    Abstract: Mechanical thrombectomy (MT) in ischemic stroke patients with poor prestroke conditions remains controversial. We aimed to analyze the frequency of previously disabled patients treated with MT in clinical practice, the safety and clinical response to MT of patients with preexisting disability, and the disabled patient characteristics associated with a better response to MT. Methods: We studied all consecutive patients with anterior circulation occlusion treated with MT from January 2017 to December 2019 included in the Codi Ictus Catalunya registry—a government-mandated, prospective, hospital-based data set. Prestroke disability was defined as modified Rankin Scale score 2 or 3. Functional outcome at 90 days was centrally assessed by a blinded evaluator of the Catalan Stroke Program. Favorable outcome (to return at least to prestroke modified Rankin Scale at 90 days) and safety and secondary outcomes were compared with patients without previous disability. Logistic regression analysis was used to assess the association between prestroke disability and outcomes and to identify a disabled patient profile with favorable outcome after MT. Results: Of 2487 patients included in the study, 409 (17.1%) had prestroke disability (313 modified Rankin Scale score 2 and 96 modified Rankin Scale score 3). After adjustment for covariates, prestroke disability was not associated with a lower chance of achieving favorable outcome at 90 days (24% versus 30%; odds ratio, 0.79 [0.57–1.08]), whereas it was independently associated with a higher risk of symptomatic intracranial hemorrhage (5% versus 3%; odds ratio, 2.04 [1.11–3.72] ) and long-term mortality (31% versus 18%; odds ratio, 1.74 [1.27–2.39]) compared with patients without disability. Prestroke disabled patients without diabetes, Alberta Stroke Program Early CT Score 〉 8 and National Institutes of Health Stroke Scale score 〈 17 showed similar safety and outcome results after MT as patients without prestroke disability. Conclusions: Despite a higher mortality and risk of symptomatic intracranial hemorrhage, prestroke-disabled patients return as often as independent patients to their prestroke level of function, especially those nondiabetic patients with favorable early ischemic signs profile. These data support a potential benefit of MT in patients with previous mild or moderate disability after large anterior vessel occlusion stroke.

    Abstract: Lakes are integrators of past climate and ecological change. This information is stored in the sediment record at the lake bottom, and to make it available for paleoclimate research, potential target sites with undisturbed and continuous sediment sequences need to be identified. Different geophysical methods are suitable to identify, explore, and characterize sediment layers prior to sediment core recovery. Due to the high resolution, reflection seismic methods have become standard for this purpose. However, seismic measurements cannot always provide a comprehensive image of lake-bottom sediments, e.g., due to lacking seismic contrasts between geological units or high attenuation of seismic waves. Here, we developed and tested a complementary method based on water-borne electrical-resistivity tomography (ERT) measurements. Our setup consisted of 13 floating electrodes (at 5 m spacing) used to collect ERT data with a dipole–dipole configuration. We used a 1D inversion to adjust a layered-earth model, which facilitates the implementation of constraints on water depth, water resistivity, and sediment resistivity as a priori information. The first two parameters were readily obtained from the echo-sounder and conductivity-probe measurements. The resistivity of sediment samples can also be determined in the laboratory. We applied this approach to process ERT data collected on a lake in southern Mexico. The direct comparison of ERT data with reflection seismic data collected with a sub-bottom profiler (SBP) showed that we can significantly improve the sediment-thickness estimates compared to unconstrained 2D inversions. Down to water depths of 20 m, our sediment thickness estimates were close to the sediment thickness derived from collocated SBP seismograms. Our approach represents an implementation of ERT measurements on lakes and complements the standard lake-bottom exploration by reflection seismic methods.

