    Abstract: Syngnathia is a rare anomaly involving fusion between the maxilla and mandible. The fusion may be due to soft tissue adhesion between the two or true bony fusion between maxilla and mandible. This condition ranges from only soft tissue fusion between gums known as synechia to bony union known as synostosis with different grades of fusion. It was first reported in 1936, with only 41 cases reported till 2009. In this case a 11 years old girl reported to the maxillofacial department of Dhaka Dental College Hospital, with the complaint of inability to open the mouth and as a diagnosed case of alveolar bone fusion of mandible with palatal alveolar bone in the left side & bone fusion of alveolar process of mandibular angle region with maxilla in the right side. She was also bone fusion of the coronoid process with zygoma in the right side. Immediate after birth she had a history of synechia (Bilateral) & it was correct by making stoma by maxillofacial surgeon. The patient was subjected to a surgical procedure, after successful operation she enabled to open her mouth up to 35 mm.Medicine Today 2014 Vol.26(2): 114-117

    Abstract: Facial defect rehabilitation is a challenging task. Individualized technique is required for each patient to accomplish the task. Due to any cause loss of an eye with disfigurement of face makes severe physical and psychological distress. A 65-year-old male Patient complaint with missing left eye was referred to the Department of Prosthodontics, Military Dental Centre Dhaka for artificial eye replacement. A defect with compressed orbit and unharmed tissue bed was found by clinical examination. Mode of rehabilitation for the missing eye is ocular prosthesis. Here I tried to describe a simple technique for the fabrication of ocular prosthesis. With stock iris and customized sclera, we have fabricated a semi customized ocular prosthesis.     Abstract: Aims: Patients’ satisfaction is an important health outcome, which is regarded as a determinant measure for quality of health care services. The aim of the study is to evaluate patients’ satisfaction regarding eye care services in a multi-specialty eye hospital of northern Bangladesh. Methods: This cross sectional descriptive study was conducted among the patients who took the eye care services in OPD and admitted as in-patients in the hospital. Randomly selected patients were interviewed by trained staff. A semi-structured questionnaire was used to conduct these   interviews. Their responses were grouped into one of five categories and evaluated to determine satisfaction for different components of eye care services. There were also open-ended questions regarding feedback from participants’ view and recommendations for the improvement of the quality of services. Results: Four hundred and forty nine participants were interviewed. Among these, 71 (15.81%) were very satisfied and 358 (79.73%) were satisfied, constituting 95.54% satisfaction with overall services received at the hospital. 99.55% of respondents were satisfied with the doctors and 98.66% of respondents were satisfied with other hospital staff regarding patient care services. 86.1% patient had satisfaction with the cost of services, while 28.57% expressed that the surgery cost was high. Though the participants expressed dissatisfaction for the long waiting time in front of refraction and doctors’ consultation room during pick hours, 442 (98.4%) patients expressed their willingness to utilize the services again. Conclusion: Although eye care services both in OPD and in the IPD were satisfactory according to the respondents, there are scopes for improvement. Patient satisfaction surveys should be encouraged in hospitals for better accountability and also for strengthening the quality of eye care services.

