    Abstract: Conservative treatment of duodenal trauma in children has long been the first line of treatment for duodenal wall hematomas. However, it has rarely been described in duodenal perforations. Our purpose is to highlight the possibility of conservative treatment in selected cases of duodenal perforation. In the period between 2009 and 2022, 6 children were treated for duodenal injury following abdominal blunt trauma in the pediatric surgical emergency department. The clinical presentation, diagnosis and treatment are reported and analyzed. Three patients presented with duodenal hematomas, they were treated non-operatively with hospital stays between 12 and 20 days and good clinical outcome. One child presented with duodenal hematoma and retroperitoneal air bubbles; non-operative conservative treatment was carried with favorable results. The fifth patient had a duodenal perforation; he underwent a primary duodenal 2-layers closure. The last patient had a combination of duodenal hematoma and perforation involving 75% of the duodenal diameter for which he underwent a gastro-jejunostomy with pyloric exclusion. An isolated duodenal lesion can be subject to a conservative treatment whenever allowed by a stable clinical condition and the availability of appropriate clinical and radiological monitoring.

    Abstract: Injuries related to everyday accidents are very common in children and are the main reasons for hospitalization in pediatric surgery. They have similarities with neglect. The main objective is to assess the frequency and circumstances of the occurrence of unintentional accidents in children under 6 years old. The secondary objective is to specify the associated factors and eventually determine if those accidents can be related to neglect. Material and methods : This is a cross-sectional study carried out at Pediatric Surgical Emergencies Department of Rabat Children's Hospital in 2017. Results : We identified 363 cases of unintentional accidents. The variables were collected on questionnaire. 155 children were aged 6 and under. The median age was 2, 83 years with a predominance of boys (56%). Children over 2 years old were the most affected (55.48%). Low and middle-income families dominated socio-demographic factors. The mothers in almost half of the cases never went to school. In 65.4% of cases, an adult was present during the accident, often the mother (62%). The lesions involved the limbs (64%) and the head (47%). Hospitalization was 24 hours (60.8%). There was no complication in 96.8% of cases. Some risk factors associated with unintentional accidents are also found in negligence.

    Abstract: Tibial tubercle avulsion fractures are an uncommon injury occurring due to strong contraction of the quadriceps femoris muscle. Of all proximal tibial fractures, approximately 3% are tibial tubercle avulsion fractures. We report 3 cases of anterior tibial tubercle fractures in 3 adolescents treated at the pediatric surgical emergency department in Rabat. Standard radiographs of the knee will reveal the avulsed tibia tubercle and also allow fracture classification. There are multiple classification systems with multiple modifiers. The most commonly used is the modified Ogden classification. Tibial tubercle avulsion fractures can be associated with concomitant soft tissue damage, periosteal damage, and compartment syndrome leading to extensor mechanism disruption, joint laxity, or vascular compromise. Treatment can be nonsurgical or surgical, and indications depend on fracture type. Most fractures are surgical candidates and can be repaired with open reduction and internal fixation (ORIF) or arthroscopy. Satisfactory results are usually achieved.

    Abstract: Background: The term bezoar refers to an intraluminal masse in the gastrointestinal system caused by the accumulation of indigestible ingested materials. Trichobezoar is a compact conglomeration of swallowed hair and constitutes less than 6% of all bezoars. The stomach is the common site of occurrence and many patients may remain asymptomatic or present a mild form of the disease. Intestinal obstruction due to trichobezoar is an extremely rare complication. The clinical findings of bezoar-induced ileus do not differ from those of mechanical bowel obstruction due to other causes and they are rarely reported in the pediatric age group. Case presentation: we report a case of acute small-bowel obstruction due to trichobezoar in a three years old boy with no history of eating disorder which presented an abdominal distention. Conclusion: This case serves to raise awareness of trichobezoar as a diagnosis in young children who present with abdominal pain, palpable mass, and signs of acute small-bowel obstruction