    Abstract: Abstract. Karst water resources play an important role in drinking water supply but are highly vulnerable to even slight changes in climate. Thus, solid and spatially dense geological information is needed to model the response of karst hydrological systems to such changes. Additionally, environmental information archived in lake sediments can be used to understand past climate effects on karst water systems. In the present study, we carry out a multi-methodological geophysical survey to investigate the geological situation and sedimentary infill of two karst lakes (Metzabok and Tzibaná) of the Lacandon Forest in Chiapas, southern Mexico. Both lakes present large seasonal lake-level fluctuations and experienced an unusually sudden and strong lake-level decline in the first half of 2019, leaving Lake Metzabok (maximum depth ∼25 m) completely dry and Lake Tzibaná (depth ∼70 m) with a water level decreased by approx. 15 m. Before this event, during a lake-level high stand in March 2018, we collected water-borne seismic data with a sub-bottom profiler (SBP) and transient electromagnetic (TEM) data with a newly developed floating single-loop configuration. In October 2019, after the sudden drainage event, we took advantage of this unique situation and carried out complementary measurements directly on the exposed lake floor of Lakes Metzabok and Tzibaná. During this second campaign, we collected time-domain induced polarization (TDIP) and seismic refraction tomography (SRT) data. By integrating the multi-methodological data set, we (1) identify 5–6 m thick, likely undisturbed sediment sequences on the bottom of both lakes, which are suitable for future paleoenvironmental drilling campaigns, (2) develop a comprehensive geological model implying a strong interconnectivity between surface water and karst aquifer, and (3) evaluate the potential of the applied geophysical approach for the reconnaissance of the geological situation of karst lakes. This methodological evaluation reveals that under the given circumstances, (i) SBP and TDIP phase images consistently resolve the thickness of the fine-grained lacustrine sediments covering the lake floor, (ii) TEM and TDIP resistivity images consistently detect the upper limit of the limestone bedrock and the geometry of fluvial deposits of a river delta, and (iii) TDIP and SRT images suggest the existence of a layer that separates the lacustrine sediments from the limestone bedrock and consists of collapse debris mixed with lacustrine sediments. Our results show that the combination of seismic methods, which are most widely used for lake-bottom reconnaissance, with resistivity-based methods such as TEM and TDIP can significantly improve the interpretation by resolving geological units or bedrock heterogeneities, which are not visible from seismic data. Only the use of complementary methods provides sufficient information to develop comprehensive geological models of such complex karst environments

    Abstract: Introducción: Los determinantes sociales de la salud aluden al impacto que tienen sobre la salud y la enfermedad en la población diversos componentes demográficos. Objetivo: identificar los determinantes sociales de salud que influyen en la complicación de pacientes hipertensos y diabéticos que padecen insuficiencia renal, Metodología: estudio de tipo descriptivo, transversal, con enfoque cuantitativo en una muestra de 86 pacientes. Resultados: en el componente Biología Humana, prevalecieron pacientes entre 41 y 85 años de edad predominantemente en la década 60-69 años, sexo masculino; raza mestiza, con ambas patologías y mayor incidencia hipertensos, IRC en nivel de riesgo III y alto frecuencia de estrés. En el componente socio-ambiental el ingreso económico y el nivel educativo fueron bajos repercutiendo en escasas oportunidades laborales, no se apreció que los sistemas de saneamiento básico e infraestructura de las viviendas tengan significación sobre las complicaciones presentadas. El componente Estilo de Vida es el que presenta la mayor frecuencia, no solo por las conductas de los pacientes sino por la influencia de las condiciones económicas y sociales, que repercuten en una dieta adecuada. El componente Procesos Administrativos de Salud presenta la frecuencia más baja y los usuarios manifiestan una buena satisfacción con la calidad de la atención de la institución. La frecuencia observada para los determinantes sociales fue para Estilo de Vida 53,49%; Socio-Ambiente 18,60%; Biología Humana 17,44% y Sistema de Salud 10,47%. Conclusión. El estilo de vida es el determinante con mayor influencia y el Sistema de Salud es el determinante con menor influencia sobre las complicaciones en los pacientes hipertensos y diabéticos que presentan insuficiencia renal crónica.

    Abstract: Más de un millón de personas mueren en accidentes de tránsito cada año en todo el mundo. Según la Organización Mundial de la Salud (OMS), este tipo de muertes son comparables a la crisis global producto del VIH o el SIDA. Las cifras que manejan la OMS y el Banco Mundial indican que otras 50 millones de personas resultan heridas o discapacitadas por tales accidentes cada año. Objetivo: Conocer la atención integral de las secuelas físicas en pacientes de 18 a 25 años víctimas de accidentes de tránsito atendido en la clínica San Juan de Dios en la ciudad de Cartagena durante el año 2011 a través de la realización de encuestas, para establecer pertinencia y oportunidad de la misma. Materiales y Métodos: Estudio descriptivo cualitativo, transversal, con muestreo no probabilístico por conveniencia, se entrevistaron 61 pacientes de ambos sexos, con edades entre 18 y 25 años, que cumplieron con los criterios de inclusión definidos. Resultados: Se encontró que la mayoría de la población era de sexo masculino, que el mayor grupo de edad estuvo en las personas de 22 a 24 años, no tuvieron ninguna secuela permanente. Conclusiones: Ninguno sufrió secuelas permanentes, así como se evidenció que la atención no fue integral.

