Abstract: This article describes the surgical management of a young, female patient with severe gingival enlargement of unknown etiology. Background Gingival enlargement frequently occurs as gingival hyperplasia, representing a reaction to a known stimulus or agent and, histopathologically, implies an increase in both extracellular matrix and cell numbers. The enlargement may range from mild, in which gingival architecture is minimally affected, to severe, in which the gingiva becomes bulbous and covers the clinical crowns of teeth. A number of local and systemic factors, such as plaque, hormonal changes, drug ingestion, and heredity, can cause or influence gingival enlargement. Mild to moderate increase in gingival bulk is relatively common, but massive gingival enlargement with associated bone resorption is rare. Case Description This case involved a 19-yearold female patient who presented with generalized severe gingival enlargement with aggressive periodontitis, a condition of some five to six months in duration. Results Based on a thorough clinical and radiographic examination, laboratory tests, and oral hygiene instructions, an internal bevel gingivectomy was performed to remove excess gingival tissue and areas of bone loss were debrided properly. There was no recurrence eight months following the last surgery. Summary Although we were not able to identify the exact cause of the gingival enlargement, amelioration of the unusual very soft, friable, enlarged gingivae and the severe periodontal attachment loss was observed. Clinical Significance Before initiating any periodontal management of a case of severe gingival enlargement with aggressive periodontitis, it is recommended to perform a complete extraoral and intraoral examination with radiographs, take a family and medical history, and determine if any medications may be responsible for the gingival enlargement. Additional testing and analysis, as described in this case, may be necessary. Citation Vishnoi SL, Phadnaik MB. Unusual Gingival Enlargement with Aggressive Periodontitis: A Case Report. Abstract: Introduction: Pericoronitis refers to inflammation of the gingiva in relation to the crown of a partiallyerupted tooth. It mostly affects third molars. It may be acute, subacute or chronic.Aims: To study the association of angular position of third molar and to evaluate predisposing factorsassociated with occurrence of pericoronitis in third molars.Materials and Methods: The sample consisted of 92 patients with 110 sites affected by pericoronitis.Clinico-radiographic examination was carried out. Clinical findings consisted of age, sex, amount oferuption, predisposing factors signs and symptoms. Radiographic examination was done to assessangular position using Winter’s classification.Statistical Analysis: By using Descriptive statistical analysis, findings were summarized using numbersand percentages,Results: The peak age of occurrence of third molar pericoronitis was 23-28 years with femalepredominance. Mandibular molars were involved more commonly. Vertical positioned and partiallyerupted third molars were most commonly affected by pericoronitis in both arches. Upper respiratorytract infection was most frequently associated predisposing factor with periocoronitis. Conclusion: Vertically positioned partially erupted third molars are more likely to be affected by frompericoronitis. Considering high risk, early treatment should be rendered.

Abstract: Introduction:-The frenum is a mucous membrane fold that attaches the lip , cheek and the tongue to the alveolar mucosa, the gingiva, and the underlying periosteum .Aesthetic problems are caused by high maxillary frenum which may compromise the orthodontic treatment results in the midline diastema cases, thus causing a recurrence after the treatment. Ankyloglossia, is also called as tongue-tie, is an embryological anatomical malformation of the tongue, characterized by an abnormally short and a thick lingual frenum. Tongue-tie restricts the physiologic movements of the tongue and results in various functional, behavioural and speech abnormalities. Material method :- The present article consists of a series of clinical cases of an aberrant frenum which were approached by various surgical frenectomy techniques. Result:- Different techniques have their own advantages and disadvantages. Laser gives advantages of less post operative pain and discomfort. Lingual frenectomy help in improvement in tongue movement and speech. Conclusion:- An aberrant frenum can be removed by any of the modification techniques that have been proposed, and a functional and an aesthetic outcome can be achieved by a proper selection of technique which is based on the type of the frenal attachment.
