    Abstract: Leading experts in the field of dermatovenereology, cosmetology, and allergology took part in the meeting of the expert board. Following the discussion, the working groups of the Expert board assessed the role of filaggrin deficiency in the pathogenesis of atopic dermatitis and other diseases/conditions accompanied by xerosis. In addition, they formed recommendations for the use of emollient Admera, taking into account the role of filaggrin in the development of atopic dermatitis and xerosis. The experts comprehensively analyzed the best basic therapy for atopic dermatitis, xerosis of diverse etiologies secondary preventive options for atopic dermatitis and developed unified recommendations on the principles of managing such patients. In addition, they also established the place of emollients in clinical practice. Additional educational, informational, and organizationalactivities were proposed to help patients and doctors understand the problem of using emollients in atopic dermatitis and xerosis of diverse etiologies.     Abstract: This article reflects the main issues discussed at the Advisory Board with the participation of leading dermatovenereologists, allergists, and immunologists. The Advisory Board has become a platform for discussing the accumulated clinical and organizational problems in the field of managing patients with chronic urticaria and other allergic dermatoses. The Advisory Board also discussed the possibility of long-term use of cetirizine and levocetirizine and the use of their high doses in clinical practice. The subject of discussion was the question of the algorithm for escalation and subsequent de-escalation of the dose of non-sedating H1 antihistamines. An algorithm for escalation and de-escalation of doses of antihistamines was created based on the discussion, which can be recommended for use in clinical practice.     Abstract: Pyoderma gangrenosum is an autoinflammatory neutrophilic dermatosis. Diagnosis of the disease remains a difficult task to date, due to the lack of a standard of examination and differential diagnostic signs. The primary elements in the development of PG may be papules, pustules or bullae the dissection of which subsequently leads to the formation of ulcers with irregular, violaceous, undermined borders. In rare cases, the diagnosis of the disease can also be complicated by the rapid development of internal organs damage symptoms, which must be regarded as extracutaneous manifestations of PG. Extracutaneous lesions can occur before, during or after the appearance of skin rashes, and the detection of sterile neutrophil infiltrates in the defeat of internal organs confirm the concept of PG as a multisystemic disease. The presented case of a rare course of PG with multiple skin lesions and extracutaneous manifestations, simulating systemic vasculitis, emphasizes the importance of a detailed examination of patients in order to make a correct diagnosis and prescribe timely adequate treatment.

    Abstract: BACKGROUND: Psoriasis is a widespread chronic genetically determined inflammatory dermatosis that affects approximately 125 million people worldwide. In the vast majority of cases, the first rashes appear in people younger than 35 years old who are just starting to build a career and start a family. Localization of psoriatic rashes on visible areas of the skin and stigmatization in society, regular exacerbations and multiple courses of expensive and insufficiently effective treatment negatively affect the quality of life and emotional state of patients up to the development of depressive and anxiety disorders, and even suicide attempts. An imbalance of mental status leads to the progression of psoriasis, which further aggravates the course of the skin process. This necessitates a personalized approach to the choice of a therapy method, taking into account high efficiency and maximum comfort for patients, which will improve their psychoemotional state. AIM: to study the effect of phototherapy (PUVA and UVB-311 nm) on the mental status of psoriasis patients MATERIALS AND METHODS: A prospective study was conducted on the basis of FGKU Central Polyclinic and V.A. Rakhmanovs clinic of skin and venereal diseases of Sechenov University with the participation of 228 patients with moderate-to-severe psoriasis who received UVB-311 nm (n=116) and PUVA therapy (n=112), including psoriasis vulgaris (n=193), guttate psoriasis (n=14), exudative psoriasis (n=21). Before and after treatment, patients were assessed for the severity of psoriasis (PASI), quality of life (DQLI), as well as the presence of depression and anxiety disorders (panic disorder, social phobia) using PHQ-9 and GAD-7 questionnaire tests. The initial radiation dose was 1 J/cm2 at PUVA and 0.10.2 J/cm2 at UVB-311 nm with a gradual increase in the dose every 12 sessions by 0.5 J/cm2 and 0.1 J/cm2, respectively. As photosensitizers ammifurin and oxoralen were used. The treatment was carried out according to the method of four-time irradiation per week using the UV-7001K cabin (Waldmann, Germany). RESULTS: Among 228 patients there were 108 women and 120 men aged 438.2 years. The UVB-311 nm group included 102 patients with moderate psoriasis vulgaris and 14 guttate psoriasis, the second group (PUVA) included patients with severe recalcitrant psoriasis vulgaris (n=91) and exudative psoriasis (n=21). In the first group, the course of therapy consisted of 282 procedures of UVB-311 nm. Before treatment, minimal depression was observed in 19 (16%) patients, mild depression in 87 (75%), moderate depression in 10 (9%). A high level of anxiety was observed in 94 (81%), the average level in 22 (19%) patients. At the end of the course of UVB, PASI 90 was achieved in 94% (n=109) of patients, PASI 100 in 88% (n=102); at the same time, on average, anxiety decreased by 80%, depressive symptoms decreased by 73%, quality of life increased by 92%. In the second group, the PUVA course consisted of 253 irradiation sessions. Before starting therapy, minimal depression was observed in 12 (11%) cases, mild depression in 73 (65%), moderate depression in 27 (24%). A high level of anxiety was characteristic of 100% of patients. As a result of therapy, PASI 90 was achieved in 95.5% (n=107) of cases, PASI 100 in 90% (n=101). The average severity of anxiety, depression, and quality of life improved by 88%, 83%, and 96% respectively. CONCLUSION: PUVA and UVB-311 nm have demonstrated high efficacy and safety in moderate-to -severe psoriasis, contributing to the rapid cleansing of the skin from rashes and improving the psycho-emotional state, which not only significantly improves the quality, but also increases the life expectancy of patients. .

