    Abstract: Đặt vấn đề: Beta-thalassemia là nhóm bệnh thiếu máu di truyền gen lặn theo quy luật Mendel do đột biến gen globin làm giảm hoặc mất tổng hợp các chuỗi β-globin từ đó gây ra tình trạng thiếu máu tán huyết. Mục tiêu nghiên cứu: Mô tả đặc điểm lâm sàng beta-thalassemia thể nhẹ, thể trung gian và thể nặng. Đối tượng và phương pháp nghiên cứu: Nghiên cứu mô tả cắt ngang trên 44 bệnh nhân mắc beta-thalassemia đến khám và điều trị tại Bệnh viện Huyết học-Truyền máu Cần Thơ và Bệnh viện Trường Đại học Y Dược Cần Thơ từ tháng 06/2021-12/2022. Kết quả: Độ tuổi trung bình biểu hiện triệu chứng đầu tiên là 16,7± 9,787 (tháng tuổi) ở thể nặng, 13,91± 19,835 (tuổi) ở thể trung gian và 17± 3,13 (tuổi) ở thể nhẹ. Về các triệu chứng cơ năng, bệnh nhân beta-thalassemia thể nhẹ có 72,7% bệnh nhân có hoa mắt-chóng mặt, 45,5% có mệt mỏi; thể trung gian có 95,7% bệnh nhân biểu hiện mệt mỏi, chóng mặt chiếm 78,3%, ăn/uống kém chiếm 73,9%; 100% bệnh nhân có mệt mỏi ở thể nặng, 80% biểu hiện chóng mặt, ăn/bú kém chiếm 70%. Về các triệu chứng thực thể, 100% bệnh nhân thể nặng có triệu chứng thiếu máu (da xanh, niêm nhợt), sạm da 50%, khuôn mặt thalassemia 50%, gan to chiếm 80%, lách to 50%, đã cắt lách 50%; thể trung gian có triệu chứng tương đồng với thể nặng nhưng mức độ nhẹ hơn, niêm nhợt chiếm 91,3%, da xanh 78,3%, sạm da 56,5%, khuôn mặt thalassemia 30,4%, gan to 52,2%, lách to 39,1%, đã cắt lách 21,7%; thể nhẹ có da xanh, niêm nhợt chiếm 9,1%. Kết luận: Các đặc điểm lâm sàng có mối liên hệ với các thể bệnh beta-thalassemia, thể bệnh càng nặng biểu hiện triệu chứng lâm sàng càng rõ rệt, càng nặng nề hơn.

    Abstract: Interpreting point-of-care lung ultrasound (LUS) images from intensive care unit (ICU) patients can be challenging, especially in low- and middle- income countries (LMICs) where there is limited training available. Despite recent advances in the use of Artificial Intelligence (AI) to automate many ultrasound imaging analysis tasks, no AI-enabled LUS solutions have been proven to be clinically useful in ICUs, and specifically in LMICs. Therefore, we developed an AI solution that assists LUS practitioners and assessed its usefulness in  a low resource ICU. Methods This was a three-phase prospective study. In the first phase, the performance of four different clinical user groups in interpreting LUS clips was assessed. In the second phase, the performance of 57 non-expert clinicians with and without the aid of a bespoke AI tool for LUS interpretation was assessed in retrospective offline clips. In the third phase, we conducted a prospective study in the ICU where 14 clinicians were asked to carry out LUS examinations in 7 patients with and without our AI tool and we interviewed the clinicians regarding the usability of the AI tool. Results The average accuracy of beginners’ LUS interpretation was 68.7% [95% CI 66.8–70.7%] compared to 72.2% [95% CI 70.0–75.6%] in intermediate, and 73.4% [95% CI 62.2–87.8%] in advanced users. Experts had an average accuracy of 95.0% [95% CI 88.2–100.0%] , which was significantly better than beginners, intermediate and advanced users ( p   〈  0.001). When supported by our AI tool for interpreting retrospectively acquired clips, the non-expert clinicians improved their performance from an average of 68.9% [95% CI 65.6–73.9%] to 82.9% [95% CI 79.1–86.7%] , ( p   〈  0.001). In prospective real-time testing, non-expert clinicians improved their baseline performance from 68.1% [95% CI 57.9–78.2%] to 93.4% [95% CI 89.0–97.8%] , ( p   〈  0.001) when using our AI tool. The time-to-interpret clips improved from a median of 12.1 s (IQR 8.5–20.6) to 5.0 s (IQR 3.5–8.8), ( p   〈  0.001) and clinicians’ median confidence level improved from 3 out of 4 to 4 out of 4 when using our AI tool. Conclusions AI-assisted LUS can help non-expert clinicians in an LMIC ICU improve their performance in interpreting LUS features more accurately, more quickly and more confidently.

