Abstract:
- Đặt vấn đề: Nuôi ăn sớm qua đường miệng ở bệnh nhân sau phẫu thuật nội soi cắt đại tràng chương trình là một phần trong chương trình hồi phục sớm sau mổ giúp bệnh nhân nhanh hồi phục sau phẫu thuật từ đó rút ngắn thời gian nằm viện của bệnh nhân.
- Mục tiêu: Nghiên cứu nhằm khảo sát thời điểm phục hồi nhu động ruột, tai biến biến chứng và số ngày nằm viện của bệnh nhân cho ăn sớm qua đường miệng sau phẫu thuật nội soi (PTNS) cắt đại tràng chương trình. - Đối tượng và phương pháp nghiên cứu: Nghiên cứu tiến cứu, mô tả loạt ca trên đối tượng là bệnh nhân được áp dụng phương pháp nuôi ăn đường miệng sớm sau PTNS cắt đại tràng chương trình từ tháng 01/2019 đến tháng 05/2020 tại khoa ngoại tiêu hóa bệnh viện Bình Dân
- Kết quả: Trong nghiên cứu có 72 trường hợp thỏa tiêu chuẩn chọn vào, độ tuổi trung bình là 58,36 ± 11,899 tuổi, trong đó có 43 TH là nam (59,7%). Có 2 TH được rút thông mũi dạ dày (TMDD) ngay trong phòng mổ, tất cả TH còn lại đều được rút sớm trong vòng 24 giờ. Thời gian trung vị trung tiện lần đầu và đi tiêu sau mổ lần lượt là 43,24 ± 15,149 giờ và 4,36 ± 1,303 ngày. Tỷ lệ biến chứng chung sau mổ là 26,4%, trong đó có 4 TH xì miệng nối sau mổ, không có TH nào mổ lại hay tử vong. Thời gian nằm viện trung vị là 7 ngày. - Kết luận: Nuôi ăn sớm qua đường miệng ở bệnh nhân sau PTNS cắt đại tràng chương trình là một phương pháp an toàn và hiệu quả có thể được áp dụng thường quy cho tất cả các trường hợp PTNS cắt đại tràng chương trình.
Abstract - Background: Early oral nutrition in patients after elective laparoscopic colectomy is part of an early recovery after surgery (ERAS) to help patients recover quickly from surgery, thereby shortening the hospital stay.
- Objective: Investigate the time of intestinal motility recover y, complications and hospital stay of patients with early oral feeding after elective laparoscopic colectomy.
- Methods: Prospective study, cases serie on patients who were applied early oral nutrition after elective laparoscopic colectomy from January 2019 to May 2020 at the Department of Gastroenterology, Binh Dan Hospital - Results: In the study, there were 72 cases that met the inclusion criteria, the average age was 58.36 ± 11,899 years old, of which 43 subjects were male (59.7%). There were 2 cases who were withdrawn the nasogastric tube in the operating room, all remaining cases were withdrawn early within 24 hours. The median time of first bowel movement and postoperative bowel movement was 43.24 ± 15.149 hours and 4.36 ± 1.303 days, respectively. The overall complication rate after surgery was 26.4%, of which there were 4 cases of postoperative anastomosis, none of which were re - operated or died. Median hospital stay was 7 days.
- Conclusion: Early oral nutrition in patients after elective laparoscopic colectomy is a safe and effective method that can be routinely applied to all cases of elective laparoscopic colectomy.