    Abstract: On February 19, 2022, a working meeting of experts on the problem of atopic dermatitis was held in Moscow under the chairmanship of the Director of the State Scientific Center for Dermatovenereology and Cosmetology of the Ministry of Health of Russia, Doctor of Medical Sciences, Professor, Corresponding Member of the Russian Academy of Sciences A.A. Kubanov. Also in attendance was the Academician of the Russian Academy of Sciences, Chief Freelance Childrens Specialist in Preventive Medicine, Head of the Research Institute of Pediatrics and Childrens Health of the Central Clinical Hospital of the Russian Academy of Sciences of the Ministry of Science, and Higher Education of the Russian Federation, Doctor of Medical Sciences, Professor, Academician of the Russian Academy of Sciences, L.S. Namazova-Baranova, and also the Deputy Director for Clinical Work ― Chief Physician of the Federal State Budgetary Institution SSC Institute of Immunology FMBA of Russia Doctor of Medical Sciences, Professor N.I. Ilyina. The meeting raised germane issues of providing medical care, reducing the burden of the disease, and improving the quality of life of patients with atopic dermatitis. New possibilities and principles of systemic therapy in patients with moderate and severe atopic dermatitis were discussed. We analyzed data on a new representative of the small molecule class the selective Janus kinase 1 inhibitor, upadacitinib, which was registered in the Russian Federation in June 2021. This analysis aimed to determine its place in atopic dermatitis therapy algorithms and the possibility of the impact of its use as a therapy on the quality of life of patients in relation to the burden of diseases.     Abstract: Currently, there is no generally accepted terminology and classification of vasculitis and vascular cutaneous disorders. In Russia there are various approaches to the classification of cutaneous vasculitis vascular lesions are classified according to clinical signs, etiology and pathogenesis. Significant difficulties are caused by the lack of a unified terminology, clear diagnostic criteria for vasculitis and the existence of a large number of duplicate names, among which there are many eponymous terms. This issue is one of the most complex, confusing and debatable not only in dermatovenereology, but also in other disciplines. Modern principles of diagnosis of cutaneous vasculitis are based on an integrated assessment of the data of the disease history, clinical picture, laboratory and instrumental methods of examination. For standardization of definitions and diagnostic criteria, it is necessary, first of all, to adopt a unified interdisciplinary classification of vasculitis, which will be based on the etiopathogenetic principle. The applied unified classification of cutaneous vasculitis is proposed for discussion.

