    Notes: Abstract. Soil clean-up policies have been a major focus of environmental concern in the Federal Republic of Germany. With the unification of the two Germanies there is an increased need for these policies. The paper briefly describes the nature of the soil contamination problem, and the development of a soil clean-up programme. Clean-up policies are evaluated by comparison with practices in the USA.

    Notes: Summary.  Menorrhagia is a very common clinical problem among women of reproductive age and recent studies have suggested that underlying bleeding disorders, particularly von Willebrand's deficiency and platelet function defects, are prevalent in women presenting with menorrhagia. The objective of this study was to determine the utility of the platelet function analyser (PFA-100) and bleeding time (BT) as initial screening tests for underlying bleeding disorders in women with menorrhagia. In this study, 81 women with a physician diagnosis of menorrhagia underwent PFA-100 testing, BT and comprehensive haemostatic testing. The effectiveness of the PFA-100 and BT as screening tools in women with menorrhagia was assessed using results of haemostatic testing for von Willebrand's disease (VWD) and platelet dysfunction. In women presenting with menorrhagia, the PFA-100 had a sensitivity 80%, specificity 89%, positive predictive value (PPV) 33%, negative predictive value (NPV) 98% and efficiency 88% for VWD. For platelet aggregation defects, the PFA-100 closure time had a sensitivity 23%, specificity 92%, PPV of 75%, NPV of 52% and efficiency 55%. The data suggest that the PFA-100 may be useful in stratifying women with menorrhagia for further von Willebrand testing; however, neither the PFA-100 nor the BT tests are effective for purposes of classifying women for standard platelet aggregometry testing in women presenting with menorrhagia.

    Description / Table of Contents: In this study the course of osteochondritis dissecans at two often affected areas, the femoral condyles of the knee joint and the talus at the upper ankle joint, is evaluated by studying and comparing the medium-term outcome in 40 patients treated conservatively and 72 patients treated surgically by arthroscopy. The criterion of assessment was magnetic resonance imaging (MRI), which shows a close correlation to the arthroscopic stages of osteochondritis. The stages on MRI follow the morphology of the interface, i.e., the zone between the osteochondral fragment and the parent bone, on T1- and T2-weighted images. After 18 and 24 months there were no substantial differences visible on MRI between the two different treatment groups. In fact, in the majority of cases (〉 60%) the MRI result was the same in spite of the treatment that had been carried out. Arthroscopic treatment compared to conservative treatment resulted in a small percentage in a reintegration in the sense of upgrading on the MRI (36% to 30%); nevertheless, a disintegration could not be avoided in all cases (4%). In both treatment groups the age of the patient seems to play an important role in the course of osteochondritis, juvenile forms following a milder course than adult forms.
    Notes: In der vorliegenden Studie wird die Entwicklung der Osteochondrosis dissecans (OD) im Bereich 2er häufiger Lokalisationen, an den Femurkondylen des Kniegelenks und der Talusrolle des oberen Sprunggelenks, einerseits nach konservativer Therapie (40 Patienten) und andererseits nach operativer, arthroskopischer Therapie (72 Patienten) im mittelfristigen Verlauf geprüft und miteinander verglichen. Als Beurteilungskriterium wurde die MRT herangezogen, die eine enge Korrelation mit den arthroskopischen Stadien der OD aufweist. Die Stadieneinteilung in der MRT erfolgte anhand der Morphologie des sog. Grenzsaums, d. h. der Zone zwischen osteochondralem Herd und Mausbett, in der T 1 - und T 2 -gewichteten Spinechosequenz. Wesentliche Unterschiede zwischen den beiden Behandlungsgruppen bezüglich der Darstellung in der MRT zeigten sich im mittelfristigen Verlauf (18 bzw. 24 Monate) nicht. In der Mehrzahl der Fälle, (〉 60%) blieb der kernspintomographische Befund trotz der durchgeführten Therapie unverändert. Die arthroskopische Behandlung führte, im Vergleich zur konservativen Therapie, in einem geringfügig höheren Prozentsatz (36% im Vergleich zu 30%) zu einer kernspintomographisch nachgewiesenen Reintegration im Sinn eines upgradings, sie konnte eine Desintegration der OD jedoch nicht in allen Fällen verhindern (4%). In beiden Behandlungsgruppen nahm das Lebensalter Einfluß auf den Verlauf der OD mit günstigeren Verläufen bei der juvenilen als bei der adulten Form.

