    Kurzfassung: Introduction: Diabetic Retinopathy (DR) is a common microvascular complication of diabetes mellitus. Optical Coherence Tomography Angiography (OCTA) is a new method for visualizing the microvasculature of the retina and choroid, which is based on detecting the movement of circulating erythrocytes. Objective: To present OCTA findings in patients with diabetes mellitus with different microvascular changes to diagnose diabetic retinopathy before the onset of clinical signs of the disease, as well as to monitor changes in clinically visible retinopathy, with or without Diabetic Macular Edema (DME). Methods: In this study, OCTA findings were compared in three groups of selected subjects - 5 patients with Non-Proliferative Diabetic Retinopathy (NPDR) without DME, 5 patients with NPDR and DME, and 5 patients with diabetes but without NPDR and DME. Results: Patients in all three groups showed decreased vessel diameter (VD) in the foveal, parafoveal, and perifoveal segments of the Superficial Capillary Plexus (SCP) and Deep Capillary Plexus (DCP) and an increase in Foveal Avascular Zone (FAZ). Conclusion: OCT angiography represents a new technique for visualizing and quantifying vascular changes and is increasingly important in the early diagnosis of subclinical retinopathy, as well as in monitoring existing changes in the retina in people with type 2 diabetes mellitus.

    Kurzfassung: Disturbed redox balance in heart failure (HF) might contribute to impairment of cardiac function, by oxidative damage, or by regulation of cell signaling. The role of polymorphism in glutathione transferases ( GSTs ), involved both in antioxidant defense and in regulation of apoptotic signaling pathways in HF, has been proposed. We aimed to determine whether GST genotypes exhibit differential risk effects between coronary artery disease (CAD) and idiopathic dilated cardiomyopathy (IDC) in HF patients. GSTA1 , GSTM1 , GSTP1 , and GSTT1 genotypes were determined in 194 HF patients (109 CAD, 85 IDC) and 274 age- and gender-matched controls. No significant association was found for GSTA1 , GSTM1 , and GSTT1 genotypes with HF occurrence due to either CAD or IDC. However, carriers of at least one variant GSTP1 ∗ Val (rs1695) allele were at 1.7-fold increased HF risk than GSTP1 ∗ Ile/Ile carriers ( p = 0.031 ), which was higher when combined with the variant GSTA1 ∗ B allele ( OR = 2.2 , p = 0.034 ). In HF patients stratified based on the underlying cause of disease, an even stronger association was observed in HF patients due to CAD, who were carriers of a combined GSTP1 (rs1695)/ GSTA1 “risk-associated” genotype ( OR = 2.8 , p = 0.033 ) or a combined GSTP1 ∗ Ile/Val+Val/Val (rs1695)/ GSTP1 ∗ AlaVal+ ∗ ValVal (rs1138272) genotype ( OR = 2.1 , p = 0.056 ). Moreover, these patients exhibited significantly decreased left ventricular end-systolic diameter compared to GSTA1 ∗ AA/ GSTP1 ∗ IleIle carriers ( p = 0.021 ). Higher values of ICAM-1 were found in carriers of the GSTP1 ∗ IleVal+ ∗ ValVal (rs1695) ( p = 0.041 ) genotype, whereas higher TNF α was determined in carriers of the GSTP1 ∗ AlaVal+ ∗ ValVal genotype (rs1138272) ( p = 0.041 ). In conclusion, GSTP1 polymorphic variants may determine individual susceptibility to oxidative stress, inflammation, and endothelial dysfunction in HF.

    Kurzfassung: The aim of the study was to analyze changes of systolic and diastolic blood pressure values over five and ten years separately boys and girls and to estimate correlation between them. Three age groups from 8 centers in Serbia were evaluated: Group 1: 10 year old patients, Group 2: 15 year old and Group 3: 20 year old. Group with normal blood pressure values, prehypertensive and hypertensive group were analyzed. Regarding the period of follow-up we analyzed: 10/15 years period-children between 10 and 15 years, 15/20 years period-children between 15 and 20 years, and 10/20 years period-children between 10 and 20 years. Significant increase of diastolic blood pressure was noticed for both genders in 10/15 years period of prehypertensive population, while in hypertensive children, boys showed decline in frequency for systolic and diastolic blood pressure and girls only for diastolic. In 15/20 years period there was significant decrease of prehypertensive and significant increase of hypertensive diastolic blood pressure frequency. In 10/20 years period significant reduction in frequency of prehypertensive systolic blood pressure was noticed, while only hypertensive group of boys showed significant reduction regarding systolic blood pressure frequency. Prehypertensive diastolic and hypertensive systolic blood pressure fluctuations are more related to age.

