    Notes: Abstract  Objectives: Dental care for children is available in France, but the method of administration is cumbersome: care is generally provided by private dentists, and parents can claim reimbursement of at least 70% of the costs afterwards. In the Département of Val d'Oise (north of Paris and including some densely populated suburbs), a scheme is in place to provide 100% reimbursement. To encourage participation, schoolchildren are screened by dentists who advise if treatment is needed. The objective of this study was to ascertain the effectiveness of screening as a stimulus for seeking dental care.Methods: In two towns, 6–7-year-old schoolchildren were cluster-sampled to give 507 participants. At screening, about half of the participants were found to be in need of operative treatment. Questionnaires about demographic and other background factors were sent home. From those needing treatment, 186 (77%) sets of questionnaires were returned. These persons were the analytical basis of the study. Six months after screening, the 186 participants were examined to estimate whether they had sought and received treatment as advised.Results: The majority of the participants came from low socioeconomic and deprived backgrounds. Most of the parents were born in countries outside Europe. Only about one-quarter of the children advised to seek treatment had actually done so after 6 months. Slightly under half of those who were treated had claimed the reimbursable amount after the treatment. Of the 186 participants needing treatment, 24% had more than 4 dt + DT but 70% of these had not received any care.Conclusion: These findings suggest that even when costs are totally reimbursed, most children in need of care do not receive it; the greater the need of care, the lower the likelihood of getting it. The deprived people and immigrants frequently inhabiting the Parisian suburbs seem to experience cultural, financial, linguistic, and administrative barriers to care, which impede uptake and which must be changed if these children are to enjoy improved dental health.

    Notes: Abstract Three cases of pericardial tamponade caused by central venous catheters are reported. Two essential factors have been recognized. First, the catheter material should be extremely soft and remain soft. Today the best material available seems to be a silicone rubber elastomer. Polyvinylchloride or polyethylene, which is widely used, is too rigid. These materials also contain phthalate esters to soften the plastic, and these softeners are gradually washed out into the bloodstream, rendering the catheters even more rigid in time. Second, a central venous catheter should enter the venous system proximal to the shoulder. When the catheter is inserted through a vein in the arm, the tip of the catheter will be able to progress 6–10 cm farther down in the venous system by extreme movements of the arm.     Notes: Abstract Excitonic photoluminescence spectra in the ZnS-ZnSe strained-layer superlattices with imperfect heterointerfaces were studied experimentally. It is shown that the energy states of excitons in these structures are affected both by interface imperfections and internal strain caused by the lattice mismatch between the semiconductors composing the superlattice layers.

    Description / Table of Contents: Résumé Les auteurs rapportent une expérience homogène de 10 ans d'une prothèse totale de genou cimentée semi-contrainte en acier inoxydable 316 L coulé et coussin tibial amovible en polyéthylène. Sur 68 patients opérés, 50 ont pu être suivis avec un recul moyen de 6 ans. Les résultats obtenus, selon le score 100 points, sont sensiblement comparables à ceux publiés dans la Littérature pour ce type de prothèse avec 84 % de malades entre 80 et 100 et un indice de satisfaction positif dans 94 % des cas. Toutefois, il est noté sur le plan anatomique une fréquence non négligeable de défauts de positionnement (5 %) et sur le plan fonctionnel, 40 % des cas présentent un sentiment d'insécurité à la descente des escaliers. Cette insécurité est liée principalement à des problèmes techniques en rapport avec un ancillaire très approximatif à l'époque étudiée et qui est maintenant amélioré. Malgré ses résultats relativement moyens, ce type de prothèse, en raison de sa facilité et rapidité de mise en place, doit encore trouver des indications chez des malades de grand âge aux exigences physiques modestes.
    Notes: Abstract The authors report an homogeneous series over ten years of a cemented total prosthesis of the knee, semi-constraint, in a stainless steel, 316 L cast, and a removable tibial cushion in polyethylene. On 68 patients operated on, 50 have been followed up with an average follow-up of 6 years. The results obtained, allowing a score of 100 points, are comparable to those published in the literature for this type of prosthesis, with 84 % of patients between 80 and 100, and a positive index of satisfaction in a 94 % of cases. Nevertheless, from an anatomical point of view, we noticed a non negligible frequency of a defect in position (5%) and from a fonctional point of view, 40% of the patients have a feeling of insecurity in going down steps. This feeling of insecurity is particulary linked to technical problems in relation with the instrumentation, very approximate at the time it was studied, and which is now improved. In spite of its average results, this type of prosthesis, thanks to its ease and speed of placement, must find some indications for elderly people with a modest requirements.

    Description: The Kelvin-Helmholtz instability in an ionized plasma is studied with a focus on the magnetic field generation via the Biermann battery (baroclinic) mechanism. The problem is solved by using direct numerical simulations of two counter-directed flows in 2D geometry. The simulations demonstrate the formation of eddies and their further interaction and merging resulting in a large single vortex. In contrast to general belief, it is found that the instability generated magnetic field may exhibit significantly different structures from the vorticity field, despite the mathematically identical equations controlling the magnetic field and vorticity evolution. At later stages of the nonlinear instability development, the magnetic field may keep growing even after the hydrodynamic vortex strength has reached its maximum and started decaying due to dissipation.
