    Abstract: Although chronic thromboembolic pulmonary hypertension (CTEPH) and chronic thromboembolic pulmonary disease (CTEPD) are known to be accompanied by symptoms associated with sleep-disordered breathing (SDB) and nocturnal hypoxemia, the sex-specific differences of SDB and nocturnal hypoxemia in patients with CTEPH and CTEPD remain unknown. Methods Between July 2020 and August 2022, data were retrieved from 57 males and 63 female patients with CTEPH and CTEPD who underwent sleep study at Shanghai Pulmonary Hospital. Nocturnal mean SpO 2 (mean SpO 2 ) & lt; 90% was defined as nocturnal hypoxemia. Logistic and linear regression analysis was performed to assess the predictive value of sleep study indices to hemodynamic parameters. Receiver operating characteristic (ROC) curve was applied to analyze the specific parameters to predict the risk of CTEPH. Results SDB was similarly present in males and females, and both sexes predominantly had obstructive sleep apnea (OSA); more women were diagnosed with nocturnal hypoxemia (32 vs. 7%, p = 0.002). SaO 2 was negatively associated with mean pulmonary arterial pressure (mPAP) in men ( p & lt; 0.001), whereas the ratio of nocturnal SpO 2 & lt; 90% of the total monitoring time (T90%) was positively correlated with mPAP. Mean SpO 2 was an independent predictor for pulmonary vascular resistance and cardiac output in women ( p = 0.001, p & lt; 0.001, p = 0.001, respectively). T90%, SaO 2 , and minimal SpO 2 were combined to develop a new composite parameter: hypoxemia scoring index (HSI). ROC curve analysis indicated that HSI levels of 0.55 could discriminate CTEPH from CTEPD with a sensitivity of 92.3% and specificity of 87.5% in female patients (an area under the curve, 0.937; 95% CI: 0.879–0.995, p & lt; 0.001). Conclusion Sex-specific nocturnal hypoxemia was present in patients with CTEPH or CTEPD. In female patients, the HSI showed high capacity for predicting the risk of CTEPH.     Abstract: Stress cardiomyopathy is a major clinical complication after severe burn. Multiple upstream initiators have been identified; however, the downstream targets are not fully understood. This study assessed the role of the plasma membrane in this process and its relationship with the protease μ-calpain and tumor necrosis factor-alpha (TNF-α). Here, third-degree burn injury of approximately 40% of the total body surface area was established in rats. Plasma levels of LDH and cTnI and cardiac cell apoptosis increased at 0.5 h post burn, reached a peak at 6 h, and gradually declined at 24 h. This effect correlated well with not only the disruption of cytoskeletal proteins, including dystrophin and ankyrin-B, but also with the activation of μ-calpain, as indicated by the cleaved fragments of α-spectrin and membrane recruitment of the catalytic subunit CAPN1. More importantly, these alterations were diminished by blocking calpain activity with MDL28170. Burn injury markedly increased the cellular uptake of Evans blue, indicating membrane integrity disruption, and this effect was also reversed by MDL28170. Compared with those in the control group, cardiac cells in the burn plasma-treated group were more prone to damage, as indicated by a marked decrease in cell viability and increases in LDH release and apoptosis. Of note, these alterations were mitigated by CAPN1 siRNA. Moreover, after neutralizing TNF-α with rhTNFR:Fc, calpain activity was blocked, and heart function was improved. In conclusion, we identified μ-calpain as a trigger for severe burn-induced membrane disruption in the heart and provided evidence for the application of rhTNFR:Fc to inhibit calpain for cardioprotection.

    Abstract: Significantly improved survival in patients with pulmonary hypertension (PH) has raised interest in maintaining a good quality of long-term survivorship. In this study, health-related quality of life (HRQOL) measurement was used to assess the long-term changes of physical and mental outcomes. Methods A total of 559 consecutive inpatients with PH completed generic HRQOL (Short Form-36) who were diagnosed with PH by right heart catheterization. Assessments were carried out at short-term (1 year), midterm (3 years), and long-term (5 years) follow-ups. Results Patients with PH suffered more severe impairments in both physical and emotional domains than the U.S. population normative values. Patients with PH due to chronic lung disease had the worst physical component summary (PCS) score, but there was no difference in mental component summary (MCS) score among different PH types. A reduced PCS score was correlated with WHO FC severity and pulmonary vascular resistance (PVR). The Z score showed that the changing trend of mental conditions continuously declined from baseline to midterm and long-term follow-ups, but the PCS score seemed to be stable or improved. Cox regression analysis indicated increased baseline PVR and WHO FC III and IV, and decreased physical subscale of role physical, mental subscale of social functioning, and the MCS score have increased risk of mortality in the long-term follow-up. Conclusion Patients with PH have poor HRQOL. The long-term change of physical status seemed to be stable, but the mental state was continuously worse. These suggested identifying and intervening mental health progresses is a noteworthy issue in PH chronic management.

