    Abstract: Alle 35-vuotiaiden työssä käyvien aikuisten mielenterveyden haasteet ovat kasvaneet voimakkaasti Suomessa ja muissa länsimaissa. Tutkimus on painottunut työelämän riskitekijälähtöiseen tarkasteluun, ja ratkaisujen etsinnässä on korostunut sekä lääketieteellinen että yksilölähtöinen viitekehys. Tässä artikkelissa tarkastellaan nuorten työntekijöiden työkykyä työelämän ja kulttuurisen muutoksen näkökulmasta. Lisäksi pyritään rakentamaan uudenlaista tutkimusperustaista näkökulmaa mielenterveyteen liittyvän työkyvyn tukemiseksi. Aineistona tutkimuksessa on käytetty Työterveyslaitoksen ”Mitä jos mielen hyvinvointia rakennettaisiin uudestaan?” -hankkeessa pääkaupunkiseudun kunta-alalta vuonna 2021 kerättyä haastatteluaineistoa (N = 70), jossa on haastateltu nuoria aikuisia, työterveyshuoltojen edustajia sekä esihenkilöitä. Tutkimusaineiston sisältöanalyysissä hyödynnetään Margaret Archerin teoriaa toimijuuden muodostumisesta refleksiivisen harkinnan kautta. Tuloksissa esitellään seitsemän erilaista nuorten työntekijöiden mielenterveyden toimijahahmoa. Kukin toimijahahmo pyrkii mielekkääseen toimintaan ja ratkaisuun omista arvoistaan, normeistaan ja moraalisista lähtökohdistaan käsin. Toimintaa raamittavat käytettävissä olevat resurssit. Hahmotypologia osoittaa, että mielenterveyskategorian alle on kertynyt joukko erilaisia yhteiskuntaan, työelämään ja yksilön elämänkulkuun kietoutuvia haasteita, joita ratkotaan työterveyshuollossa yksilötasolla. Hahmojen sosiaalisen ja rakenteellisen taustan moninaisuus korostaa sitä, kuinka mielenterveyteen liittyvä työkyky jäsentyy osaksi sosiaalista ja institutionaalista toimintaa. Tällöin on syytä myös kysyä, onko perusteltua pyrkiä ratkomaan nuorten työntekijöiden mielenterveyden haasteita pääsääntöisesti yksilötasolla diagnostisin käsittein ja hoitomuodoin. Abstract Social characters of mental health. A qualitative study of young employees in the municipal sector. The mental health challenges faced by working adults aged under 35 have grown rapidly in Finland and other Western countries. Studies have emphasized risk-based observation of work life, and the search for solutions has centered around two frameworks: medical and individual oriented. This article examines the work ability of young employees from the viewpoint of work life and cultural change. It also attempts to build a new kind of study-based viewpoint to support mental health-related work ability. The data used in this study were collected during the What if mental health could be constructed differently? project of the Finnish Institute of Occupational Health, in which young adults, occupational health service representatives, and supervisors (N=70) from the municipal sector in the capital area were interviewed in the year 2021. Content analysis of the study data used formation through reflexive deliberation from Margaret Archer’s theory of agency. Seven different social characters of young employee’s mental health are presented in the results. Every social character strives towards meaningful action and a solution based on their own values, norms, and moral starting points. Action takes place within the limits of available resources. The character typology shows that the mental health category has accumulated many different challenges, which are intertwined with society, work life and the individual life course, and are solved on an individual level by occupational health services. The diversity of a characters’ social and structural background highlights how mental health-related work ability is structured as part of social and institutional action. Thus, we may ask whether striving to resolve the mental health challenges of young employees on mainly an individual level, using diagnostic terms and types of treatment, is justified.     