    Abstract: The aim of this study was to produce an eco-innovative gluten-free bread with a pleasant taste and a unique formulation that includes the highest quality grains and pseudocereals (buckwheat; rice; and millet); and okara; a by-product of soy milk production. The mixture of pseudocereal and cereal flour contained buckwheat flour 45%, rice flour 33%, and millet flour 22%. Three gluten-free breads; each containing different contents of gluten-free flour (90%, 80%, and 70%, respectively); okara (10%, 20%, and 30%, respectively); and a control sample (without okara); were prepared and subjected to sensory evaluation. The okara-enriched gluten-free bread with the highest sensory score was selected for further analysis of physico-chemical (total proteins; total carbohydrates; insoluble fiber; soluble fiber; sugars; total lipids; saturated fatty acids; and salt) and functional properties (total phenolic content and antioxidant properties). The highest sensory scores were obtained for 30% okara-enriched gluten-free bread including taste; shape; odor; chewiness; and cross-section properties; classifying this bread in the category of very good quality and excellent quality (mean score 4.30 by trained evaluators and 4.59 by consumers). This bread was characterized by a high content of dietary fiber (14%), the absence of sugar; low content of saturated fatty acids (0.8%), rich source of proteins (8.8%) and certain minerals (e.g.,; iron; zinc); and low energy value (136.37 kcal/100g DW). Total phenolic content was 133.75 mgGAE/100g FW; whereas ferric reducing power; ABTS radical cation; and DPPH radical scavenging activity were 119.25 mgAA/100g FW; 86.80 mgTrolox/100g FW; and 49.92 mgTrolox/100g FW; respectively. Okara addition in gluten-free bread production enables the formulation of high-nutritive; good antioxidative; low-energy bread; and better soy milk waste management.

    Abstract: Background/Aim. Achieving good metabolic control, which plays a key role in reducing or preventing macrovascular and microvascular complications of diabetes mellitus (DM), requires continuous patient involvement in the self-management of DM. This continued engagement, which makes type 2 DM (T2DM) one of the most physically and emotionally demanding diseases, can become, at certain periods of life, extremely severe and lead to emotional distress (symptoms of depression and DM-related distress) and d-terioration of metabolic control. The aim of this study was to examine the association and influence of behavioral and psychological factors on the metabolic control of patients with T2DM. Methods. The research was conducted as a descriptive-analytic cross-sectional study. The method of random sampling included 324 subjects with T2DM in the research. The values of biochemical parameters of metabolic control were measured by standard laboratory methods. Blood pressure was measured two times, and the arithmetic mean was calculated. Anthropometric measurement was performed, and body mass index (BMI) was calculated. Attitudes toward medication adherence, adherence to dietary recommendations, level of physical activity, presence of depressive symptoms, and level of DM-related distress were examined using standardized questionnaires. Results. The target values of metabolic control parameters were reached by 21.6% of respondents. Multivariate analysis as predictors of poor metabolic control identified obesity, non-adherence toward dietary recommendations, insulin therapy, low level of physical activity, and clinically significant DM-related distress. Conclusion. Routine application of the questionnaire used in this study in the initial stages or critical moments of the disease can assess patients? attitudes and knowledge about behavioral determinants of DM self-management and timely detect psychological conditions that affect them. It would be realistic to expect that such a comprehensive holistic approach would contribute to a lower incidence of complications and better metabolic control of T2DM.

    Abstract: Background/Aim. The role of the complex sepsis-related immune response has not been fully clarified, and still remains a subject matter of investigations. Nowadays, sepsis is considered a dynamic syndrome characterised by many, often antagonistic, phenomena from hyperinflammation to anergy, or immunoparalysis. On the basis of pro- and anti-inflammatory mediators in the critically ill with sepsis, the aim of the study was to determine whether the cytokine profile differs according to the type of bacterial causative agent, as well as to assess the prognostic value regarding the outcome. The outcome measure has been hospital mortality. Methods. Blood serum samples have been taken from 125 critically ill patients with severe secondary sepsis as a consequence of peritonitis, pancreatitis, or trauma, who had been admitted to the surgical intensive care unit (SICU). The average age of the patients was 57.7?17.3. Of the total number of patients, 84 (67,2%) were males, and 41 (32,8%) were females. The levels of pro-inflammatory ? interleukin 1-alpha (IL-1?), IL-1?, IL-6, IL-8, IL-12?70, IL-17?, tumor necrosis factor alpha (TNF?), interferon-gamma (IFN-?), interferon-gamma-inducible protein-10 (IP 10), monocyte chemoattractant protein-1(MCP-1), macrophage inflammatory protein alpha and beta (MIP-1? and MIP-1?), as well as anti-inflammatory mediators IL-4, IL-10, IL-13, IL-27, IL-31 and IL-33 were determined at three time intervals - on the day of admission (the first day), and then on the third and fifth day. Through standard microbiological analyses, the type of the bacterial causative agent has been determined. Results. On the third day of measurement, significant differences in the cytokine levels with regard to the nature of bacteremia were determined in all pro- and anti-inflammatory cytokines, except for IL-8. Generally, the lowest levels were observed in patients with polymicrobial blood culture. On the first and fifth days of measurement, no significant differences in the cytokine levels with regard to the nature of bacteriemia was found. The only significant predictor of the lethal outcome on the first measurement day was IL-17?, AUR ROC of 0,665 (95% confidence interval of 0,519-0,791, ?=0.034) with secondary sepsis as a complication of peritonitis. Conclusion. According to the type of the bacterial causative agent, the lowest levels of cytokines have been observed in patients with the polymicrobial blood culture. IL- 17? is a good predictor of the outcome in patients with peritonitis as an underlying condition of secondary sepsis. The low level of IL-17? in these patients predicted a lethal outcome on the first day of measurement. On the other hand, the levels of other cytokines correlated with the outcome only on the fifth day of measurement, and they were higher in survivors than in non-survivors.

