    Abstract: The importance of the rare microbial biosphere in maintaining biodiversity and ecological functions has been highlighted recently. However, the current understanding of the spatial distribution of rare microbial taxa is still limited, with only a few investigations for rare prokaryotes and virtually none for rare fungi. Here, we investigated the spatial patterns of rare and abundant fungal taxa in alpine grassland soils across 2,000 km of the Qinghai-Tibetan plateau. We found that most locally rare fungal taxa remained rare (13.07%) or were absent (82.85%) in other sites, whereas only a small proportion (4.06%) shifted between rare and abundant among sites. Although they differed in terms of diversity levels and compositions, the distance decay relationships of both the rare and the abundant fungal taxa were valid and displayed similar turnover rates. Moreover, the community assemblies of both rare and abundant fungal taxa were predominantly controlled by deterministic rather than stochastic processes. Notably, the community composition of rare rather than abundant fungal taxa associated with the plant community composition. In summary, this study advances our understanding of the biogeographic features of rare fungal taxa in alpine grasslands and highlights the concordance between plant communities and rare fungal subcommunities in soil. IMPORTANCE Our current understanding of the ecology and functions of rare microbial taxa largely relies on research conducted on prokaryotes. Despite the key ecological roles of soil fungi, little is known about the biogeographic patterns and drivers of rare and abundant fungi in soils. In this study, we investigated the spatial patterns of rare and abundant fungal taxa in Qinghai-Tibetan plateau (QTP) alpine grassland soils across 2,000 km, with a special concentration on the importance of the plant communities in shaping rare fungal taxa. We showed that rare fungal taxa generally had a biogeographic pattern that was similar to that of abundant fungal taxa in alpine grassland soils on the QTP. Furthermore, the plant community composition was strongly related to the community composition of rare taxa but not abundant taxa. In summary, this study significantly increases our biogeographic and ecological knowledge of rare fungal taxa in alpine grassland soils.

    Abstract: To assess the incidence, risk factors, and clinical characteristics of perinatal stroke in Beijing. Methods This multicenter prospective study included all the live births from 17 representative maternal delivery hospitals in Beijing from March 1, 2019 to February 29, 2020. Neonates with a stroke were assigned to the study group. Clinical data, including general information, clinical manifestations, and risk factors, were collected. Up until 18 months after birth, neonates were routinely assessed according to the Ages and Stages Questionnaire (ASQ) and/or the Bayley scale. Statistical analysis was done using the chi-squared, t -tests, and logistic regression analysis using SPSS version 26.0. Outcomes In total, 27 cases were identified and the incidence of perinatal stroke in Beijing was 1/2,660 live births, including 1/5,985 for ischemic stroke and 1/4,788 for hemorrhagic stroke. Seventeen cases (62.96%) of acute symptomatic stroke and convulsions within 72 h (10 cases, 37.04%) were the most common presentations. Ten patients showed no neurological symptoms and were found to have had a stroke through routine cranial ultrasonography after being hospitalized for non-neurological diseases. The risk factors include primiparity, placental or uterine abruption/acute chorioamnionitis, intrauterine distress, asphyxia, and severe infection. In the study group, 11.1% (3/27) of patients had adverse neurodevelopmental outcomes. The patients in the study group had lower scores for the ASQ than those in the control group in the communication, gross, and fine motor dimensions. Conclusion The incidence of perinatal stroke in Beijing was consistent with that in other countries. Routine neuroimaging of infants with risk factors may enable identification of asymptomatic strokes in more patients. Patients who have suffered from a stroke may have neurological sequelae; therefore, early detection, treatment, and regular follow-ups are beneficial for improving their recovery outcomes.

