Abstract:
Bệnh nhân sảng có thời gian nằm viện lâu hơn, tỷ lệ tử vong sau 6 tháng cao hơn bệnh nhân không sảng. Xác định, điều trị và phòng ngừa sảng ngày càng được coi là ưu tiên chính trong việc chăm sóc sức khỏe toàn diện bệnh nhân điều trị hồi sức tích cực vì nó là một biến chứng thường xuyên xảy ra, có thể lên đến 80% ở nhóm bệnh nhân phải thở máy. Để bổ sung vào nguồn dữ liệu về sảng ở Việt Nam, chúng tôi thực hiện nghiên với 2 mục tiêu: mô tả thực trạng sảng ở bệnh nhân thở máy và tìm hiểu một số yếu tố liên quan đến tình trạng sảng ở nhóm bệnh nhân này. Thiết kế nghiên cứu mô tả cắt ngang 34/84 bệnh nhân thở máy thỏa mãn điều kiện được đưa vào nghiên cứu trong 6 tháng (từ 03 đến 08/2022) tại Khoa Cấp cứu và Hồi sức tích cực - Bệnh viện Đại học Y Hà Nội. Kết quả nghiên cứu cho thấy tỷ lệ sảng trong nghiên cứu là rất cao (61,76%), chủ yếu là sảng giảm động (66,67%) và sảng tăng động là (33,33%); trung bình thời gian từ khi vào viện tới khi bị sảng là 12,19 ± 12,76 ngày; thời gian từ khi vào ICU tới khi bị sảng 6,14 ± 6,41 ngày; thời gian BN sảng nằm tại ICU trung bình khá cao (13,88 ± 8,42 ngày). Bệnh lý chính của các bệnh nhân có sảng hay gặp nhất là nhiễm trùng (42,86%). Đặc điểm nhân khẩu học, tình trạng bệnh lý không liên quan tới sảng. Điểm SOFA, số ngày sử dụng thuốc an thần có liên quan tới sảng với p 〈 0,05 và tỷ suất chênh lần lượt là OR = 1,42 (95%CI: 1,03 - 1,95) và OR = 1,56 (95%CI: 1,09 - 2,24). Tỷ lệ bệnh nhân thở máy bị sảng sảng chiếm tỷ lệ cao đặc biệt ở những bệnh nhân sử dụng thuốc an thần dài ngày, do đó cần đánh giá và theo dõi chặt tình trạng sảng ở những bệnh nhân này. Đặc biệt việc quản lý đau không tốt và rối loạn chu kỳ ngủ thức của bệnh nhân cũng làm tăng tỷ lệ mắc sảng ở bệnh nhân ICU. Ngoài ra, những bệnh nhân bị sảng cần phải theo dõi và đánh giá điểm SOFA của bệnh nhân để có can thiệp và xử trí kịp thời đảm bảo an toàn cho bệnh nhân.
