Abstract: Introduction:  Prevalence of active smokers in Malaysia remains high despite the availability of stop-smoking services. Rather than focussing on patients’ demotivating factors, possible poor performance from providers should be evaluated. Thus, knowledge, attitude and practice of the healthcare professional on smoking cessation intervention needs to be assessed, especially among primary care doctors.  Materials and Methods:  A cross-sectional study was conducted among primary care medical doctors attending the 19th Family Medicine Scientific Conference 2016 held at the Grand Dorsett, Subang on the 10th to the 13th August 2016. Validated questionnaire of Knowledge, Attitude and Practice (KAP) of Medical Doctors on Smoking Cessation Guidelines consisting 17 questions were used to determine the KAP score. The difference between the median KAP scores between age, gender, ethnicity, religion, profession, location of clinic, availability of specialist in clinic, numbers of smokers seen and presence of smoking cessation training were assessed using Mann-Whitney U Test and Kruskal Wallis H Test. Results:  A total of 145 primary care doctors involved in the study. Majority were female (84.1%), Malay (73.1%) and haven’t undergone any kind of smoking cessation training (47.6%). The KAP score were statistically associated with presence of specialist, type of profession, history of attending training program, ethnicity and religion (p 〈 0.05). Conclusion: Majority of the involved primary care doctors had low KAP score. Being a family medicine specialist, in charge of stop smoking clinic and having attended smoking cessation training program were significantly associated with high KAP score. Therefore, more smoking cessation training should be provided to the primary care doctors.

Abstract: Anaemia in pregnancy is one of the most common conditions encountered in antenatal follow-up at primary care. Some targets need to be achieved and managed by healthcare professionals, especially at the earliest possible time and optimal level of haemoglobin in the pregnancy period. Nevertheless, some patients may end up with severe anaemia that was not successfully treated even at term. Therefore, an audit was conducted among antenatal women in a local health clinic to assess the patient’s haemoglobin level at booking and term, the management plan and to formulate strategies for improvement. This audit was conducted for two weeks using a universal sampling method in November 2022. All antenatal women who were already at term gestational period and attended the clinic during this period were included. The patients’ sociodemographic and health characteristics, haemoglobin level at booking and term as well as management plan were recorded. The data were entered into the statistical software SPSS version 23 for analysis. A total of 115 antenatal women were recruited and 87% of them were able to achieve target haemoglobin at term. Twenty-three of them had concomitant iron deficiency anaemia since the pre-pregnant state. Associated factors for successful anaemia treatment were those without concomitant IDA, those without side effects on  iron supplements and those who did not require changes in the treatment course (P 〈 0.05). The majority of patients can achieve normal values of haemoglobin at term. Pre-pregnancy clinics to treat concomitant anaemia before pregnancy selective individualized iron supplementation to avoid side effects and maintaining the same medication groups would help treat morenumbers of antenatal women with anaemia.

Abstract: Breast cancer is among the commonest cancers that affect women worldwide. Nevertheless, many patients were diagnosed with breast cancer in the terminal stage due to their late presentation to healthcare centres despite the advanced development of screening modalities. Therefore, a systematic review was conducted to summarize the trend of knowledge, attitude, and practice level of breast cancer screening among the public globally. Review identification was performed in database search using PubMed, Scopus and CINAHL. Based on the criteria consisting of full papers in English published from January 2010 to September 2020, 27 articles were eligible for review. The systematic review revealed that the public had inadequate knowledge, negative attitudes, and poor practice on breast cancer screening. Educational programmes should be implemented among communities. Communication skills training must be instilled among healthcare workers to raise knowledge of breast cancer that can influence the attitude and practice of the public on breast cancer screening positively.

Abstract: Introduction: Prevalence of active smokers in Malaysia is remaining high (22.8%) despite the wide availability of quit-smoking services. Rather than look at the patients’ factors alone, the possible negative factors from health care providers should also be explored. Therefore, knowledge, attitude and practice of the healthcare professional on smoking cessation intervention needs to be assessed, especially among the primary care doctors. This can be done during national conference gathering all doctors from Malaysia. Materials and Method: A crosssectional study was conducted among medical doctors attending First East Coast Symposium on Specialized Care at Primary Care Level at Grand DarulMakmur, Kuantan on 1st to 2nd December 2019, using validated questionnaire of Knowledge, Attitude and Practice of Medical Doctors on Smoking Cessation Guidelines; consisting 17 questions to determine the KAP score. The difference between the median KAP scores between age, gender, ethnicity, religion, profession, location of clinic, availability of specialist at clinic, numbers of smokers seen and presence of smoking cessation training were assessed using Independent sample T-test and One-way ANOVA. Results: A total of 131 primary care doctors involved in the study. Majority were female (80.9%), Malay (96.2%), medical officers (74.8%), government doctors (90%) and haven’t undergone any kind of smoking cessation training (65.5%). The higher KAP score were statistically associated (p 〈 0.05) with history of attending training program (mean 35.7 score) and in charge of quit smoking clinic (mean 35.4 score). No significant difference of score between government (mean 32.0 score) and private doctors (mean 29.9 score). Conclusions : Majority of the involved primary care doctors had low KAP score. Being a family medicine specialist, in charge in stop smoking clinic and have attended smoking cessation training program were significantly associated with high KAP score. Therefore, more smoking cessation training should be provided for doctors.

