    Abstract: We review key design elements of positive randomized controlled trials (RCTs) in acute ischemic stroke (AIS) treatment and summarize their main characteristics. Method We searched Medline, Pubmed and Cochrane databases for positive RCTs in AIS treatment. Trials were included if (1) they had a randomized controlled design, with (at least partial) blinding for endpoints, (2) they tested against placebo (or on top of standard therapy in a superiority design) or against approved therapy; (3) the protocol was registered and/or published before trial termination and unblinding (if required at study commencement); (4) the primary endpoint was positive in the intention to treat analysis; and (5) the study findings led to approval of the investigational product and/or high ranked recommendations. A topical approach was used, therefore the findings were summarized as a narrative review. Findings Seventeen positive RCTs met the inclusion criteria. The majority of trials included less than 1000 patients (n = 15), had highly selective inclusion criteria (n = 16), used the modified Rankin score as a primary endpoint (n = 15) and had a frequentist design (n = 16). Trials tended to be national (n = 12), investigator-initiated and performed with public funding (n = 11). Discussion Smaller but selective trials are useful to identify efficacy in a particular subgroup of stroke patients. It may also be of advantage to limit the number of participating countries and centers to avoid heterogeneity in stroke management and bureaucratic burden. Conclusion The key characteristics of positive RCTs in AIS treatment described here may assist in the design of further trials investigating a single intervention with a potentially high effect size.

    Abstract: Tomato leaf curl Bangalore virus (ToLCBaV), a monopartite begomovirus transmitted by the whitefly Bemisia tabaci , has become a major constraint in tomato production in the Indian subcontinent. Earlier breeding efforts in India led to the adoption of tomato cultivars carrying the resistance gene Ty‐2 . However, it has been observed recently that such cultivars/hybrids are susceptible to begomoviruses. This requires the identification of additional or new sources of resistance against ToLCBaV. The present study was undertaken to assess the effectiveness of several Ty genes in providing resistance against infection by ToLCBaV. The kinetics of virus multiplication in different Ty resistance gene stocks of tomato were estimated and compared using quantitative PCR data. Accumulation of viral genomic units and symptom severity were lower in tomato lines carrying Ty‐3 and Ty‐2  +  Ty‐3 compared with those carrying Ty‐2 alone. All tested lines carrying Ty‐2 showed typical tomato leaf curl disease symptoms. Tomato lines carrying Ty‐2  +  ty‐5 and ty‐5  +  Ty‐6 combinations had a significantly higher disease severity index and viral genomic units compared to those carrying Ty‐3 and Ty‐2  +  Ty‐3 at 30 days postinoculation. The accumulation level of ToLCBaV genomic units serves as a good indicator for resistance selection along with other parameters of disease. We present here a comprehensive overview of the effectiveness of Ty‐2 , Ty‐3 , and ty‐5 alone as well as the combinations Ty‐2  +  Ty‐3 and Ty‐2 + ty‐5 against ToLCBaV.

    Abstract: This study evaluated some new class insecticides viz., chlorantraniliprole 18.5SC, flubendiamide 480SC, spinosad 45%SC, indoxacarb 14.5SC and emamectin benzoate 5%SG in cotton under high density planting system (HDPS). These were evaluated against Helicoverpa armigera (Hubner) and their impact on natural enemies were also observed. Experiments were conducted at the ICAR-Central Institute for Cotton Research, Nagpur during 2013-14. Each of the new molecules were sprayed in eight different windows 60 days after sowing (DAS), 60 and 80 DAS, 60, 80 and 100 DAS, 60, 80 100 and120 DAS, 100 and 120 DAS, 80,100 and 120 DAS, 120 DAS and unsprayed control along with three replicates each were analysed in randomized complete block design. The damage by H. armigera was observed by counting healthy and damaged squares and bolls, and % worked out. The whole plant incidence was recorded in case of major predators sampled including spiders and coccinellids. The spray regime 2 (60 and 80 DAS), regime 3 (60, 80 and 100 DAS), regime 4 (60, 80, 100 and 120 DAS) and regime 6 (80, 100 and 120 DAS) were found superior in managing bollworms with by recording least square damage. The results on natural enemies’ populations (spiders and coccinellids) clearly revealed that, all spray regimes were more or less safe to the natural enemies. However, spray regime 1 (spray at 60 DAS), regime 2 (spray at 60 and 80 DAS) and regime 3 (spray at 60, 80 and 100 DAS) recorded maximum natural enemies. Maximum natural enemies was reported at 80 DAS in all the insecticidal spray regimes. Chlorantraniliprole, flubendiamide, spinosad, indoxacarb and emamectin benzoate through the spray regime 2 (spray at 60 and 80 DAS) was observed to be the best option against H. armigera by controlling it in early stage itself and to sustain natural enemies in the cotton ecosystem.

