    Abstract: Human immunodeficiency virus (HIV) infection cannot be completely eliminated from the body because the virus integrates its genetic code into that of the host cell. The prevalence of pregnancy in women with HIV infection has increased due to the efficacy of antiretroviral therapy (ART). Placental insufficiency is associated with a reduction in blood flow and circulatory redistribution, resulting in fetal hypoxia and nutrient deprivation as a consequence of an altered placental function, and it can result in a lower birthweight. The aim of the study was to determine the combined effect of HIV infection and ART on the anthropometric parameters of infants born to HIV-positive pregnant women under ART compared to the values of these parameters in a control group of infants born to healthy mothers. There are no significant differences between the two groups in terms of gestational age at birth. We found a statistically significant lower birth weight in infants born from HIV-positive mothers under ART, with 3041 g in the control group compared to 2758 g in the group of HIV positive pregnant women (p 〈 0.01). There were statistically significant differences in all anthropometric parameters, these showing higher values in the control group (seronegative pregnant women).

    Abstract: Introduction. Despite significant advances in diagnosis, medical management and antimicrobial therapy, sepsis in the puerperium remains an important cause of maternal morbidity and mortality. The abnormalities associated with the clinical syndrome of sepsis result from a nonspecific innate inflammatory response. This is due to the fact that sepsis represents a systemic inflammatory response syndrome (SIRS) to infection or injury; therefore, it can rapidly progress to septic shock and death despite aggressive treatment. Severe sepsis with MODS has a mortality rate of 20– 40%, rising to around 60% if septicemic shock develops. Symptoms of sepsis may be less distinctive than in the non-pregnant population and are not necessarily present in all cases; therefore, a high index of suspicion is necessary. The major pathogens causing sepsis in the puerperium are: group A streptococcus (GAS), also known as Streptococcus pyogenes, Escherichia coli, Staphylococcus aureus, Streptococcus pneumonia, methicillin-resistant S. aureus (MRSA), Clostridium septicum, Morganella morganii and antibiotic-resistant Stenotrophomonas Moltrophilia. Multiple risk factors for maternal sepsis have been identified: obesity, impaired glucose tolerance/diabetes, impaired immunity/immunosuppressant medication, anemia, vaginal discharge, history of pelvic infection, amniocentesis and other invasive procedures, cervical cerclage, prolonged spontaneous rupture of membranes, caesarean section, wound hematomas, retained products of conception, GAS infection, vaginal trauma. Material and methods. Presentation of case report using the following key-words: „infection”, „systemic inflammatory response syndrome (SIRS)”, „severe sepsis”, „septic shock”, „multiorgan dysfunction syndrome (MODS)”. Results. We present a case of perinatal infections complicated with MODS. Conclusions. The presence of pelvic hematomas triggers the sepsis caused by multiple infections in perinatal period and can significantly increase the morbidity related to bleeding, infection, surgery and blood product transfusion. The clinical situation may worsen in the presence of pre-existing pathological conditions before pregnancy.

    Abstract: Aim : The aim of this study is to identify correlations between total antioxidant status values of mothers and their infants and compare these values in accordance to the presence or absence of intrauterine growth restriction. Material and methods : This is a prospective, comparative study performed over a period of 3 years on a number of 52 infants and their mothers. Thirty-six of them had intrauterine growth restriction and 16 were appropriate for their gestational age and were used for comparative purposes. General information regarding the mother, infant and pregnancy were recorded. In addition, total antioxidant status was assessed from blood samples, taken right before delivery from mothers and from the cord blood in infants. Results : We found significant differences between total antioxidant status both between mothers and neonates with IUGR (intrauterine growth restriction) versus without IUGR (p=0.018, and p 〈 0.001, respectively). In addition, in both groups, there was a significant direct correlation between maternal and neonatal values of serum total antioxidant status (TAS) (p 〈 0.001). In conclusion , we can say that TAS values, as an important marker of the oxidative status of patients, are correlated with the presence of IUGR and values recorded from blood samples of the mother may be predictive for the oxidative status of the infant, thus of IUGR.

    Abstract: Because of its histological and embryological complex development, the ovary can be the source of a large variety of tumors, ovarian cancer being the fifth female neoplasia as a frequency. A major clinical significance is that the ovarian tumor is asymptomatic for a long period of time.Ovarian proliferative pathology raises a number of important issues from both theoretical and practical point of view. The change in the therapeutic attitude, especially in the areas of surgery and chemotherapy, has led in the last decades to an improvement in the survival rate. Our retrospective study included the analysis of 64 patients with ovarian cancer. Ovarian neoplasms should have a multidisciplinary approache with individualized treatment for each patient. The long-term survival remains, however, the detection of the tumor at an early stage, the undetectment in the initial stages leading to a survival rate of 5 years less than 20%.

    Abstract: Endometriosis is a multifactorial disease that is manifested by infertility and pelvic pain. The purpose of the study was to evaluate the effect of progesterone treatment on the serum level of osteopontin, a multipotent cytokine, in patients with endometriosis. The study was prospective and we evaluated osteopontin levels that were measured in the serum of 40 patients with endometriosis and 12 healthy women using a standardized Enzyme-Linked Immunosorbent Assay (ELISA) kit. Osteopontin seric levels were lower in endometriosis patients and increased after progesterone treatment. Because of the large dispersion of data even in the control group, we find the association between osteopontin and endometriosis questionable.