    Abstract: Given the widespread use of dietary supplements (DS) and herbal products (HP), healthcare professionals (HCPs) will increasingly encounter patients who use these preparations with conventional drugs and who need their services to reduce the consequences of adverse therapeutic outcomes. The aim of our survey was to assess the knowledge and behaviors of HCPs regarding the risk of potential drug–dietary supplement (DDSIs) and drug–herbal product (DHPIs) interactions. This cross-sectional survey collected data via on paper-based questionnaire among general practitioners (GPs) (n = 105), specialty doctors (n = 87) and nurses (n = 154). The HCPs were mostly familiar with the interaction of doxycycline with magnesium (83%) and were least familiar with interaction of warfarin with glucosamine (14%). The results on DDSIs and DHPIs knowledge showed that GPs scored significantly higher than nurses (p 〈 0.001 and p = 0.003, respectively), while specialty doctors scored significantly higher than nurses only on DDSIs knowledge (p 〈 0.001). Only 28% of respondents reported that they often or always ask patients on drug therapy about the use of DS or HP, and 25% of respondents record such data in the medical documentation of patients. Our results showed that HCPs have sufficient knowledge about most major DDSIs and DHPIs, but insufficient knowledge about most moderate interactions. However, their overall knowledge and behavior regarding the risk of these interactions indicate the need for further continuing education and training.

    Abstract: Uvod Kvalitet i trajnost veze gledji i kompozitnih materijala zavisi, izmedju ostalog, i od koriscenog adhezivnog restaurativnog sistema. Cilj rada Cilj istrazivanja je bio da se analizom pomocu skening elektronske mikroskopije (SEM) proceni kvalitet ivicnog pripoja kompozitnih adhezivnih sistema i gledji, odnosno prouce morfoloske osobine gledjne povrsine duz ivica kompozitnih restauracija nakon delovanja kariogenog rastvora. Metode rada Kao materijal u istrazivanju korisceno je 56 ekstrahovanih humanih trecih molara. Na svim zubima su preparirani kaviteti pete klase s ivicama u gledji. Kaviteti su restaurirani kompozitnim adhezivnim sistemima: Single Bond/Z250 i Single Bond/Filtek Flow, koriscenjem tehnike totalnog nagrizanja, i Prompt-L-Pop/Z250 i Prompt- L-Pop/Filtek Flow, primenom tehnike sa samonagrizajucim adhezivnim sistemima. Posle postupka restauracije zubi su izlagani demineralizaciji tokom sedam i 28 dana potapanjem u kariogeni rastvor (mlecna kiselina pH 4,5; 0,1 M) na 37?C ili drzani u dejonizovanoj vodi (kontrolna grupa). Ivice restauracija i perimarginalna gledj ispitivane su pomocu SEM. Ivicna pukotina oko restauracija merena je u predelu vrata zuba, okluzivno i na aproksimalnim stranama. Rezultati Primena tehnike totalnog nagrizanja je pokazala znacajno manju sirinu ivicne pukotine oko kompozitnih restauracija u odnosu na samonagrizajuci sistem (p〈0,01). SEM ispitivanja su takodje otkrila izrazitu demineralizaciju i eroziju ivicne zone gledji (ostecenja prizmi, poroziteti) nakon delovanja kariogenog rastvora kod oba adhezivna sistema. Na gledjnim povrsinama van perimarginalne zone uocena su neznatna ostecenja. Zakljucak Tretman kaviteta sa 35-procentnom fosfornom kiselinom kod totalno nagrizajuceg postupka znacajno poboljsava vezu kompozitnih smola za gledj. Rastvaranje ivicne gledji u kariogenom rastvoru uoceno je oko svih restauracija i kod oba adhezivna sistema.