    Abstract: Neuromuscular diseases (NMDs) affect ∼15 million people globally. In high income settings DNA-based diagnosis has transformed care pathways and led to gene-specific therapies. However, most affected families are in low-to-middle income countries (LMICs) with limited access to DNA-based diagnosis. Most (86%) published genetic data is derived from European ancestry. This marked genetic data inequality hampers understanding of genetic diversity and hinders accurate genetic diagnosis in all income settings. We developed a cloud-based transcontinental partnership to build diverse, deeply-phenotyped and genetically characterized cohorts to improve genetic architecture knowledge, and potentially advance diagnosis and clinical management. We connected 18 centres in Brazil, India, South Africa, Turkey, Zambia, Netherlands and the UK. We co-developed a cloud-based data solution and trained 17 international neurology fellows in clinical genomic data interpretation. Single gene and whole exome data were analysed via a bespoke bioinformatics pipeline and reviewed alongside clinical and phenotypic data in global webinars to inform genetic outcome decisions. We recruited 6001 participants in the first 43 months. Initial genetic analyses ‘solved’ or ‘possibly solved’ ∼56% probands overall. In-depth genetic data review of the four commonest clinical categories (limb girdle muscular dystrophy, inherited peripheral neuropathies, congenital myopathy/muscular dystrophies and Duchenne/Becker muscular dystrophy) delivered a ∼59% ‘solved’ and ∼13% ‘possibly solved’ outcome. Almost 29% of disease causing variants were novel, increasing diverse pathogenic variant knowledge. Unsolved participants represent a new discovery cohort. The dataset provides a large resource from under-represented populations for genetic and translational research. In conclusion, we established a remote transcontinental partnership to assess genetic architecture of NMDs across diverse populations. It supported DNA-based diagnosis, potentially enabling genetic counselling, care pathways and eligibility for gene-specific trials. Similar virtual partnerships could be adopted by other areas of global genomic neurological practice to reduce genetic data inequality and benefit patients globally.

    Abstract: Field experiments were conducted from 1985 through 2001 on medium deep vertisol to determine the effect of the continuous application of manure and fertilizers on a two-year cotton-sorghum rotation. The results indicate that the response to N was greatest during the initial years, while after five to six rotation cycles the yield levels declined to the zero level in the N and NK plots. The application of P along with N prevented the decline in seed cotton and sorghum grain yields. The effect was more pronounced at the higher level. K application did not result in any yield increase. Balanced fertilizer at the higher level (N 90 P 19 K 37 ) resulted in a significant yield increase over the recommended dose (N 60 P 13 K 25 ); however, the percentage increase declined with duration, indicating a decline in factor productivity. Seed cotton yields were the highest when part of the fertilizer N was applied from an organic source (farmyard manure: FYM). Of the eight years, a significant response was observed in four years at the lower level (N 30 P 13 K 25 + 5 t FYM ha -1 ) and six years at the higher level (N 45 P 19 K 37 + 7.5 t FYM ha -1 ), while in sorghum a response was only observed in two years. The cotton crop should, therefore, be preferred to sorghum for the application of organic manure. In areas where no organic manure is available, N 60 P 13 K 25 is sufficient for cotton, while sorghum needs to be fertilized at 1.5 times the recommended dose (N 90 P 19 K 37 ).

