    Abstract: Finerenone reduced the risk of kidney and cardiovascular events in people with chronic kidney disease (CKD) and type 2 diabetes in the FIDELIO-DKD and FIGARO-DKD phase 3 studies. Effects of finerenone on outcomes in patients taking sodium–glucose cotransporter 2 inhibitors (SGLT2is) were evaluated in a prespecified pooled analysis of these studies. RESEARCH DESIGN AND METHODS Patients with type 2 diabetes and urine albumin-to-creatinine ratio (UACR) ≥30 to ≤5,000 mg/g and estimated glomerular filtration rate (eGFR) ≥25 mL/min/1.73 m2 were randomly assigned to finerenone or placebo; SGLT2is were permitted at any time. Outcomes included cardiovascular composite (cardiovascular death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure) and kidney composite (kidney failure, sustained ≥57% eGFR decline, or renal death) end points, changes in UACR and eGFR, and safety outcomes. RESULTS Among 13,026 patients, 877 (6.7%) received an SGLT2i at baseline and 1,113 (8.5%) initiated one during the trial. For the cardiovascular composite, the hazard ratios (HRs) were 0.87 (95% CI 0.79–0.96) without SGLT2i and 0.67 (95% CI 0.42–1.07) with SGLT2i. For the kidney composite, the HRs were 0.80 (95% CI 0.69–0.92) without SGLT2i and 0.42 (95% CI 0.16–1.08) with SGLT2i. Baseline SGLT2i use did not affect risk reduction for the cardiovascular or kidney composites with finerenone (Pinteraction = 0.46 and 0.29, respectively); neither did SGLT2i use concomitant with study treatment. CONCLUSIONS Benefits of finerenone compared with placebo on cardiorenal outcomes in patients with CKD and type 2 diabetes were observed irrespective of SGLT2i use.

    Abstract: China has experienced one of the fastest increases in the incidence of acute lymphoid leukemia (ALL). The aim of this study was to assess the long-term trends of the incidence and mortality of ALL in mainland China between 1990 and 2019 and to project these trends through 2028. Methods Data on ALL were extracted from the Global Burden of Disease Study 2019; population data were extracted from World Population Prospects 2019. An age–period–cohort framework was used in the analysis. Results The net drift for the incidence of ALL was 7.5% (95% confidence interval [CI]: 7.1%, 7.8%) per year in women and 7.1% (95% CI: 6.7%, 7.6%) in men, and local drift was found to be higher than 0 in every studied age group (p & lt;0.05). The net drift for mortality was 1.2% (95% CI: 1.0%, 1.5%) in women and 2.0% (95% CI: 1.7%, 2.3%) in men. Local drift was lower than 0 in boys aged 0–4 years and girls aged 0–9 years and higher than 0 in men aged 10–84 years and women aged 15–84 years. The estimated period relative risks (RRs) for both incidence and mortality showed increasing trends in the recent period. The cohort RRs for incidence showed increasing trends in both sexes; however, the cohort RR for mortality was decreased in the recent birth cohort (women born after 1988–1992 and men born after 2003–2007). Compared with that in 2019, the incidence of ALL in 2028 is projected to increase by 64.1% in men and 75.0% in women, and the mortality is predicted to decrease by 11.1% in men and 14.3% in women. The proportion of older adult/adults individuals with incident ALL and ALL-related death was projected to increase. Conclusions Over the last three decades, the incidence and mortality rates of ALL have generally increased. It is projected that the incidence rate of ALL in mainland China will continue to increase in the future, but the associated mortality rate will decline. The proportion of older adult/adults individuals with incident ALL and ALL-related death was projected to increase gradually among both sexes. More efforts are needed, especially for older adult/adults individuals.

    Abstract: Schizophrenia is an important public health problem in China. This study aims to assess the long-term trends in the incidence and disability-adjusted life years (DALYs) rate of schizophrenia in China between 1990 and 2019. Methods The incidence and DALYs data were drawn from the Global Burden of Disease Study 2019, and an age–period–cohort model was used in the analysis. Results The age-standardized incidence rate (ASIR) and age-standardized DALYs rate (ASDR) of schizophrenia increased by 0.3 and 3.7% for both sexes between 1990 and 2019. For males, the local drift for incidence was higher than 0 ( P & lt; 0.05) in those aged 10 to 29 years (local drifts, 0.01 to 0.26%) and lower than 0 ( P & lt; 0.05) in those aged 35 to 74 years (local drifts, −1.01 to −0.06%). For females, the local drift was higher than 0 ( P & lt; 0.05) in those aged 10 to 34 years (local drifts, 0.05 to 0.26%) and lower than 0 ( P & lt; 0.05) in those aged 40 to 74 years (local drifts, −0.86 to −0.11%). The local drift for DALYs rate was higher than 0 ( P & lt; 0.05) in the age group from 10 to 69 years (local drifts, 0.06 to 0.26% for males and 0.06 to 0.28% for females). The estimated period and cohort relative risks (RR) for DALYs rate of schizophrenia were found in monotonic upward patterns, and the cohort RR for the incidence increased as the birth cohort moved forward starting with those born in 1972. Conclusion Although the crude incidence of schizophrenia has decreased in China, the ASIR, ASDR, and crude DALYs rate all showed a general increasing trend over the last three decades. The DALYs rate continue to increase as the birth cohort moved forward, and the increasing trend of incidence was also found in individuals born after 1972. More efforts are needed to promote mental health in China.

