    Abstract: Background.  Although it is the best method to detect early therapeutic failure, viral load (VL) monitoring is still not widely available in many resource-limited settings because of difficulties in specimen transfer, personnel shortage, and insufficient laboratory infrastructures. Dried blood spot (DBS) use, which was introduced in the latest World Health Organization recommendations, can overcome these difficulties. This evaluation aimed at validating VL measurement in DBS, in a laboratory without previous DBS experience and in routine testing conditions. Methods.  Human immunodeficiency virus (HIV)-infected adults were observed in a HIV care site in Hanoi, and each patient provided 2 DBS cards with whole blood spots and 2 plasma samples. Viral load was measured in DBS and in plasma using the COBAS Ampliprep/TaqMan and the Abbott RealTime assays. To correctly identify those with VL ≥ 1000 copies/mL, sensitivity and specificity were estimated. Results.  A total of 198 patients were enrolled. With the Roche technique, 51 plasma VL were ≥1000 copies/mL; among these, 28 presented a VL in DBS that was also ≥1000 copies/mL (sensitivity, 54.9; 95% confidence interval [CI], 40.3–68.9). On the other hand, all plasma VL & lt; 1000 copies/mL were also & lt;1000 copies/mL in DBS (specificity, 100; 95% CI, 97.5–100). With the Abbott technique, 45 plasma VL were ≥1000 copies/mL; among these, 42 VL in DBS were also ≥1000 copies/mL (sensitivity, 93.3%; 95% CI, 81.7–98.6); specificity was 94.8 (95% CI, 90.0–97.7). Conclusions.  The Abbott RealTime polymerase chain reaction assay provided adequate VL results in DBS, thus allowing DBS use for VL monitoring.

    Abstract: (1) Background: The present study measures the fear of COVID-19 among hospital healthcare workers and identifies several factors associated with increasing fear of COVID-19. (2) Methods: A cross-sectional, hospital-based survey was conducted on healthcare workforce recruited from the National Hospital of Tropical Diseases from 1 October 2021 and 20 October 2021. We selected the participants who have been directly involved in diagnosing, treating, or providing nursing care to patients with COVID-19. The primary data was collected via sending the invitation directly to the participants, utilizing structured self-completed questionnaires. The seven-item fear of COVID-19 scale was used to measure the data. The responses of 208 hospital healthcare workers were included in the final analysis. (3) Results: Total score of COVID-19 fear was 19.62 (SD = 5.22). The COVID-19 fear score of 7 items ranged from 2.38 (SD = 0.83) to 3.21 (SD = 0.96). The lowest and highest scores were the item ‘My hands become clammy when I think about Corona’ and the item ‘I am most afraid of corona’ was the highest, respectively. Linear regression of the COVID-19 fear showed that the factors positively correlated with the fear of COVID-19 among hospital healthcare workers were: being influenced by the community (p = 0.001), feeling at very high risk of COVID-19 (p = 0.03), and experiencing traumatic stress with an academic event (p = 0.042). (4) Conclusions: Although these findings merit further elaboration, these preliminary findings suggest relatively great fear of the COVID-19 pandemic among Vietnamese hospital healthcare workers and that social and personal connections are necessary for maintaining the mental wellbeing.

    Abstract: (1) Background: This study aims to assess the magnitude of, and factors associated with, depression and anxiety among Vietnamese frontline hospital healthcare workers in the fourth wave of COVID-19; (2) Methods: A hospital based cross-sectional study was carried out within two weeks, October 2020, at a central COVID-19 treatment hospital. Depression and anxiety were measured with PHQ-9 and GAD-7, respectively. Bivariate and multivariate logistic regression analysis were applied to recognize variables related to depression and anxiety, respectively; (3) Results: Among 208 frontline hospital healthcare workers, overall prevalence of depressive symptoms, anxiety symptoms, and both symptoms of depression and anxiety was 38.94%, 25.48% and 24.04%, respectively, in healthcare workers. In a reduced model after using multivariate stepwise logistic regression, age (OR = 0.9, p = 0.001), marital status (OR = 7.84, p = 0.027), profession (OR = 0.39, p = 0.028), having experienced traumatic stress following a work event (OR = 46.24, p 〈 0.001), feeling at very high risk for COVID-19 (OR = 0.02, p 〈 0.04), and affected by workplace conditions (OR = 5.36, p 〈 0.001) were associated with the symptoms of depression. With regard to symptoms of anxiety, single status (OR: 12.18, p = 0.002), being medical technician (OR: 68.89, p 〈 0.001), alcohol use (OR: 6.83, p = 0.014), using pain relief medications (OR: 25.50, p = 0.047), having experienced traumatic stress following a family event (OR: 130.32, p = 0.001), having experienced traumatic stress following a work event (OR: 181.55, p = 0.002), reporting at very high risk for COVID-19 (OR: 29.64, p = 0.011), treating moderate (OR: 6.46, p = 0.038) and severe (OR: 18.96, p = 0.004) COVID-19 patients, and being significantly affected by the community (OR: 6.33, p = 0.003) were increased risk factors for the symptoms of anxiety. Meanwhile, those living with 4–5 people (OR: 0.15, p = 0.011), specializing in infectious disease (OR: 0.13, p = 0.044)/resuscitation and emergency medicine (OR: 0.04, p = 0.046), and having knowledge preparation before participating in COVID-19 (OR: 0.008, p = 0.014) were less associated with the symptoms of anxiety; (4) Conclusions: There was a relatively high prevalence among Vietnamese hospital healthcare workers exhibiting symptoms of depression and anxiety during the ongoing pandemic. Greater attention to training in psychological skills should be suggested for those belonging to a younger age group, being single/widowed/divorced, treating moderate and severe COVID-19 patients, feeling at very high risk for COVID-19, being significantly affected a lot the community or workplace conditions, or experiencing traumatic stress following a family/work event in the past week.