    Abstract: Abstract. Multi dimensional data (multi frequency, incident angle and polarisation) measurements of σ0 provided better estimates of soil moisture over those derived from single. This particular paper explains a new methodology for soil moisture estimation with the use of multi angle and multi polarisation RISAT-1 data. The roughness component was derived by correlating root mean square height with the differences of cross polarisation and like polarisation backscatter values (σ HV - σ HH) and differences of low and high incidence backscatter values (σ HH high (θ) - σ HH Low). The derived roughness was inputted to the modified dubois model (MDM). The results show both the σ HV - σ HH & σ HH high (θ) - σ HH Low are sensitive to roughness. The derived soil moisture using the MDM model is shows reasonable correlation with ground soil moisture.

    Abstract: Primary CNS Vasculitis (PCNSV) is a rare inflammatory disorder affecting the blood vessels of the central nervous system. Patients present with a combination of headaches, seizures, and focal neurological deficits. There is usually a diagnostic delay. Treatment is based on observational studies and expert opinion. Our objective was to identify clinical, laboratory, neuroimaging, pathologic or management-related associations with 2 year outcome in patients with primary CNS vasculitis. We conducted a cohort study at a single tertiary care referral centre of prospectively (2018-2019) and retrospectively (2010-2018) identified individuals with primary CNS vasculitis (diagnosis was proven by either brain biopsy or cerebral digital subtraction angiography). Clinical, imaging and histopathologic findings, treatment, and functional outcomes were recorded. Univariate and stepwise multiple logistic regression were applied. P-value 〈 0.05 was considered statistically significant. The main outcome measures were documentation of clinical improvement or worsening (defined by mRS scores) and identification of independent predictors of good functional outcome (mRS 0-2) at 2 years. We enrolled eighty-two biopsy and/or angiographically proven PCNSV cases. The median age at presentation was 34 years with a male predilection and a median diagnostic delay of 23 months. Most patients presented with seizures (70.7%). All patients had haemorrhages on MRI. Histologically lymphocytic subtype was the commonest. Corticosteroids with cyclophosphamide was the commonest medication used. The median mRS at follow-up of 2 years was 2 (0-3), and 65.2% of patients achieved a good functional outcome. Myelitis and longer duration of illness before diagnosis were associated with poorer outcomes. The presence of hemorrhages on SWI sequence of MRI might be a sensitive imaging marker. Treatment with steroids and another immunosuppressant probably reduced relapse rates in our cohort. We have described the third largest PCNSV cohort and multi-centre randomised controlled trials are required to study the relative efficacy of various immunosuppressants.     Abstract: Primary CNS Vasculitis (PCNSV) is a rare, diverse, and polymorphic CNS blood vessel inflammatory condition. Due to its rarity, clinical variability, heterogeneous imaging results, and lack of definitive laboratory markers, PCNSV diagnosis is challenging. This retrospective cohort analysis identified patients with histological diagnosis of PCNSV. Demographic data, clinical presentation, neuroimaging studies, and histopathologic findings were recorded. We enrolled 56 patients with a positive biopsy of CNS vasculitis. Most patients had cerebral hemisphere or brainstem symptoms. Most brain MRI lesions were bilateral, diffuse discrete to confluent white matter lesions. Frontal lobe lesions predominated, followed by inferior cerebellar lesions. Susceptibility-weighted imaging (SWI) hemorrhages in 96.4% (54/56) of patients, either solitary microhemorrhages or a combination of micro and macrohemorrhages. Contrast-enhanced T1-WIs revealed parenchymal enhancement in 96.3% (52/54 patients). The most prevalent pattern of enhancement observed was dot-linear (87%), followed by nodular (61.1%), perivascular (25.9%), and patchy (16.7%). Venulitis was found in 19 of 20 individuals in cerebral DSA. Hemorrhages in SWI and dot-linear enhancement pattern should be incorporated as MINOR diagnostic criteria to diagnose PCNSV accurately within an appropriate clinical context. Microhemorrhages in SWI and venulitis in DSA, should be regarded as a potential marker for PCNSV.
