    Abstract: A newly obtained foraminiferal assemblage from the Guadalupian−Lopingian, and Induan of the Western Salt Range (Pakistan) comprises 47 species (including 7 species of fusulinoideans) of 24 genera and 71 species (including 9 species of fusulinoideans) of 41 genera, in Nammal and Zaluch sections, respectively. Our results show that only 4 species went extinct during the Guadalupian−Lopingian mass extinction (GLME), suggesting the GLME was minor for smaller foraminifers in the Salt Range. Most species went extinct during the Permian-Triassic mass extinction (PTME), except the two survivor genera, Nodosinelloides and Planiinvoluta. During the end-Permian mass extinction, majorities of the species became extinct in lower part of Chhidru Formation, beacuse they are not tolerant to the terigenious-clastic input in the upper part of Chhidru Formation. The extinction process of foraminifers in the Nammal and Zaluch sections has been documented for the first time. Five foraminiferal assemblages are established in the shallow marine shelf deposits of Western Salt Range, i.e., Wordian Geinitzina araxensis assemblage in tide-influenced subaqueous delta to middle shelf environment, Capitanian Baisalina pulchra assemblage and Wuchiapingian Codonofusiella schubertellinoides assemblage in inner-outer shelf carbonate deposits, Changhsingian Colaniella pseudolepida assemblage and Induan Nodosinelloides-Planiinvoluta assemblage in mixed siliciclastic–carbonate shelf setting. The Baisalina pulchra and Codonofusiella schubertellinoides assemblages have wide geographic distribution and have been found in Transcaucasus, China, Turkey, Crimea and Oman. The other three assemblages (i.e., Geinitzina araxensis, Colaniella pseudolepida, and Nodosinelloides-Planiinvoluta assemblages) found in this study are likely local/regional assemblages. The foraminifers were observed with the standard microscopic system Leica-DM500. A total of 681 specimens of foraminifers are reported in the Zaluch Group and Mianwali Formation of Nammal and Zaluch sections. Five hundred ninety-nine smaller foraminifers specimens were recovered. Among the smaller foraminifera, Nodosinelloides, Pachyphloia, Geinitzina, Colaniella, Hemigordius, Tuberitina, and Langella are the most dominant genera. Larger foraminifera genera include Nankinella, Codonofusiella, Reichelina, Necdetina and Pisolina, represented by 82 specimens. A total of 3 classes (e.g., Nodosariata, Fusulinata, and Tubothalamea) and seven orders (i.e., Miliolida, Spirillinida, Lagenida, Parathuramminida, Endothyrida, Fusulinida, and Earlandiida) are identified in the current study. Among them, Lagenida becomes the most dominant by diversity and abundance. Shannon diversity and other indices rise progressively from Wordian to Changhsingian in the shallow marine sediments before dropping to zero in the Early Triassic mixed siliciclastic–carbonate shelf deposits.

    Abstract: Blood transfusion has been proven as a common cause for contracting infectious diseases with 1.5 million donors each year in Pakistan carrying 1% risk of transfusion transmitted infections. Syphilis is one of them whose seroprevalence has been increasing in recent years. Thus, evaluation of its seroprevalence will give information about current burden of the disease. Objectives: To determine seroprevalence of syphilis among healthy blood donors of Lahore to assess disease burden for year 2016 and 2017. Study Design: Retrospective single centre cross-sectional study. Setting: Tertiary care hospital (Mayo Hospital) in Lahore with a referral base from all over Pakistan but predominantly from the Punjab province. Period: Two years (from January 2016 to December 2017). Material & Methods: The study was based on immune-chromatographic assay of antibodies to Treponema pallidum in human serum using Nantong Egens Syphilis detection Kits. This is a qualitative test and provides rapid screening for syphilis in blood donors. Data was analyzed using SPSS 20 for prevalence study and mathematically for future prediction of its percentage. Results: A total of 76530 blood donors reported in the blood bank in two years. Out of these donors, 1720 were positive for TP antibody showing an overall percentage of 2.25%, 755 were positive in 2016 & 965 in 2017 with their percentages being 2.02% & 2.46% respectively. Conclusion: Percentage of syphilis has increased in year 2016 and 2017. This needs to be reduced by taking preventive measures and education to masses. Further work up of the patients who were TP positive is required.

    Abstract: Deep neck space infections are uncommon in infants. Retropharyngeal abscess (RPA) is a deep space neck infection that can present with subtle signs and symptoms. Delay in diagnosis can lead to life-threatening complications. Here we describe life-threatening complication of retropharyngeal abscess. Case presentation We report a life-threatening complication of retropharyngeal abscess in 10-month old Asian infant weighing 8.2 kg. The patient presented with fever, right-sided neck swelling, hoarseness of voice, and respiratory distress. The clinical and radiological findings were suggestive of airway obstruction complicated by retropharyngeal abscess. The patient was urgently taken to the operating room and underwent (grade 3) intubation. After stabilization, the patient underwent endoscopic trans-oral incision and drainage, during which 5 cc pus was aspirated. Antibiotics were prescribed for 2 weeks following the procedure based on the sensitivity result of the pus culture. Conclusion In retropharyngeal abscess, a delay in diagnosis can result in life-threatening complications. This report highlights the importance of prompt recognition of a threatened airway and the management of retropharyngeal abscess by emergency physicians. If an emergency physician suspects RPA with airway obstructions, the airway should be immediately secured in a secure environment by otolaryngologist and an anesthesiologist.

