    Abstract: Personalized medicine has become an important direction to offer better solutions for health problems. In implantology, this trend was materialized through customizing dental abutments to each clinical situation. The demands for better esthetics and function of implant-supported restorations have imposed a more personalized variety of prosthetic abutments. This retrospective study compared clinical efficiency of personalized implant abutments with standard implant abutments in multiple implant restorations. Clinical data of patients who were admitted in a private clinic between 2011 and 2022 and received dental implant treatments were collected. All complications and undesired events from the patients’ medical record charts were statistically analyzed. The implants were loaded using either standard or customized abutments. For complete arch rehabilitations with the SKY Fast & Fixed protocol, standard titanium prosthetic abutments were used. Our results suggest that the abutments choice for patients has moved throughout the years more towards the use of customized abutments. The number of customized abutments (414) was higher compared with the number of standard abutments (293). In our database, the most used abutments for the anterior area implants were made of titanium and zirconia, whereas for the posterior area, the preferred abutments were mostly titanium. The standard abutments were used almost entirely for immediate loading and implantation in both anterior and posterior areas (Fast & Fixed protocol). Complications were encountered mainly in restorations with standard abutments (9.22%) compared to customized abutments (2.7%), with titanium abutments being the most reliable, having only 1.79% complications.

    Abstract: Recently, due to the high demand for dental implants, the use of dental implant stock abutments has increased significantly, especially dental custom CAD/CAM implant abutments milled by dental technicians in their laboratories. The purpose of this study is to analyze the surface quality of the emergence profile of dental custom CAD/CAM implant abutments made by a non-industrial milling machine, compared to original and compatible dental implant stock abutments made by industrial machines. Thirty dental implant abutments were divided into six study lots. Lot 1 (control group): original dental implant stock abutments—industrial machined; lot 2 (study group): compatible dental implant stock abutments—industrial machined; lots 3, 4, 5, and 6 (study groups): compatible custom CAD/CAM dental implant abutments—non-industrial milled with hyperDENT CAM software and Paragon Tools. The Nikon SMZ745T stereomicroscope was used to analyze the emergence profile surface of each dental implant abutment. The structure of the analyzed surfaces did not show significant differences between original and compatible abutments that were industrially machined. As for the customized dental implant abutments, the greatest similarity with the original was obtained for lot 6, and a significant statistical difference was obtained for lot 4. Stepover and Feed Rate parameters of the milling process influenced the surface roughness of the emergence profile for the customized dental implant abutments. The digital technology of non-industrial milling compatible custom CAD/CAM dental implant abutments is reliable and within the correct milling parameters.

    Abstract: BACKGROUND: Coronavirus disease 2019 (COVID-19) is a highly infectious disease. The pathogenic mechanism of COVID-19 pneumonia involved excessive immune reaction in the host - a "cytokine storm", that determined extensive tissue damage associated with coagulation abnormalities. Severity of this disease is associated with comorbidities such as hypertension, obesity, pulmonary disease or age. Thrombotic complications are reported during COVID-19 evolution even with prophylaxis. OBJECTIVE: In this study, we evaluate hematological and coagulation parameters in order to obtain predictors for an unfavourable evolution of the patient. DESIGN AND SETTING: We performed a prospective study that included all patients admitted in our hospital in Hematology, Pneumology, and Intensive Care Unit (ICU) Department at Colentina Clinical Hospital during April - July 2020. The study group included 144 patients that were split into ICU and non-ICU patients. All patients were SARS-CoV-2-positive by molecular test. The distribution according to gender was as follows: 67 male with median age: 61 (min 22, max 88) and 77 female with median age: 57 (min 17, max 92). RESULTS: Age is an important risk factor for the severity; 50 patients admitted in ICU with median age 67 (min 34, max 92) and 94 non-ICU patients with median age 52.5 (min 17, max 92), p=0.00003. Associated comorbidities were important and were present in both groups. In ICU patients, we obtained lower level of lymphocytes compared with non ICU group median: 1 x 103/L (min 0.04 x 103/L max 3.28 x 103/L) vs 1.57 x 103/L (min 0.39 x 103/L max 23.35 x 103/L), p=0.0001. There are no significant differences between groups for the rest of hematological parameters. The neutrophil/lymphocytes ratio (NLR) is with statistical difference between ICU and non-ICU groups: 2.34 (min 1.06, max 31.90) vs 7.94 (min 2.18, max 90.04), p & lt;0.000001. This indicator seems to be predictor for severe evolution; a high correlation with IL-6 level (r=0.73, p & lt; 0.001) was obtained. NLR in association with IL-6, CRP and ferritin level are important factors in severe evolution of COVID-19 (severe pneumonia-more than 50% of lung evaluated by CT-scan and presence of any complications during COVID-19 evolution), coefficient of determination-R2 =0.69, R2 - adjusted 0.67, p & lt;0.0001). The ICU patients with unfavourable evolution had a higher level of D-Dimers at the admission in hospital compared with ICU patients who were discharged from the hospital (3.42 mg/ml FEU vs 1.09 mg/ml FEU, p=0.01). Patients with thrombosis (stroke, myocardial infarction, deep venous thrombosis, thromboembolism) during COVID-19 evolution have higher level of D Dimers compared with patients without thrombotic complications (7.35 mg/ml FEU vs 0.82 mg/ml FEU), p=0.0001. There are no difference for another coagulation tests (APTT, Quick time) or Protein C, Protein S and Antithrombin III level. CONCLUSIONS: We conclude that NLR in association with feritin, CRP and IL-6 assessment are important to be evaluated in COVID-19 patient in order to expect a severe evolution of the disease. D- Dimer should be an important parameter to be evaluated for all COVID-19 patients in order to identify COVID-19 patients with high risk of thrombotic complications.     Abstract: The selection of patch test haptens contained in oral rehabilitation materials is not an easy task, therefore baseline series and additional dental series are used for screening and to increase the chance for the detection of patient's contact sensitizations. Moreover, the clinical relevance of positive patch tests must be evaluated. We conducted a biomedical literature search, covering a period of thirty years, using the keywords: contact, allergy, dental, allergens, patch, patients, screening, in order to evaluate many studies including large numbers of patients, tested to various hapten series, in different departments, for the detection of the most common contact allergens involved in dental contact hypersensitivity. We propose a short patch test series of oral rehabilitation materials for patients including six metal haptens present in dental alloys for inlays, crowns, bridges and dentures, one monofunctional methacrylate and one epoxy prepolymer present in dental composite resins, dentin bonding agents and prosthetic materials, one epoxy resin type present in root canal sealants, three odorant constituents contained in provisional restorative and root canal filling materials, periodontal dressings and dental impression materials. Contact allergy to dental haptens is a constant studied subject, but the cost of patch testing with screening and additional or extensive series is relevant, therefore the proposal of a shorter dental material series for patients is aimed to promote sustainability of oral rehabilitation and contact hypersensitivity care and to reduce costs in some real-world settings.