    Kurzfassung: We present three different cases of patients with papilledema: one case with a primary tumour of the optic nerve, and two cases with secondary involvement of the optic nerve (breast carcinoma and non-Hodgkin lymphoma). The visual acuity varies from sudden, painless, decrease of vision, to blurred vision and proptosis on the affected eye. A characteristic ophthalmoscopic examination shows a swollen optic disc and promonence with tortuosity of blood vessels. A fluorescein angiography shows dilated capillaries of the optic disc with leakage in the early phase and diffuse hyperfluorescence during the late phase. A ultrasonography B-scan of the affected eye shows prominent optic nerves with widened optic nerve sheath. An exhaustive history and complete ophthalmological examination are essential for the diagnosis, to which fluorescein angiography, B-scan ultrasonography, ocular coherent tomography, computerized tomography, and magnetic resonance can be used as useful additional tests. Treatment and prognosis depend on the primary process. We emphasize the crucial role of ophthalmologist in the early diagnosis of CNS neoplasia.

    Kurzfassung: Background and Objectives: The main cause of the vision loss in diabetics is the development of diabetic macular edema, regardless of the stage of diabetic retinopathy. The paper aimed to examine whether the additional intravitreal application of triamcinolone acetonide to continuous anti-vascular endothelial growth factor therapy could improve therapeutic outcomes for pseudophakic eyes with persistent diabetic macular edema. Materials and Methods: twenty-four pseudophakic eyes with refractory diabetic macular edema, that had appeared despite three previously administered intravitreal injections of aflibercept, were divided into two groups (twelve eyes in each group). The first group continued to have aflibercept administered according to a fixed dosing regimen (once in two months). Triamcinolone acetonide 10 mg/0.1 mL (administered once per four months) was included for the second group, i.e., their treatment continued with a combination of aflibercept + triamcinolone acetonide. Results: The reduction in central macular thickness was higher in the eyes treated with combined therapy (aflibercept + triamcinolone acetonide) compared with the use of aflibercept alone during the entire 12-month follow-up period (3rd month p = 0.019; 6th month p = 0.023; 9th month p = 0.027; 12th month p = 0.031). As was evident from the p-values, the differences were statistically significant. No statistically significant difference was recorded for visual acuity: 3rd month p = 0.423; 6th month p = 0.392; 9th month p = 0.413; 12th month p = 0.418. Conclusions: Combined anti-vascular endothelial growth factor and steroid therapy leads to a better anatomical outcome of persistent diabetic macular edema in pseudophakic eyes, but does not lead to a more significant improvement in visual acuity than continuous anti-VEGF therapy alone.

    Kurzfassung: The aim of the study was to investegate the correlation between the levels of CRP and YKL-40 in blood samples with morphometric parameters of retinal blood vessels in patients with diabetic retinopathy. Blood laboratory examination of 90 patients included the measurement of glycemia, HbA1C, total cholesterol, LDL-C, HDL-C, triglycerides and CRP. Levels of YKL-40 were detected and measured in serum by ELISA (Micro VueYKL-40 EIA Kit, Quidel Corporation, San Diego, USA). Morphmetric analysis was performed with ImageJ software (http://rsbweb.nih.gov/ij/) for digital retinal photography. We measured the number, diameter of retinal blood vessels in five different parts concentric to the optic disc. Differences between the morphometric parameters and the blood test analysis results were evaluated using the Student’s t – test. One Way ANOVA was used to establish the significance of differences. CRP and YKL-40 levels were moderately higher in the group of patients with severe diabetic retinopathy. Levels of YKL-40 correlated positively with diameter and negatively with number of retinal blood vessels. The average number of the blood vessels per retinal zone was significantly higher in the group of patients with mild non-proliferative diabetic retinopathy than in the group with severe form in the optic disc and all five retinal zones. The average outer diameter of the evaluated retinal zones and optic disc vessels was significantly higher in the group with severe compared to the group with mild diabetic retinopathy. Morphological analysis of the retinal vessels on digital fundus photography and correlation with YKL-40 may be valuable for the follow-up of diabetic retinopathy.
