    Analysis of clinical features and identification of risk factors in patients with non-alcoholic fatty liver disease based on FibroTouch

    Abstract: Our aim was to explore the correlation between ultrasound attenuation parameter (UAP) and liver stiffness measurement (LSM) based on FibroTouch (China) and clinical features in patients with non-alcoholic fatty liver disease (NAFLD), so as to provide a certain basis for the clinical application of FibroTouch in NAFLD. Hepatic steatosis and fibrosis in patients with NAFLD were graded according to FibroTouch, and the relationship between steatosis and fibrosis levels and clinical characteristics was retrospectively analyzed. Hepatic steatosis was positively related with weight, BMI, waist, hyperlipidemia, hyperuricemia, FBG, UA, TG, ALT, AST, GGT, LSM and hepatic fibrosis grading, and was negatively related with gender (male), age and AST/ALT ratio. Hepatic fibrosis was positively related with age, BMI, waist, hypertension, FBG, ALT, AST, GGT, NFS, APRI, FIB-4, UAP and hepatic steatosis grading, and was negatively related with blood platelet (PLT) counts. Moreover, BMI, waist, TG, ALT and LSM were independent risk factors of hepatic steatosis, while decreased PLT counts, AST and UAP were independent risk factors of hepatic fibrosis. Body mass parameters, metabolic risk factors and liver function indicators increase the risk of hepatic steatosis and fibrosis in patients with NAFLD, and UAP and LSM can interact with each other.

    Abstract: Background: Unhealthy diet is a common risk factor threatening dental and general health. Conflicting dietary advice persists among different healthcare professions, despite some shared goals, causing mystification to patients and the public. This qualitative study aimed to understand the perspectives of dentists, physicians, and dietitians in targeting unhealthy diet as a common risk factor, their experiences and barriers in addressing conflicting dietary advice, and possible ways for improving cross-professional coordination.Methods and Findings: A purposive sample of 40 dentists, physicians, and dieticians was recruited from different service sectors and joined in semi-structured interviews, which were subjected to thematic content analysis. Participants supported the common risk factor approach and suggested improving cross-professional cooperation by maximizing potentials of multidisciplinary care, engaging auxiliary/allied staff, refining electronic systems, and incorporating cutting-edge communication technologies. Inconsistent dietary advice stemmed from different treatment focuses and lack of mutual understanding and well-followed guidelines. Inconsistencies can be resolved by striking a balance for the patient’s best interest, well-informing patients, respecting patients’ autonomy, acquiring cross-professional knowledge, and conforming to shared guidelines.Conclusions: Views solicited from three healthcare professions endorsed the importance of cross-professional partnership in preventing/managing dietrelated health problems. Educators, professional bodies, and administrators share the responsibility to dispel conflicting health messages and promote better practice in dietary counselling.

    Abstract: A total of 702 1‐day‐old male Jiangnan White goslings were used to study the effects of varying levels of Na + and Cl − on their growth performance and blood parameters. In this experiment, goslings were randomly allotted to nine treatments, with six pens per treatment in a 3 × 3 factorial arrangement, and fed diets with three concentrations of added Na + (0.10%, 0.15% and 0.20%) and three concentrations of added Cl − (0.15%, 0.20% and 0.25%). The purpose of this experiment was to investigate the sensitivity of goslings to Na + and Cl − during brooding to determine the appropriate levels of Na + and Cl − in their feed. The results are as follows: (a) Different levels of Na + and Cl − in the diet, especially low Na + and Cl − , significantly affected the body weight (BW), average daily gain (ADG) and feed/gain ratio (F/G) of the 28‐day‐old goslings ( p   〈  .05). But Na +  × Cl − has no significant effect on water consumption ( p   〉  .05). (b) Serum concentrations of urea increased linearly with Na + content ( p   〈  .05). Serum concentrations of creatinine (CR) and uric acid (UA) increased linearly with Cl − content ( p   〈  .05). (c) Na +  × Cl − has a significant impact on the serum sodium (Na) and chlorine (Cl; p   〈  .05). The content of serum Na and Cl increased linearly with increasing levels of Na + and Cl − . These results show that low levels of Na + and Cl − had significant adverse effects on the growth of 1‐ to 28‐day‐old goslings. The results of the experiment support a recommendation to supplement the diet of goslings with 0.20% Cl − and not 〈 0.15% Na + .

    Abstract: The Rho family of GTPases is strictly regulated by a large family of GTPase-activating proteins (GAPs) that stimulate the relatively weak intrinsic GTP-hydrolyzing activity of Rho GTPases. p190A is a potent and widely expressed GAP that acts on RhoA GTPases. p190A is frequently mutated in endometrial cancer, but the contribution of p190A mutations to endometrial tumorigenesis remains unclear. Here we identified that p190A is an upstream regulator of the Hippo-YAP signaling pathway, which is a critical regulator of cell proliferation, apoptosis, and cell fate. p190A knockout in endometrial cancer cells promoted cell proliferation, migration, and epithelial–mesenchymal transition (EMT), which were partially dependent on YAP activation. Wild-type p190A, but not endometrial cancer-associated mutants, suppressed the nuclear localization, transcriptional activity, and malignant transformation function of YAP. Moreover, the nuclear localization of YAP was enhanced in p190A-mutated endometrial cancer. These findings reveal novel molecular mechanisms underlying Hippo-YAP pathway-driven endometrial tumorigenesis and elucidate the potential for therapy targeting the Hippo-YAP pathway in p190A-mutated endometrial cancer.