Abstract: Number of active smokers in Malaysia is increasing despite availability of stop smoking clinics and smoking cessation medications. Thus, the practice of the healthcare professionals involved in providing smoking cessation intervention using evidence-based guidelines needs to be assessed using validated assessment tool. Newly invented reliable and valid questionnaire locally is needed. This study aimed to develop and validate a newly modified questionnaire to assess the knowledge, attitude and practice of medical doctors based on national smoking cessation guidelines and factors contributing to the score. The 22 items consists of mixture of true/false choice questions and Likert scaling response based on domain of 5A’s (ask, assess, advice, assist, arrange) and 5R’s (relevant, risks, rewards, roadblocks, repetitions) of the national stop-smoking guideline. The questionnaires were distributed to 131 primary care doctors. Reliability was determined using Cronbach’s alpha for internal consistency while construct validity was assessed using factor analysis. A high degree of internal consistency was observed for this 22 items (Cronbach’s alpha = 0.87), with good knowledge subscale (0.72), attitude subscale (0.58) and practice subscale (0.92). Factor analysis showed five meaningful components which represent the smoking cessation framework. This study indicates that this questionnaire is reliable and valid tool to assess the knowledge, attitude and practice on stop smoking guidelines compared to previous questionnaire.

Abstract: The number of elderly people in Malaysia is increasing in trend due to better healthcare services provided by the government and increased awareness of the community towards disease prevention. Therefore, the knowledge of future medical doctors towards geriatric care should be upgraded at par to fulfill the needs of the older population. As a result, their level of knowledge should be assessed, particularly in terms of the fundamental concept of geriatrics. A new, invented, reliable and valid questionnaire is needed. This study aimed to develop and validate a newly developed questionnaire to assess the knowledge of medical students in geriatrics. The 23 items consist of true-false questions and one-based answer responses based on five domains of Giant I’s geriatric, namely immobility, instability, incontinence, cognition, and iatrogenic. The sample size is calculated using a five-to-one item ratio. The questionnaires were distributed to 143 medical students. Reliability was determined using Cronbach’s alpha for internal consistency, while construct validity was assessed using factor analysis. An acceptable internal consistency was observed (Cronbach’s alpha = 0.62). Factor analysis showed two meaningful finalized components, which represent the geriatric general knowledge and the Giant I’s. According to the findings of this study, this questionnaire is a reliable and valid tool for assessing medical students' knowledge of geriatric. Abstract: Introduction: The number of elderly diabetes patients is rising all over the world. Therefore, the quality of life of older people as well as the factors that influence it are aimed to be evaluated in this study. Method: A cross-sectional study was carried out on 269 senior diabetes patients from government health clinic in Kuantan, Pahang. Their sociodemographic information was gathered, and the Diabetes Quality of Life Instrument (DQoL) was used to assess their quality of life. The connection between DQoL domains and covariates was determined by Mann-Whitney U test and the Kruskal-Wallis test. Linear regression analysis was used to find the predictors. Results: : The majority of patients in the B40 groups had at least a primary level of education (52.2%) and a source of income (97.3%).The majority of the clinics (61.3 %) were located in rural areas and did not have a family medicine specialist on staff (77.7 %). The majority of the patients had uncontrolled diabetes (71.7%), uncontrolled blood pressure (63.2%), uncontrolled cholesterol levels (74.0%), and abnormal abdominal circumferences (84.8%). Clinic location, diabetes control status, and old age were statistically significant in predicting the quality of life. Conclusion: Despite the perception of a significant risk of problems, this study found that decreasing the HbA1c goal further improved the quality of life of older patients. Those clinic headed by family medicine specialist gave the highest degree of patient quality of life. As a result, additional primary care physicians should be trained, as well as local treatment measures.

Abstract: Introduction: Prevalence of active smokers in Malaysia is increasing despite availability of stop smoking clinics. Thus, practice of the healthcare professionals in providing smoking cessation intervention needs to be assessed. This study aimed to develop and validate a questionnaire to assess the knowledge, attitude and practice of medical doctors on national smoking cessation guidelines  and factors contributing to the score. Methods: The 26 items consists of mixture of                        true/false questions; strongly-agree/agree/don’tknow/disagree/strongly-disagree response; and always/frequent/seldom/never response were created based on domain of 5A’s (ask, assess, advice, assist, arrange) and 5R’s (relevant, risks, rewards, roadblocks, repetitions). The questionnaires were distributed to 141 medical doctors. Reliability was determined using Cronbach’s alpha for internal consistency while construct validity was assessed using factor analysis. Results: A high degree of internal consistency was observed for this 26 items (Cronbach’s alpha= 0.824). Subsequently, 2 items with poor inter-item correlations were removed. Factor analysis extracted 7 meaningful components from this remaining 24 items, in which 3 components with least items were deleted due to overlapping subscale with other components, leaving 4 meaningful components of (1st) practice ask, advice, assess and practice 5R’s, (2nd) practice of assist and arrange for those willing to quit, (3rd) knowledge and (4th) attitude. These final 17 items still demonstrate high internal consistency with Cronbach’s alpha of 0.832. Conclusions: This study indicates that this questionnaire is a reliable and valid tool to assess the knowledge, attitude and practice on stop smoking guidelines. Improvement can be made for attitude items in the future.

Abstract: Comprehensive care towards geriatric health can be sustained by having good knowledge and attitude among the providers. Hence, this research aims to measure the knowledge and attitude of the final year medical students towards geriatric health and determine the associated factors. A cross-sectional study was conducted among the students using structured questionnaires of Geriatric-Attitude-Scale and Palmore-Facts-on-Aging-Quiz was used. Data was analysed using Mann Whitney U test, Chi square test and Spearman correlation coefficient analysis for the association between the score and studied variables. Median score of students’ knowledges is at 14.00 (IQR 13.00) and mean score for attitude is 48.52 (+5.53) respectively. Place of residence (P= 0.01) and presence of grandparents (P= 0.04) were the significant factors that affected knowledge of students towards geriatrics while number of siblings (P= 0.05) influenced attitudes of students towards geriatrics. Improvement of current geriatric curriculum should be done with more refined implementation of gerontology course in the basic medical learning.
