    Abstract: To develop and validate the prognostic prediction model DU-VASC to assist the clinicians in decision-making regarding the use of platelet inhibitors (PIs) for the management of digital ulcers in patients with systemic sclerosis. Secondly, to assess the incremental value of PIs as predictor. Methods We analysed patient data from the European Scleroderma Trials and Research group registry (one time point assessed). Three sets of derivation/validation cohorts were obtained from the original cohort. Using logistic regression, we developed a model for prediction of digital ulcers (DUs). C-Statistics and calibration plots were calculated to evaluate the prediction performance. Variable importance plots and the decrease in C-statistics were used to address the importance of the predictors. Results Of 3710 patients in the original cohort, 487 had DUs and 90 were exposed to PIs. For the DU-VASC model, which includes 27 predictors, we observed good calibration and discrimination in all cohorts (C-statistic = 81.1% [95% CI: 78.9%, 83.4%] for the derivation and 82.3% [95% CI: 779.3%, 85.3%] for the independent temporal validation cohort). Exposure to PIs was associated with absence of DUs and was the most important therapeutic predictor. Further important factors associated with absence of DUs were lower modified Rodnan skin score, anti-Scl-70 negativity and normal CRP. Conversely, the exposure to phosphodiesterase-5 inhibitor, prostacyclin analogues or endothelin receptor antagonists seemed to be associated with the occurrence of DUs. Nonetheless, previous DUs remains the most impactful predictor of DUs. Conclusion The DU-VASC model, with good calibration and discrimination ability, revealed that PI treatment was the most important therapy-related predictor associated with reduced DU occurrence.

    Abstract: The prevalence and characteristics of SSc-associated interstitial lung disease (SSc-ILD) vary between geographical regions worldwide. The objectives of this study were to explore the differences in terms of prevalence, phenotype, treatment and prognosis in patients with SSc-ILD from predetermined geographical regions in the EUSTAR database. Material and methods Patients were clustered into seven geographical regions. Clinical characteristics and survival of patients with SSc-ILD were compared among these pre-determined regions. Results For baseline analyses, 9260 SSc patients were included, with 6732 for survival analyses. The prevalence of SSc-ILD in the overall population was 50.2%, ranging from 44.0% in ‘Western Europe and Nordic countries’ to 67.5% in ‘Eastern European, Russia and Baltic countries’. In all regions, anti-topoisomerase antibodies were associated with SSc-ILD. Management also significantly differed; mycophenolate mofetil was prescribed at baseline in 31.6% of patients with SSc-ILD in ‘America (North and South)’ and 31.7% in ‘Middle East’ but only 4.3% in ‘Asia and Oceania’ (P  & lt;0.0001). Patients from ‘America (North and South)’ and ‘Middle East’ had the highest survival rate at the end of follow-up (85.8% and 85.2%, respectively). Conclusions Our study highlights key differences among regions in terms of clinical presentation and prognosis of SSc-ILD. This work also demonstrates that the management of SSc-ILD is highly variable among the different regions considered, suggesting that efforts are still needed for the standardization of medical practice in the treatment of this disease.

    Abstract: Persistent and adequate treatment of patients with arterial hypertension leads to more favorable disease outcome. Hypothesis Aside for the present left ventricular hypertrophy (LVH), there are other non‐invasive parameters which can represent additional predictors of unfavorable prognosis in patients with essential arterial hypertension during the 10‐year follow‐up. Methods A hypertensive group with LVH (124 patients; age 57.0 ± 8.0; 84 males and 40 females) was included in the study and examined noninvasively. Patients used regular medication therapy during the follow‐up period. Results During the 10‐year follow‐up period, unfavorable outcome was recorded for 40 (32.3%) patients. Patients with unfavorable outcome had higher baseline values of left ventricular mass index (178.9 ± 29.5 g/m 2 vs 165.5 ± 29.5 g/m 2 ; P 〈 0.05) and QTc dispersion (64.1 ± 24.7 ms vs 54.8 ± 19.4 ms; P 〈 0.05). Frequency of positive Cornell product was higher in the group of patients with unfavorable outcome (35% vs 22.2%; P 〈 0.01). Positive Lyon‐Sokolow score did not show statistical significance (25% vs 11.9%; P = 0.06). Cornell product (β = 0.234; P 〈 0.01) and QTc dispersion 〉 65 ms (β = 0.184; P 〈 0.05) had prognostic significance in LVH (multiple regression analysis: R = 0.314, R = 0.099, adjusted R = 0.084, standard error of the estimate = 0.449, P 〈 0.05). Conclusions Patients with a positive Cornell product and larger QTc dispersion had more unfavorable 10‐year outcomes compared with other patients with LVH.

