    Abstract: The protection of World Natural Heritage Sites (WNHSs) has become a global undertaking, wherein Outstanding Universal Value (OUV) is an important aspect of heritage protection. Focusing on the protection of the aesthetic and bioecological values of Bogda (one component of Xinjiang Tianshan WNHS), 17 indicators are selected to construct a Heritage Value Protection Significance Index (HVPSI) and a related Heritage Value Protection Index (HVI). From these, the level of OUV conservation is obtained according to the five aspects of integrity, vulnerability, sensitivity, degree of human impact and environmental factors. The spatial autocorrelation analysis method was applied to the OUV conservation indices to comprehensively reflect the spatial-pattern characteristics of the heritage value in the study area. According to the spatial-division of the HVPSI, the plant community composition and diversity characteristics of different HVPSI zones are analyzed by a field survey (July 2018 and 2019). To verify the rationality of space partition and identify the OUV elements in each HVPSI district, a spatial-econometric model is then used to explore the relationship among HVPSI, HVI and community diversity. This study provides a scientific basis for management of heritage sites and a theoretical basis for further investigation into heritage site indicators.

    Abstract: Roads are the link between geographic space and human socio-economic activities, promoting local economic development, and simultaneously causing various negative effects, such as segmentation, interference, destruction, degradation, and pollution. In China, the construction of roads is rapid, which might affect wildlife movement, landscape pattern, and land use change, thereby, affecting the conservation of heritage sites. In the present study, the minimum cumulative resistance model, along with geographic information system technology, was adopted to compute the ecological corridor for wildlife movement between the source patches and to analyze ecological corridor changes under two conditions (road presence/absence) at two time points in Kanas, nominated as a World Natural Heritage site. The relationships between the ecological corridor changes and various factors, including the cutting index of the ‘road-effect zones’, terrain, and road geometric characteristics, were examined using the geographical detector model to identify the influencing factors and mechanisms of the corridor changes, in order to rationally simulate the potential ecological corridors. In addition, the detached and fragmented ecological patches can be connected to effectively protect the biodiversity, biological habitats, and species, which are important means to achieve regional sustainable development and ecological construction.

    Abstract: Mature T-cell lymphomas (MTCLs), a group of diseases with high aggressiveness and vulnerable prognosis, lack for the accurate prognostic stratification systems at present. Novel prognostic markers and models are urgently demanded. Aberrant lipid metabolism is closely related to the tumor progression but its prognostic significance in MTCLs remains unexplored. This study aims to investigate the relationship between dysregulated lipid metabolism and survival prognosis of MTCLs and establish a novel and well-performed prognostic scoring system for MTCL patients. Methods A total of 173 treatment-naive patients were enrolled in this study. Univariate and multivariate Cox regression analysis were performed to assess the prognostic significance of serum lipid profiles and screen out independent prognostic factors, which constituted a novel prognostic model for MTCLs. The performance of the novel model was assessed in the training and validation cohort, respectively, by examining its calibration, discrimination and clinical utility. Results Among the 173 included patients, 115 patients (01/2006–12/2016) constituted the training cohort and 58 patients (01/2017–06/2020) formed the validation cohort. Univariate analysis revealed declined total cholesterol (TC, P = 0.000), high-density lipoprotein cholesterol (HDL-C, P = 0.000) and increased triglycerides (TG, P = 0.000) correlated to inferior survival outcomes. Multivariate analysis revealed extranodal involved sites ≥ 2 (hazard ratio [HR]: 2.439; P = 0.036), β2-MG ≥ 3 mg/L (HR: 4.165; P = 0.003) and TC  〈  3.58 mmol/L (HR: 3.338; P = 0.000) were independent predictors. Subsequently, a novel prognostic model, EnBC score, was constructed with these three factors. Harrell’s C-index of the model in the training and validation cohort was 0.840 (95% CI 0.810–0.870) and 0.882 (95% CI 0.822–0.942), respectively, with well-fitted calibration curves. The model divided patients into four risk groups with distinct OS [median OS: not available (NA) vs. NA vs. 14.0 vs. 4.0 months, P  〈  0.0001] and PFS (median PFS: 84.0 vs. 19.0 vs. 8.0 vs. 1.5 months, P  〈  0.0001). Time-dependent receiver operating characteristic curve and decision curve analysis  further revealed that EnBC score provided higher diagnostic capacity and clinical benefit, compared with International Prognostic Index (IPI). Conclusion Firstly, abnormal serum lipid metabolism was demonstrated significantly related to the survival of MTCL patients. Furthermore, a lipid-covered prognostic scoring system was established and performed well in stratifying patients with MTCLs.

