    Abstract: ResumoObjetivo:verificar a ocorrência e as causas de reinternações até um ano após a alta de hospitalizações, devido a acidentes de trânsito.Métodos:foram incluídas vítimas de traumas múltiplos, por acidentes de trânsito, admitidas numa Unidade de Terapia Intensiva. Dados sociodemográficos, circunstâncias do acidente, regiões corporais atingidas e causa da readmissão foram coletados dos prontuários dos pacientes.Resultados:das 109 vítimas de acidentes de trânsito, a maioria eram homens jovens e adultos. O maior número de internações por acidentes envolveu motociclistas (56,9%). As causas de retorno ao hospital foram: necessidade de continuidade do tratamento cirúrgico (63,2%), infecção de sítio cirúrgico (26,3%) e queda relacionada às sequelas físicas do trauma (10,5%), sendo a taxa de reinternação de 174/1.000 pessoas/ano.Conclusão:a taxa de readmissão hospitalar na população estudada é semelhante às encontradas em outros estudos. Vítimas de traumas graves de extremidade necessitam de múltiplos procedimentos cirúrgicos, maior tempo de internação hospitalar e reabilitação prolongada.

    Abstract: Las finalidades fueran evaluar el estado de salud percibido y la adhesión farmacológica y, verificar la correlación entre esas medidas en pacientes sometidos a intervención coronaria percutánea, tras el alta. Estudio transversal, realizado en el periodo de mayo/2011 a julio/2012. Los instrumentos utilizados fueron: SF-36 y Medida de Adhesión a los Tratamientos, con 101 pacientes, 54 (53,5%) hombres; promedio de edad 59,5±10,3; con tratamiento cardiaco previo 32 (32,7%). Todos estaban usando medicación antihipertensiva, la mayoría utilizaba antiplaquetario, 99 (98%); reductores de colesterol, 98 (97%); y vasodilatadores coronarios, 59 (58,4%). El número promedio de medicamentos utilizados fue 6,8±2,1. Se observó la adherencia farmacológica en 98 (97%) pacientes. Los participantes mostraron mejor estado de salud en los Aspectos Sociales y Capacidad Funcional. Fueron encontradas correlaciones positivas y moderadas entre la medida de adhesión farmacológica y Capacidad Funcional, Salud General y Aspectos Sociales. Hubo correlación entre adhesión farmacológica y estado de salud percibido.

    Abstract: OBJETIVO: comparar possíveis diferenças quanto à presença de sintomas depressivos, segundo o diagnóstico clínico da síndrome coronariana aguda, sexo e faixa etária, uma semana antes do primeiro evento cardíaco. MÉTODO: estudo transversal, descritivo e exploratório, que utilizou o Inventário de Depressão de Beck. A amostra foi composta por 253 pacientes. RESULTADOS: os pacientes com diagnóstico clínico de angina instável, do sexo feminino e com idade inferior a sessenta anos relataram, com maior frequência, a presença de sintomas depressivos. CONCLUSÕES: elevada porcentagem de pacientes apresentava sintomas depressivos no momento da internação pelo primeiro episódio da síndrome coronariana aguda, e essa prevalência foi significativamente maior entre as mulheres, com menos de 60 anos e com angina instável. Esses resultados deverão embasar o atendimento na internação, alta e planejamento da reabilitação desses pacientes, pois se sabe que a depressão dificulta o controle da coronariopatia.

    Abstract: ABSTRACT Objective: to assess the association between perceived health status and the mental health of patients submitted to percutaneous coronary intervention after hospital discharge. Method: a quantitative and cross-sectional study involving 101 participants. The following instruments were used: a sociodemographic and clinical characterization instrument, the Medical Outcomes Study 36-Item Short-Form Health Survey (SF-36), and the Hospital Anxiety and Depression Scale (HADS). Participants were divided into three groups: no anxiety and no depression (G1); anxiety or depression (G2); and both (G3). The ANOVA test was used for the intergroup comparison of means on the SF-36 domains. Results: There was an association between perceived health status and mental health. The participants in G1 presented higher scores in all SF-36 domains in comparison with participants in G2 and G3. Conclusion: Participants with no anxiety and depression presented better-perceived health status in comparison with those with anxiety or depression, or both.

    Abstract: ABSTRACT Objective: to compare the health status of traffic accident victims, at hospital discharge and after six months, and to analyze the predictive variables of their health status and return to work. Method: observational, longitudinal study. Data were collected through interviews and medical records of 102 patients with a mean age of 33 years; with the majority being men and victims of motorcycle accidents. The variables were analyzed by means of validated tools, student's t-test, multiple linear regression, and logistic regression. Results: there was an improvement of perception in the patients' health status six months after hospital discharge and functional capacity. The individuals who returned to work showed better health-related quality of life evaluation. Conclusion: improvement of the perceived health status six months after hospital discharge was found. Factors that influenced the patients' return to work were not identified.

