    Notes: A frequent cause of contact urticaria is skin exposure to the common stinging nettle (Urtica dioica). The urticaria is accompanied by a stinging sensation lasting longer than 12 h. Little is known of the cellular and molecular mechanism of stinging-nettle urticaria. After preliminary pharmacological analysis of pro-inflammatory activity in nettle stings, the cellular response of mononuclear cells, polymorphonuclear cells and mast cells was examined in six people 5 min and 12 h after nettle contact. Only mast cell numbers were significantly increased at 12 h. Ultrastructurally, some mast cells showed evidence of degranulation at 5 min and 12 h. At 12 h mast cells were closely associated with dermal dendritic cells and lymphocytes suggesting a functional unit. The mean histamine and serotonin contents of a nettle hair were found to be 6.1 ng and 33.25 pg, respectively. Nettle-sting extracts did not demonstrate histamine release from dispersed rat mast cells in vitro. These results suggest that part of the immediate reaction to nettle stings is due to histamine introduced by the nettle. However, the persistence of the stinging sensation might suggest the presence of substances in nettle fluid directly toxic to nerves or capable of secondary release of other mediators.

    Notes: Abstract Radiographic contrast media are commonly used diagnostic aids to improve imaging, e.g. in computerized tomography. However, the routine application of these agents may cause adverse allergic/pseudoallergic reactions. In order to understand more completely the underlying mechanisms involved in these reactions, experiments on histamine release bothin vivo andin vitro are necessary. Using canine mast cell suspensions from lung and liver, we have investigated the histamine release caused by six commonly used preparations. The dog is an ideal model for bothin vitro andin vivo studies not only by virtue of its size but also because of its similarity to man with respect to e.g. cardiovascular reactions after drug-induced histamine release. The two non-ionic preparations (Solutrast, Ultravist) released little histamine from both cell types (ca. 4–6%). The ionic contrast media (Angiographin, Hexabrix, Telebrix, Rayvist) dose-dependently released histamine from the liver cells and pulmonary cells (maximum release between 18–35%). The liver cells (the liver is the shock organ in the dog) reacted more strongly to these agents than the pulmonary cells, thus providing further evidence for mast cell heterogeneity and the importance of selecting the appropriate mast cell model for the investigation.

    Notes: Abstract Radiographic contrast media in clinical use cause unwanted allergic and pseudoallergic reactions. To investigate the mechanisms of these reactions, studies on isolated mast cells from different species and sites are necessary. In this study, the effect of six commonly used contrast media on rat (peritoneal, lung) and human (lung) mast cells was investigated. The three preparations with low osmolalities (Hexabrix, Solutrast, Ultravist) released little or no histamine from the cells examined. In contrast, the three preparations with high osmolalities (Angiographin, Telebrix, Rayvist) were potent releasing agents. However, the degree of release and the order of potency was different depending on the cells investigated. Indeed, rat peritoneal mast cells required much higher concentrations before release was observed. Since the contrast media with low osmolality also cause histamine release and reactionsin vivo, other systems (e.g. complement) must be additionally involved.

    Notes: Abstract Cyclosporin A (CSA) is a strong immunosuppressive substance which is widely used for prevention of transplant rejection. Moreover, this compound is clinically active in different cutaneous inflammatory reactions. In the present study the effects of CSA on mediator release from activated human basophils and isolated human skin mast cells, as important effector cells of such skin diseases, were investigated. CSA exterted a dose-dependent inhibition of the IgE-mediated histamine release from both cell populations. The maximum inhibition was 64.4±9.3% and 85.8±14.2% for basophils and skin mast cells, respectively. Non-immunological histamine release induced by calcium ionophore A23817 was also inhibited. The present data suggest, therefore, that the anti-allergic properties of CSA on basophils and skin mast cells may be partially responsible for the strong anti-inflammatory effects of CSAin vivo.

    Notes: Abstract Catecholamine levels and associated cardiovascular responses were determined in 21 control and 30 term infants with meconium-stained amniotic fluid (MSAF). Cord arterial blood was obtained for determination for norepinephrine (NE) and epinephrine (EPI) levels; cardiovascular measurements included heart rate and systolic blood pressure at 30 min. Pregnancy was uncomplicated in all cases. The total length of labor 964±402 versus 555±312 min (P〈0.001) and active phase of labor 300±261 versus 135±104 min (P〈0.001) were significantly longer in MSAF infants when compared to controls. The 1 min Apgar score was 〈7 (range 1–6) in 11 of 30 MSAF infants versus 0 of 21 control infants; these 11 infants required positive pressure ventilation for approximately 60 s secondary to transient respiratory depression (RD). The 5 min Apgar score was 〉7 in 49 of the 51 infants. Infants with MSAF exhibited a higherP aCO2 (6.89±1.17 vs 6.24±1.17 mm Hg;P〈0.02) and lower pH (7.25±0.06 vs 7.29±0.05;P〈0.02) compared to controls. Infants with transient RD demonstrated the most marked arterial blood gas differences, i.e.,PCO2 7.41±1.30 mm Hg (P〈0.001), pH 7.21±0.07 (P〈0.001) and base deficit −6±4 versus −3±2.8 (P〈0.001) when compared to controls. NE and EPI levels were significantly higher in MSAF versus control infants, i.e., 12600±3040 versus 3740±1000 pg/ml (SEM) (P〈0.02) and 1550±250 versus 620±130 pg/ml (P〈0.001) respectively. The highest values were observed in infants with transient RD. NE levels were related to base deficit (r=−0.45,P〈0.0001) and the presence of meconium (r=0.26,P〈0.05), while EPI levels were related to pH (r=0.44,P〈0.001) and meconium (r=0.28,P〈0.05). MSAF infants exhibited a higher systolic blood pressure at 30 min, i.e., 59±10 versus 54±8 mm HgP〈0.002 when compared to controls. No differences in heart and respiratory rates were noted. These data indicate that infants delivered in the presence of MSAF exhibit significant elevations in catcholamine levels when compared to control infants, which may reflect an adaptive respone to in utero stress.
