Abstract:
El cannabis es una planta con larga historia y gran tradición de uso medicinal. Las sustancias activas de la planta (fitocannabinoides, FC), los receptores y los mediadores del sistema endocannabinoide (SEC) se descubrieron a finales del siglo XX. Hasta el momento se han identificado varios FC y endocannabinoides (EC), y se han sintetizado algunos cannabinoides de uso farmacológico. Los compuestos no cannabinoides de la planta incluyen una amplia variedad de terpenos y flavonoides. El SEC incluye receptores a FC, EC, así como las enzimas que se encargan de la síntesis y degradación de dichos compuestos. La actividad biológica de los compuestos cannabinoides depende de las interacciones con los receptores del SEC y con otras vías de señalización: los receptores CB1 y CB2. Éstos son los principales mediadores de la respuesta a cannabinoides y receptores del SEC. En este documento se exponen las vías de biosíntesis de los EC. Igualmente, se describe la utilización de cannabinoides en distintas patologías o situaciones clínicas tales como epilepsia, dolor, náusea y vómito inducidos por quimioterapia, espasticidad por esclerosis múltiple, insomnio y trastorno por estrés post traumático. También se incluye la evidencia de los efectos secundarios de estos compuestos. Los objetivos de este artículo de perspectiva son ofrecer un resumen de la evidencia de los efectos farmacológicos del cannabis y mostrar la importancia que ha tenido y puede cobrar en cuanto a su uso médico.