    Abstract: One hundred days after severe acute respiratory syndrome coronavirus 2 (SARS-CoV-2) was first reported in Vietnam on 23 January, 270 cases were confirmed, with no deaths. We describe the control measures used by the government and their relationship with imported and domestically acquired case numbers, with the aim of identifying the measures associated with successful SARS-CoV-2 control. Methods Clinical and demographic data on the first 270 SARS-CoV-2 infected cases and the timing and nature of government control measures, including numbers of tests and quarantined individuals, were analyzed. Apple and Google mobility data provided proxies for population movement. Serial intervals were calculated from 33 infector-infectee pairs and used to estimate the proportion of presymptomatic transmission events and time-varying reproduction numbers. Results A national lockdown was implemented between 1 and 22 April. Around 200 000 people were quarantined and 266 122 reverse transcription polymerase chain reaction (RT-PCR) tests conducted. Population mobility decreased progressively before lockdown. In total, 60% (163/270) of cases were imported; 43% (89/208) of resolved infections remained asymptomatic for the duration of infection. The serial interval was 3.24 days, and 27.5% (95% confidence interval [CI], 15.7%-40.0%) of transmissions occurred presymptomatically. Limited transmission amounted to a maximum reproduction number of 1.15 (95% CI, .·37–2.·36). No community transmission has been detected since 15 April. Conclusions Vietnam has controlled SARS-CoV-2 spread through the early introduction of mass communication, meticulous contact tracing with strict quarantine, and international travel restrictions. The value of these interventions is supported by the high proportion of asymptomatic and imported cases, and evidence for substantial presymptomatic transmission.

    Abstract: Previous studies regarding malloapelta B (malB), a natural compound isolated from the Vietnamese medicinal plant, showed a strong NF-κB inhibitory effect, making it a promising source for the development of novel anticancer drugs. However, similar to many other natural compounds from plants, malB has several disadvantages for clinical applications, including high toxicity and low solubility. To improve its bioavailability, malB was conjugated into nanoliposomes, which are ideal drug carriers. The formulations with 1,2-dipalmitoyl-sn-glycero-3-phosphocholine, mPEG-cholesterol, malB, with or without cholesterol exhibited nanoliposomes with an average diameter of approximately 76.98 nm, PDI of 0.28, zeta potential of −5.53 mV, and the highest encapsulation efficiency of 78.73% ± 9.5%. These malB-nanoliposomes inhibited the survival of all lung cancer cell lines examined with IC50 values ranging from 11.86 to 13.12 µM. Moreover, malB-nanoliposomes showed stronger inhibition of A549 colony-forming activity compared to that of the free compound. The effects of malB and its nanoliposomal formulation may be mediated through activation of apoptosis by the significant induction of caspase 3 activity. The nanoliposomal formulations also showed potential to inhibit tumor growth (37.03%) and prolong survival (32.20 days) of tumor-bearing mice compared with the unloaded drug (p 〈 0.05). The improved antitumor activity of malB-nanoliposomes suggests their promising clinical applications.

