    Abstract: We aimed to investigate the performance of a chest X-ray (CXR) scoring scale of lung injury in prediction of death and ICU admission among patients with COVID-19 during the 2021 peak pandemic in HCM City, Vietnam. CXR and clinical data were collected from Vinmec Central Park-hospitalized patients from July to September 2021. Three radiologists independently assessed the day-one CXR score consisting of both severity and extent of lung lesions (maximum score = 24). Among 219 included patients, 28 died and 34 were admitted to the ICU. There was a high consensus for CXR scoring among radiologists (κ = 0.90; CI95%: 0.89–0.92). CXR score was the strongest predictor of mortality (tdAUC 0.85 CI95% 0.69–1) within the first 3 weeks after admission. A multivariate model confirmed a significant effect of an increased CXR score on mortality risk (HR = 1.33, CI95%: 1.10 to 1.62). At a threshold of 16 points, the CXR score allowed for predicting in-hospital mortality and ICU admission with good sensitivity (0.82 (CI95%: 0.78 to 0.87) and 0.86 (CI95%: 0.81 to 0.90)) and specificity (0.89 (CI95%: 0.88 to 0.90) and 0.87 (CI95%: 0.86 to 0.89)), respectively, and can be used to identify high-risk patients in needy countries such as Vietnam.

    Abstract: Giới thiệu và Mục tiêu: Chúng tôi khảo sát chỉ số định lượng độ nặng tổn thương trên X-quang phổi (CXR) lúc nhập viện nhằm dự đoán tử vong ở các bệnh nhân mắc COVID-19 điều trị tại bệnh viện Vinmec Central Park (TP.HCM, VN) trong đợt dịch cao điểm năm 2021. Đối tượng và Phương pháp nghiên cứu: Thiết kế hồi cứu; dữ liệu X-quang và lâm sàng được thu thập từ tất cả bệnh nhân nhập viện và dương tính với SARS-CoV-2 PCR từ tháng 7 đến tháng 9 năm 2021. Ba bác sĩ X quang đánh giá độc lập điểm CXR lúc nhập viện, bao gồm mức độ nặng và mức độ lan rộng của tổn thương phổi trên bốn góc phần tư phổi (thang điểm 0-24). Mối liên hệ giữa điểm CXR và nguy cơ tử vong được ước lượng bằng phân tích hồi quy thời gian/sự kiện với phân phối logarit-logistic. Kết quả: Có 219 bệnh nhân được đưa vào nghiên cứu (28 bệnh nhân tử vong). Có sự đồng thuận tốt giữa 3 bác sĩ về ước lượng điểm CXR (κ = 0,90; CI95%: 0,89-0,92). Phân tích PCA cho thấy vai trò tương đương giữa điểm CXR và chỉ số CRP. Điểm CXR còn là thông số có khả năng dự báo tử vong tốt nhất (tdAUC 0,85; CI95% 0,69–1) trong giai đoạn 3 tuần đầu so với những thông số lâm sàng và xét nghiệm thường quy khác. Sau khi hiệu chỉnh cho tuổi, vẫn có liên hệ ý nghĩa giữa sự gia tăng điểm CXR và nguy cơ tử vong (HR = 1.15; CI95%:1.04-1.27, p=0.009). Ở ngưỡng 16 điểm, điểm CXR cho phép dự đoán tử vong với độ nhạy và đặc hiệu cao (0,82; CI95%: 0,78 - 0,87 và 0,89; CI95%: 0,88 - 0,90). Kết luận: Điểm số CXR trong ngày đầu nhập viện là yếu tố dự báo độc lập và hiệu quả cho nguy cơ tử vong trong COVID-19 và có thể được sử dụng để xác định những bệnh nhân có nguy cơ cao ở những nước như Việt Nam.

