    Abstract: Objectives: To determine the relationship of Neutrophils to lymphocytes ratio (NLR) values with the benign and malignant thyroid nodules. Methods: In this cross-sectional clinical study conducted from September 1st 2020 to February 28th 2021, we included 216 patients who underwent thyroidectomy at Jinnah Postgraduate Medical Centre Karachi Pakistan. After thyroidectomy specimens were sent for pathologic examination. Patients were divided into two categories based on histopathologic results; Malignant nodule and benign nodules. Data of complete blood count was obtained from the pre-operative lab investigations and NLR was calculated. Results: There were 42 (26%) men, and 116 (74%) women of 158 in the BTN group, 18 (31%) men, and 40 (69%) women of 58 in the MTN group. The mean age of 48±6 years in the BTN group as well as 47 ±8 years in the MTN group (p-value 0.32). The mean neutrophil count in the BTN group was 4.26±2.8 versus 4.41±2.2 (x 1000/mm3) the malignance thyroid group (p-value = 0.71). The mean lymphocyte count was 3.81±0.9 (x 1000/mm3) in the BTN group and 3.61±1.2 (x 1000/mm3) in the malignance group (p-value = 0.18). The mean NLR value for the benign thyroid nodular group was 1.19±2.2 and 1.22±1.8 in the malignant thyroid nodular group (p-value = 0.92). Conclusion: According to the results of this study, we concluded that preoperative period biochemistry laboratory results such as neutrophils count, lymphocyte count, and NLR value don’t provide enough evidence to differentiate between benign and malignant thyroid carcinoma. doi: https://doi.org/10.12669/pjms.37.7.4503 How to cite this:Haider N, Mahmood Z, Khalid F, Razzak SA.     Abstract: Gastrointestinal bleeding can be manifested as a variety of symptoms and, often, it is difficult to classify as upper or lower gastrointestinal bleeding on mere symptomatology. This is a case report of a similar kind of patient who initially was diagnosed with fresh per rectum bleeding, subsequently diagnosed as bleeding angiodysplasia in duodenal diverticulum by a series of investigations and management including urgent oesophageal-gastroduodenoscopy (OGD), laparotomy, followed by computerized tomography-angiogram. As diagnosis was established after laparotomy, the patient was kept intubated and IR selective embolization was performed. Keeping this case report in view, it can be suggested that bleeding vascular malformation in any part duodenum should be considered as a cause of massive upper GI bleeding. Furthermore, if operative intervention is indicated, it should be preceded by OGD, not only for a therapeutic purpose but also as an adjunct for guidance for the operative plan.

    Abstract: Objective: The objective of the study is to determine better mode of learning for medical graduates by comparing team-based learning (TBL) and lecture-based learning methods. Study Design: Comparative analytical study. Place and Duration of Study: Surgical Ward 25 of Endocrine and General surgery, Jinnah Postgraduate Medical Center, Karachi, in April 2019. Methodology: This comparative study was based on the principles of TBL; the control program used the traditional lecture-based approach. Both programs were aimed at investigating the knowledge and performance of the two groups. Thirty surgical interns were included in this study. Two groups were made by random selection of surgical interns, 15 in TBL group and other 15 in traditional teaching group. TBL group (Group A) was given the topic of thyroid diseases for self-study followed by 1 h discussion amongst the group members. Lecture-based group (Group B) was given 1 h powerpoint presentation on similar topic. As the main outcome measures, questionnaire containing twenty best choice questions was given to both groups. Performance of the two groups was checked and results calculated as total, average, and standard deviation. Results: Group A participants’ total score (147) was higher than Group B (131) but the p-value was not found to be significant (0.144). Conclusion: Both forms of learning methods are effective and productive in medical education.