    Abstract: Ultrasound-guided hydrostatic reduction of acute intussusception is a nonoperative technique, simple, performed under ultrasound guidance without risk of irradiation. Purpose: Our study aimed to evaluate the efficiency and utility of Ultrasound-guided hydrostatic reduction in the management of infant intussusception. Material and Methods: We conducted a retrospective study of cases of intussusception in infants (1-24 months) treated in the pediatric surgical emergency service of Rabat children’s Hospital during the period 2016-2020. Of the 135 cases collected during the study, 109 patients received ultrasound-guided hydrostatic reduction while 26 infants inderwent primary surgery. Results: This condition was most common in the age group (3-9 months) with 78% of cases. The sex ratio M/F was 2.29. The ileocolic type was the most encountered (56%). The complete ultrasound hydrostatic reduction was achieved for 88 infants with a success rate of 81% and an average hospitalization time of 1.4 days. 21 cases (19%) of which 16 failed hydrostatic reduction and 05 recurrences within 24 hours, were taken up by surgery with manual reduction and bowel resection for respectively 19 and 02 cases. Conclusion: Ultrasound guided hydrostatic reduction is a safe, quick and efficient method with a high success rate, a decrease in the occurrence of recurrences and complications and should be recommended first.

    Abstract: The Covid-19 pandemic has put stress on the healthcare system around the world and has had an impact on people’s access to healthcare. This study aims to underline the activities at the Rabat Children’s Hospital (HER), during the year 2020 which experienced a state of health emergency due to the Covid-19 pandemic. Material and methods: We have listed the data from the admission and billing service, about the activities of the hospitalization department of the children’s hospital, Rabat. Data was entered on WPS and analyzed by Jamovi software 1.6.23. We also calculated some performance indicators: Average occupancy rate, average length of stay, and bed turnover interval (BOR, LOS, and BTOI). Results: We distinguished between services providing « urgent care » (intensive care unit ICU and emergency rooms ER) and those providing « non-urgent care » (pediatry and surgery) department. The mean number of hospital admissions (NHA) was around 8123 + /− 3942. The average BOR, LOS and BTOI were respectively 72.0 days (SD: 32.6), 66.83 days (SD: 2.68) and 3.89 days (SD: 4.34). Conclusion : The COVID-19 pandemic had an impact on our hospital activities, disturbing its normal functioning. This study tried to have an overview on the current situation and should lead to other studies in the future, allowing a better managmnt of our hospital.

    Abstract: Pierre Robin’s sequence PRS is a congenital malformative sequence with a retrognathism, a glossoptosis and an upper respiratory obstruction with an associated cleft palate. This malformative association can be disabling for both the child and his parents, because of its aesthetic, functional, psychological and social impacts. Patients and methods: We have investigated 14 patients with PRS presenting cleft palates, collected over a period of 5 years, from March 2014 to November 2019 in the pediatric surgical emergencies department (PSE) in Rabat Children’s Hospital (HER). Results: Nine of the 14 patients had a complete cleft palate. The average age of intervention was 23 months ranging from 13 months to 4 years. The study showed a slight female predominance (n = 8, 57%). Regarding the treatment, 3 patients were treated with the Bardach’s technique, 3 had the Von Langenbeck’s technique, 3 had the Push-back technique and one patient who suffered from a post-operative persisting velar division was treated by veloplasty 3 rd generation. The hospital stay was around 24 hours and the post-operative follow-up was uncomplicated for all our patients. Conclusion: Although we had a relatively high success with our approach, the complexity and lack of consensus regarding the management of Pierre Robin’s sequence still requires the involvement of a multidisciplinary team. The main aim is to give the best aesthetical and functional results in order to reduce the suffering of the PRS children and families.

    Abstract: Unintentional injuries (UI) correspond to “unplanned” events; they include everyday life injuries (EDL), road traffic accident (RTA) and work accidents. In our pediatric context, the latter are excluded. Aim: Draw up an epidemiological profile of UI in children hospitalized for at least 24 hours and make a comparison between EDL and RTA. Material and Methods: A cross-sectional study conducted during four years, about children hospitalized for UI, in the pediatric surgical emergency department (PSE) of the children's hospital of Rabat, Morocco. Sociodemographic and medical data were analyzed by statistical software (JAMOVI). Results: 545 files were retained. EDL represented 81.9 % with a male predominance (70.4%). The median age was 8 years [4; 12] with predominance of school children (63.5%). Infants predominated in EDL (15.1%). Spring has a predominance in RTA (33%), while there is a reversal of the trend f or the other seasons (p 〈 0.05). As regards the accident, the majority occurred on weekdays. The injured body parts were multiple for the RTA (39.1%) and affected the limbs (58.5%) for the EDL (p 〈 0.05). Radiological abnormalities were found in 79.6% of cases in EDL. The 24-hour hospital stay was for EDL (66.7%) while RTA is hospitalized beyond 48 hours (53.7%).