    Abstract: The ideal method for buccal defects reconstruction should provide good outcome of both function and appearance; our goal is to highlight the reliability of radial forearm flap in buccal reconstruction. This retrospective study was conducted from July 2013 to June 2014, 04 radial forearm flaps were used to repair the defects. We analyzed the superiority and reliability of the flap; in addition, we reviewed some related literature. All radial forearm flaps totally survived.     Abstract: Introduction: Zygomatic bone is closely associated with the maxilla, frontal, temporal bones which are usually involved when a zygomatic bone fracture occurs. The most common causes of these fractures are from the assaults, road traffic accidents and falls. Aesthetic beauties and functions are greatly hampered with the displacement of the fractured fragments. Objective: To analyze the aetiology, clinical presentations, surgical intervention and complications associated with zygomatic complex (ZC) fractures in a tertiary level healthcare facility. Materials and Methods: This descriptive observational study was carried out in the Oral and Maxillofacial Department, Military Dental Centre, Dhaka during the period of January 2016 to December 2016. Data were obtained from clinical notes and surgical records of the patients using standardized data collection form specifically designed to examine the variables and features of zygomatic complex fractures. Results: Road traffic accidents were the leading cause (75%) of zygomatic fractures. The mean operation time was found 99.5±31.2 minutes with a range from 60 to 125 minutes. Open reduction and two-point internal fixation were employed in more than two-third (67.5%) of the cases. Four (10.0%) patients developed postoperative complications; among them 2(5.0%) cases suffered paresis of temporal branch of facial nerve and trismus. Other complications included paraesthesia, ectropion, scar and palpability of plate. Twenty-five (62.5%) patients were found having limited mouth opening preoperatively and postoperative improvement occurred in 21(52.5%). Thirty three (82.5%) patients had facial asymmetry preoperatively and postoperative improvement occurred in 29(72.5%). Conclusion: More than two-third of the patients received two point fixations. Only four patients out of forty in this study were found having post operative complications. The functional and Aesthetic outcome was found to be fair in terms of improvement in limitation of mouth opening and correction of facial asymmetry.     Abstract: Introduction: Mandibular third molar is most commonly dental impacted and it’s removal is not without morbidity. Post operative pain, swelling and trismus are universal. This study was carried out to compare the effect of co-administered corticosteroid and diclofenac Na+/K+ with diclofenac Na+/K+ alone on the post operative morbidity like pain, swelling and trismus after surgical removal of impacted lower third molars. Materials and Methods: The study was a prospective study done over a period of 01.01.13 to 30.06.14 in Department of Oral & Maxillofacial Surgery (OMS), Military Dental Center, Combined Military Hospital, Dhaka and the department of OMS Dhaka Dental College and Hospital, Dhaka. Sixty patients were included in the study and were randomly divided into two study groups: Control group received only diclofenac Na+/K+, Experimental group received corticosteroid and diclofenac Na+/K+. Pain, swelling and trismus were evaluated before, 1st, 2nd and 7th POD after surgery. Results: Among the 60 cases common type of impaction was horizontal and vertical type (30%) followed by Mesioangular (23%) and Distoangular (17%). All corticosteroid groups showed statistically significant (p 〈 0.01) improvement in pain and trismus at 1st and 2nd pod, but statistically significant (p 〈 0.01) improvement of swelling occur only on 2nd pod as compared to control group. Conclusion: Corticosteroid and diclofenac Na+/K+ combination was found effective for early recovery and the patients who used this combination suffered less pain, swelling and trismus after impacted mandibular third molar surgery.     Abstract: Perforations are significant iatrogenic complications of endodontic treatment and could lead to endodontic failure. The prognosis is generally excellent if the problem is well-diagnosed and repair is well-performed with a material that can provide proper sealing ability and biocompatibility. In this case report, the patient presented with the complaint of continuous dull pain in his right mandibular first molar tooth. The tooth was attempted for endodontic treatment and perforation has occurred at that time. Radiographic examination revealed, destruction in the furcal area with a little radiolucency and periradicular lesion involving apical region of the roots. Considering the above conditions, endodontic treatment and nonsurgical repair of furcal perforation with mineral trioxide aggregate followed by permanent restoration was planned and performed successfully. Follow up after one month, the patient was symptomless and the affected area was found healthy. However, long term clinico- radiographic follow-up is essential.     Abstract: Introduction: Temporomandibular joint disorders (TMDs) are among the most common disorders in the maxillofacial region which usually present with joint pain, decreased jaw mobility, joint clicking, headache, tinnitus, neck pain etc. There are different modalities of treatment used for the TMDs. These include some conservative and surgical procedures. The main aim is to improve the jaw function and reduction of joint pain. Arthroscopy is a modern diagnostic and effective therapeutic procedure for the TMDs. Objective: To assess the outcome of arthroscopic lavage of Temporomandibular Joint (TMJ) in terms of improvement of jaw function and joint pain in patients suffering from TMDs. Materials and Methods: This prospective study was carried out at the department of Oral and Maxillofacial Surgery, Military Dental Centre, Combined Military Hospital of Dhaka Cantonment from 01 October 2014 to 31 March 2016. Patients were selected consecutively having TMDs and unresponsive to conservative nonsurgical therapy. Total thirty patients of TMDs were selected for study and all the patients were treated with arthroscopic lavage. Objective data were collected including, inter incisal opening, lateral excursion, deviation on opening, tenderness on palpation. A questionnaire in the form of visual analogue scale relating to pain, joint clicking and Jaw mobility were completed by each patient at 1 week,1 month and 6 months post operatively. Results: Among thirty patients, 10 patients were male and 20 patients were female. Mean age of the patient was 42.6±5.79 years. Maximum patients were in the age group of 41-45 years. There was statistically significant improvement in maximum incisal opening and pain score. There was also reduction in Joint clicking and tenderness in maximum patients. Conclusion: The findings of the present study suggested potential utility of arthroscopic lavage in the management of TMDs. Journal of Armed Forces Medical College Bangladesh Vol.12(1) 2016: 88-93

    Abstract: Borage oil that is extracted from (Borago officinalis Linn.) is a well-known medicinal plant having various medicinal benefits. In this work, an affordable, simple, reliable, rapid and easily accessible high-performance thin-layer chromatography (HPTLC) method was developed for the estimation of gamma-linolenic acid (GLA) in borage oil. HPTLC method employs thin-layer chromatography (TLC) aluminum plates precoated with silica gel (G60F254) as the stationary phase, and the mixture of hexane:toulene:glacial acetic acid (3:7:1, v/v/v) was used as the mobile phase. Densitometric analysis of the TLC plates was carried out at 200 nm. The developed method showed well-resolved spots with retention factor (Rf) value of 0.53 ± 0.04 for GLA. Various experimental conditions like saturation time for chamber, solvent phase migration and width of the band were studied intensely for selecting the optimum conditions. The method validation was performed for parameters like linearity, accuracy, specificity and precision. The values of limit of detection and limit of quantification for GLA were found to be 0.221 and 0.737 μg/band, respectively. In nutshell, the developed HPTLC method was found to be highly sensitive for the estimation of GLA in the herbal oil samples and formulations.