    Abstract: Introduction: Primary breast diffuse large B cell lymphoma(PB-DLBCL) is a rare subtype of DLBCL with limited data on treatment outcome. The impact of rituximab on survival and the role of central nervous system(CNS) prophylaxis in patients(pts) with PB-DLBCL remain controversial. The aim of this study was to define the clinical features, treatment outcome and patterns of relapse of Chinese pts with PB-DLBCL. Methods: Data on pts with PB-DLBCL was retrospectively collected from 20 main Chinese medical centers. Between 2000 and 2015, 110 pts were included. Eligibility criteria required confirmed pathological diagnosis of DLBCL and disease localized to one or both breasts±ipsilateral regional lymph nodes. Patients with systemic disease with breast involvement or transformed DLBCL from low-grade lymphoma were excluded. PFS and OS were estimated using the Kaplan-Meier method, and prognostic factors were analyzed using the log-rank test and Cox proportional hazards model. Results: All of 110 pts were female, with a median age of 47years(yrs)(range 16-85yrs). 4.5% presented with B-symptoms; 94.5% presented with unilateral disease (right 58.7%; left 36.7%), and 6 pts(5.5%) presented with bilateral breast involvement. The median tumor size was 4cm(range 1.2-12.8cm); 56.4% had stage IE and 38.2% had stage IIE disease; pts with bilateral breast disease(5.5%) were classified as stage IV. Among the 72 pts in whom immunohistochemistry was available, 68.1% were classified as non-germinal center B-cell like(GCB) immunophenotype and 31.9% as GCB type based on the algorithms of Hans and Visco-Young. The median Ki-67 expression was 80%(range 20-98%). Among the 86 pts with available international prognostic index (IPI), the score was 0 in 51.2% of pts, 1 in 32.6%, 2 in 12.8%, and 3-4 in 3.5%. Most pts(98%) received anthracycline-containing chemotherapy, and 60% received rituximab; 44% received CNS prophylaxis with intrathecal chemotherapy(IT); 36% received radiotherapy (RT); 19% underwent mastectomy. At a median follow-up of 3.2 yrs (range 0.1 - 11.5 yrs), the Kaplan-Meier estimated median PFS was 6.3 yrs (95% CI 4.2 - 8.4 yrs) and the median OS was not reached. The 5-yr PFS and 5-yr OS were 61.2% (95% CI 49.0-73.4%) and 77.3% (95% CI 66.1-88.5%) respectively. In univariate analysis, rituximab was associated with improved PFS(5yrPFS 75.8% vs 38.4%, P=0.03) but not an advantage in OS. RT was associated with a significant benefit in PFS(P=0.02) and a borderline significant advantage in OS(p=0.052). In multivariate analysis, IPI was the only significant prognostic factor for OS(HR=5.16, P=0.01). The 5-yr OS was 94.7% in pts with a IPI score of 0, 76.0% in pts with a score of 1, and 54.3% in those with a score of 2-4. There was no difference in OS in pts with tumors less than versus more than 5cm, or between pts with non-GCB versus GCB subtype. 3 pts with bilateral disease had early progression within 1 yr from diagnsosis, and all died within 4yrs. A total of 35 (31.8%) pts had relapsed, with the breast(16 cases, 14.5%) and CNS(11 cases, 10%) as the most common sites of extra-nodal relapse. Although 71.4% of first relapses occurred within the first 3 yrs, late relapse was frequently observed in the contralateral breast and CNS, with 57% of contralateral breast relapses and 55% of CNS relapses occurring beyond 3 yrs from diagnosis. Among the 11 pts with CNS relapse, 3 pts received prophylactic IT during first-line therapy. There was a continuous risk of CNS relapse up to 8.2 yrs from initiation of treatment(median time to relapse: 3.1yrs), with 72% of relapses occurring in the brain parenchyma. In univariate analysis, elevated LDH(P 〈 0.001) and bilateral breast involvement(P=0.014) were associated with a higher risk of CNS relapse. Neither prophylactic IT nor rituximab was associated with a significant reduction in the cumulative risk of CNS relapse. Conclusions: PB-DLBCL is a very rare subtype of DLBCL among Chinese lymphoma pts. Among the 110 pts collected from 20 main medical centers, PB-DLBCL appears to have worse PFS with distinct patterns of extra-nodal relapse. Rituximab and RT were both associated with improved PFS, but no significant beneficial effect on OS was observed. A continuous pattern of CNS relapse was often, especially in pts with bilateral breast involvement or elevated LDH. Prophylactic IT had limited value in preventing CNS relapses in these PB-DLBCL pts.     Abstract: Poststroke depression is a serious and common complication of stroke, especially the ischemic poststroke depression. Antidepressants are used in poststroke depression, and acupuncture may be an alternative approach. However, the efficacy and mechanism of acupuncture for poststroke depression has not been confirmed. Methods/design: This is a multicenter, central-randomized, single-blind, sham-controlled clinical trial. We will allocate 208 subjects aged between 40 and 80 years old, diagnosed with initial poststroke depression (PSD) within 6 months to 2 groups randomly in a ratio of 1:1. Patients in the experimental group will be treated with traditional acupuncture and placebo pills, whereas the others in the control group will be treated with sham-acupoints acupuncture and antidepressant (fluoxetine hydrochloride