    Impact of Evidence‐Based Stroke Care on Patient Outcomes: A Multilevel Analysis of an International Study

    Abstract: The uptake of proven stroke treatments varies widely. We aimed to determine the association of evidence‐based processes of care for acute ischemic stroke ( AIS ) and clinical outcome of patients who participated in the HEADPOST (Head Positioning in Acute Stroke Trial), a multicenter cluster crossover trial of lying flat versus sitting up, head positioning in acute stroke. Methods and Results Use of 8 AIS processes of care were considered: reperfusion therapy in eligible patients; acute stroke unit care; antihypertensive, antiplatelet, statin, and anticoagulation for atrial fibrillation; dysphagia assessment; and physiotherapist review. Hierarchical, mixed, logistic regression models were performed to determine associations with good outcome (modified Rankin Scale scores 0–2) at 90 days, adjusted for patient and hospital variables. Among 9485 patients with AIS, implementation of all processes of care in eligible patients, or “defect‐free” care, was associated with improved outcome (odds ratio, 1.40; 95% CI, 1.18–1.65) and better survival (odds ratio, 2.23; 95% CI , 1.62–3.09). Defect‐free stroke care was also significantly associated with excellent outcome (modified Rankin Scale score 0–1) (odds ratio, 1.22; 95% CI , 1.04–1.43). No hospital characteristic was independently predictive of outcome. Only 1445 (15%) of eligible patients with AIS received all processes of care, with significant regional variations in overall and individual rates. Conclusions Use of evidence‐based care is associated with improved clinical outcome in AIS . Strategies are required to address regional variation in the use of proven AIS treatments.     Combination of Cladribine, Cytarabine and Topotecan (CLAT) for Relapsed or Refractory Acute Myeloid Leukemia

    Abstract: Xiaomei Chen and Jianyu Weng contributed equally to this study. The outcomes ofrelapsed or refractory acute myeloid leukemia (RR-AML) are poor and effective salvage regimens are urgently needed. We present a study of 14 patients with RR-AML (median age 42years, range 18-65years; male n=12, female n= 2) treated with CLAT regimen, which consisted of cladribine 5mg/m² per day i.v. 2-3 hour on days 1-5, cytarabine 1.0g/m² per day i.v. 4 hours after cladribine on days 1-5, topotecan 1.25mg/m² per day i.v. 4 hours after cytarabine on days1-5 and G-CSF 300ug per day subcutaneous injection on days until neutrophile granulocyte recovery. Total of fourteen patients were included into the study from June 2013 to June 2015, Two (14.3%) patients were relapsed and twelve (85.7%) patients were refractory, 4 of 14 patients were relapsed or refractory after allogeneic-HSCT. Two patients died of invasive fungal infection before the assessment. Seven patients (58.3%) achieved complete remission (CR), and one patient (8.3%) achieved partial remission (PR), the rest patients (33.3%) did not respond (NR). The overall response rate was 66.7%. Following CLAT treatment, four patients with CR underwent allogeneic hematopoietic stem cell transplantation (HSCT) or microtransplantation. The median relapse-free survival (RFS) for RR-AML patients receiving CLAT regimen was 8.6 (range 2-16) months. Thirteen patients developed grade 4 granulocytopenia and thrombocytopenia, the median duration was 13(range 2 to 21) days and12 (range 2 to 21) days, respectively. The most common non-hematological side effects included nausea, vomiting, diarrhoea, and were grade 1/2. The CLAT regimen seems promising for the treatment of patients, and it was well tolerated. This regimen offers an alternative treatment for those patients with RR-AML who have received severe intensive treatment, especially with anthracycline-containing chemotherapy.     Abstract: Although the concept of entropy in landscape evolution was proposed over 40 years ago, previous studies of geomorphic entropy paid little attention to the applications of geomorphic entropy in the erosional watershed geomorphic system on the Loess Plateau in China. Therefore, we propose a new concept of entropy called watershed geomorphic entropy (WGE) and its method of calculation based on a digital elevation model and the principles of system theory. To study the geomorphic significances of WGE, we applied the WGE to an artificial rainfall experiment that was originally designed to study erosional processes in a small open watershed geomorphic system on the Loess Plateau. Our study shows that the decrease of WGE in an open watershed geomorphic system means a gradual erosional or erosion-dominated landscape evolutional process and the change of WGE shows a perfectly positive linear correlation with the measured sediment yields of the outlet of the watershed system under our experimental conditions. In addition, to some extent, the decrease of the change of WGE also reflects the reduction of total potential energy of a specific erosional, or erosion-dominated, open watershed geomorphic system.
