    Abstract: The aim of this experimental study is to compare the effect of mini‐incision flapless versus flap technique of implant placement on the amount of vascular structures and blood vessel elements in peri‐implant soft tissue, using immunohistochemical analysis. Method The experiment was conducted on five domestic pigs. Each animal received six implants in mandible according to the split‐mouth design. On one randomly chosen jaw side, mini‐incision flapless surgery was performed, whereas on the opposite jaw side, flap was raised. After 3 months of implant healing through submerged approach, the experimental animals were sacrificed and samples for immunohistochemical analyses were taken from the buccal side of peri‐implant mucosa next to the neck of implants, from three levels. The study outcome was the presence of vascular structures and elements of the blood vessels in the peri‐implant mucosa per microscopic field, estimated through ordinal scores from 0 to 2. Effects of surgical approach, site of implantation, and their interaction on vascular scores of peri‐implant mucosa were assessed by Brunner and Langer nonparametric analysis of longitudinal data. Results Statistically significant effect of surgical approach on vascularity of peri‐implant mucosa has been revealed in the second mucosal layer, where flapless approach provided higher vascularity compared with flap approach ( P  = 0.002). In the remaining two layers, surgical approach did not affect mucosal vascularity significantly (layer 1: P  = 0.071; layer 3: P  = 0.433). Conclusion The flapless surgical implant placement approach using mini‐incision provides better vascularization of peri‐implant mucosa after 3 months of healing compared with flap surgery.

    Abstract: This study aims to determine the correlation between the mean value of bone density measured on the CBCT device and the primary stability of dental implants determined by resonant frequency analysis. An experimental study was conducted on a material of animal origin: bovine femur and pig ribs. Two types of implants were used in this study: self-tapping and non-self-tapping of the same dimensions. Results of the experimental study showed a statistically significant correlation between bone density expressed in HU units and the primary stability of self-tapping and non-self- tapping dental implants expressed in ISQ units in bovine femur bones and self-tapping implants and pig rib bones. There was no statistically significant correlation between non-self-tapping dental implants in pig rib bones. Self-tapping and non-self-tapping implants did not show statistical significance in the primary stability in bones of different qualities. The analysis of bone density from CBCT images in the software of the apparatus expressed in HU units can be used to predict the degree of primary stability of self-tapping and non-self-tapping dental implants in bones of densities D1 and D2, and self-tapping dental implants in bones of the lower quality D4.

    Abstract: To evaluate by histopathological analysis the peri‐implant bone inflammation degree, in certain time intervals (7, 14, 21 and 28 days), following mini‐incision flapless and flap implant placement. Material and methods The experiment was conducted on four domestic pigs. Nine weeks prior to implant insertion, second and third mandibular premolars were extracted. Each animal received six implants in lower jaw. On one randomly chosen side of jaw flapless technique using mini‐incision was performed, while on the other side implants were inserted after flap raising. After 7, 14, 21, and 28 days, the experimental animals were sacrificed. Following mandibular resection and decalcification, the samples for histopathological analysis of the peri‐implant bone were obtained in the empty implant bed area, from the buccal side of the mandible, adjacent to implant neck region and parallel to crestal edge of implant bed. The degree of inflammatory response of the peri‐implant bone was estimated through ordinal scores from 0 to 2. Results Seven days after the surgery all samples in the flap group had score 2 indicating high inflammation degree, in contrast to lower inflammatory reaction in flapless group. On the 14th and 21st postoperative day decreasing of inflammation degree was noted in all samples of the flapless group (score 1), while in flap group samples presented scores 1 and 2. Twenty‐eight days after the implant placement, further reduction of inflammation in the flapless group (33% of samples had score 0) was observed. Conclusion Flapless technique in comparison to conventional flap procedure minimizes postoperative bone inflammatory reactions.

    Abstract: Uvod/Cilj. Implantatna hirurgija flapless postaje aktuelna poslednjih nekoliko godina zahvaljujuci kompjuterizaciji stomatologije i softverskog planiranja ugradnje implantata. Cilj rada bio je da se radiografskom analizom i analizom rezonantne frekvencije uporede hirurske tehnike flap i flapless ugradnje implantata. Metode. Eksperiment je obavljen na pet domacih svinja. Devet nedelja nakon ekstrakcije zuba, svinjama su ugradjeni implantati, sa desne strane hirurskom tehnikom flap, a sa leve hirurskom tehnikom flapless. Uradjeni su digitalni radioloski snimci i izmerena primarna stabilnost implantata (ISQ). U odredjenim vremenskim intervalima do tri meseca, eksperimentalne zivotinje su zrtvovane, a neposredno pre radjeni su kontrolni rendgenski snimci i merena stabilnost implantata. Rezultati. Radiografskom analizom uoceno je da je periimplantatna kostana resorpcija u prve cetiri nedelje bila zanemarljiva kod obe hirurske tehnike. Nakon tri meseca srednja vrednost periimplantatne kostane resorpcije kod flap implantata bila je 1,86 mm, a kod flapless implantata 1,13 mm. U odnosu na primarnu stabilnost implantata, u prvoj i drugoj nedelji doslo je do ocekivanog pada vrednosti ISQ, ali je taj pad kod flapless implantata bio manji. U trecoj nedelji rasle su vrednosti ISQ kod flap i flapless implantata, ali je porast kod flapless implantata bio veci (7,4 ISQ) u odnosu na flap implantate (1,5 ISQ). Tendencija rasta se nastavila i u cetvrtoj nedelji i nadalje, a nakon tri meseca vrednosti stabilnosti implantata kod flap implantata bile su vece u odnosu na primarnu stabilnost za 7,1 ISQ, a kod flapless implantata za 10,1 ISQ. Zakljucak. Na osnovu rezultata radiografske analize i analize rezonantne frekvencije mozemo zakljuciti da hirurska tehnika ugradnje implantata flapless daje bolje rezultate od hirurske tehnike flap.