    Abstract: Biotic and abiotic factors control aboveground biomass (AGB) and the structure of forest ecosystems. This study analyses the variation of AGB and stand structure of evergreen broadleaved forests among six ecoregions of Vietnam. A data set of 173 1-ha plots from 52 locations in undisturbed old-growth forests was developed. The results indicate that basal area and AGB are closely correlated with annual precipitation, but not with annual temperature, evaporation or hours of sunshine. Basal area and AGB are positively correlated with trees 〉  30 cm DBH. Most areas surveyed (52.6%) in these old-growth forests had AGB of 100–200 Mg ha −1 ; 5.2% had AGB of 400–500 Mg ha −1 , and 0.6% had AGB of 〉  800 Mg ha −1 . Seventy percent of the areas surveyed had stand densities of 300–600 ind. ha −1 , and 64% had basal areas of 20–40 m 2 ha −1 . Precipitation is an important factor influencing the AGB of old-growth, evergreen broadleaved forests in Vietnam. Disturbances causing the loss of large-diameter trees (e.g., 〉  100 cm DBH) affects AGB but may not seriously affect stand density.

    Abstract: As reported, cancer stem cells (CSCs) are in charge of dangerous characteristics including drug resistance, metastasis, recurrence and therapeutic effectiveness. Therefore, CSCs are an important target for discovering of novel effective and specific anticancer drugs. In Vietnam, the Stichopus chloronotus sea cucumber is found as a potential biological source with various active ingredients. Particularly, the active triterpene saponin stichoposide D, which was isolated from S. chloronotus, showed strong cytotoxic activity in leukemias. Herein, stichoposide D was further studied its potential anti-CSCs activities on pluripotent human embryonic carcinomas NTERA-2 cells. The compound exhibited its promising and specific cytotoxic activities in NTERA-2 cells with the IC50 = 0.26 ± 0.02 µM, in comparison with that ranging from 0.35 ± 0.02 µM to 0.53 ± 0.03 µM (P 〈 0.05), tested on non-CSCs cancer cell lines, which were breast carcinoma (MCF-7) and lung adenocarcinoma (SK-LU-1), respectively. The working fashion of the compound on NTERA-2 cells could be apoptotic induction. Significantly, treatment of stichoposide D at 1 µM induced 76.4% of apoptotic cells as well as 1.72 relatively fold change of caspase-3 activation in comparision with the control (P 〈 0.05). Meanwhile, stichoposide D was the first time recorgnized its positive efficacy on reducing the number of highly expressed CD44+/CD24+ cells, which were reported as typically CSCs characterized population. The compound also exhibited some effects on NTERA-2 cell cycle of which it arrested cells at sub-G1 phase (15.03%) and prevented those CSCs to enter the S-phase for DNA synthesis. In conclusion, stichoposide D presents potential anti-CSCs activities and should be further studied for future applications.

    Abstract: Currently, multimode treatment is current therapy for cancer that causes various unwanted side effects and costliness for patients. Therefore, finding new natural ingredients derived from plants of which are less toxic and more targeted, are considered to be urgent for development of potential effective drugs to treat cancer due to safety and economic concerns. Physagulin P (VPA22) was known as a novel structural compound, isolated from the species Physalis angulata of the genus Physalis in Vietnam. This active substance has demonstrated cytotoxic effect on lung cancer cell line (A549) with the IC50 value = 13.47±2.73 µM. In our study, VPA 22 was investigated for apoptotic inducing activity on A549 cells by using Hoechst staining, flowcytometry analysis and caspase-3 colorimetric assay. VPA 22 treatment changed the morphology of A549 tending to apoptotic cells whose percentage ranged from 10.09% to 19.18% at the concentration of 5 µM and 15 µM, respectively. In addition, the active ingredient at concentration of 5 µM to 15 µM also significantly activated caspase-3 in turn from 1.30 folds to 2.59 folds. These results together indicated that VPA 22 activated the caspase pathway to trigger out apoptosis on A549 cells and subsequently resulted in inhibition of cancer cell proliferation.
