    Abstract: Neuroendokrine Neoplasien (NEN) umfassen eine seltene Tumorentität mit heterogener Biologie, Prognose und therapeutischen Optionen. In Zusammenhang mit der kürzlichen Publikation der ersten deutschen Leitlinie zur Diagnostik und Therapie von NEN erfolgte die Analyse der Kohorte des Deutschen NET-Registers der Deutschen Gesellschaft für Endokrinologie (DGE). Hierzu wurden 2686 Fälle extrahiert und ihre Patientencharakteristika wie Alter, Geschlecht, Primärtumorlokalisation, Grading und Staging dargestellt sowie das Gesamtüberleben berechnet. Zusätzlich wurden die systemischen Behandlungsstrategien in den beiden größten Untergruppen, NEN des Dünndarms und Pankreas, im Stadium der Metastasierung analysiert. Die Verteilung der Primärtumoren, die histopathologische Charakterisierung, das Tumorstadium sowie das Gesamtüberleben waren vergleichbar mit den Ergebnissen internationaler Registerstudien. Somatostatinanaloga (SSA) und die Peptid-Rezeptor-Radionuklid-Therapie (PRRT) waren die häufigsten systemischen Therapieverfahren bei Dünndarm-NEN. Hingegen wurde eine Chemotherapie – in Übereinstimmung mit der neuen Leitlinie – vor allem bei pankreatischen NEN eingesetzt und kam in der Erstlinie in ähnlicher Frequenz wie die SSA-Therapie bzw. in der Zweitlinie ähnlich häufig wie eine PRRT zum Einsatz. Prognostisch relevante Parameter waren die WHO-Klassifikation 2010 und das TNM-Staging. Die aktuelle Analyse des deutschen NET-Registers charakterisiert damit eine multizentrische, interdisziplinäre, deutschlandweite Kohorte von NEN-Patienten und beschreibt die angewendeten systemischen Therapieverfahren, das Gesamtüberleben und die prognostische Bedeutung der WHO-Klassifikation 2010 sowie des TNM-Stadiums.

    Abstract: Finerenone reduced the risk of kidney and cardiovascular events in people with chronic kidney disease (CKD) and type 2 diabetes in the FIDELIO-DKD and FIGARO-DKD phase 3 studies. Effects of finerenone on outcomes in patients taking sodium–glucose cotransporter 2 inhibitors (SGLT2is) were evaluated in a prespecified pooled analysis of these studies. RESEARCH DESIGN AND METHODS Patients with type 2 diabetes and urine albumin-to-creatinine ratio (UACR) ≥30 to ≤5,000 mg/g and estimated glomerular filtration rate (eGFR) ≥25 mL/min/1.73 m2 were randomly assigned to finerenone or placebo; SGLT2is were permitted at any time. Outcomes included cardiovascular composite (cardiovascular death, nonfatal myocardial infarction, nonfatal stroke, or hospitalization for heart failure) and kidney composite (kidney failure, sustained ≥57% eGFR decline, or renal death) end points, changes in UACR and eGFR, and safety outcomes. RESULTS Among 13,026 patients, 877 (6.7%) received an SGLT2i at baseline and 1,113 (8.5%) initiated one during the trial. For the cardiovascular composite, the hazard ratios (HRs) were 0.87 (95% CI 0.79–0.96) without SGLT2i and 0.67 (95% CI 0.42–1.07) with SGLT2i. For the kidney composite, the HRs were 0.80 (95% CI 0.69–0.92) without SGLT2i and 0.42 (95% CI 0.16–1.08) with SGLT2i. Baseline SGLT2i use did not affect risk reduction for the cardiovascular or kidney composites with finerenone (Pinteraction = 0.46 and 0.29, respectively); neither did SGLT2i use concomitant with study treatment. CONCLUSIONS Benefits of finerenone compared with placebo on cardiorenal outcomes in patients with CKD and type 2 diabetes were observed irrespective of SGLT2i use.

    Abstract: Rebars made of carbon fibres for the use in civil engineering: part 2: bond behaviour – bond tests with different carbon rebars Carbon reinforced concrete is becoming more and more accepted in the building industry. The use of mat carbon reinforcement is well researched in regard of the tensile and the bond behaviour. But there is still a considerable need for research in the case of carbon fibre rods and here in particular on the bond behaviour of the rods in concrete. For this reason, 8 various carbon rods with different surface profiles were evaluated in bond tests. The results were compared with the bond behavior of a conventional steel rod. Based on the results, a preferred rod variant was recommended, An in‐depth examination of these will be part of a further publication.

    Abstract: Impact safety of building structures through mineral‐bonded composites: Structural level The aim of the Research Training Group GRK 2250 funded by the German Research Foundation is interdisciplinary research to increase impact resistance of existing structures by strengthening. The article at hand provides insight into research at structural level. Mineral‐bonded strengthening layers were investigated on the face of the structure which was not hit by impactor. As material for these layers, fine‐grained matrices without and with short fibres were used. Their effectiveness was varied by embedding different textiles. Strengthened reinforced concrete plates were subjected to impact loading in a drop tower facility. An important indicator for the effectiveness of strengthening is the degree of damage. For this purpose, a method for the analysis of crack patterns by means of image sequence analysis was developed. Furthermore, the experiments serve as a reference for the simulation of crack propagation in the strengthened plates by the phase field method.

