    Abstract: This prospective multicenter study, established by the Japanese Ministry of Health, Labour and Welfare and involving 27 institutions, aimed to compare postoperative outcomes between laminoplasty (LM) and posterior fusion (PF) for cervical ossification of the posterior longitudinal ligament (OPLL), in order to address the controversy surrounding the role of instrumented fusion in cases of posterior surgical decompression for OPLL. 478 patients were considered for participation in the study; from among them, 189 (137 and 52 patients with LM and PF, respectively) were included and evaluated using the Japanese Orthopaedic Association (JOA) scores, the JOA Cervical Myelopathy Evaluation Questionnaire (JOACMEQ), and radiographical measurements. Basic demographic and radiographical data were reviewed, and the propensity to choose a surgical procedure was calculated. Preoperatively, there were no significant differences among the participants in terms of patient backgrounds, radiographical measurements (K-line or cervical alignment on X-ray, OPLL occupation ratio on computed tomography, increased signal intensity change on magnetic resonance imaging), or clinical status (JOA score and JOACMEQ) after adjustments. The overall risk of perioperative complications was found to be lower with LM (odds ratio [OR] 0.40, p = 0.006), and the rate of C5 palsy occurrence was significantly lower with LM (OR 0.11, p = 0.0002) than with PF. The range of motion (20.91° ± 1.05° and 9.38° ± 1.24°, p  〈  0.0001) in patients who had PF was significantly smaller than in those who had LM. However, multivariable logistic regression analysis showed no significant difference among the participants in JOA score, JOA recovery rate, or JOACMEQ improvement at two years. In contrast, OPLL progression was greater in the LM group than in the PF group (OR 2.73, p = 0.0002). Both LM and PF for cervical myelopathy due to OPLL had resulted in comparable postoperative outcomes at 2 years after surgery.

    Abstract: The Japanese Clinical Practice Guidelines for Management of Sepsis and Septic Shock 2020 (J-SSCG 2020), a Japanese-specific set of clinical practice guidelines for sepsis and septic shock created as revised from J-SSCG 2016 jointly by the Japanese Society of Intensive Care Medicine and the Japanese Association for Acute Medicine, was first released in September 2020 and published in February 2021. An English-language version of these guidelines was created based on the contents of the original Japanese-language version. The purpose of this guideline is to assist medical staff in making appropriate decisions to improve the prognosis of patients undergoing treatment for sepsis and septic shock. We aimed to provide high-quality guidelines that are easy to use and understand for specialists, general clinicians, and multidisciplinary medical professionals. J-SSCG 2016 took up new subjects that were not present in SSCG 2016 (e.g., ICU-acquired weakness [ICU-AW], post-intensive care syndrome [PICS] , and body temperature management). The J-SSCG 2020 covered a total of 22 areas with four additional new areas (patient- and family-centered care, sepsis treatment system, neuro-intensive treatment, and stress ulcers). A total of 118 important clinical issues (clinical questions, CQs) were extracted regardless of the presence or absence of evidence. These CQs also include those that have been given particular focus within Japan. This is a large-scale guideline covering multiple fields; thus, in addition to the 25 committee members, we had the participation and support of a total of 226 members who are professionals (physicians, nurses, physiotherapists, clinical engineers, and pharmacists) and medical workers with a history of sepsis or critical illness. The GRADE method was adopted for making recommendations, and the modified Delphi method was used to determine recommendations by voting from all committee members. As a result, 79 GRADE-based recommendations, 5 Good Practice Statements (GPS), 18 expert consensuses, 27 answers to background questions (BQs), and summaries of definitions and diagnosis of sepsis were created as responses to 118 CQs. We also incorporated visual information for each CQ according to the time course of treatment, and we will also distribute this as an app. The J-SSCG 2020 is expected to be widely used as a useful bedside guideline in the field of sepsis treatment both in Japan and overseas involving multiple disciplines.