    Abstract: Electrocardiography is the first‐choice technique for detecting left ventricular hypertrophy in patients with arterial hypertension. It is necessary to know the probable outcome for every patient during the treatment, with the aim of improving cardiovascular event prevention. Hypothesis Certain electrocardiographic criteria for left ventricular hypertrophy may predict outcomes of patients with left ventricular hypertrophy during a 15‐year follow‐up. Methods Fifteen‐year prospective study of 83 consecutive patients (53 male and 30 female; mean age 55.3 ± 8.1) with echocardiographic left ventricular hypertrophy (left ventricular mass index 170.3 ± 31.6 g/m 2 ). Electrocardiographic left ventricular hypertrophy was determined by means of Gubner‐Ungerleider voltage, Lewis voltage, voltage of R wave in aVL lead, Lyon‐Sokolow voltage, Cornell voltage and Cornell product, voltage RV 6 and RV 5 ratio, Romhilt‐Estes score, Framingham criterion and Perugia criterion. Results One or more composite events were registered in 32 (38.5%) patients during 15‐year follow‐up. Positive Lyon‐Sokolow score (17.6% vs. 47.3%; P   〈  0.05), Lewis voltage (9.8% vs. 21.9%; P   〈  0.05), Cornell voltage (15.7% vs. 37.5%; P   〈  0.05), and Cornell product (9.8% vs. 34.4%; P   〈  0.01) were more frequent in a group of patients with composite events. Odd ratio for Cornell product was 4.819 (95% CI 1.486‐15.627). Conclusion Patients with echocardiographic left ventricular hypertrophy who had positive Lewis voltage, Lyon‐Sokolow voltage, Cornell voltage, and Cornell product showed worse 15‐year outcome. The strongest predictor of cardiovascular events was positive result of Cornell product.

    Abstract: This work aimed to analyse the effect of non-Saccharomyces yeasts on the volatile composition, aromatic profile and sensory properties of wine made from autochthonous Serbian grape variety Prokupac (Vitis vinifera). Fermentation was performed with two commercial yeasts, Metschnikowia pulcherrima and Torulaspora delbrueckii, in pure and sequential fermentation with Saccharomyces cerevisiae. The standard analysis of produced wines indicated that the application of both non-Saccharomyces yeast (in pure or in sequential fermentation) significantly reduces the content of alcohol when compared to the control. The use of M. pulcherrima in both pure and sequential fermentation resulted in a higher content of different groups of polyphenols, anthocyanins and flavonoids or better colour characteristics compared to the wines obtained in fermentations with T. delbrueckii. HS-SPME GC/MS analysis revealed the presence of 27 different compounds in the wine samples, with higher alcohols and ethyl esters being the most dominant. It was proven that the presence of non-Saccharomyces yeasts in the initial stage of fermentation with later inoculation with S. cerevisiae contributes to increased complexity of the wine, as well as increased higher alcohol content and total extracts. Wines produced from sequential fermentation with T. delbrueckii showed floral and spice attributes, probably due to 1-pentanol and 2-phenylethanol detected in the wine, as well as to the large amount of ethyl acetate. The use of M. pulcherrima in pure fermentation resulted in wines with the lowest sensorial characteristics (i.e., lacking fruity and floral aroma) probably due to the lowest relative contribution of ethyl esters. The control wine in this study (from pure fermentation with S. cerevisie), had the most intense ‘red berries’ and ‘black berries’ note and low intensity of spices or vegetable aroma, which may be explained by the highest relative contribution of ethyl octanoate and ethyl decanoate. The PCA results also clearly differentiated the analysed samples produced in the sequential, pure and control fermentation trials, leading to the conclusion that the aromatic profile of Prokupac wines produced in sequential fermentation was more complex compared to the wines produced in pure fermentation.