    Abstract: Background and Aim: Many studies have reported on the phenomenon of co-infections involving two or more pathogens (bacteria or viruses) over the past few years. However, very few studies on this issue were conducted in Vietnam. Therefore, this study aimed to determine the circulation of single and multiple porcine parvovirus (PPV) (e.g., PPV1, PPV2, PPV3, and PPV4), porcine bocavirus (PBoV), and torque teno virus (TTV) (TTV1 and TTV2) infections in Vietnamese pigs. Materials and Methods: A total of 174 porcine circovirus 2-positive samples from pigs (n = 86 for 2017 and n = 88 for 2021), including from the sera and internal organs, across 11 provinces were examined by polymerase chain reaction. Results: This study demonstrated the wide distribution of DNA viruses among pig farms in Vietnam in 2021, with the detection rate for PPV ranging from 3.4% to 27.3% among PPV1-PPV4. Moreover, the detection rates of TTV genotypes were confirmed to be 14.8% (TTV1) and 63.6% (TTV2), respectively, and the positive rate of PBoV was 65.9%. The most frequent combinations were double and triple infections. Double infection was found in 16/86 (18.6%) in 2017 and 26/88 (29.5%) in 2021, while triple infection was found at 19/86 (22.1%) in 2017 and 26/88 (29.5%) in 2021. The incidence of simultaneous detection of more than three viruses was low. Conclusion: These results provide at least partial information about the occurrence of three viruses, including PPV (including PPV1 to 4), PBoV, and TTV (TTV1 and TTV2), in pigs. Determination of particular viruses in pigs will help to prevent the porcine respiratory disease complex caused by DNA viruses in Vietnamese pigs in the future.

    Abstract: Introductions: Pre-hospital care, especially for severe trauma which contributes to reduce the risk of morbidities and mortalities. We conducted a study for having an over view of the situation of pre-hospital emergency in Hanoi city through traumatic patients have been treated at Dong Da General Hospital, one of the second level hospitals in Hanoi. Materials and methods: All traumatic patients have been admitted at the emergency department of Dong Da General Hospital during the period from March 2022 to October 2022, regardless of gender, age, had complete medical records, and including severe cases were discharged to die or dead in hospital. Data were collected by Epidata 3.1 and processed using SPSS 20.0 software. Results: Characteristics of target population: A total of 236 cases of traumatic patients were enrolled, of them, the ISS scores in the mild range accounted for the majority of 97.0%; Lesion located in limbs and pelvis accounts for the largest proportion of 43.1%. Current situation of pre-hospital care: Patients who did not receive first aid before coming to the hospital accounted for 71.6%, the first aid people were mainly accompanied by the person next to patient at the scene (65.7%). Transportation by ambulance accounts for only 1.7%, the average waiting time was 4.54 7.68 minutes. Patients receiving first aid to step Disability (D) accounted for the largest proportion with 59.7%, 34.3% of patients received first aid to step Exposure (E). Wound washing and bandaging are the two most commonly used techniques, accounting for 28.6%, with 1 patient requiring CPR accounting for 1.0%. Conclusions and recommendations: Through the study of pre-hospital care for trauma showed that most patients were initially managed by the person accompanying the patient. A low percentage of ambulance transportation can be potentially high risk for the patient. We recommend strengthening the network of collaborators and communities for pre-hospital care in emergency.