Abstract: To examine the association between cognitively stimulating work and subsequent risk of dementia and to identify protein pathways for this association. Design Multicohort study with three sets of analyses. Setting United Kingdom, Europe, and the United States. Participants Three associations were examined: cognitive stimulation and dementia risk in 107 896 participants from seven population based prospective cohort studies from the IPD-Work consortium (individual participant data meta-analysis in working populations); cognitive stimulation and proteins in a random sample of 2261 participants from one cohort study; and proteins and dementia risk in 13 656 participants from two cohort studies. Main outcome measures Cognitive stimulation was measured at baseline using standard questionnaire instruments on active versus passive jobs and at baseline and over time using a job exposure matrix indicator. 4953 proteins in plasma samples were scanned. Follow-up of incident dementia varied between 13.7 to 30.1 years depending on the cohort. People with dementia were identified through linked electronic health records and repeated clinical examinations. Results During 1.8 million person years at risk, 1143 people with dementia were recorded. The risk of dementia was found to be lower for participants with high compared with low cognitive stimulation at work (crude incidence of dementia per 10 000 person years 4.8 in the high stimulation group and 7.3 in the low stimulation group, age and sex adjusted hazard ratio 0.77, 95% confidence interval 0.65 to 0.92, heterogeneity in cohort specific estimates I 2 =0%, P=0.99). This association was robust to additional adjustment for education, risk factors for dementia in adulthood (smoking, heavy alcohol consumption, physical inactivity, job strain, obesity, hypertension, and prevalent diabetes at baseline), and cardiometabolic diseases (diabetes, coronary heart disease, stroke) before dementia diagnosis (fully adjusted hazard ratio 0.82, 95% confidence interval 0.68 to 0.98). The risk of dementia was also observed during the first 10 years of follow-up (hazard ratio 0.60, 95% confidence interval 0.37 to 0.95) and from year 10 onwards (0.79, 0.66 to 0.95) and replicated using a repeated job exposure matrix indicator of cognitive stimulation (hazard ratio per 1 standard deviation increase 0.77, 95% confidence interval 0.69 to 0.86). In analysis controlling for multiple testing, higher cognitive stimulation at work was associated with lower levels of proteins that inhibit central nervous system axonogenesis and synaptogenesis: slit homologue 2 (SLIT2, fully adjusted β −0.34, P 〈 0.001), carbohydrate sulfotransferase 12 (CHSTC, fully adjusted β −0.33, P 〈 0.001), and peptidyl-glycine α-amidating monooxygenase (AMD, fully adjusted β −0.32, P 〈 0.001). These proteins were associated with increased dementia risk, with the fully adjusted hazard ratio per 1 SD being 1.16 (95% confidence interval 1.05 to 1.28) for SLIT2, 1.13 (1.00 to 1.27) for CHSTC, and 1.04 (0.97 to 1.13) for AMD. Conclusions The risk of dementia in old age was found to be lower in people with cognitively stimulating jobs than in those with non-stimulating jobs. The findings that cognitive stimulation is associated with lower levels of plasma proteins that potentially inhibit axonogenesis and synaptogenesis and increase the risk of dementia might provide clues to underlying biological mechanisms.

    Abstract: There is a recent and growing migrant population in Northern Ireland. However, rigorous research is absent regarding access to mental health care by different migrant groups. In order to address this knowledge gap, this study aimed to identify the relative use of psychotropic medication between the largest first generation migrant groups in Northern Ireland and the majority population. Methods Census (2011) data was linked to psychotropic prescriptions for the entire enumerated population of Northern Ireland using data linkage methodology through the Administrative Data Research Centre Northern Ireland (ADRC-NI). Results Lower prescription dispensation for all psychotropic medication types, particularly antidepressants (OR = 0.35, CI 95% 0.33–0.36) and anxiolytics (OR = 0.42, CI 95% 0.40–0.44), was observed for all migrant groups with the exception of migrants from Germany. Conclusions It is likely that the results reflect poorer access to services and indicate a need to improve access and the match between resources, services and the health and social care needs of migrants. Further research is required to identify barriers to accessing primary care and mental health services.