    Abstract: Vorgestellt wird der Fall einer 57-jährigen Frau mit einem Adenokarzinom des Pankreaskorpus. Dies erforderte eine operative Entfernung des Pankreas und in der Folge eine Substitution der Pankreasenzyme und des Insulins.     Abstract: Hintergrund Die endoskopische Submukosa-Dissektion (ESD) ist in Asien Standard zur Behandlung von Magenfrühkarzinomen (MFK) im Rahmen von definierten Kriterien. Auch in Europa wird sie immer häufiger benutzt. Bei Risikokonstellationen (z. B. Submukosa-Invasion, Lymphangio-Invasion, schlecht differenziertes Karzinom) ist das Auftreten von Lymphknotenmetastasen möglich. Wir definierten eine „Low-Risk-Gruppe“ (differenzierte, nur mukosal gelegene MFK ohne Ulzeration und Gefäßinfiltration, größenunabhängig) um das endoskopische Ergebnis und den Langzeitverlauf dieser prognostisch günstigen Gruppe zu untersuchen. Patienten und Methoden Patienten mit nicht vorbehandelten Low-Risk-MFK wurden in die retrospektive Auswertung aufgenommen. Ausgewertet wurden Patientendaten (Alter, Geschlechterverteilung), Karzinomparameter (Anzahl, Größen, Lokalisation, Paris-Klassifikation), histologische Parameter, Resektionsdaten (u. a. En-bloc-Resektion, R-Status), Nachsorgen (u. a. Lokalrezidive, Überleben) und Komplikationsdaten. Ergebnisse Durch ESD wurden bei 55 Patienten 61 Low-Risk-MFK entfernt. In 93,4 % gelang die En-bloc-, in 91,8 % die R0-Resektion. In 8 Fällen (13,1 %) kam es zu interventionsabhängigen Komplikationen: 5 verzögerte Nachblutungen (8,2 %), 2 Mikroperforationen (3,3 %, keine Operation notwendig) und ein Nicht-ST-Hebungsinfarkt (1,6 %).Während der Nachbeobachtungszeit (Mittelwert: 54,3 Monate) kam es in 4 Fällen (6,6 %) zu Lokalrezidiven, von welchen zumindest zwei erfolgreich endoskopisch behandelt werden konnten. Die anderen beiden Patienten verstarben nach endoskopischer Re-Therapie vor der ersten Kontrolle. Insgesamt verstarben 4 Patienten (7,8 %) während des Follow-ups, jeweils nicht tumorassoziiert. Bei 9 Patienten (16,4 %) wurden syn- oder metachrone Läsionen nachgewiesen, welche alle endoskopisch behandelt werden konnten. Lymphknotenmetastasen traten nicht auf. Fazit In der von uns definierten Low-Risk-Gruppe von mukosalen MFK wurde mittels ESD eine hohe Rate von En-bloc- und R0-Resektionen erreicht. Die festgestellten Lokalrezidive konnten endoskopisch beherrscht werden. Metastasen wurden nicht festgestellt, kein Patient verstarb tumorassoziiert. ESD ist für diese Gruppe daher als Standard-Therapie zu empfehlen. Hinsichtlich des klinisch-endoskopischen Verlaufs bei Vorhandensein von Risikofaktoren sowohl bei Mukosakarzinomen als auch der übrigen MFK aus der sogenannten Expanded-Group (u. a. Lymphangio-Invasion, submukosainvasive MFK, schlecht differenzierte G3-Karzinome) liefert die Studie keine Daten.

    Abstract: Introduction: A systems perspective was used to describe U.S. Department of Defense (DoD) Global Health Engagement (GHE). This description was based on DoD instructions and higher-level documents related to DoD GHE. A complex system of systems such as health care can benefit from using modeling approaches to help understand the interactions among major components. Models (conceptual, computer-based programs, realistic simulations, or training exercises) can be used to help stakeholders prioritize options and to identify important components and gaps for making performance improvements. Based on the cited documents, we identified major DoD GHE components to create a conceptual model. Materials and methods: Components were selected from DoDI 2000.30 for DoD GHE. Definitions of these interacting components are given and assigned to our network model that consists of three levels: strategic, operational, and tactical. These levels are linked by critical nodes (decision points) that allow feedback to previous levels for modifying appropriate components. A network that is reminiscent of this structure is Boyd’s observe-orient-decision-act diagram. Acceptable strategic and operational plans are linked to the tactical level. Acceptable tactical components lead to the desired outcome of accomplishing the DoD GHE goals. Complex systems also have feedback loops to allow for component evaluations and modifications. Accomplishing DoD GHE goals need to have adaptable components in dynamic permissive environments. Results: The network that we considered is multicomponent and interdisciplinary. The network uses DoD GHE goals as the input (observing in the observe-orient-decide-act loop) to provide relevant information. It consists of three levels of adaptable, interacting (orienting) components that are linked by critical nodes (decision points) involving the evaluation of plans and desired outcomes. Strategic components (eg, sharing, personal interactions, agreements, planning, readiness, etc.) are required to develop strategic plans, the first critical node. If these plans are acceptable, the information is passed on (feed forward, action) to the operational components (define problems, understand strategic direction and guidance, understand the operational environment, etc.). At the second critical node, the decision is made about moving the operational plans to the tactical components (eg, evaluation, data, training, etc.). Tactical components are essential to provide further information to the third critical node, desired outcomes, in accomplishing DoD GHE goals. Feedback from all critical nodes is essential to allow modifications of various components and to attain health-related interoperability in supporting security policies and military strategies. Conclusions: Examining the composition of DoD GHE and creating a defined model can help identify interacting features of this complex system. All of the identified components have been associated with gaps, such as the need for monitoring and measuring tools, and standards. The current state of this system is dynamic and is evolving when confronting gaps. DoD GHE represents an intersection of global health and security in supporting U.S. national security objectives by establishing access and influence with partner nations and among health care-related government and non-government organizations, and as a result, improves the readiness, health, and safety of our military personnel.