    Abstract: BACKGROUND: Rotator cuff tear (RCT) is a common injury of the shoulder, especially middle-aged people. Nonoperative treatment, cortisone injections are only effective at an early stage. Open surgery causes postoperative atrophy of the deltoid muscle, so results are limited. Arthroscopic rotator cuff repair surgery has been performed in Vietnam for about ten years, with many advantages such as the ability to accurately assess the lesions and less invasive procedure. In order to have a clearer view, we performed a mid-term assessment of the effectiveness of this surgery. AIM: Evaluate results over 2 years of patients with rotator cuff tears treated with arthroscopic surgery and their quality of life. METHOD: A group of 30 patients were diagnosed with RCT and surgery by arthroscopy to treat at Hanoi Medical University Hospital and Saint Paul Hospital between Jun 2015 and April 2017. The results of the surgeries were assessed by the degree of pain, muscle power, motion of the shoulder joint according to UCLA shoulder score. Evaluate the quality of life through the Rotator Cuff-Quality of Life (RC-QoL) index. RESULTS: The average age was 60.7 years. Female / male ratio was 1.3. Thirty-six months ± 6.41 was the average follow-up time (min 27 – max 50 months). The shoulder function is recorded according to UCLA has an average score of 30.9, therein good and excellent result were 90 %. The mean RC-QoL index was 91.5%. CONCLUSION: Treatment of RCT by arthroscopic surgery that has been evaluated for a minimum of 2 years follow-up showed good results and high quality of patient’s life.

    Abstract: Mục tiêu: Đánh giá giá trị của siêu âm 2D và siêu âm Doppler màu trong chẩn đoán hẹp động mạch cảnh trong đoạn ngoàisọ, đối chiếu với chụp mạch số hoá xoá nền (DSA).Đối tượng và phương pháp: Nghiên cứu hồi cứu với mẫu 18 bệnh nhân được chẩn đoán hẹp động mạch cảnh trong đoạnngoài sọ bằng siêu âm, dựa trên 2 phương pháp là tính chỉ số đường kính (phương pháp NASCET) trên siêu âm 2D và dựa theothay đổi của các chỉ số vận tốc trên siêu âm Doppler màu. Kết quả có đối chiếu với DSA, được xem là tiêu chuẩn vàng trongxác định mức độ hẹp.Kết quả: Lấy DSA làm tiêu chuẩn vàng, tỉ lệ hẹp ĐM cảnh có ý nghĩa ( 〉 70%) trên siêu âm theo chỉ số đường kính (phươngpháp NASCET) và theo các chỉ số vận tốc phát hiện được lần lượt 77,8% và 50%, tỉ lệ này là 77,8% khi kết hợp cả 2 phươngpháp, bằng với đánh giá theo đường kính. Có sự tương quan chặt chẽ giữa tỉ lệ hẹp trên siêu âm tính theo phương pháp NASCET và trên DSA với r = 0.674 (p = 0.002). Không có sự tương quan tuyến tính giữa chỉ số PSV trên siêu âm và trên chụp DSA (p 〉 0.05). Mức độ đồng hợp giữa hai phương pháp siêu âm 2D và Doppler trong xác định mức độ hẹp mạch cảnh là thấp, với giá trị Kappa = 0.44 (p=0.002).Kết luận: Siêu âm là phương pháp dễ tiếp cận, rẻ tiền, không xâm lấn nhưng đáng tin cậy trong việc ước tính mức độ hẹp độngmạch cảnh trong đoạn ngoài sọ và siêu âm 2D sử dụng phương pháp đo NASCET có độ chính xác cao hơn siêu âm Doppler.

    Abstract: Partial differential equations, integral, differential, or other equations describe multi-motor automatic electric drive systems containing elastic conveyor belts. Because of the elastic and distributive nature of the system parameters, the transfer function describing them is often a complex expression, containing not only the arguments as a linear system but also the inertial and transcendental components. This makes the precise control of tension and speed synchronously much more complicated than the centralized parameter system. A promising numerical solution based on the real interpolation method will simplify the procedure for synthesizing control loops while preserving the characteristic properties of objects with distributed parameters. The objective of the study is to propose a feasible solution for synthesizing the regulators based on the real interpolation method; it allows direct operation with the original transfer function containing the inertial and transcendental components. In this paper, we proposed an approach to synthesize the control system for objects with distributed parameters using the real interpolation method to reduce computational capacity and synthesis error while preserving the properties of this object class. Building an experimental model of the two-motor electric drive system containing an elastic conveyor to verify the effectiveness of the proposed algorithm. The simulation and experimental results indicate that the control system with the received regulators operating stably and meets the required quality criteria. It proves the efficiency of the synthesis algorithm based on the real interpolation method.

