    Abstract: The study was conducted from March until June 2021. The number of nurses involved were: 39 first nurses and 33 second nurses. The research data were analysed using SPSS program version 25. The implementation of IDC on 5895 injected medication affected right time (p=0.000) and right documentation (p=0.041) in preparation of medication. IDC has a partial or simultaneous effect with the characteristics of the first nurse on the right dose and right time in preparation of drugs (p = 0.000). IDC also affects the right dose, right time and right documentation in administration of injected medication (p 〈 0.05). The characteristics of the first nurse's education had a partial effect on right documentation in preparation and administration of drugs, and right time in administration of injected medication (p 〈 0.05). Nurses' work experience also had a partial effect on right documentation in administration of injected medication (p= 0.001). IDC implementation reduces ME 〈 5%.

    Abstract: 〈 p 〉 Efforts to ensure patient safety during drug administration are to prevent drug administration errors. The characteristics of the nurse and the five right principles are factors that can ensure patient safety in the hospital. The Ministry of Health stated that there should not be an error in drug administration (0%), which is a very difficult application in the field. One Hospital in West Java has set a rate of & lt;5% per month as the tolerable limit for drug administration errors. The reality in the field is that the average incidence of drug administration errors is still high compared to the set standards, namely 8.2% per month from January to June 2018. The purpose of this study was to analyze the relationship between nurses’ factors and the true incident of injection drug preparation by nurses at one Hospital in West Java. A total of 22 nurses were involved in this study, grouped into nurse 1 who prepared the medication and nurse 2 who applied the Independent Double Check /IDC. Each nurse recorded the results of the five principles of correct injection drug preparation in separate records independently. The results showed that the observed agreement value (Po) and the expected agreement value (Pe) regarding the principle of five steps were very good ( & gt; 0.960) but the kappa value among nurses 1 and 2 were in the moderate category ( & lt;0.50). In addition, this study revealed that the clinical competence of the nurse 1 was related to the correct injection drug (p = 0.005), their work experience was related to the correct route of drug injection (p = 0.017). It is showed that the drug injection was influenced by the characteristics of nurse 1, especially by the clinical competence (p = 0.046). This study recommends that it is needed of further in-depth analysis of the factors that influence the correctness of drug administration. 〈 /p 〉 〈 p 〉   〈 /p 〉 〈 p 〉 〈 strong 〉 BAHASA INDONESIA  〈 /strong 〉 Upaya dalam menjamin keselamatan pasien selama pemberian obat-obatan adalah dengan mencegah terjadinya kesalahan pemberian obat. Karakteristik perawat dan prinsip lima benar merupakan faktor yang bisa menjamin keselamatan pasien di rumah sakit. Kementerian Kesehatan menyatakan bahwa tidak boleh ada kejadian kesalahan pemberian obat (0%), dimana angka tersebut sangat sulit untuk diterapkan di lapangan. Satu rumah sakit di Jawa Barat telah menetapkan angka & lt; 5% per bulan sebagai batas kesalahan pemberian obat yang bisa ditoleransi. Kenyataan dilapangan bahwa rata-rata kejadian kesalahan pemberian obat masih tinggi dibanding standar yang ditetapkan yaitu 8,2% per bulan dari Januari sampai Juni 2018. Tujuan penelitian ini untuk menganalisis hubungan antara faktor perawat dan kejadian kebenaran penyiapan obat injeksi oleh perawat di rawat inap satu rumah sakit di Jawa Barat. Sebanyak 22 perawat terlibat dalam penelitian ini, dikelompokkan menjadi perawat satu yang menyiapkan obat dan perawat dua yang melakukan 〈 em 〉 Independent Double Check/IDC 〈 /em 〉 . Setiap perawat mencatat hasil prinsip lima benar penyiapan obat injeksi dalam catatan yang terpisah secara independen. Hasil penelitian menunjukkan nilai kesepakatan teramati dan nilai kesepakatan harapan prinsip lima benar sangat baik ( & gt;0,960) namun nilai kappa antara perawat satu dan dua dalam kategori sedang ( & lt;0,50). Selain itu, hasil penelitian ini juga menunjukkan bahwa kompetensi klinik perawat satu berhubungan dengan benar obat injeksi (p=0,005), pengalaman kerja perawat satu berhubungan dengan benar rute obat injeksi (p=0,017). Selain itu, hasil penelitian ini menunjukkan bahwa benar obat injeksi dipengaruhi oleh karakteristik perawat satu terutama oleh kompetensi kliniknya (p=0,046).     Abstract: AbstractTrombocytopenia is related with severity of malaria. Parasitaemia and immune system have important roles for the reactions that occur in malaria infections including thrombocytopenia. Therefore, we conducted a study to determine the association between thrombocytopenia, parasitaemia and cytokines in Plasmodium falciparum and P.vivax. This was a cross-sectional and hospital based study at Mitra Masyarakat Hospital,Timika, Papua, in 2010. Subjects were all age groups with uncomplicated malaria. On admission, characteristics subjects were recorded, parasitaemia was calculated by using microscope, platelets were measured by Hematology analyzer (Sysmex), and cytokines were measured by Multiplex Flow Cytometry Assay (Luminex).Thrombocytopenia was defined if platelet count 〈 150,000/mm . Cytokines were presented in ratio of TNFα/IL10, IFNγ/IL10, and IL12/IL10. The association thrombocytopenia, parasitaemia and cytokines were determined by logistic multivariate analysis. A total 76 subjects were recruited, 51.3% infected with P. falciparum and 48.7% infected with P. vivax infection. The mean age of subjects was 15 (range: 5 – 55) years and 82.9% with thrombocytopenia (platelet range:18958-341123/mm ). The geometric mean of parasitemia was 3393 (43–412503) /mm , while the median of ratios TNFα/IL10, IFN γ/IL10 and IL12/IL10 were 1.05; 0.99; and 0.99, respectively. Bivariate anaylsis showed that trombocytopenia was associated with low level of TNFα/IL10 (p=0.015) and IL12/IL10 (0.020). The multivariate anaysis data also showed a relationship between thrombocytopenia and ratio of TNFα/IL10 with adjusted OR of 7,33 (95%CI: 1,5–35,8), p=0.014. Thrombocytopenia is associated with low ratio of TNFα/IL10 in patients with uncomplicated infection of P. falciparum and P.vivax. AbstrakTrombositopenia berhubungan dengan tingkat keparahan malaria. Parasitemia dan sistem kekebalan tubuh memiliki peran penting pada manifestasi infeksi malaria termasuk trombositopenia. Oleh karenanya penelitian ini bertujuan untuk menentukan hubungan antara trombositopenia, parasitemia, dan sitokin pada Plasmodium falciparum dan P.vivax. Disain studi adalah potong lintang, dilaksanakan di Rumah Sakit Mitra Masyarakat, Timika, Papua pada tahun 2010. Subjek penelitian adalah semua kelompok umur dengan malaria tanpa komplikasi. Pada perekrutan, karakteristik subjek akan dicatat, parasitemia dihitung dengan mikroskop, trombosit diukur dengan Hematology analyzer (Sysmex) dan sitokin diukur dengan Multiplex Flow Cytometry Assay (Luminex). Trombositopenia didefinisikan jika jumlah trombosit 〈 150.000 mm . Sitokin yang dipaparkan adalah rasio TNFα/IL10, IFNγ/IL10, dan IL12/IL10. Hubungan trombositopenia, parasitemia, dan sitokin ditentukan dengan analisis multivariat logistik. Sebanyak 76 subjek malaria mono-infeksi direkrut, 51,3% terinfeksi P. falciparum dan 48,7% P. vivax. Rata-rata umur subjek adalah 15 (kisaran : 5-55) tahun dan 82,9% dengan trombositopenia (kisaran : 18.958-341.123/ mm 3 3 ). Rerata geometrik parasitemia adalah 3393 (43-412503)/mm , sedangkan median masing–masing rasio TNFα/IL10, IFNγ/IL10 dan IL12/IL10 yaitu 1,05, 0,99; dan 0,99. Analsis bivariat menunjukkan bahwa trombositopenia berhubungan dengan rendahnya tingkat TNFα/IL10 (p = 0,015) dan tingginya rasio IL12/ IL10 (p=0,020). Hasil analisis multivariat menunjukkan adanya hubungan antara trombositopenia dan rasio TNFα/IL10 rendah ( 〈 1,05) dengan OR: 7,33 (95% CI: 1,5-35,8), p = 0,014. Trombositopenia dikaitkan dengan rendahnya rasio TNFα/ IL10 pada pasien infeksi dari P. falciparum dan P. vivax tanpa komplikasi.