    Abstract: Objective: To assess the risk factors for breast cancer in patients attending oncology OPD of civil hospital Karachi, Pakistan. Introduction: Breast cancer is the single largest cause of death among women1,2. The probability of American woman developing breast cancer in their life is 7 in 11. Studies from subcontinent show that the incidence of breast cancer is increasing, with an estimated 80,000 new cases diagnosed annually. Breast cancer is the second most common type of cancer after lung cancer in Pakistan and ranked first in women. Only 10% women are diagnosed, out of them, 75% women do not get treatment and die within 5 years6. Data from Pakistan about the risk factors or association is not only scanty but also does not comment on the use of fatty diet in breast cancer patients. Method: A cross-sectional descriptive study conducted at Oncology OPD of civil hospital Karachi (CHK) from October 2009 -April 2011. One Hundred and Fifty consecutive patients having histopathalogical diagnosis of breast cancer were assessed for different risk factors that included marital status, parity, age, menopausal status, family history of breast cancer, prolong use of oral contraceptives, breast feeding, , early menarche, trauma to the breast and fatty diet. Result: Mean age of patients was 48 years. Three fourth (73%) of these female were above the age of 40 years. Consumption of fatty diet was found in 62.67% while positive family history of breast cancer was present in 34% of the cases. Early menarche and being nulliparous were not as strong risk factors as in previous studies. Conclusions: Our study has highlighted the need of further exploration in this area that would not only help this population but also enhance our understanding of different risk factors. This will have important implications for the overall management of breast cancer.

    Abstract: Background: Patients with acute ischemic stroke frequently test positive forhyperglycemia, which is associated with a poor clinical outcome. This association between poorglycemic control and an unfavorable prognosis is particularly evident in patients with persistenthyperglycemia, patients without a known history of diabetes mellitus (DM), and patients withcortical infarction. Objective: To determine the frequency of raised hemoglobin A1c in patientspresenting with ischemic stroke. Material & Methods: Study design: Retrospective study.Setting: Department of Medicine, Independent University Hospital, Faisalabad. Duration:6 months. Data collection procedure: Reports of 150 patients fulfilling selection criteriawere selected from medical record section of Independent University Hospital, Faisalabad.Demographic details were obtained. Then medical record of patients was assessed for presenceof HbA1c. If HbA1c of patients was reported ≥ 6.5%, then raised HbA1c was labeled. Results:In this study raised HbA1C level in patients presenting with ischemic stroke was 25%. Amongthese patients the most affected age group of patients was 41-60 years. Female patients weremore effected than male patients. Among these patients only 27% were diabetic while 73%were non-diabetic, 54% were smokers, 32% patients were hypertensive and in 14% patientshyperlipidemia was seen. All these factors were not significantly associated with raised HbA1clevel in patients presenting with ischemic stroke. Conclusion: Results of this study demonstratea high frequency of raised HbA1c level in patients presenting with ischemic stroke. i.e. 25%. Sokeeping in mind these findings it is recommended that stroke patients should also be screenedfor hyperglycemia or increased HbA1c so that patients can be prevented from development ofpermanent hyperglycemia or other complications associated with it.

    Abstract: This study aims to investigate and compare the characteristics of three topologies of moving-magnet linear oscillating actuator (LOA) based on their mover position. Positive aspects and consequences of every topology are demonstrated. Three topologies of axially magnetized moving-magnet LOA; outer mover, inner mover (IM) and dual stator (DS) are designed and examined. Due to its characteristically high thrust density and more mechanical strength, axially magnetized tubular permanent magnets (PMs) are used in these topologies. Design/methodology/approach LOAs are designed and optimized using parametric sweep, in term of design parameters and output parameters like thrust force, stroke and operating resonance frequency of the LOA. All the pros and cons of each topology are investigated and compared. Output parameters of the LOAs are compared using same size of the investigated LOAs. Mover mass, which plays a vital role in resonant operation, is analyzed for IM and DS designs. Investigated LOAs are compared with conventional designs of LOA for compressor in refrigeration system with regards of motor constant, stroke and thrust per PM mass. Findings This paper analyzes three topologies of moving-magnet LOAs. The basic difference between investigated LOAs is the radius of tubular-shaped mover from its central axis. All the design parameters are compared and concluded that thrust per PM mass of IMLOA is maximum. OMLOA provides maximum motor constant of value 180 N/A. DSLOA provides thrust force with motor constant 120 N/A and required intermediate materials of PMs. All the three designs give the best results in terms of motor constant and thrust per PM mass, compared to conventional designs of LOA. Originality/value This paper determines the impact of mover position from its central axis in a tubular-shaped moving-magnet LOA. This work is carried out in correspondence of latest papers of LOA.
