Abstract: The newly-developed cosmic-ray method for measuring area-average soil moisture at the hectometer horizontal scale is being implemented in the COsmic-ray Soil Moisture  Observing  System  (or  the  COSMOS). The  stationary cosmic-ray  soil  moisture  probe  measures  the  neutrons  that are generated by cosmic rays (or Cosmic-Ray Neutron Sensing-CRNS) within air and soil and other materials, moderated by mainly hydrogen atoms located primarily  in  soil  water,  and  emitted  to  the  atmosphere where they mix instantaneously at a scale of hundreds of meters and whose density is inversely correlated with soil moisture. The CRNS technique has been known for more than 10 years and validated by many scientific publications in the world. In Vietnam, a country greatly affected by global climate change, this is the first time this technique has been implemented and applied. Together with other traditional methods for soil moisture determination such as point measurement and remote sensing, CRNS technique will effectively contribute to the real-time monitoring of drought, prediction of flash floods and landslides, or planning irrigation in smart agriculture. This paper is written to introduce the results of the COSMOS-VINATOM project of the Ministry of Science and Technology. We describe the cosmic-ray soil moisture measurement method and the design of a specific COSMOS using advanced technologies in neutron and muon detector, signal processing and data transmission. This system has also been tested in the field in Xuan Canh commune, Dong Anh district, Hanoi, Vietnam with initial positive results.

Abstract: Abstract Introduction: Laparoscopic pancreaticoduodenectomy is one of the most complicated major surgeries. The postoperative mortality and morbidity are still a burden of hepato-pancreato-biliary surgery. Aim of this study is to assess the short-term outcome of laparoscopic Whipple procedure for periampullary cancer. Materials and methods: Case series report. Results: From 1/2018 to 1/2019, we had performed laparoscopic Whipple procedure in 21 patients. The intracorporeal hepatico-jejunal anastomosis had been formed in all the cases. The pancreaticojejunal and gastro-jejunal anastomosis had been formed extracorporeally through small 5cm midline incision. The pancreatic duct had been drained with 8F drainage. The average operation time was 364 minutes. The average intraoperation blood loss was 175ml. Postoperative mean length of stay was 12.1 days. Pancreaticojejunal anastomosis accounted for 19,0%. Postoperative bleeding occurred in 2 patient (9%) whom were converted to open surgery for control bleeding. In our series, there was no postoperative mortality. Conclusions: Laparoscopic-assisted pancreaticoduodenectomy is safe with low postoperative morbidity and mortality. This procedure is feasible for treatment of periampullary cancer.

Abstract: Introduction: Coronary artery disease is the leading cause of death in patients with chronic kidney disease. Despite many advances in diagnosis and treatment, percutaneous coronary intervention in patients with chronic kidney disease remains challenging. Objectives: (1) Describe clinical, subclinical and coronary artery lesions in patients with chronic kidney disease requiring percutaneous coronary angiography at the Vietnam Heart Institute. (2) Evaluation the changes in clinical, laboratory and glomerular filtration rate after coronary angiography or intervention in thesse patients.Subjects and methods: A descriptive prospective study of 59 chronic kidney disease patients assigned to take percutaneous coronary angiography with or without intervention at the Vietnam Heart Institute from August 2020 to October 2021. The study collected and analyzed information of clinical and laboratory characteristics, percutaneous coronary angiography results and changes in renal function after intervention. Result: The average SYNTAX score was 21,9 ± 13,0. Coronary artery lesions accounted for the majority of branches (84.8%), lesions of 1 branch (11.9%). The most common site of injury was the left anterior descending artery (88.1%) followed by the right coronary artery (71.2%) and the circumflex artery (50.8%). The culprit lesions were often complex, type B2 and type C were the most common types (76.2%), followed by type B1 (14.3%), type A (9.5%). The rate of chronic total occlusion was also quitehigh (25.4%), lesions at the bifurcation (25.4%) were less common in left coronary artery (13.6%). 17,3% of patients had complications of acute kidney injury after the intervention, of which the CKD 3 and CKD 4-5 group were accounted for 12.9% and 23.8% of the patients respectively. Of these, 3 patients had to undergo emergency dialysis after the intervention. Conclusion: Coronary artery lesions in patients with chronic kidney disease was often complicated, often with multiple coronary artery lesions and tended to be more severe with the decline of glomerular filtration rate. Percutaneous coronary intervention in patients with chronic kidney disease had safe results in terms of procedures, however, the rate of complications of acute kidney injury was quite high in patients with CKD.

Abstract: Based on the first order shear deformation shell theory, this paper presents an analysis of the nonlinear dynamic response and vibration of imperfect eccentrically stiffened functionally graded material (ES-FGM) cylindrical panels subjected to mechanical, thermal, and blast loads resting on elastic foundations. The material properties are assumed to be temperature-dependent and graded in the thickness direction according to simple power-law distribution in terms of the volume fractions of the constituents. Both functionally graded material cylindrical panels and stiffeners having temperature-dependent properties are deformed under temperature, simultaneously. Numerical results for the dynamic response of the imperfect ES-FGM cylindrical panels with two cases of boundary conditions are obtained by the Galerkin method and fourth-order Runge–Kutta method. The results show the effects of geometrical parameters, material properties, imperfections, mechanical and blast loads, temperature, elastic foundations and boundary conditions on the nonlinear dynamic response of the imperfect ES-FGM cylindrical panels. The obtained numerical results are validated by comparing with other results reported in the open literature.