    Abstract: Severe malaria is associated with decreased nitric oxide (NO) production and low plasma concentrations of l -arginine, the substrate for NO synthase. Supplementation with l -arginine has the potential to improve NO bioavailability and outcomes. We developed a pharmacokinetic model for l -arginine in moderately severe malaria to explore the concentration-time profile and identify important covariates. In doses of 3, 6, or 12 g, l -arginine was infused over 30 min to 30 adults with moderately severe malaria, and plasma concentrations were measured at 8 to 11 time points. Patients who had not received l -arginine were also assessed and included in the model. The data were analyzed using a population approach with NONMEM software. A two-compartment linear model with first-order elimination best described the data, with a clearance of 44 liters/h (coefficient of variation [CV] = 52%) and a volume of distribution of 24 liters (CV = 19%). The natural time course of l -arginine recovery was described empirically by a second-order polynomial with a time to half recovery of 26 h. The half-life of exogenous l -arginine was reduced in patients with malaria compared with that for healthy adults. Weight and ethnicity were significant covariates for clearance. MATLAB simulations of dosing schedules for use in future studies predicted that 12 g given over 6, 8, or 12 h will provide concentrations above the K m of endothelial cell CAT-1 transporters in 90%, 75%, and 60% of patients, respectively.

    Abstract: Individuals living in regions of intense malaria transmission exhibit natural immunity that allows them to be without fever and other symptoms for most of the time despite frequent parasitization. Although this tolerance of parasitemia appears to be more effective in children than in adults (as evidenced by lower parasitemia fever thresholds with age), adults do exhibit a degree of tolerance but the mechanism(s) underlying this are unclear. Asymptomatic malaria-exposed children have higher levels of nitric oxide (NO) than children with severe disease, and NO has been proposed as a mediator of malarial tolerance. However, the ability of highly malaria-exposed asymptomatic adults to generate high-level basal NO is unknown, as is the relationship between NO and malaria tolerance in adults. The relationship between NO and malaria parasitemia was therefore determined in asymptomatic adults from Papua, Indonesia. Adults with Plasmodium falciparum parasitemia had markedly increased basal systemic NO production relative to aparasitemic Papuan controls, who in turn produced more NO than healthy controls from a region without malaria. Immunoglobulin E levels were universally elevated in malaria-exposed Papuan subjects, suggesting that the prevalence of intestinal parasitosis may be high and that nonmalarial infection may also contribute to high basal NO production. Basal peripheral blood mononuclear cell (PBMC) NO synthase activity was elevated in Papuans but poorly correlated with systemic NO production, suggesting that NO production in this setting arises not only from PBMCs but also from other tissue and cellular sources. NO production was associated with and may contribute to malaria tolerance in Papuan adults.