    Abstract: Up to now, the disease situation caused by COVID-19 has spread to most areas of the world with high rates of infection and deaths. Facing the complicated developments of the epidemic, healthcare workers (HCWs) adherence of COVID-19 prevention can be directly influenced by inadequate knowledge and negative attitude. The study aimed to learn about the knowledge, attitudes and practices of COVID-19 epidemic prevention and some factors related among HCWs at Dong Da General Hospital, 2020. Method: This was a cross-sectional study conducted among 295 HCWs in Dong Da General Hospital from 04/2020 to 07/2020. Results: Majority of participants (94.9%) had good knowledge and positive attitude (96.6%) to epidemic prevention but the proportion of healthcare workers who have good preventive practices was not significant (62.0%) of COVID-19. Healthcare workers with good knowledge, working at clinical departments tend to have better preventive practices toward COVID-19 (p = 0.002 and p = 0.002). Conclusions: The percentage of healthcare workers with good practice toward COVID-19 prevention was not significant (62.0%). Appropriate practice among healthcare workers corelates directly with factors such as working department and knowledge towards COVID-19 infection.     Abstract: Nhiễm HIV/AIDS là gánh nặng về bệnh tật cho người bệnh và gia đình bệnh nhân cũng như toàn xã hội. Điều trị bằng các thuốc (ARV-anti retroviral drugs) đang là biện pháp điều trị tối ưu cho nhóm bệnh nhân nhiễm HIV/AIDS. Bộ Y tế Việt Nam mới đây phê chuẩn và khuyến cáo sử dụng phác đồ TDF+3TC+DTG. Mục tiêu: Đánh giá hiệu quả điều trị và tác dụng không mong muốn của phác đồ TDF + 3TC + DTG trên bệnh nhân HIV/AIDS tại bệnh viện đa khoa Đống Đa. Đối tượng: Nghiên cứu trên 61 bệnh nhân được chẩn đoán nhiễm HIV/AIDS và được điều trị bằng phác đồ TDF+3TC+DTG theo dõi đủ 24 tuần, trong thời gian từ 2/202- đến 6/2021. Phương pháp: nghiên cứu mô tả cắt ngang hồi cứu kết hợp tiến cứu. Kết quả: Cân nặng, BMI và số lượng CD4 tăng có ý nghĩa thống kê (p 〈 0,05) tại thời điểm sau 24 tuần điều trị. Có 45/61 (73,8%) bệnh nhân đạt mức vi rút ức chế tối đa (dưới ngưỡng phát hiện). Tuân thủ điều trị có liên quan đến kết quả điều trị (OR=2,32 CI95% 0,21-8,643, p 〈 0,05). Kết luận: Phác đồ thuốc ARV TDF+3TC+DTG có hiệu quả điều trị tốt trên bệnh nhân nhiễm HIV/AIDS (cân nặng, CD4 tăng sau 24 tuần điều trị và vi rút được ức chế đạt tỷ lệ cao). Nhóm bệnh nhân tuân thủ điều trị tốt có khả năng ức chế vi rút ở mức tối đa cao hon 2,32 lần với nhóm tuân thủ điều trị không tốt.

    Abstract: Mục tiêu: Đánh giá hiệu quả điều trị phác đồ Sofosbuvir/ Dalatasvir ở đối tượng nhiễm HCV mạn tính  đồng nhiễm HIV tại Bệnh viện Đa khoa Đống Đa, Hà Nội. Đối tượng và phương pháp: Nghiên cứu mô tả, tiến cứu 60 đối tượng đồng nhiễm HCV/HIV được điều trị phác đồ Sofosbuvir/Daclatasvir từ tháng 6 năm 2018 đến tháng 5 năm 2020. Kết quả: Tại thời điểm kết thúc theo dõi, 100% bệnh nhân giai đoạn lâm sàng 1 , các triệu chứng lâm sàng và nhiễm trùng cơ hội thay đổi rõ rệt ngay sau 4 tuần điều trị, 100% bệnh nhân theo dõi không có biểu hiện lâm sàng và nhiễm trùng cơ hội ngay tại T4 và ổn định các thời điểm sau đó. Số lượng tế bào CD4 trung bình tại thời điểm kết thúc theo dõi tăng rõ rệt với thời điểm bắt đầu điều trị (p 〈 0,01). Tải lượng HIV-RNA tại thời điểm kết thúc theo dõi nhóm xơ gan và viêm gan 100% đạt mức dưới ngưỡng phát hiện. Đáp ứng vi rút bền vững sau khi kết thúc điều trị 12 tuần (SVR 12) của cả 2 nhóm là 100%. Giá trị trung bình các enzym gan các thời điểm sau điều trị giảm so với thời điểm T0 (p 〈 0,01). 100% bệnh nhân có enzyme gan bình thường tại thời điểm SVR12. chỉ số APRI giảm rõ rệt so với thời điểm T0 (p 〈 0,01); căn cứ vào kết quả Fibroscan, tỷ lệ viêm gan và xơ gan không đổi.