    Abstract: Along with the findings of recent years in medical imaging, a wide range of contrast agents have been developed and used in order to improve the quality of the images acquired and to get a diagnosis with the highest accuracy. Modern substances used in nuclear magnetic resonance imaging contain ions with one or more free electrons, which have fewer side effects and are eliminated by the kidney in a short time. This is used in MRI practice in T1 section, where these substances reduce relaxation time and increase signal strength. Our study included a group of 17 patients diagnosed with cervical cancer, who were also investigated by MRI technique with paramagnetic contrast agents. We followed the cervix-related aspects in the T1 and T2 sections and applied the EMVI score criteria to determine the level of extramural invasion of the blood vessels. The results are conclusive in the advanced stages of the disease when EMVI staging criteria are similar to those of TNM.

    Abstract: Toxaemia is a pathological condition specific to the period of pregnancy which begins and is indissolubly related to the presence of placenta. Antiangiogenic factors, such as sFLT-1 (soluble tyrosine kinase receptor fms-like) and sEng (soluble endoglin) play an important role in the first part of pregnancy. They are linked to physiological vascular neoformation, and, in the second part of the pregnancy, grant the endothelial functionality and physiological vascular remodeling. The aim of the study is to try to establish the levels of the sFLT� 1/PIGF ratio, as a prognostic tool in the patient with pre-eclampsia, depending on the influence of the cumulative risk factors. The sFLT-1 / PIGF report is a potential prognostic parameter in monitoring preeclampsia. The results of our study confirm the importance of deterring these markers for the diagnosis and monitoring of hypertensive pregnancies and at the same time to emphasize that the sFLT-1/PIGF ratio is a good predictor of preeclampsia.

    Abstract: Endometriosis (EMS) is a benign condition characterized by a systemic inflammation that affects fertile women at reproductive age. Ultrasound became, in recent years, the method of choice for both effective diagnostic and preoperative planning. Therefore, accurate characterization and mapping of endometriotic lesions is imperative in such circumstances to enable optimal approach of treatment, whether surgical or non-surgical based on the severity of the findings. This pictorial essay outlines a practical approach to evaluating patients with deep endometriosis by means of transvaginal ultrasound. The technical aspects are in conjunction with both consensus of the International Deep Endometriosis Analysis (IDEA) group and the hands-on experience acquired through daily clinical practice.

    Abstract: (1) Background: Recurrent pregnancy loss (RPL) and recurrent implantation failure (RIF) have in common a deficient maternal adaptation to the semi-allogeneic fetus, in which killer immunoglobulin-like receptor (KIR) family expressed by natural killer (NK) cells play an important role. The aim of this study was to evaluate the influence of maternal KIR haplotype on the reproductive outcomes after single embryo transfer in IVF cycles in patients with RPL and RIF. (2) Methods: Patients with RIF and RPL who presented at Origyn Fertility Center from Iasi, Romania, were prospectively enrolled between January 2020 and December 2022. Clinical and paraclinical data was examined. Descriptive statistics and a conditional logistic regression model were used to analyze our data. (3) Results: Patients with a KIR AA haplotype had significantly more chances of miscarriage if they underwent an IVF procedure (aOR: 4.15, 95% CI: 1.39–6.50, p = 0.032) compared with those who spontaneously achieved a pregnancy. Moreover, it appeared that the same haplotype increased the chances of obtaining a pregnancy for patients who underwent an IVF procedure (aOR: 2.57, 95% CI: 0.85–6.75, p = 0.023). (4) Conclusions: Determination of KIR haplotype could be beneficial for patients with RPL or RIF in order to offer an individualized management.

    Abstract: Triploidy is a numerical chromosomal anomaly characterized by the presence of three sets of haploid chromosomes. The incidence is hard to evaluate, because usually it causes 1st trimester miscarriage. At 20 weeks of amenorrhea the incidence of triploidy is estimated at 1/250,000 cases. We present 4 cases of triploidy diagnosed during the decade 2003-2013 in the Prenatal Diagnosis Department of Maternity “Cuza-Vodă” Iasi, Romania, all registered in one year. The analysis of pathological cases identified in the last 11 years by prenatal diagnosis has shown that triploidies represented only 5.7% of numeric chromosomal anomalies, but in 2013 the four cases of triploidy represented 36% of numeric chromosomal anomalies. The karyotypes were recommended after discovering different congenital anomalies by ultrasound scan. In all cases, an intrauterine growth retardation (IUGR) was present but with no placental changes. Also, we discovered anomalies of limbs, congenital anomalies of heart and some dysmorphic features. This series demonstrates that triploidy may be discovered in the 2nd trimester of pregnancy and has a heterogeneous aspect at ultrasound scan, which can generate diagnostic difficulties. Therefore, the detection by ultrasound scan, at 18-22 weeks of pregnancy, of complex foetal morphological abnormalities should be an important reason for amniocentesis to search chromosomal anomalies
