    Abstract: Wstęp: Celem pracy było zbadanie osłony rogówkowo-twardówkowej i systemu tkanki łącznej (CT) u dzieci z nabytą oraz wrodzoną krótkowzrocznością postępującą, a także zidentyfikowanie informacyjnych kryteriów diagnostycznych wspomagających funkcję twardówki.Materiał i metody: Przebadano 155 dzieci w wieku 9–17 lat. 18 miało emetropię lub hiperopię, 20 miało niską krótkowzroczność, 32 średnią i 85 wysoką krótkowzroczność. 32 dzieci miało krótkowzroczność powikłaną (CM) z powodu obwodowego zwyrodnienia siatkówki. Mierzono histerezę rogówki (CH), gęstość akustyczną twardówki (SAD), topografię kręgosłupa promieniami X, plantografię, łączną hipermobilność, poziom kortyzolu w surowicy krwi (SC) i autonomiczny balans.Wyniki: Histereza rogówki, mmHg i SAD (jednostki względne; średnia ±SE) były niższe w wysokiej krótkowzroczności; w niskiej krótkowzroczności CH wynosiła 13,0 ±0,3, w średniej krótkowzroczności 11,9 ±0,3#, w wysokiej krótkowzroczności 10,7 ±0,3#; SAD wynosił odpowiednio 215,9 ±5,2, 204,9 ±3,7# i 192,8 ±5,8# (#: p 〈 0,05 dotyczy niskiej krótkowzroczności). Najniższe CH (10,3 ±0,4) i SAD (186,5 ±7,3) znaleziono w nabytej CM. Wrodzona krótkowzroczność z PRD wykazywała CH i SAD większe niż nabyta CM (p 〈 0,05). Poziom kortyzolu w surowicy krwi (nmol/L) w hiperopii wynosił 335,8 ±40,9 i spadał z wysokiej krótkowzroczności: w niskiej krótkowzroczności – 290,7 ±58,6, w średniej – 250,9 ±26,4, w wysokiej – 243,9 ±20,5. Najniższe SC znaleziono w nabytej CM, złożonej z CH i SAD. Postęp dysplazji CT z wysoką krótkowzrocznością stwierdzono: u 76,9% dzieci z niską, 82,4% ze średnią, 89,2% z wysoką krótkowzrocznością nabytą i 91,7% z krótkowzrocznością wrodzoną. Biochemiczne defekty CT i hormonalne zaburzenie balansu były związane z zaburzeniami balansu w wegetatywnym układzie nerwowym: w wysokiej krótkowzroczności jedynie 20,5% dzieci było eutonicznych, 61,5% – sympatykotonicznych i 17,5% parasympatykotonicznych.Wnioski: Uczniowie z postępującą krótkowzrocznością wykazują biochemiczne zaburzenia rogówkowo-twardówkowe wraz z dysplazją CT, spadkiem SC i zaburzeniem balansu wegetatywnego układu nerwowego, bardziej wyrażone w nabytej krótkowzroczności powikłanej. Nasze obserwacje przyczyniają się do zrozumienia patogenezy krótkowzroczności i diagnostyczno-prognostycznej oceny krótkowzroczności dzieci, a zwłaszcza wówczas, gdy potrzebne jest wzmocnienie twardówki.     Abstract: To investigate eye care practitioners' attitudes and perceptions toward potential interventions that can enhance contact lens (CL) practice across the world, and how this is influenced by their practice setting. Methods: A self-administered, anonymized survey was constructed in English and then forward and backward translated into six more languages. The survey was distributed online via social media platforms and mailing lists involving reputed international professional bodies. Results: In total, 2,222 responses from 27 countries with sufficient responses were analyzed (53% females, median age- 37 years). Most of the respondents were optometrists (81.9%) and 47.6% were from stand-alone/independent practices. Median working experience in CL prescribing was 11.0 years (IQR: 18.0, 4–22 years). Over two-third of them declared themselves to be very hopeful (22.9%) or hopeful (45.1%) about the future of their CL practice. Among the potential interventions proposed, continuous update of knowledge and skills and competently managing CL-related complications were rated the most important (median score: 9/10 for each). Practitioners working in national/regional retail chains expressed higher proactivity in recommending CLs (9/10) than those in local chains, hospitals, and universities (for all 8/10, P 〈 0.05). National differences were also identified in eye care practitioner attitudes and perceptions ( P 〈 0.05). Conclusions: The study provided important information to delineate a variety of elements characterizing CL practice across the world. These insights can serve as a basis to design strategies at national and international levels.