tablets). All will be given acupuncture and/or medication treatment for 12 weeks, and then received 12-week follow-up. Patients will be evaluated with the 17-item Hamilton Depression Scale and Se1f-rating Depression Scale for depression state, National Institute of Health Stroke Scale for neurological deficit, Modified Barthel Index for activities of daily living, Treatment Emergent Symptom Scale for side effects of treatments, diagnosis and evaluation criteria of traditional Chinese medicine for stroke (try out) for curative effects of stroke, and clinical global impression for synthesize effect before and the 2nd, 4th, 8th, and 12th week of treatment, 24th week of follow-up. Study on mechanisms of acupuncture will be revealed through the diversity of brain metabolites (choline-containing compounds [Cho], N-acetylaspartate [NAA] , myoinositol, glutamine and glutamate complex, creatine [Cr], Cho/Cr, Cho/NAA, Cr/NAA) in bilateral dorsolateral prefrontal cortex and anterior cingulate cortex monitored by proton magnetic resonance spectroscopy, and serum monoamine neurotransmitters (5-hydroxytryptamine, norepinephrine, dopamine) and cytokines (brain-derived neurotrophic factor [BDNF] , interleukin [IL]-4, IL-6, IL-10, IL-18, IL-1β, tumor necrosis factor alpha) before and the 12th week of treatment. Baseline characteristics of patients will be summarized by groups and compared with chi-square for categorical variables, and 2-sample t tests or Wilcoxon rank-sum test for the continuous variables. Primary and secondary outcomes according to the measurement times are applicable to univariate repetitive measurement deviation analysis or 2-sample t tests, or Wilcoxon rank-sum test. Conclusion: The present research is designed to investigate efficacy and mechanism of traditional acupuncture therapy on ischemic PSD, also to explore the correlation between cerebra metabolic and serologic factors, and ischemic PSD. With this research, we are looking forward to find out an appropriate alternative nondrug therapy for PSD people to alleviate the adverse effects and drug dependence caused by antidepressants.

    Abstract: A disturbance of energy metabolism reduces cardiac function in acute severe hemorrhagic patients. Alternatively, adequate energy supply reduces heart failure and increases survival. However, the approach to regulating energy metabolism conductive to vital organs is limited, and the underlying molecular mechanism remains unknown. This study assesses the ability of histone deacetylase inhibitors (HDACIs) to preserve cardiac energy metabolism during lethal hemorrhagic injury. In the lethally hemorrhagic rat and hypoxic myocardial cells, energy metabolism and heart function were well maintained following HDACI treatment, as evident by continuous ATP production with normal cardiac contraction. Valproic acid (VPA) regulated the energy metabolism of hemorrhagic heart by reducing lactate synthesis and protecting the mitochondrial ultrastructure and respiration, which were attributable to the inhibition of lactate dehydrogenase A activity and the increased myeloid cell leukemia-1 (mcl-1) gene expression, ultimately facilitating ATP production and consumption. MCL-1, the key target of VPA, mediated this cardioprotective effect under acute severe hemorrhage conditions. Our results suggest that HDACIs promote cardioprotection by improving energy metabolism during hemorrhagic injury and could therefore be an effective strategy to counteract this process in the clinical setting.

    Abstract: Primary breast diffuse large B‐cell lymphoma ( PB ‐ DLBCL ) is a rare subtype of DLBCL with limited data on patterns of failure. This multicenter study aimed to define the optimum treatment strategy and patterns of failure for PB ‐ DLBCL patients. We retrospectively reviewed data on 108 PB ‐ DLBCL patients from 21 Chinese medical centers. Only patients with localized disease (involvement of breast and localized lymph nodes) were included. After a median follow‐up of 3.2 years, 32% of patients developed progression or relapse. A continuous pattern of relapse was observed, characterized by frequent late relapses in the contralateral breast and central nervous system ( CNS ). Although rituximab significantly reduced the overall cumulative risk of progression or relapse (5‐year cumulative risk 57% vs 24%, P  =   .029), it had limited effect on the reduction of breast relapse ( P  =   .46). Consolidative radiotherapy significantly decreased the risk of breast relapse, even in the subgroup of patients treated with rituximab (5‐year cumulative risk 21.2% vs 0%, P  =   .012). A continuous risk of CNS progression or relapse up to 8.2 years from diagnosis was observed (10‐year cumulative risk 28.3%), with a median time to CNS relapse of 3.1 years. Neither rituximab nor prophylactic intrathecal chemotherapy significantly decreased the risk of CNS relapse. In summary, our study indicates that PB ‐ DLBCL has a continuous pattern of relapse, especially with frequent late relapses in the CNS and contralateral breast. Rituximab and RT confer complementary benefit in the reduction of relapse. However, neither the addition of rituximab nor prophylactic intrathecal chemotherapy could effectively prevent CNS relapse for PB ‐ DLBCL patients.