    Abstract: Introduction: Sinusitis is an inflammation of the paranasal sinuses and it can be infectious, allergic or autoimmune. Acute rhinosinusitis commonly has viral origin and occurs as part of the upper respiratory tract infections. The most common pathogens are Streptococcus, Pneumococcus and Haemophilus influenzae. Some inflammations may start as viral, but develop into bacterial superinfection, i.e., subsequent colonization of the bacteria in 0.5-2% of cases. Viral sinusitis lasts for 7-10 days, whereas bacterial may take longer. The aim: The aim of this study was to compare three therapeutic protocols for the treatment of acute bacterial rhinosinusitis. Materials and Methods: A prospective clinical study was conducted at the Ear, Nose and Throat Clinic, Clinical Center Nis from October 2019 to January 2020, and it involved patients with acute bacterial rhinosinusitis in whom we compared the efficiency and safety of levofloxacin administration for five and 10 days with the efficiency of amoxicillin clavulanate treatment. The study included 62 patients with documented clinical and radiological symptoms. Results: Confirmation of bacterial etiology is not routinely performed in clinical practice since it requires antral puncture or endoscopic examination of the middle nasal meatus. Consequently, the choice of antibiotic therapy is empiric. In our study, we used levofloxacin for five (500 mg once a day) and 10 days (500 mg once a day) and amoxicillin-clavulanatefor 10 days (500 mg-125 mg three times a day), and the results showed no statistically significant difference in regard to the choice of antibiotics and the duration of therapy. Conclusion: The findings of this study suggest that a short course of antibiotic treatment has similar efficiency compared to a longer course of treatment of patients with uncomplicated acute bacterial sinusitis when treatment is indicated.

    Abstract: Aim of the study: This study aimed to formulate a novel, commercially applicable biofertilizer, to optimize the microencapsulation procedure of Bacillus subtilis NCIM 2063 and examine the stability and phytostimulatory effects of obtained formulation. Area of the study: Southestern Serbia. Material and methods: Microbial powder formulations were prepared using spray drying with maltodextrin as a carrier. The spray drying conditions were set according to Box-Benkhen experimental desing. The effect of the formulation was tested on green pepper (Capsicum annuum) seeds in controled conditions. Main results: Response surface models were developed. All of the models were statistically significant, adequately fitted and reproducible. The maximum achieved values of viability and yield in a formulation were 1.99·109 CFU/g and 96.8%, respectively, whilst the driest formulation had 1.44% moisture. The following optimum conditions were proposed for the spray drying procedure: an inlet air temperature of 133 °C, maltodextrin concentration of 50 g/L and a feed flow rate of 6.5 mL/min. The obtained microbial formulation had a high survival rate after being stored at room temperature over a 1--year period. Its application on green pepper seeds had beneficial effect on plant height, leaf dry weight and chlorophyll content of the seedlings. Research highlights: B. subtilis was successfully microencapsulated on maltodextrin as a carrier. Interaction effects between the process variables were fully explained and statistically significant models were developed. In addition to biocontrol properties formulation had a phytostimulatory effect, excellent stability and satisfactory physical properties.

    Abstract: Nowadays, the appropriate control of rheumatoid arthritis (RA) involves the absence of clinical disease activity, delaying joint destruction as long as possible and adequate quality of life of the affected. With currently available therapeutic modalities, this therapeutic goal can be achieved in a large number of patients. The aim of this research was to determine the effectiveness of an IL-6 blocker (Tocilizumab) in patients with RA in everyday clinical practice. We also analyzed whether a change in the mode of drug administration (switching from intravenous to subcutaneous drug formulation) had an impact on drug effectiveness (using the DAS 28 SE and CDAI indexes) and quality of life of patients with RA (HAQ, Beck Depression Inventory, FACIT F score and SF 36 questionnaire). The study included 53 subjects diagnosed with RA, treated with Tocilizumab. After a six-month use of subcutaneous Tocilizumab, we concluded that the method of drug administration did not have an impact on its effectiveness and on all the examined parameters of quality of life assessment.