    Abstract: AIM: To develop a method for evaluating peripheral spatial contrast sensitivity and to use it in comparative studies of peripheral spatial contrast sensitivity in children with myopia under correction conditions with glasses with a highly aspherical lenslet (HAL) and single vision lenses (SVL). MATERIAL AND METHODS: A method for evaluating peripheral spatial contrast sensitivity was developed, in which visual control was carried out with the patients gaze fixed in the forward direction using a remote binocular autorefractometer, and the test image was sent to the selected area of the retina periphery. The SVL values were evaluated in three ranges of spatial frequencies, namely, low (0.52.0 cycle/deg), medium (4.0 to 8.0 cycle/deg), and high (11.3116.0 cycle/deg). Peripheral spatial contrast sensitivity was examined descriptively in 20 patients with low and moderate myopia. The age of the patients ranged from 8 to 13 (average, 11.41.5) years, and the degree of myopia ranged from 1.0 to 3.75 (average, 2.661.5 D). The patients were examined in glasses with HAL and in a trial frame with SVL correction. RESULTS. In HAL, which induces volumetric myopic defocus on the periphery of the retina, the peripheral spatial contrast sensitivity was lower than that in the SVL. This decrease was most pronounced at medium and high spatial frequencies, where it reached 3050%. The revealed differences in the peripheral spatial contrast sensitivity at high frequencies (8; 11.31 and 16 cycles/deg) were statistically significant: 7.5 and 10.8 at the frequency of 8 cycles/deg, 5.8 and 9.05 at the frequency of 11.31 cycles/deg, and 4.4 and 9.2 at the frequency of 16 cycles/deg, respectively. CONCLUSION: The peripheral spatial contrast sensitivity in glasses with rings of HAL, compared with the peripheral spatial contrast sensitivity in SVL, is significantly reduced at high spatial frequencies.

    Abstract: Aim: To conduct a correlation analysis of the parameters of visual fixation, light sensitivity of the retina in the central region, and the thickness of the central region of the retina and choroid with the best corrected visual acuity (BCVA), refraction (SER), and axial length of the eye (AL) in nystagmus and amblyopia of various origins. Material and methods: The study included 45 patients aged 544 years (mean 11.26.14 years). All patients were divided into six groups depending on the type of amblyopia and associated conditions (with nystagmus). The retinal photosensitivity and fixation parameters were studied using an MP-3 Nidek microperimeter (Japan). Chorioretinal parameters were studied using an RS-3000 Advance 2 spectral optical coherence tomograph (Nidek, Japan). The axial length of the eye was measured using a Galilei G6 Scheimpflug analyzer. Correlation analysis was performed using the linear Pearson correlation coefficient (r). Results: In nystagmus, refractive and dysbinocular amblyopia, a strong direct correlation was found between the indicators of BCVA and the characteristics of fixation. In the groups associated with congenital myopia, relationships were found between BCVA, retinal thickness, and light sensitivity of the retina in the central region. There was no relationship between the subfoveal choroidal thickness and functional parameters of the eyes. Conclusions: In nystagmus and amblyopia associated with strabismus and hyperopic refraction, the primary amblyogenic factor is a decrease in the density and amplitude of fixation. In relative amblyopia due to congenital myopia, the decrease in visual acuity correlates with the photosensitivity of the retina and does not correlate with fixation parameters, which indicates the partially organic nature of relative amblyopia. No relationship was found between the choroidae thikness and the degree of amblyopia, which does not cofine its participation in the pathogenesis of the disease.