    Abstract: Abstract Introduction: Laparoscopic pancreaticoduodenectomy is one of the most complicated major surgeries. The postoperative mortality and morbidity are still a burden of hepato-pancreato-biliary surgery. Aim of this study is to assess the short-term outcome of laparoscopic Whipple procedure for periampullary cancer. Materials and methods: Case series report. Results: From 1/2018 to 1/2019, we had performed laparoscopic Whipple procedure in 21 patients. The intracorporeal hepatico-jejunal anastomosis had been formed in all the cases. The pancreaticojejunal and gastro-jejunal anastomosis had been formed extracorporeally through small 5cm midline incision. The pancreatic duct had been drained with 8F drainage. The average operation time was 364 minutes. The average intraoperation blood loss was 175ml. Postoperative mean length of stay was 12.1 days. Pancreaticojejunal anastomosis accounted for 19,0%. Postoperative bleeding occurred in 2 patient (9%) whom were converted to open surgery for control bleeding. In our series, there was no postoperative mortality. Conclusions: Laparoscopic-assisted pancreaticoduodenectomy is safe with low postoperative morbidity and mortality. This procedure is feasible for treatment of periampullary cancer.

    Abstract: A biochemical experiment on four marine fish species commonly farmed in Vietnam, sea bass Lates calcarifer(Bloch, 1790), cobia Rachycentron canadum (Linnaeus, 1766), orange-spotted grouper Epinephelus coioides(Hamilton, 1822), and short yellowfin pompano Trachinotus ovatus (Linnaeus, 1758), was carried out at Cat Ba island, Hai Phong province. The mean weight of fish used in the experiment were 2, 5, 2, and 0.6 kg.ind-1, respectively. The experiment aimed to determine the biological parameters of the farmed fish (Ap, Af, Ac) and related nutritional parameters (Pp, Pf, Fp, Ff, Fc), for assessment of the holding capacity of a location. The results showed that the apparent digestibility coefficient of protein and fat in trash fish for sea bass were 92.90%, 93.84%, respectively; cobia 91.17%, 95.53%; orange-spotted grouper 95.20%, 96.40%. The apparent digestibility coefficient of protein, fat and carbohydrate in pellet feed for short yellowfin pompano were 85.83, 76.47, and 13.68%, respectively. The protein content and fat content in the muscle of sea bass were 19.28%, 6.27%; cobia 19.13%, 13.85%; orange-spotted grouper 17.37%, 7.32%; and short yellowfin pompano 18.31%, 15.08%, respectively. The protein content and fat content of trash fish were 16.99%, 7.19%, respectively. The proportion of protein, fat and carbohydrate of pellet feed were 42.01, 9.93, and 27.82%, respectively. There are no carbohydrates in the muscle of farmed fish as well as trash fish.

    Abstract: Huperzine A, isolated from Huperzia serrata, is a potent, specific, and reversible inhibitor of acetylcholinesterase with high efficiency and low toxicity. To evaluate the presence of huperzine A in Vietnamese H serrata, a reliable capillary zone electrophoresis method was developed. The analytical conditions were established using 80 mM ammonium acetate buffer, pH 6.0, hydrodynamic injection at 50 mbar for 5 s, applied voltage of 20 kV, temperature at 25 °C, uncoated fused-silica capillary, 56 cm (50 cm effective length) × 70 µm inner diameter, and ultraviolet detection at 310 nm. The recovery rates ranged from 98.05% to 100.64%, with a relative standard deviation 〈 2%. Good linear regression was observed in the concentration range of 1 to 500 µg/mL, with a correlation coefficient of 0.9994. The limit of detection and limit of quantification were 0.33 and 1.0 µg/mL, respectively. These results demonstrate that this method is simple, selective, and suitable for performing quality control for huperzine A derived from Vietnamese H serrata.

    Abstract: The “Globalization 4.0” has challenged Vietnamese Higher education to meet the new requirements of generating qualified labor force: self-direct their learning and make their own decision, to adjust and develop their perspectives and their actions consequently. The current teaching and learning approaches mostly based on behaviorism are not enough to allow Higher education to achieve the new objective. Meanwhile, though constructivist learning has been implemented for years, its use was limited to activate student learning and equipped them with professional skills and competencies instead of focusing on developing their perspectives and worldview. To enhance the success of higher education in terms of achieving new educational objective, this paper will introduce transformative learning as a perspective that would be contribute to radical and comprehensive change in this educational level.