    Abstract: Zirconia is a metal used in dental implantology. Its biocompatibility was studied in vitro and in vivo, results of the studies being analyzed in reviews and meta analyses. The aim of this systematic review was to evaluate biocompatibility of zirconia in animal studies in vivo expressed as results of histomorphometric tests. Databases were searched from 1980 until February 2016, with different combination of the following MeSH terms: zirconium, biocompatibility, dental implants, in vivo, animal studies. Letters to the editors, case reports, commentaries, review articles and articles published in other languages then English were excluded. The search of PubMed, ScienceDirect and Google Scholar databases yielded 690 titles. After abstract screening and duplicate discarding 50 articles were identified and finally, 40 were included in the review. Most of the studies compared zirconia with titanium, a well established material for dental implants. In majority of the studies zirconia showed a similar osseointegration with titanium. Surface implant treatments, like sandblasted and etched zirconia (ZrO2-SLA), alumina toughed zirconia (ATZ), and powder injection molding (PIM) were used to improve osseointegration of zirconia with good results. In the light of histomorphometric test, zirconia, no matter physical and structural forms tested, is a biocompatible material.

    Abstract: Polycythaemia vera ( PV ), essential thrombocythaemia ( ET ) and primary myelofibrosis ( PMF ) represent typical myeloproliferative neoplasms ( MPN ), usually characterized by specific somatic driver mutations ( JAK 2 V617F, CALR and MPL ). JAK 2 46/1 haplotype and telomerase reverse transcriptase gene ( TERT ) rs2736100 A 〉 C single nucleotide polymorphism ( SNP ) could represent a large fraction of the genetic predisposition seen in MPN . The rs10974944 C 〉 G SNP , tagging the JAK 2 46/1 haplotype, and the TERT rs2736100 A 〉 C SNP were genotyped in 529 MPN patients with known JAK 2 V617F, CALR and MPL status, and 433 controls. JAK 2 46/1 haplotype strongly correlated to JAK 2 V617F‐positive MPN and, to a lesser extent, CALR ‐positive MPN . The TERT rs2736100 A 〉 C SNP strongly correlated to all MPN , regardless of the phenotype ( PV , ET or PMF ) and major molecular subtype ( JAK 2 V617F‐ or CALR ‐positive). While both variants have a significant contribution, they have nuanced consequences, with JAK 2 46/1 predisposing essentially to JAK 2 V617F‐positive MPN , and TERT rs2736100 A 〉 C having a more general, non‐specific effect on all MPN , regardless of phenotype or major molecular subtype.

    Abstract: Hypertension is worldwide spread, affects 40.41% of Romanians, and represents the leading cause of death. Epidemiological evidence suggests increasing proportion of elderly people. One half/two thirds of them have hypertension. Suboptimal adherence to treatment is considered to be the main impediment in controlling chronic diseases and the most common precipitating factors for hospitalization. Among the new concepts in 2018 ESC/ESH Guidelines for the Management of Arterial Hypertension is a strong emphasis on evaluating treatment adherence as a major cause of poor BP control. The aim of our study was to assess adherence to therapy in uncontrolled middle-old hypertensives admitted in an Internal Medicine Department Design and method: We enrolled 192 consecutive treated but uncontrolled patients with hypertension (BP 〉 140/90mmHg) and age between 75 and 85 years old. All patients completed the Morisky questionnaire for adherence to treatment. Results: 90 patients (53.57%) were females, and 78 (46.43%) males. Morisky score 0 (maximum adherence to therapy) was recorded in only 15.47% of the patients, 36.92 % had intermediate adherence (Morisky score 1–2) and 47.61% low adherence (Morisky score 3–4). Adherence to treatment was low in both sexes. Men had a sleight better maximum adherence than women (17.94% vs 13.33%). Women had a sleight lower low adherence to therapy than men (46.66% vs 50%). Conclusions: In-hospital middle-old uncontrolled patients with hypertension have a very poor adherence to therapy. Low adherence is present in both sexes. Since adherence to therapy is crucial in controlling chronic diseases, effective strategies to increase adherence to therapy are mandatory in uncontrolled middle-old hypertensives.
