    Abstract: : The microbiome is important in the development and progression of breast cancer. This study investigated the effects of microbiome derived from Klebsiella on endocrine therapy of breast cancer using MCF7 cells. The bacterial extracellular vesicles (EVs) that affect endocrine therapy were established through experiments focused on tamoxifen efficacy. Methods : The microbiomes of breast cancer patients and healthy controls were analyzed using next-generation sequencing. Among microbiome, Klebsiella was selected as the experimental material for the effect on endocrine therapy in MCF7 cells. MCF7 cells were incubated with tamoxifen in the absence/presence of bacterial EVs derived from Klebsiella pneumoniae and analyzed by quantitative real-time polymerase chain reaction and Western blot. Results : Microbiome derived from Klebsiella is abundant in breast cancer patients especially luminal A subtype compared to healthy controls. The addition of EVs derived from K pneumoniae enhances the anti-hormonal effects of tamoxifen in MCF7 cells. The increased efficacy of tamoxifen is mediated via Cyclin E2 and p -ERK. Conclusion : Based on experiments, the EVs derived from K pneumoniae are important in hormone therapy on MCF7 cells. This result provides new insight into breast cancer mechanisms and hormone therapy using Klebsiella found in the microbiome.

    Abstract: Introduction: The microbiota is known to be involved in the development and progression of breast cancer, although few experiments have been conducted to determine which species in the microbiota are involved in the disease. We have examined the effects of symbiotic bacteria in breast cancer under hormone therapy and have identified bacteria that affect anti-hormonal therapy focusing on the estrogen receptor. The intestinal symbiotic bacteria secrete bacterial extracellular vesicles to the blood, lymphatic fluid, and communicate with distant organs through these vesicles which are including metabolites and bacterial materials. After circulation of the body, all the extracellular vesicles are collected to urine. Therefore, we analyzed the microbiome of breast cancer patients and normal controls through urine samples. Materials and Methods: A total of 347 urine samples from females (127 breast cancer patients and 220 normal individuals) were collected from September 2014 to August 2015, at Ewha Womans University Mokdong Hospital and Inje University Haeundae Paik Hospital. These samples analyzed by NGS using a universal bacterial primer of 16S rDNA. A t-test and Kaplan-Meier analysis were performed to find out the discrepancy between cancer and control groups. Cell experiments confirmed the effect of bacterial EVs on breast cancer cells. Results: We have found that there is a significant difference in the number of microorganisms between breast cancer patients and healthy controls. Among the microorganisms in breast cancer patients, a certain bacteria was found to be abundant in luminal A subtype tumors: Klebsiella. We selected Klebsiella pneumoniae for subsequent experiments. Over a mean 5-year follow-up period, The Klebsiella-abundant and -lacking groups included 50 and 48 patients, respectively, and the disease-free survival outcome of breast cancer patients was determined in both groups. Extracellular vesicles derived from Klebsiella showed anti-hormonal effects in MCF7 cells during treatment with tamoxifen. K. pneumoniae EVs doubled the efficacy of tamoxifen. K. pneumoniae EVs co-treated with tamoxifen down-regulated cyclin E2, which in turn decreased the expression of p21. Moreover, in this process, the ERK pathway appears to be involved. Conclusions: Based on clinical data and in vitro experiments with cell lines, we conclude that symbiotic microorganisms affect hormone therapy for breast cancer. These findings provide new horizons for breast cancer mechanisms and hormone therapy.     Abstract: Breast cancer (BC) in the Asia Pacific regions is enriched in younger patients and rapidly rising in incidence yet its molecular bases remain poorly characterized. Here we analyze the whole exomes and transcriptomes of 187 primary tumors from a Korean BC cohort (SMC) enriched in pre-menopausal patients and perform systematic comparison with a primarily Caucasian and post-menopausal BC cohort (TCGA). SMC harbors higher proportions of HER2+ and Luminal B subtypes, lower proportion of Luminal A with decreased ESR1 expression compared to TCGA. We also observe increased mutation prevalence affecting BRCA1 , BRCA2 , and TP53 in SMC with an enrichment of a mutation signature linked to homologous recombination repair deficiency in TNBC. Finally, virtual microdissection and multivariate analyses reveal that Korean BC status is independently associated with increased TIL and decreased TGF-β signaling expression signatures, suggesting that younger Asian BCs harbor more immune-active microenvironment than western BCs.

    Abstract: With the aging demographic of today’s society, the need for robotic exoskeletons is expected to increase as they can compensate for declining physical strength in the physically impaired. In this study, an assistive robotic exoskeleton for the knee joint with fairly low energy consumption is proposed for industrial applications. The knee-assistive robotic exoskeleton (KARE-1) was designed to support a human body during production line tasks. The KARE-1 is based on a four-bar link mechanism with a rotary actuator and gas spring to accommodate a high power-to-weight ratio. By taking advantage of the utilized singular configuration of the four-bar linkage, this novel design is able to efficiently support the weight of the human body. The selected singular configuration allows this device to support the knee joint in the load-bearing stages of static sitting as well as during the motion between standing and sitting. The proposed device is further able to move freely along with the knee during walking movements. The proposed design was verified through a series of numerical simulations and through human subject testing at an industrial workplace.

    Abstract: Intratumoral heterogeneity has emerged as a problem to be resolved for precise diagnosis and effective treatment of cancer. Breast cancer is one of the most heterogeneous cancer in its molecular subtypes and its intratumoral genomic diversity has been demonstrated as well. Single-cell sequencing approach would reveal heterogeneity in gene expression signatures and the common gene expression characteristics across tumor cells within a tumor. In this study, we isolated live individual cells and amplified cDNAs using C1™Single-cell Auto Prep System (Fluidigm) then performed RNA sequencing using HiSeq2500 System (Illumina). Single-cell transcriptomes were acquired from four primary tumor (ER+/HER2-, ER-/HER2+, ER+/HER2+ and triple negative breast cancer) and two matched lymph node metastases (ER+/HER2+ and triple negative breast cancer). Because the tumor tissue isolates may include stromal and immune cells in the tumor microenvironment, we defined ‘epithelial cell adhesion molecule signature’ to filter out non-tumor populations (Poster# presented by Woosung Chung et. al) After the removal of non-tumor cells, collected tumor single-cell transcriptomes largely recapitulated bulk tumor-expression profiles. The ER+/HER2- tumor cells showed highly activated estrogen receptor-associated pathways. Although both ER-/HER2+ and ER+/HER2+ tumors had the HER2 gene amplification and overexpression, HER2/HER3 pathway is activated only in ER-/HER2+ tumor cells. The association of gene expression for cancer stem cells and chemoresistance, such as IL8 and CXCL1, with HER2/HER3 signaling pathway activation was prominent. By contrast, the ER+/HER2+ tumor cells had highly activated estrogen receptor signaling, presenting them as the luminal type. The triple negative breast cancer cells highly expressed adhesion molecules and proteases suggesting their invasive nature, yet demonstrated extensive gene expression heterogeneity. Collectively these data revealed the power of single cell gene expression profiling in the precise characterization of tumor cell properties. The explicit gene expression signatures for patient specific tumors may be defined as subtype-specific signatures and helpful to unmask targets for molecular directed therapies.