    Abstract: Health is largely determined by environmental factors. A number of skin diseases, as well as its age-related changes, are a poorly studied medical and social problem. The procedures provided in cosmetology clinics can be considered a relative indicator of an improvement in the quality of life, directly related to the restoration and maintenance of balance and harmony of individual and public health of a person with the natural and social environment. The analysis of the quality of life of patients with cosmetology profile was carried out. It is noted that despite the rapid development of cosmetology, information proving the impact of surgical and non-surgical cosmetic procedures on the patients quality of life is limited; there are no effective, reliable rating scales for the patients self-assessment of the results of cosmetic therapy. Sociocultural and quality of life factors that force patients to undergo these procedures are not well understood. The results of a sociological survey of patients with a cosmetology profile allow cosmetologists to plan a complex of interventions for the prevention and correction of age-related changes in appearance, emphasize the professional image of a specialist. The development of rating scales and methods for assessing the quality of botulinum therapy and the patients self-assessment of the results of medical care provided on the principles of evidence-based medicine is an urgent issue.

    Abstract: BACKGROUND: Phototherapy using different wavelengths of the ultraviolet (UV) spectrum has been successfully used to treat a variety of dermatoses. Along with the positive effects of this intervention, there are both acute and long-term side effects that should be taken into account by the clinicians. AIM: to study the tolerability and safety of phototherapy (PUVA and UVB-311 nm) in patients with moderate to severe psoriasis. MATERIALS AND METHODS: There were 920 patients with psoriasis under our supervision from 2005 to 2021, of which 756 (82%) patients suffered from psoriasis vulgaris, 36 (4%) ― guttate psoriasis, 73 (8%) ― exudative psoriasis, 55 (6%) ― inverse psoriasis. 249/920 (27%) were diagnosed with psoriatic arthritis. Patients with moderate psoriasis received 311 nm UVB therapy (n=473), with severe psoriasis ― PUVA therapy (n=447). RESULTS: Out of 920 patients with psoriasis during the entire follow-up period, 385 (41.8%) noted one or more early side effects, which were short-lived and disappeared after the end of the PUVA course if the recommendations were followed. Among the early side effects that occurred during the procedures from taking a photosensitizer, the most common were transient nausea, vomiting, dizziness, headache, transient increase in hepatic transaminases. Side effects from the action of ultraviolet rays were reduced to the manifestation of itching, photodermatitis in the form of erythema, photodermatitis in the form of blisters, photoallergic reaction in the form of papular rash, dry skin, photoconjunctivitis, herpes simplex and even photoonycholysis. Patients treated with UVB-311 nm phototherapy complained on xerosis of the skin, itching, photodermatitis in the form of erythema, burning of the skin as early side effects. Long-term side effects in patients treated with PUVA therapy included lentigo (persistent mottled pigmentation), growth of melanocytic neoplasms, PUVA keratosis. In 11 (2.3%) patients with psoriasis who received UVB-311 nm therapy, new nevi appeared. In 42 (9.6%) patients treated with PUVA therapy and in 17 (3.8%) patients treated with UVB-311 nm, signs of photoaging were detected as well. In our study, only 1 (0.2%) patient was diagnosed with facial skin basal cell carcinoma, who received 5 courses of PUVA (119 procedures). CONCLUSION: Our study showed that side effects, especially long-term ones, were fewer and significantly less pronounced in patients treated with UVB-311 nm therapy. Comparing the benefits and risks of phototherapy sessions, we can conclude in favor of the effectiveness, good tolerability and safety of PUVA and UVB-311 nm therapy for the treatment of patients with moderate to severe psoriasis. A dose-dependent effect of photoaging was established: the more phototherapy courses were received, the more pronounced the signs of photoaging were, especially after PUVA therapy.

    Abstract: Atopic dermatitis (AD) is a worldwide disease with a complex aetiology. Both genetic and environmental factors cause a predisposition to AD. DNA methylation may be an additional predisposing factor. The goal of our study was to investigate genome‐wide methylation profiles of skin from patients with AD and healthy persons. This case–control study included 12 AD patients and 6 healthy volunteers. DNA methylation levels in skin samples were analysed using the Illumina Infinium HumanMethylation450 BeadChip. We found that the methylation profile of skin from patients with AD was significantly different from that of healthy controls. Differential DNA methylation was observed for genes involved in a number of AD‐related processes including regulation of the immune response, activation of lymphocytes, cell proliferation, apoptosis and differentiation of the epidermis. Our study indicates the involvement of epigenetic regulation in the development of atopic dermatitis.