    Abstract: Objective: The effects of lifestyle interventions on the prevention of a decline in work ability and mental health are not well known. The aim of this randomized controlled trial was to examine the effects of healthy lifestyle changes on work ability, sleep, and mental health. Methods: Workers aged 18–65 years, who were free from cardiovascular diseases, diabetes, and malignant diseases, and did not use medication for obesity or lipids were included (N = 319). Based on their cholesterol balance, participants were classified into medium-risk and high-risk groups and were randomized into four arms: group lifestyle coaching (N = 107), individual lifestyle coaching (N = 53), the control group for group coaching (N = 106), and the control group for individual coaching (N = 53). The intervention groups received eight sessions of mostly remote coaching for 8 weeks about healthy diet, physical activity, other lifestyle habits, and sources/management of stress and sleep problems, and the control groups received no intervention. In individual coaching, the coach focused more on individual problem solving and the possibilities for motivation and change. The intention-to-treat principle was applied, and missing data on the outcomes were imputed using multiple imputation. Results: After the completion of the intervention, the risk of depressive symptoms was lower by 53% (95% CI 1–77%) in participants who received individual lifestyle coaching compared with the control group. The intervention had no beneficial effects on anxiety, work ability, sleep duration, or daily stress. In subgroup analyses, group lifestyle coaching had beneficial effects on depressive symptoms and work ability in participants with less tight schedules or less stretching work, whereas individual lifestyle coaching lowered the risk of depressive symptoms in those with fewer overlapping jobs, less tight schedules, or less stretching work. Conclusion: Short but intensive remote lifestyle coaching can reduce depressive symptoms and improve work ability, and time-related resources at work may improve mental health in the context of individual lifestyle intervention. However, further randomized controlled trials are needed to confirm the findings.

    Abstract: In the last few decades, suicide rates have decreased in most European countries. However, periodic changes in risk by occupational class have not been studied as much in detail. Methods Representative cohorts of Finnish working-age men were followed for nine years on suicide mortality starting from five different census years (1970, 1980, 1990, 2000, 2010). Each cohort included between 300 970 and 332 318 men. Cox regression modelling was used to estimate hazard ratios by census year, occupational class and their interactions. Further models adjusted for age and its interactions with census year and occupational class. Results The risk of male suicide has more than halved between 1991 and 2019. The relative hazard ratio of suicide in manual workers compared to managers and professionals was around 1.6 to 1.8 times higher. The period when the suicide risk started to decline differed by occupational class: a significant decrease compared to 1970s’ levels was seen for managers and professionals already in the 1990s and for lower non-manual employees around 10 years later (in the 2000s). Manual workers only reached the 1970s suicide risk of managers and professionals in the 2000s and 2010s. Conclusion A delayed reduction of suicide rates among lower occupational classes suggests that the impact of social changes can occur at different speed in different population groups.

    Abstract: Drawing from the sociological and organizational scholarship of emotion management in human service work, our historical analysis elaborates on how external pressures and efficiency requirements jeopardize employees’ ability to carry out emotional labour in the public sector. Building on the Finnish social workers’ accounts published in a professional trade paper in 1975–2009, the article provides an insight into the emotional challenges and dissonances of interactive work. Findings The article suggests that social workers’ emotional job requirements are deeply embedded in and influenced by the broader social context and changes in the welfare state. The article indicates how workers’ emotion management underwent changes and readjustments from the heyday of the expansive welfare state of the mid-1970s, through the severe economic recession of the 1990s, and was bound by the ideological reform of the Finnish welfare policy in the 1990s and 2000s. Applications Historical prospect to emotion management shows social workers, social work students and the academic community how employees’ emotion management has been gradually recognized as a crucial element of relational client work over the decades, but also how workers’ emotional job requirements and demeanours are intertwined with broader societal changes. Second, emotion management research provides a solid framework to elaborate emotional and ethical dissonances which are embedded in social work more profoundly.