    Abstract: Abstract: Objective: The focus of this review is to summarize the recent advancement to understand the molecular pathogenesis of diabetic nephropathy (DN). Also, to highlight the role of abnormal aldosterone secretion on the development and progression of diabetic nephropathy. Background: Diabetic Nephropathy (DN) is a progressive disease of nephron due to slow progressive failure of kidney tubule to perform its filtration process. It is often associated with proteinuria and glomerular stiffening which eventually leads to low glomerular filtration rate, finally succumbs the patient toward the end stage of kidney disease. The abnormal level of aldosterone in diabetes mellitus is a fatal combination to combat because of progressive development of diabetic nephropathy. Methods: We reviewed the literatures for implications of aldosterone in diabetic nephropathy. The literatures that were related to different aspects of diabetic nephropathy in relation to aldosterone were analyzed and summarized in this review article. Result & Conclusion: Aldosterone, a mineralocorticoid of corticosteroid hormones releases under the influence of adrenocorticotrophic hormone (ACTH) from zona glomerulosa of adrenal gland of kidney. In normal physiological condition it regulates the renin-angiotensin system of kidney, which regulates the resorption and conservation of sodium (Na+), potassium (K+) ions and water (H2O) from distal tubule. Aldosterone secretion, despite an important regulator to maintain the equilibrium of water and ions in the body, its non-regulated higher secretion gives rise to various pathological conditions. Additionally, we also discuss the risk factors associated with the use of mineralocorticoid receptor antagonist and renin-angiotensin-aldosterone therapy, and suggesting the need of robust and controlled methods to administer these drugs.     Abstract: This study aimed to measure the impact of implementing mechanisms of accounting data governance, represented by International Accounting Standards, internal auditing, external auditing, audit committees, disclosure and transparency, and performance evaluation, on the quality of financial reporting data for the commercial banks listed on the Amman Stock Exchange, totaling (15) banks. To achieve the objectives of this study, a descriptive-analytical approach was adopted by developing a questionnaire to collect the primary data measuring the study variables. The questionnaire was distributed to employees in the financial and control departments of these banks, with a total of (375) respondents from the total study population of (733) individuals. Appropriate statistical methods were used to analyze the data, test hypotheses, and the results of this study revealed a strong positive impact of five variables of accounting data governance mechanisms on achieving the quality of financial reporting data. These variables are ranked from highest to lowest in terms of the strength of impact and correlation with the quality of financial reports: disclosure and transparency, external auditing, International Accounting Standards, internal auditing, and audit committees. However, there was no impact of the performance evaluation governance variable on achieving the quality of financial reporting data. These results call on the management of commercial banks in the study to commit to the objective implementation of the requirements of accounting data governance mechanisms as stipulated by international professional assemblies.

Clients will have direct access to Physical Therapists in all milieus for patient/client management, expectation, and wellness services. Physiotherapist will be practitioners of choice in clients’/patients’ health networks and will hold all rights of autonomous practice4. Physical Therapists may be assisted by Physical Therapy assistants, who are erudite and qualified to provide Physical Therapist–directed and controlled, components of intervention. Physical Therapy profession in Pakistan is rising with a great pace. Every passing minute brings extraordinary revolution in this field and now it is a high time to have some institution takes the responsibility on its shoulder to curtail the nourishing elements of quackery and bring autonomy and sovereignty to the field.

    Abstract: Malignant peritoneal mesothelioma is a rare cancer originating primarily from the peritoneum with a poor prognosis and non-specific clinical presentation. We present a case of a 60-year-old male, retired metallurgy engineer who initially presented with shortness of breath, lethargy, weight loss, vague abdominal pain and night sweats. Extensive workup for almost 2 months finally leads to the diagnosis of primary malignant peritoneal mesothelioma based on immunohistochemical analysis of loss of BAP1 gene. The patient was deemed non-suitable for surgical management and started on palliative carboplatin and pemetrexed. In conclusion, histological diagnosis is essential for peritoneal diseases before considering it as a metastasis from other primary tumours. Furthermore, immunohistochemical analysis and genetic profiling may also guide towards the diagnosis and possible treatment.