    Abstract: Identificar la asociación entre obesidad gestacional y desenlaces perinatales adversos en una población del nororiente colombiano.Pacientes y Método: Estudio multicéntrico transversal con relación prospectiva de datos. Se seleccionaron pacientes que consultaron y finalizaron su embarazo en la Clínica Materno Infantil San Luis y el Hospital Universitario de Santander entre Enero-2019 y Marzo-2020. Se clasificó el estado nutricional según la curva de Rosso-Mardones y se incluyeron gestantes obesas y normales. El desenlace primario fue: prematurez, y los secundarios: restricción del crecimiento intrauterino, grande para la edad gestacional e hipoglucemia neonatal transitoria. Se realizó el análisis por subgrupo de gestantes sin comorbilidades. Se estimó la fuerza de asociación mediante regresión binomial (RR) y su intervalo de confianza del 95%, ajustado por la edad materna como variable de confusión según un modelo contrafactual. Se consideró significancia estadística una p 〈 0,05.Resultados: Fueron incluidas 283 gestantes obesas y 276 con estado nutricional normal. Se encontró asociación entre obesidad gestacional y el nacimiento pretérmino (RR 2,5; IC95% 1,4- 4,2), hipoglucemia neonatal transitoria (RR 7,1; IC95% 2,1-23,7) y grande para la edad gestacional (RR 6,6; IC95% 3,3-13,1). Hallazgos similares se encontraron en gestantes sin trastorno hipertensivo asociado al embarazo, ni diabetes.Conclusiones: La obesidad gestacional se asocia con la prematurez, recién nacido grande para la edad gestacional e hipoglucemia neonatal transitoria, incluso en aquellas gestantes que no cursan con trastorno hipertensivo asociado al embarazo o diabetes.

    Abstract: Introduction: Prematurity causes great morbidity and mortality in childhood, which is why the identification and prevention of risk factors is essential. Objective: To identify the association of non-infectious risk factors with prematurity in two reference centers in northeastern Colombia. Patients and Methods: Multicenter prospective cohort study carried out between January 2019 and March 2020. A non-probabilistic convenience sampling was carried out in pregnant women with a single pregnancy, greater than 20 weeks who were then followed up monthly until the moment of birth. Preterm birth was considered as an outcome variable, after the normality analysis, based on Shapiro-Wilk Test; Chi2 or Fisher tests were performed for qualitative variables, and t-test or Wilcoxon test for quantitative variables, p 〈 0.05 was considered significant and OR was calculated by regression logistics. Results: 559 pregnant women were studied, with 63 preterm births. An association was found between prematurity and gestational obesity (OR 2.71; 95% CI 1.52-4.81), hypertensive disorder associated with pregnancy (OR 3.88; 95% CI 2.18-6.90), premature rupture of membranes (OR 3.86 95% CI 1.60-9.29) and previous preterm labor in the current pregnancy (OR 17.39 95% CI 9.57-31.51). Conclusions: Prematurity is significantly associated with gestational obesity, pregnancy-associated hypertensive disorder, premature rupture of membranes, and previous preterm labor in current pregnancy. Further studies would be needed to support these findings.

    Abstract: Las enfermedades crónicas no transmisibles, incluido el cáncer, están sobrepasando a las enfermedades infecciosas como la principal amenaza de salud en países como Colombia. La economía colombiana está creciendo rápidamente, conduciendo al aumento en la calidad de vida. Este crecimiento se acompaña de cambios como: aumento del tabaquismo, estilos de vida sedentarios, consumo de alcohol, hábitos alimentarios no saludables, mayor exposición a contaminantes ambientales cancerígenos y envejecimiento de la población, lo que explica el aumento de los casos de cáncer relacionados con factores de riesgo prevenibles. Esto diverge de lo encontrado en países desarrollados, en donde la mortalidad específicamente para el cáncer gástrico muestra tendencia al descenso, ya sea como resultado de su detección temprana, por intervenciones preventivas, o por acceso a terapias efectivas. Por esto es importante desarrollar estudios que ayuden a determinar y caracterizar todas las variantes implicadas en la aparición y desarrollo de esta enfermedad en el país. El cáncer gástrico es un problema de salud pública en Colombia, presentando la mortalidad más alta por cáncer. Esta enfermedad es multifactorial y de larga evolución, se caracteriza por no presentar síntomas o signos específicos. Son necesarias las investigaciones orientadas a disminuir la carga de este cáncer, mediante la reducción del riesgo y por medio de la identificación de los posibles factores involucrados. Considerando la necesidad de recopilar información que permita identificar los factores involucrados en el desarrollo del cáncer gástrico en la región, se realizó esta revisión que busca recopilar la literatura publicada sobre factores de riesgo.