    Abstract: Background/Aim. In planning the extraction of posterior teeth, it is necessary to determine their position and correlation with the surrounding anatomical structures. The aim of this study was to perform Cone Beam Computed Tomography (CBCT) analysis in order to evaluate the predictive value of specific orthopantomography (OPT) radiographic signs in determining the proximity of posterior upper teeth to the maxillary sinus and lower teeth to the mandibular canal. Methods. In a prospective study, 460 cases out of 423 patients were analyzed. Seven OPT radiographic signs were assessed to determine the correlation of the third molars to the mandibular canal, while five radiographic signs were followed to determine the correlation of the maxillary sinus floor to the upper posterior teeth. For each OPT radiographic sign, a precise analysis of the axial slices of the CBCT was performed. Results. Interruption of the white line is a radiographic sign showing statistically significant interruption of the integrity of the mandibular canal on the CBCT axial slices (p = 0.002). Also, diversion of the inferior alveolar canal at the axial sections showed statistically significant interruption of the continuity of the mandibular canal (p = 0.003). A radiographic sign, Darkening of the root, showed super-imposition of the anatomical structures, a close relationship with the tooth but often with preserved mandibular canal integrity (p 〈 0.001). Absence of lamina dura, interruption of the maxillary sinus floor's cortex, and projection of the root apices in the sinus cavity were radiographic signs that were statistically the most commonly associated with interruption of maxillary sinus integrity on CBCT axial slices (p 〈 0.001). Conclusion. The results of our study indicate that the presence of certain radiographic signs on the OPT may have predictive significance. The exact relationship between anatomical structures and posterior teeth cannot be accurately estimated on OPT. The precise position of the molars and correlation to the mandibular canal and the maxillary sinus can be accurately determined by CBCT imaging.

    Abstract: Background/Aim. Treatment of periodontitis undergoes several treatment phases. Non-surgical periodontal treatment (NSPT) represents the basic treatment stage, and it is applied to all the patients undergoing periodontal treatment. Adjunctive antimicrobial photodynamic therapy (aPDT) is one of several contemporary and relatively new possibilities with a role to inactivate microorganisms responsible for the occurrence and progression of the disease. The aim of this study was to comparatively analyze the clinical and microbiological effects of the NSPT alone, as well as combined with aPDT. Methods. A split-mouth method design was used in a prospective randomized controlled trial. The following clinical parameters were registered and monitored: plaque index (PI), bleeding on probing, probing depth (PD), and clinical attachment level (CAL). The presence of microorganisms Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and Treponema denticola was detected by the polymerase chain reaction (PCR) method. Samples were tested before the therapy, as well as three months after the therapy. Therapeutic modalities of NSPT and NSPT with adjunct aPDT were examined. Results. All of the analyzed clinical parameters proved statistically significant improvement after the application of both treatment modalities (p 〈 0.001). Microbiological analyses showed that the total number of microorganisms was statistically significantly lower after the application of both methods (p 〈 0.001). Following the treatment, there was a statistically significantly reduced number of microorganisms Aggregatibacter actinomycetemcomitans, Porphyromonas gingivalis, and Treponema denticola (p 〈 0.001). NSPT combined with aPDT led to a statistically significant improvement of both clinical parameters and microbiological status compared to NSPT applied on its own. Conclusion. The study showed improvement of all clinical indicators after the application of both treatment modalities. The total number of microorganisms was reduced as well as the number of specific microorganisms. Combining aPDT with NSPT led to a statistically significantly higher reduction in the number of microorganisms compared to NSPT alone.

According to split‐mouth design, randomly one side was used for flapless technique using mini‐incision, while on the other side, flap was raised. After 7, 14, 21, 28, and 90 days, the experimental animals were sacrificed. Samples for histopathological analyzes were taken from the buccal side of peri‐implant mucosa next to the neck of implants, from three levels. The degree of inflammatory response in the peri‐implant soft tissue was estimated through ordinal scores from 0 to 3. Results In the flap group Score 3 indicating high degree of inflammation was present from day 7 to day 21, in contrast to flapless group where Score 3 was not recorded during the entire follow‐up. Three months after implantation, there were no signs of inflammation neither around flap nor around flapless implants. Conclusion Flapless surgical implantation technique using mini‐incision decreases peri‐implant soft tissue inflammatory reaction compared with flap surgery.