    Abstract: COVID-19 has posed an enormous threat to public health around the world. Some severe and critical cases have bad prognoses and high case fatality rates, unraveling risk factors for severe COVID-19 are of significance for predicting and preventing illness progression, and reducing case fatality rates. Our study focused on analyzing characteristics of COVID-19 cases and exploring risk factors for developing severe COVID-19. Methods The data for this study was disease surveillance data on symptomatic cases of COVID-19 reported from 30 provinces in China between January 19 and March 9, 2020, which included demographics, dates of symptom onset, clinical manifestations at the time of diagnosis, laboratory findings, radiographic findings, underlying disease history, and exposure history. We grouped mild and moderate cases together as non-severe cases and categorized severe and critical cases together as severe cases. We compared characteristics of severe cases and non-severe cases of COVID-19 and explored risk factors for severity. Results The total number of cases were 12 647 with age from less than 1 year old to 99 years old. The severe cases were 1662 (13.1%), the median age of severe cases was 57 years [Inter-quartile range(IQR): 46–68] and the median age of non-severe cases was 43 years (IQR: 32–54). The risk factors for severe COVID-19 were being male [adjusted odds ratio (a OR ) = 1.3, 95% CI: 1.2–1.5]; fever (a OR  = 2.3, 95% CI: 2.0–2.7), cough (a OR  = 1.4, 95% CI: 1.2–1.6), fatigue (a OR  = 1.3, 95% CI: 1.2–1.5), and chronic kidney disease (a OR  = 2.5, 95% CI: 1.4–4.6), hypertension (a OR  = 1.5, 95% CI: 1.2–1.8) and diabetes (a OR  = 1.96, 95% CI: 1.6–2.4). With the increase of age, risk for the severity was gradually higher [20–39 years (a OR  = 3.9, 95% CI: 1.8–8.4), 40–59 years (a OR  = 7.6, 95% CI: 3.6–16.3), ≥ 60 years (a OR  = 20.4, 95% CI: 9.5–43.7)], and longer time from symtem onset to diagnosis [3–5 days (a OR  = 1.4, 95% CI: 1.2–1.7), 6–8 days (a OR  = 1.8, 95% CI: 1.5–2.1), ≥ 9 days(a OR  = 1.9, 95% CI: 1.6–2.3)]. Conclusions Our study showed the risk factors for developing severe COVID-19 with large sample size, which included being male, older age, fever, cough, fatigue, delayed diagnosis, hypertension, diabetes, chronic kidney diasease. Based on these factors, the severity of COVID-19 cases can be predicted. So cases with these risk factors should be paid more attention to prevent severity.

    Abstract: Calcium bismuth niobate (CaBi 2 Nb 2 O 9 , CBN) is considered to be one of the most promising high-temperature (HT) piezoelectric materials owing to its high Curie temperature of ∼940 °C, however, its low electrical resistance and poor piezoelectric properties at elevated temperatures limit its applications at high temperatures. In this work, we report the significantly enhanced dc electrical resistivity and piezoelectric performance of CBN ceramics through rare-earth Nd-substitution. The crystal structure, microstructure, and dielectric, electrical, ferroelectric and piezoelectric properties of Nd-modified CBN with nominal compositions of Ca 1− x Nd x Bi 2 Nb 2 O 9 (CBN-100 x Nd) have been investigated in detail. The results indicate that the substitutions of Nd 3+ ions for Ca 2+ ions increase the piezoelectric properties, and reduce the dielectric loss tan δ at high temperatures. The dc and ac conduction mechanisms indicate that the conduction mechanism is closely related to oxygen vacancies that are reduced through the donor substitutions of Nd 3+ for Ca 2+ , thereby resulting in a significant improvement in the dc electrical resistivity. The optimal composition of CBN-3Nd exhibits a high piezoelectric constant d 33 of 13.5 pC N −1 , and a high Curie temperature T c of 948 °C. More importantly, the CBN-3Nd exhibits good thermal stability of the electrical properties ( ρ = 2.6 × 10 7 Ω cm at 500 °C and 1.8 × 10 6 Ω cm at 600 °C, k p = 6.1%–6.2% at RT ∼ 500 °C), which demonstrates that the Nd-modified CBN ceramics are promising piezoelectric materials for use in HT piezoelectric sensors.