    Abstract: AIM: To evaluate the dynamics of axial and peripheral refraction in the near periphery of the retina in eyes with myopia against the background of wearing bifocal soft contact lens (BSCL) with the addition of 4 D. MATERIAL AND METHODS: Forty-three patients (84 eyes) with myopia from 0.5 to 6.5 D (average 3.530.19 D) aged 715 (average 11.30.27) years were examined before and 6 months after the start of wearing Prima BIO Bifocal BSCL (Okey Vision Retail, Russia). Visual acuity without correction, with optimal correction, and in BSCL, cycloplegic refraction, eye length, keratotopography, and peripheral defocus (PD) at 5, 10, and 15 to the nose and temple from the center of the fovea with and without lenses were assessed. RESULTS: After 6 months of wearing BSCL, the subjective refraction increased by 0.04 D, cycloplegic refraction by 0.18 D, and average BSCL strength by 0.01 D. The axial length increased by 0.03 mm (p 0.05). The initial PD without lenses was hypermetropic in all zones, and in BSCL, it was myopic in zones T5, T10, T15, and N15 and hypermetropic in zones N5 and N10. After 6 months of PD without correction, hypermetropic defocus tended to decrease in zones N10 and N15 and myopic defocus appeared in N5 (p 0.05); with lenses, myopic defocus increased in zones T5, T10, T15, and N15, and in N5 and N10, it did not change and remained hypermetropic. CONCLUSION: BSCLs provide total correction of myopia and high distant and near visual acuity in children, induce myopic defocus in the near periphery, and contribute to the inhibition of the progression of myopia in the traced period.

    Abstract: Majority of reports regarding methods of myopia control are devoted to the use of orthokeratology lens¬es (OKL) or atropine. The purpose of the study was to estimate the efficacy and safety of using OKL in com¬bination with instillation of 0.01% atropine drops for myopia control in children. Material and methods. Prospective cohort study in¬cluded 34 patients (68 eyes) aged 8 to 14 years old with acquired myopia. Groups with low (17 patients, 34 eyes), moderate (12 patients, 24 eyes) and high (5 patients, 10 eyes) myopia were identified and examined prior to and 6, 12 and 18 months after adding 0.01% atropine instillations to OKL wearing. To assess the degree of pro¬gression of myopia in dynamics the following parameters were evaluated: refraction (by Huvitz MRK 3100P, ax ial length (AL) by IOL-Master, “Carl Zeiss”, (Germany), ampli¬tude of accommodation (AA) by Grand Seiko WRK-5100K, positive-relative accommodation (PRA), pseudoaccom¬modation (PA) and annual gradient of progression (AGP). Results. The most noticeable effect was observed in patients with low myopia. In patients with low myopia, the rate of annual progression gradient decreased by 3.4 times by 18th month of atropine use; therefore a condition close to stabilization of myopia progression occurred. The data is statistically significant. In patients with moderate myopia, despite the de¬crease of the AGP within 6-month period of atropine use by 3.7 times, the increase of APG was observed by 12th month of atropine use, which continued until 18th month of observation. In spite of this, in comparison with baseline indices, the decrease in the rate of myo¬pia progression was obtained. The data is statistically significant. In patients with high myopia, change in the AGP was not ob¬served within the 6-month period of atropine use; afterwards, a gradual decrease in the rate of myopia progression was noted: AGP decreased by 1.2 times by 12th month. By 18th month of atropine use, AGP decreased by 1.5 times compared to its initial level. The data is statistically significant. Evidently, the inhibitory effect of OKL, which is de¬termined by optical factors such as peripheral myopic defocus, even in combination with atropine, is not suf¬ficient to halt the progression of high myopia, because of structural and biomechanical changes of the sclera. Conclusion. Based on the preliminary results ob¬tained, 100% efficacy of a long-term instillation of low-concentration atropine cannot be claimed, howev¬er, the positive effect does exist and therefore the study continues.