    Abstract: Whether smoking modifies the associations of diabetes and risk factor management with subsequent risk of cardiovascular disease (CVD), and whether the smoking related CVD risk differs among people with and without diabetes are unclear. This study aimed to examine the associations and interactions of smoking, diabetes, and risk factor management in relation to incident CVD. Methods This nationwide, population-based, prospective cohort study of 20 communities from various geographic regions recruited adults aged 40 years or older during 2011–2012. The follow-up survey was conducted between 2014 and 2016. This study included 126,181 participants who were free from CVD at baseline. Results Study participants included 19,397 current smokers (15.4%), 6,049 former smokers (4.8%), and 100,735 never smokers (79.8%). Mean (SD) age ranged from 55.8 (8.6) years to 60.7 (9.1) years. Compared with never smokers, heavy smokers exhibited a greater risk of CVD events among participants with diabetes (multivariable-adjusted hazard ratio [HR], 1.45; 95% CI, 1.17–1.78) than among participants without diabetes (HR, 1.20; 95% CI, 1.01–1.42; P for interaction = 0.006). Compared with participants without diabetes, participants with diabetes who were never smokers and had 5 or more controlled risk factors showed no significantly excess CVD risk (HR, 0.93; 95% CI, 0.71–1.22), but the cardiovascular benefits from risk factor management were counteracted among participants with diabetes who were current smokers (HR, 1.28; 95% CI, 0.77–2.14) or former smokers (HR, 1.22; 95% CI, 0.66–2.28). Conclusions Smoking and diabetes interacted with each other in relation to increased risk of CVD events, and the beneficial effect of risk factor management on CVD risk among participants with diabetes was attenuated by current or former smoking.

    Abstract: Cardiology professional organizations recommend considering individual risk-enhancing clinical factors in addition to the atherosclerotic cardiovascular disease (ASCVD) score when making decisions about preventive treatment. Using data from a prospective multicenter cohort of 115,366 Chinese community residents aged ≥40 years without a history of cardiovascular disease (CVD), the authors found that addition of nontraditional CVD risk factors, such as eGFR and urinary albumin-creatinine ratio (ACR), further improved stratification of future CVD risks beyond that of the ASCVD risk score calculated by traditional CVD risk factors. Adding ACR and eGFR to the ASCVD risk score also significantly improved prediction and reclassification of CVD risks. Clinical applications of the Kidney Disease Improving Global Outcomes (KDIGO) risk categories based on eGFR and ACR levels for CVD prevention should be evaluated by interventional studies and in other ethnic populations. Background The Kidney Disease Improving Global Outcomes (KDIGO) clinical practice guideline used eGFR and urinary albumin-creatinine ratio (ACR) to categorize risks for CKD prognosis. The utility of KDIGO’s stratification of major CVD risks and predictive ability beyond traditional CVD risk prediction scores are unknown. Methods To evaluate CVD risks on the basis of ACR and eGFR (individually, together, and in combination using the KDIGO risk categories) and with the atherosclerotic cardiovascular disease (ASCVD) score, we studied 115,366 participants in the China Cardiometabolic Disease and Cancer Cohort study. Participants (aged ≥40 years and without a history of cardiovascular disease) were examined prospectively for major CVD events, including nonfatal myocardial infarction, nonfatal stroke, and cardiovascular death. Results During 415,111 person-years of follow-up, 2866 major CVD events occurred. Incidence rates and multivariable-adjusted hazard ratios of CVD events increased significantly across the KDIGO risk categories in ASCVD risk strata (all P values for log-rank test and most P values for trend in Cox regression analysis 〈 0.01). Increases in c statistic for CVD risk prediction were 0.01 (0.01 to 0.02) in the overall study population and 0.03 (0.01 to 0.04) in participants with diabetes, after adding eGFR and log(ACR) to a model including the ASCVD risk score. In addition, adding eGFR and log(ACR) to a model with the ASCVD score resulted in significantly improved reclassification of CVD risks (net reclassification improvements, 4.78%; 95% confidence interval, 3.03% to 6.41%). Conclusions Urinary ACR and eGFR (individually, together, and in combination using KDIGO risk categories) may be important nontraditional risk factors in stratifying and predicting major CVD events in the Chinese population.