    Abstract: High blood pressure increases bleeding risk during treatment with antithrombotic medication. The association between blood pressure levels and the risk of recurrent stroke during long-term secondary stroke prevention with thienopyridines (particularly prasugrel) has not been well studied. Methods: This was a post hoc analysis of the randomized, double-blind, multicenter PRASTRO-I trial (Comparison of Prasugrel and Clopidogrel in Japanese Patients With Ischemic Stroke-I). Patients with noncardioembolic stroke were randomly assigned (1:1) to receive prasugrel 3.75 mg/day or clopidogrel 75 mg/day for 96 to 104 weeks. Risks of any ischemic or hemorrhagic stroke, combined ischemic events, and combined bleeding events were determined based on the mean level and visit-to-visit variability, including successive variation, of systolic blood pressure (SBP) throughout the observational period. These risks were also compared between quartiles of mean SBP level and successive variation of SBP. Results: A total of 3747 patients (age 62.1±8.5 years, 797 women), with a median average SBP level during the observational period of 132.5 mm Hg, were studied. All the risks of any stroke (146 events; hazard ratio, 1.318 [95% CI, 1.094–1.583] per 10-mm Hg increase), ischemic stroke (133 events, 1.219 [1.010–1.466] ), hemorrhagic stroke (13 events, 3.247 [1.660–6.296]), ischemic events (142 events, 1.219 [1.020–1.466] ), and bleeding events (47 events, 1.629 [1.172–2.261]) correlated with increasing mean SBP overall. Similarly, an increased risk of these events correlated with increasing successive variation of SBP (hazard ratio, 3.078 [95% CI, 2.220–4.225] per 10-mm Hg increase; 3.051 [2.179–4.262]; 3.276 [1.172–9.092] ; 2.865 [2.042–4.011]; 2.764 [1.524–5.016] , respectively). Event rates did not differ between the clopidogrel and prasugrel groups within each quartile of SBP or successive variation of SBP. Conclusions: Both high mean SBP level and high visit-to-visit variability in SBP were significantly associated with the risk of recurrent stroke during long-term medication with either prasugrel or clopidogrel after stroke.     Abstract: Structural and electrical properties of newly synthesized vinylidene fluoride (VDF) oligomer thin film have been investigated. The FTIR spectrum showed that the epitaxially grown film on KBr(001) substrate consists of form I (β phase) crystals and their c axes (molecular axes) and b axes (polar axes) are arranged parallel to the KBr substrate. To make electrical measurements possible, this film was transferred onto a gold bottom electrode without causing any changes in the crystalline structures. By using a modified atomic force microscope, we succeeded in the formation of local polarized domains as well as the clear observation of piezoresponse hysteresis curves in this sample. The coercive field and piezoelectric coefficient ( d 33 ) for the 37-nm-thick film were about 200 MV/m and -3 pm/V, respectively. It was suggested that the b axis in the as-grown film rotated from the parallel to the perpendicular direction to the film surface during the poling process. This study reveals the ferroelectric characteristics in the VDF oligomer thin films for the first time.

    Abstract: We have studied the fullerene (C 60 ) multilayer thin film deposited on the Si(111)-7×7 reconstructed surface by noncontact atomic force microscopy (NC-AFM) under an ultrahigh-vacuum (UHV) condition. The double domain structure on a terrace of the crystalline island was successfully imaged. We have also simultaneously measured the contact potential difference (CPD) by the Kelvin probe method. Small CPD variations, which are not due to coupling with the topographic image, were measured at a molecular scale.

    Abstract: A molecular electronics device (MED), which makes use of the functionality of one or several molecules, is a very attractive nanotechnology. However, it is necessary to both fabricate conductive metal nanowire circuits and insert the desired functional molecule at any position on the circuits in order to realize superintegrated circuits using molecular electronics. Therefore, a technique for fabricating nanogaps at any position on a metal nanowire is very important. We patterned an electron-beam negative resist SAL601 using the scanning probe lithography (SPL) technique and the resist patterns were transferred to the underlying titanium film by wet etching. We fabricated a continuous 20-µm-long Ti nanowire, thus this method can be combined with conventional photolithography. Using this combination, we fabricated a Ti nanowire connected to a large Pt pad electrode, and measured its current–voltage property with an atomic force microscopy (AFM) conductive tip. Moreover, we fabricated nanogaps on this line pattern by switching off the voltage bias between the AFM tip and the sample for a short time. Using these techniques, we fabricated nanogap electrodes with both nanometer-scale width and gap. It is expected that the electrodes can also be applied for electric measurement of one or several molecules.

    Abstract: Structures of self-assembled monolayers (SAMs) of alkanedithiol molecules were studied using an ultrahigh vacuum scanning tunneling microscope (UHV-STM) and by Fourier transform infrared reflection-absorption spectroscopy (FT-IR-RAS). Clearly resolved STM images of alkanedithiol SAMs revealed striped structures as bright lines and the distance between the adjacent bright lines was about 1.5 nm, which is in good agreement with the length of the alkanedithiol molecule. Furthermore, the intensities of the CH 2 stretching bands in FT-IR-RAS measurement of alkanedithiol SAMs were stronger than those of alkanethiol SAMs. We suggest that the molecules in alkanedithiol SAMs are arranged with their axes parallel to the gold substrate.

    Abstract: Dynamic force microscopy (DFM) was applied for investigating on the structures of C 60 multilayer thin films deposited on the Si(111)-7 ×7 reconstructed surface. C 60 molecules on the crystalline island have been successfully imaged with a molecular resolution for the first time in an ultrahigh vacuum environment. In addition, a difference in the contrast of phase image between the islands and the monolayer-covered Si surface was observed.