    Notes: Abstract Myotonic dystrophy (MD) is characterized by myotonia, weakness and extramuscular symptoms, including intellectual impairment. We performed magnetic resonance imaging (MRI) of brain and muscle in 25 MD patients: 81% had cerebral atrophy (severe in 36%); 68% had focal white matter lesions, which were large and multiple in 27%. Brain MRI findings correlated with mental impairment; the severity of both correlated with disease duration. Changes in brain and muscle MRI were progressive with time, but independent of each other. Muscle MRI findings were fatty degeneration and loss of bulk. In the calves, the medial gastrocnemius muscles were involved earliest and the posteriortibial muscles relatively spared. In the thighs the vastus muscles were damaged most often and the rectus femoris least. Focal muscle damage was efficiently visualized, sometimes preceding clinical detection. Muscle MRI was less sensitive than conventional methods for early diagnosis, but ideal for follow-up, owing to its non-invasiveness and examiner-independence.

    Notes: Summary We report the case of a 27-year-old woman with a prolactin-secreting macroadenoma of the pituitary gland who was under treatment with a new dopamine agonist, Sandoz CV 205-502. Immediately after diagnosis of a 12 × 10 mm intrasellar prolactinoma, treatment with CV 205–502 was begun at a daily dose of 0.075 mg. Under this low dose, prolactin in the serum normalized after 4 weeks, the initial hormone value being 189.9 ng/ml. After 6 months of therapy there was still a 6 × 6 mm tumor, and after 15 months of treatment the pathological process could no longer be observed with magnetic resonance imaging. In the 20th month of therapy the patient became pregnant. An ovulatory menstrual cycle had been present for a few months.

    Description / Table of Contents: Abstract Beta-trace protein is a lipocalin that was recently identified as prostaglandin D synthase and represents a major constituent of human cerebrospinal fluid. Beta-trace protein, similar to β2-transferrin, has been used as an immunological marker for the detection of cerebrospinal fluid. Between 1982 and 1999, 130 specimens from 101 patients with suspected cerebrospinal fluid leaks of the anterior or lateral skull base have been investigated for beta-trace protein. The specimens were analyzed by one-dimensional immunoelectrophoresis using Laurrell’s electroimmunoassay. In all, beta-trace protein could be detected in 57 specimens and was absent in 73. All case notes were studied retrospectively. In correlation with the clinical course and the findings of CT of the paranasal sinuses, high resolution CT of the petrous bones, CT cisternography, NMR, 111In-DTPA radionuclide cisternography, intraoperative findings, visualization of sodium-fluorescein stained cerebrospinal fluid using endoscopic blue light or testing for glucose, there was no false-positive result. A false-negative result occurred in two cases. These findings show that the beta-trace test has a sensitivity of 97.3% and a specificity of nearly 100%. Our findings show that analysis of beta-trace protein can be a valuable test for detecting CSF leakage and has certain advantages in comparison with the β2-transferrin assay.
    Notes: Zusammenfassung Das Beta-trace-Protein, ein Lipokalin, welches als Prostaglandin-D-Synthase (PGD-S) identifiziert werden konnte, ist ein Hauptvertreter der Proteine des menschlichen Liquor cerebrospinalis und ein immunologischer Marker für Liquor oder Liquorbeimengungen. Zwischen 1982 und 1999 wurden 130 Proben von 101 Patienten unter dem klinischen Verdacht einer Liquorfistel der vorderen oder seitlichen Schädelbasis auf Beta-trace-Protein hin untersucht. Die Proben wurden durch eine eindimensionale Immunelektrophorese (Laurrell’s electroimmunoassay) analysiert. In 57 Proben war Beta-trace-Protein nachweisbar, in 73 Proben nicht. Alle Krankenakten wurden retrospektiv ausgewertet. In Korrelation mit dem klinischen Verlauf und den Ergebnissen zusätzlicher Untersuchungen (Computertomographie (CT) der Nasennebenhöhlen, hochauflösendes Felsenbein-CT, CT-Zisternographie, Kernspintomographie, 111In-DTPA-Liquorszintigraphie, intraoperative Befunde, intrathekale Na-Fluoreszeinanwendung, Glukoseteststreifenuntersuchung) wurde die Validität des Beta-trace Tests bestimmt. Es fand sich kein falsch-positives Ergebnis. In 2 Fällen lag ein falsch-negatives Resultat vor. Der Beta-trace-Proteinnachweis hat eine Sensitivität von 97,3% und eine Spezifität von nahezu 100% und ist damit ein wertvolles Verfahren zum Aufdecken einer Liquorfistel, es bestehen Vorteile im Vergleich zum β2-Transferrin-Nachweis.
