    Kurzfassung: Psychological treatment that reduces anxiety has been found to be beneficial to benzodiazepine withdrawal. High drop-out and relapse rates, however, have also been reported. They might be due to a heightened anxiety level maintaining the drug-taking habit or to the addictive potency of the drug, which is not addressed by the intervention strategies. In the present study, initial assessment data were compared among treatment refusers, drop-outs and completers – successful as well as unsuccessful ones – of a psychological treatment programme in support of benzodiazepine withdrawal. Treatment refusers showed a more negative current mood state than those consenting to treatment. Internal locus of control was predictive of premature termination and unsuccessful completion of the treatment trial. Neither medication-related variables nor anxiety or depression were found to influence the success of treatment. Perceived control over medication intake is thought to reduce compliance with the treatment regimen – a pattern that may be consistent with the addiction hypothesis.

    Kurzfassung: 〈 i 〉 Background: 〈 /i 〉 A practice-based study was carried out to assess the comparative effectiveness and acceptability of standardised hypnosis, hypnosis with individualised imagery, cognitive behavioural treatment (CBT) and general anaesthesia (GA) in the treatment of dental phobia. 〈 i 〉 Methods: 〈 /i 〉 A 4-group design was used with 4 repeated measurement occasions. Of an initial total of 137 dental phobics, 77 completed the study with sample sizes of between 14 and 29 patients in the 4 groups. Participants completed questionnaires of dental anxiety at the beginning of the trial, before and after the first dental appointment and again before the second dental appointment a week later. 〈 i 〉 Results: 〈 /i 〉 Standardised hypnosis evidenced a significantly higher rate of premature termination of treatment than CBT. The completer analysis showed a significant reduction of dental anxiety after CBT and individualised hypnosis compared to the GA condition. The intent-to-treat analysis showed significant improvement only after CBT. 〈 i 〉 Conclusions: 〈 /i 〉 The results suggest that CBT is the treatment of choice in dental phobia when taking both effectiveness and acceptability into account.

    Kurzfassung: Abstract Contingent negative variation (CNV) and evoked potentials were recorded during the continuous performance test in 20 children (aged 7 to 12) with attention deficit/hyperactivity disorder (ADHD) and in 19 age-matched healthy children. A series of letters were displayed every 2.1 s and a button had to be pressed whenever the letter O was followed by an X. Children were also given neuropsychological tests and questionnaires assessing anxiety and depression. Unlike ADHD children, healthy children showed a more pronounced CNV after the cue - the letter O - than after the distractors. At Fz, CNV negativity was significantly correlated with IQ and executive function. Control children also exhibited a higher parietal N375 post S1 - likely to be associated with letter processing - and a higher N550 amplitude at Fz than ADHD children post S1 and S2; both groups showed a higher N550 amplitude whenever X followed non-O or O was followed by non-X. This component appears to be involved in decision-making processes regarding the initiation or inhibition of movement. ADHD children had a faster reaction time but made more errors of omission and commission. Results suggest that ADHD children show deficits in both stimulus processing and response execution.

    Kurzfassung: The most frequently reported neuropsychological deficits in schizophrenia are those of attention, executive function, and verbal memory. Whereas the former appear to be related to negative symptoms of schizophrenia, there is little agreement about which clinical symptoms are related to the verbal memory deficit. The aim of the present study was to delineate further the pattern of neuropsychological deficits in schizophrenia—especially those of verbal memory—and their relationship to clinical symptoms. One hundred patients with chronic schizophrenia and 62 healthy control subjects took part in the study. Assessments of patients took place within the first 3 weeks after admission to hospital. Nine neuropsychological tests, mainly measuring executive and memory function and attention, were administered to all subjects, and clinical symptoms, such as psychotic and negative symptoms and conceptual disorganization, were assessed in patients by means of the Positive and Negative Syndrome Scale (PANSS). Patients showed widespread cognitive deficits with verbal memory impairment best discriminating patients and controls. Conceptual disorganization was partly accounted for by poor verbal memory and a low IQ estimate, and negative symptoms by deficient word fluency; positive symptoms were not significantly related to cognitive deficits. The results indicate that there is a specific relationship between neuropsychological deficits and the more chronic of the clinical symptoms.

    Kurzfassung: Community studies found deficits of cognitive function in elderly persons with diabetes. The aim of the present study was to compare diabetes and other age-related diseases among residents of senior citizens’ care facilities. Participants were 130 residents with and 130 without a diagnosis of diabetes aged between 65 and 98 years. Measures were neuropsychological tests designed for this age group, questionnaires of mood, quality of life and daily activities and level of glycosylated hemoglobin (HbA 〈 sub 〉 1c 〈 /sub 〉 ) as an index of metabolic control. Only verbal memory was significantly more impaired in residents with than without diabetes. Otherwise, both groups showed overall impairment of cognitive function and mood. Unexpectedly, a slightly elevated HbA 〈 sub 〉 1c 〈 /sub 〉 level (6–8%) was associated with an increased level of cognitive and everyday functioning. In old age, diabetic patients might benefit from a slightly higher level of blood glucose than has hitherto been assumed.

    Kurzfassung: Theoretischer Hintergrund: Die meisten diagnostischen Instrumente stellen bei Zwangsstörung, bzw. den Symptomkategorien Zwangsgedanken und -handlungen, in erster Linie den Schweregrad und nicht deren Generalisierung fest. Fragestellung: Sind Zwangsgedanken bzw. -handlungen situationsspezifisch oder generalisiert auf viele Bereiche? Methode: 142 Patienten mit Zwangsstörung und 21 gesunde Kontrollprobanden erhielten eine deutsche Version des VOCI, das Zwangssymptome in einer Anzahl von Situationen erfragt und die Subskalen der Y-BOCS, die den Schweregrad von Zwangshandlungen und –gedanken erheben. Patienten wurden mit den Kontrollprobanden hinsichtlich der Skalen verglichen. In der Gruppe der Patienten wurden die VOCI Items einer Faktorenanalyse unterzogen und die Faktorenscores mit dem Y-BOCS korreliert. Ergebnisse: Die Patienten unterschieden sich signifikant von den Kontrollprobanden hinsichtlich beider Zwangsfragebogen. Das 6-Faktorenmodell des deutschen VOCI entsprach dem der Originalfassung. Die Y-BOCS Subskalen waren nur moderat mit den entsprechenden VOCI Faktoren korreliert. Unter Patienten mit hoher Ausprägung der Y-BOCS befanden sich solche mit hohen (generalisierten), aber auch mit niedrigen (spezifischen) VOCI Faktorscores. Schlussfolgerungen: Die Ergebnisse lassen auf individuelle Unterschiede hinsichtlich der Spezifität oder Generalisierung der Symptomkategorien unter Zwangspatienten schließen.
