we conclude that both concentrations are of equal diagnostic value in patch testing.

    Description / Table of Contents: Summary Fractionated irradiation with X-rays or megavoltage is an important therapeutic option in the palliative treatment of Kaposi’s sarcoma. We report on three HIV patients demonstrating bullous cutaneous eruptions in the irradiation field. Three homosexual men (age 45–60 y) stage of disease CDC 3C, were treated with fractionated soft X-rays with the scheme conventionally used by us (total dose 30 Gy, single doses 3 Gy). Immediately to three days after completion of treatment large blisters appeared within the irradiation field, resolving after 2–4 weeks, leaving postinflammatory hyperpigmentation. The cause of this rare reaction to soft X-ray therapy, potentially suggesting an individually altered radiation sensitivity, is so far unknown. Controlled prospective studies applying different fractionation schemes will be necessary to better elucidate this issue.
    Notes: Zusammenfassung Die fraktionierte Röntgenbestrahlung ist eine wichtige Behandlungsform des Kaposi-Sarkoms. Wir berichten über 3 HIV-positive Patienten, bei denen nach fraktionierter Röntgenweichstrahltherapie von Kaposi-Sarkomen bullöse Hautreaktionen im Bestrahlungsfeld auftraten. Drei homosexuelle männliche Patienten (45–60 Jahre, Krankheitsstadium CDC 3C) wurden mit Röntgenweichstrahlen (43 kV, 25 mA, FHA 15 u. 30 cm, Einzeldosis 3 Gy 5mal/Woche, Gesamtdosis 30 Gy) behandelt. In den ersten 3 Tagen nach Beendigung der Strahlentherapie kam es im Bestrahlungsareal zu einer bullösen Reaktion, die sich innerhalb von 2–4 Wochen unter Hinterlassung von Restpigmentierungen zurückbildete. Die Ursache dieser seltenen Reaktion nach Röntgenweichstrahltherapien, die auf eine individuell erhöhte Strahlenempfindlichkeit hinweist, ist nicht bekannt.