    Abstract: The aim of this study was to compare health status, self‐efficacy, symptoms of depression and anxiety and post‐traumatic stress between two groups of burn victims receiving information on self‐care through routine care or an educational programme with telephone reinforcement. Background Self‐care post discharge is an important factor on quality of life of burn victims. Reinforcement of self‐care programme via telephone follow‐up might improve self‐efficacy and general health perception in burn victims. Design Randomized , controlled clinical trial. Method From 2011–2012, we randomized 108 burn victims either to an educational programme on self‐care tailored for burn victims according to the cognitive social theory and reinforcement by telephone every 4‐6 weeks for 6 months post discharge, or to the routine instructions given at discharge from the hospital. During hospitalization, at discharge, at 6 and 12 months post discharge, we collected data on socio‐demographic characteristics, Burns Specific Health Scale Revised ( BSHS ‐R), Perceived Self‐Efficacy Scale ( PSE ), Hospital Anxiety and Depression Scales ( HADS ) and Impact of the Event Scale ( IES ). We analysed data using t ‐test, Chi‐square test and linear regression models. Results Groups were similar regarding clinical and demographic characteristics, except for age. At 6‐month follow‐up, there were no differences between the groups on BSHS ‐R, PSE and HADS Depression, while the intervention group showed fewer symptoms of anxiety ( HADS Anxiety) and lower IES scores than the control group. Conclusion The educational programme, when compared with usual care, reduced anxiety symptoms and post‐traumatic stress. Educational programmes with telephone follow‐up might be a viable intervention to improve self‐care for burn victims.

    Abstract: To evaluate the effect of reinforcing an educational programme through telephone follow‐up on health‐related quality of life and anxiety and depression symptoms in individuals starting warfarin therapy. Background Educational interventions have improved quality of life in individuals using warfarin. Few studies have examined the addition of telephone follow‐up to enhance educational interventions. Design Randomised controlled trial in outpatient setting. Methods Hospitalised adults starting warfarin therapy who agreed to participate received an educational programme about the warfarin treatment. At discharge, they were randomised to receive either five telephone follow‐up calls (intervention) or no telephone calls (controls). Both groups were evaluated for health‐related quality of life (using Duke Anticoagulation Satisfaction Scale) and symptoms of anxiety and depression (using Hospital Anxiety and Depression Scale) at three and six months post‐discharge. Groups were compared at each time by independent‐samples t test, and over time by repeated‐measures analysis of variance, with time (three and six months), groups (intervention and control) and an interaction between time and group as factors. Level of significance was set at 0.05. The Consolidated Standards of Reporting Trials was used for reporting. Results Fifty‐two individuals (26 per group) completed the study. There were no statistical differences between groups in health‐related quality of life, anxiety and depression symptoms, at both times post‐discharge. Participants who received follow‐up telephone calls reported better positive psychological impact (a subscale of quality of life) than controls. Conclusions Reinforcing an educational programme with telephone follow‐ups did not have an overall effect on health‐related quality of life of individuals using warfarin but promoted positive psychological impact. Relevance to clinical practice The low cost of reinforcing educational programmes with telephone calls and the improvement in positive psychological aspects indicate that this type of intervention is still a promising intervention that could be further investigated and improved.

    Abstract: RESUMO Objetivo: avaliar a qualidade de vida relacionada à saúde de pacientes com marca-passo cardíaco definitivo. Método: estudo observacional descritivo, transversal, realizado no ambulatório de arritmia de um hospital universitário do interior paulista. Amostra consecutiva e não probabilística foi constituída de pacientes de ambos os sexos, maiores de 18 anos, com marca-passo há pelo menos um mês. Foram excluídos os que não apresentaram condições cognitivas para responder aos questionários, como também aqueles que apresentaram dispneia, fraqueza e fadiga no momento da aplicação dos instrumentos e com cardioversor desfibrilador implantável. Para a avaliação da qualidade de vida relacionada à saúde, utilizou-se o instrumento genérico Medical Outcomes Study 36 - Item Short-Form Health Survey, composto por 36 questões distribuídas em oito domínios, e o instrumento específico Assessment of Quality of Life and Related Events, composto por 20 questões distribuídas em três domínios. Resultados: participaram 88 pacientes, a maioria do sexo masculino e com companheiro, com a média de idade de 64,3 (±13). Os domínios que apresentaram maiores médias, assim como melhores avaliações, foram Aspectos Sociais (78,1; ±26,8) e Saúde Mental (68,2; ±23,9), e as menores foram em Aspectos Físicos (48,2; ±41,4) e Capacidade Funcional (58,5; ±27,9), referentes ao Medical Outcomes Study 36. Quanto ao Assessment of Quality of Life and Related Events, o domínio de maior média e melhor qualidade de vida foi Arritmia (78,2; ±20,7), e o de menor, Dispneia (71,1; ±26,8). Conclusão: os pacientes apresentaram melhores avaliações da qualidade de vida relacionada à saúde nos domínios mentais e piores nos domínios físicos.

    Abstract: RESUMEN Objetivo: evaluar la relación entre los estresores percibidos en la Unidad de Terapia Intensiva y la inestabilidad hemodinámica en el post-operatorio de pacientes sometidos a la primera cirugía cardíaca. Método: estudio observacional analítico de cohorte prospectivo y desarrollado en un hospital universitario del interior paulista. Una muestra consecutiva y no probabilística fue constituida por pacientes sometidos a la primera cirugía cardíaca de revascularización del miocardio o corrección de valvulopatías. La Escala de Evaluación de los Estresores en la Unidad de Terapia Intensiva (con valores de 0-200; el mayor valor indica un mayor estrés) y los marcadores de perfusión tisular fueron utilizados en la evaluación de las variables de interés. Resultados: participaron del estudio 150 pacientes. Los pacientes con inestabilidad hemodinámica (n=91) presentaron una media de 75,6 puntos en la evaluación de los estresores y aquellos pacientes sin inestabilidad hemodinámica (n=59) presentaron una media de 72,8 puntos, no habiendo ninguna diferencia estadísticamente significativa entre ellos (p=0,398; test t de Student). Conclusión: en la muestra estudiada no fue constatada ninguna relación entre los estresores percibidos en la Unidad de Terapia Intensiva y la inestabilidad hemodinámica en el post-operatorio de las cirugías cardíacas.