Abstract: BACKGROUND: Rotator cuff tear (RCT) is a common injury of the shoulder, especially middle-aged people. Nonoperative treatment, cortisone injections are only effective at an early stage. Open surgery causes postoperative atrophy of the deltoid muscle, so results are limited. Arthroscopic rotator cuff repair surgery has been performed in Vietnam for about ten years, with many advantages such as the ability to accurately assess the lesions and less invasive procedure. In order to have a clearer view, we performed a mid-term assessment of the effectiveness of this surgery. AIM: Evaluate results over 2 years of patients with rotator cuff tears treated with arthroscopic surgery and their quality of life. METHOD: A group of 30 patients were diagnosed with RCT and surgery by arthroscopy to treat at Hanoi Medical University Hospital and Saint Paul Hospital between Jun 2015 and April 2017. The results of the surgeries were assessed by the degree of pain, muscle power, motion of the shoulder joint according to UCLA shoulder score. Evaluate the quality of life through the Rotator Cuff-Quality of Life (RC-QoL) index. RESULTS: The average age was 60.7 years. Female / male ratio was 1.3. Thirty-six months ± 6.41 was the average follow-up time (min 27 – max 50 months). The shoulder function is recorded according to UCLA has an average score of 30.9, therein good and excellent result were 90 %. The mean RC-QoL index was 91.5%. CONCLUSION: Treatment of RCT by arthroscopic surgery that has been evaluated for a minimum of 2 years follow-up showed good results and high quality of patient’s life.

Abstract: For personal radiation dose monitoring, electronic personal dosimeters (EPD), also known as active personal dosimeter (APD), using silicon diode detector have the advantage capability of measuring and displaying directly the exposure results of gamma, beta and neutron radiations in real time. They are mainly considered as good complement to passive dosimeters to satisfy ALARA principle in the radiation protection. In this paper, the meansurement methods and algorithms for evaluating personal dose equivalents such as Hp(10) and Hp(0.07) from air-kerma are studied and developed in two directions: the first, named energy correction method based on incident energy determined by the ratio of two detector responses with the different filter configurations; the second new method is carried out in the way that matching the shape of a detector’s energy response curve to the kerma-to-personal dose equivalent conversion function provides an approximate means of determining the dose equivalent without the need to resolve the actual incident energies. The algorithm has also been experimentally verified at Secondary Standards Dosimetry Laboratory (SSDL) of INST by the beam of radiation defined in ISO 4037-1. The obtained results of personal dose equivalents with errors almost less than 30% in energy range from 20 keV to 1.5 MeV are partially met the EPD design requirements according to the IEC 61526 Standard. The work and results of described in this paper are important basics for design and construction of completed electronic personal dosimeter.

Abstract: Introduction: Total anomalous pulmonary venous connection (TAPVC) is a rare condition. Mortality of TAPVC has improved dramatically in the present. Sutureless closure had initially used for congenital pulmonary venous stenosis (PVS) or post-repair PVS. Recently, it had been adopted for primary TAPVC to prevent postoperative pulmonary venous obstruction, a leading cause of death. This technique had gradually applied at our institute for primary TAPVC since 2018. Methods: Prospective case series was conducted from mid-2019. Including criteria consists of patients with primary TAPVC. These are all supracardiac, infracardiac, mixed type and intracardiac TAPVC with pulmonary venous stenosis. Excluding measures are TAPVC with single ventricle, isomerism or heterotaxy. End-point outcomes were mortality and short-term PVS. Results: Seventeen TAPVC cases were enrolled in this study from 06/2019-12/2020 at Children's Hospital 1. Male/female ratio was 12/5. Median age at admission was 44 (3-1010) days old. The median operative weight was 3.9 kg (2.4-11). Of those, fourteen (82.4%) cases were supracardiac TAPVC and 3 (17.6%) cases were infracardiac TAPVC. Nine (52.9%) cases were emergency operations. Median aortic clamp time was 66 (32-138) mins. Median bypass time was 112 (86-212) mins. There were 9 (52.9%) cases with arrhythmias, 2 (11.8%) cases with chylothorax. Mean ventilation time was 3.6±0.5 days; mean CICU time was 6.5 (1-20), median LOS was 14.5 (8-39). In-hospital mortality was 5.9% (1). Mean follow-up time was 12.3±6.4 months (range 4-24), there was no PVS with median pulmonary venous score of 0 (range 0-1). Conclusions: Sutureless technique could be a safe option for primary TAPVC repair.

Abstract: Huperzine A, isolated from Huperzia serrata, is a potent, specific, and reversible inhibitor of acetylcholinesterase with high efficiency and low toxicity. To evaluate the presence of huperzine A in Vietnamese H serrata, a reliable capillary zone electrophoresis method was developed. The analytical conditions were established using 80 mM ammonium acetate buffer, pH 6.0, hydrodynamic injection at 50 mbar for 5 s, applied voltage of 20 kV, temperature at 25 °C, uncoated fused-silica capillary, 56 cm (50 cm effective length) × 70 µm inner diameter, and ultraviolet detection at 310 nm. The recovery rates ranged from 98.05% to 100.64%, with a relative standard deviation 〈 2%. Good linear regression was observed in the concentration range of 1 to 500 µg/mL, with a correlation coefficient of 0.9994. The limit of detection and limit of quantification were 0.33 and 1.0 µg/mL, respectively. These results demonstrate that this method is simple, selective, and suitable for performing quality control for huperzine A derived from Vietnamese H serrata.