    Abstract: Lithium salts, known to ameliorate the effects of alkali-silica reaction, can make significant effects on cement setting. However, the mechanism of effects on cement hydration, especially the hydration of C 3 A which is critical for initial setting time of cement, is rarely reported. In this study, the development of pH value of pore solution, conductivity, thermodynamics, and mineralogical composition during hydration of C 3 A with or without Li 2 CO 3 are investigated. The results demonstrate that Li 2 CO 3 promotes C 3 A hydration through high alkalinity, due to higher activity of lithium ion than that of calcium ion in the solution and carbonation of C 3 A hydration products resulted from Li 2 CO 3 . Li 2 CO 3 favors the C 3 A hydration in C 3 A-CaSO 4 ·2H 2 O-Ca(OH) 2 -H 2 O hydration system and affects the mineralogical variation of the ettringite phase(s).

    Abstract: Nasopharynx cancer (NPC) is a great health burden in China. This study explored the long-term trends of NPC incidence and mortality in China. Methods We retrospectively analyzed data from the Global Burden of Disease Study 2019 using an age-period-cohort framework. Results The age-standardized incidence rate (ASIR) of NPC increased by 72.7% and age-standardized mortality rate (ASMR) of NPC decreased by 51.7% for both sexes between 1990 and 2019. For males, the local drift for incidence was higher than 0 ( P   〈  0.05) in those aged 20 to 79 years. For females, the local drift was higher than 0 ( P   〈  0.05) in those aged 30 to 59 years, and lower than 0 ( P   〈  0.05) in those aged 65 to 84 years. The local drift for mortality rates were less than 0 ( P   〈  0.05) in every age group for both sexes. The estimated period relative risks (RRs) for incidence of NPC were increased monotonically for males, and increased for females after 2000. The increasing trend of cohort RRs of incidence was ceased in recent birth cohorts. Both period and cohort effects of NPC mortality in China decreased monotonically. Conclusions Over the last three decades, the ASMR and crude mortality rate (CMR) of NPC has decreased, but the ASIR and crude incidence rate (CIR) increased in China. Although the potential mortality risk of NPC decreased, the risk of NPC incidence was found to increase as the period move forward, and suggested that control and prevention efforts should be enhanced.

    Abstract: This study explores the effects of metakaolin (MK) and silica fume (SF) on rheological behaviors and microstructure of self-compacting concrete (SCC). The rheology, slump flow, V-funnel, segregation degree (SA), and compressive strength of SCC are investigated. Microstructure characteristics, including hydration product and pore structure, are also studied. The results show that adding MK and SF instead of 4%, 6% and 8% fly ash (FA) reduces flowability of SCC; this is due to the fact that the specific surface area of MK and SF is larger than FA, and the total water demand increases as a result. However, the flowability increases when replacement ratio is 2%, as the small MK and SF particles will fill in the interstitial space of mixture and more free water is released. The fluidity, slump flow, and SA decrease linearly with the increase of yield stress. The total amount of SF and MK should be no more than 6% to meet the requirement of self-compacting. Adding MK or SF to SCC results in more hydration products, less Ca(OH)2 and refinement of pore structure, leading to obvious strength and durability improvements. When the total dosage of MK and SF admixture is 6%, these beneficial effects on workability, mechanical performance, and microstructure are more significant when SF and MK are applied together.

    Abstract: :Polyvinyl alcohol (PVA) fiber was proposed to enhance the mechanical performance of engineered cementitious composite in this research. A mixture of engineered cementitious composite with better expected performance was made by adding 2% PVA fiber. Mechanics tests, including pressure resistance, fracture resistance, and ultimate tensile strength, were conducted. They reveal that the engineered cementitious composites not only exhibit good pressure resistance, but they also exhibit excellent fracture resistance and strain capability against tensile stress through mechanics tests, including pressure resistance, fracture resistance, and ultimate tensile resistance. To further improve the engineered composites’ ductility, attempts to modify the performance of the PVA fiber surface have been made by using a vinyl acetate (VAE) emulsion, a butadiene–styrene emulsion, and boric anhydride. Results indicated that the VAE emulsion achieved the best performance improvement. Its use in fiber pre-processing enables the formation of a layer of film with weak acidity, which restrains the hydration of adjacent gel materials, and reduces the strength of transitional areas of the fiber/composite interface, which restricts fiber slippage and pulls out as a result of its growth in age, and reduces hydration levels. Research illustrates that the performance-improvement processing that is studied not only improves the strain of the engineered cementitious composites, but can also reduce the attenuation of the strain against tensile stress.
