    Abstract: Convalescent plasma therapy (CPT) and remdesivir (REM) have been approved for investigational use to treat coronavirus disease 2019 (COVID-19) in Nepal. Methods In this prospective, multicentered study, we evaluated the safety and outcomes of treatment with CPT and/or REM in 1315 hospitalized COVID-19 patients over 18 years in 31 hospitals across Nepal. REM was administered to patients with moderate, severe, or life-threatening infection. CPT was administered to patients with severe to life-threatening infections who were at high risk for progression or clinical worsening despite REM. Clinical findings and outcomes were recorded until discharge or death. Results Patients were classified as having moderate (24.2%), severe (64%), or life-threatening (11.7%) COVID-19 infection. The majority of CPT and CPT + REM recipients had severe to life-threatening infections (CPT 98.3%; CPT + REM 92.1%) and were admitted to the intensive care unit (ICU; CPT 91.8%; CPT + REM 94.6%) compared with those who received REM alone (73.3% and 57.5%, respectively). Of 1083 patients with reported outcomes, 78.4% were discharged and 21.6% died. The discharge rate was 84% for REM (n = 910), 39% for CPT (n = 59), and 54.4% for CPT + REM (n = 114) recipients. In a logistic model comparing death vs discharge and adjusted for age, gender, steroid use, and severity, the predicted margin for discharge was higher for recipients of remdesivir alone (0.82; 95% CI, 0.79–0.84) compared with CPT (0.58; 95% CI, 0.47–0.70) and CPT + REM (0.67; 95% CI, 0.60–0.74) recipients. Adverse events of remdesivir and CPT were reported in  & lt;5% of patients. Conclusions This study demonstrates a safe rollout of CPT and REM in a resource-limited setting. Remdesivir recipients had less severe infection and better outcomes. ClinicalTrials.gov identifier. NCT04570982.

    Abstract: Two field experiments were conducted at Regional Agricultural Research Station, Tarahara, Nepal during 2012 and 2013 to determine the effect of agronomic management on growth and yield of Swarna Sub1 under flash floods. The first experiment was laid out in a split plot design with three replications; and four different nutrient combinations at nursery as main plots and three age groups of rice seedlings as sub plots. The second experiment was laid out in a randomized complete block design and replicated thrice; with three post flood nutrient doses at six and 12 days after de-submergence (dad). The experiments were complete submerged at 10 days after transplanting for 12 days. The survival percentage, at 21 dad, was significantly higher in plots planted with 35 (90.25%) and 40 (91.58%) days-old seedlings compared to 30 days-old seedlings (81.75%). Plots with 35 days-old seedlings produced 5.15 t ha-1 with advantage of 18.83% over 30 days-old seedlings. Plots with 100-50-50 kg N-P2O5-K2O/ha at nursery recorded the highest grain filling of 79.41% and grain yield of 5.068 t/ha with more benefit. Post flood application of 20-20 N-K20kg/ha at 6 dad resulted in higher plant survival and taller plants, leading to significantly higher grain yield of 5.183 t/ha and straw yield of 5.315 t/ha. Hence, 35-40 days old seedlings raised with 100-50-50 kg N-P2O5-K2O /ha in nursery and the additional application of20-20 kg N-K2O /ha at 6 dad improved plant survival and enhanced yield of Swarna Sub1 under flash flood conditions. The practice has prospects of saving crop loss with getting rice yield above national average yield leading to enhanced food security in the flood prone areas of Nepal.

    Abstract: Resistance towards fluoroquinolones and treatment failure is a matter of concern in enteric fever. The present study was undertaken to analyze the susceptibility pattern of  Salmonella towards fluoroquinolones using  2006 and 2013 Clinical Laboratory Standards Institute (CLSI) breakpoint for interpretation and revisit the efficacy of Nalidixic acid resistance (NAR) as a phenotypic marker. A retrospective analysis of the zone of inhibition (ZOI) diameter data and minimum inhibitory concentration (MIC) data of 105 Salmonella was conducted. The ZOI diameter analysis showed that all isolates were susceptible to Ofloxacin and Ciprofloxacin using the previous 2006 ZOI breakpoint. However, with the revised 2013 breakpoint of Ciprofloxacin, the susceptibility percent dropped significantly and for Ofloxacin the breakpoint was not revised, so the percentage remained unchanged. The MIC analysis showed that all isolates were susceptible towards Ofloxacin and 97.14 % of isolates were susceptible to Ciprofloxacin using the previous 2006 MIC breakpoint, while the susceptibility decreased for both antibiotics with the revised 2013 MIC breakpoint. Statistically, the ZOI diameter of Nalidixic acid and MIC values of Ciprofloxacin and Ofloxacin was negatively correlated. To conclude, the revision of breakpoints addresses the problem of screening fluoroquinolones resistance but the emerging fluoroquinolones resistance situation is still a matter of concern in healthcare facilities of Nepal. Thus a reliable screening method is need of the hour as NAR cannot be considered a reliable marker to screen fluoroquinolones resistance

    Abstract: Introduction: Induction of labor is done with the aim of normal vaginal delivery. There are different methods; mechanical like intra-cervical Foley’s or pharmacological agents like misoprostol ordinoprostone. Objectives: To compare sequential use of Foley’s catheter and vaginal misoprostol in comparison with vaginal misoprostol only. Methods: Patients with period of gestation more than or equal to 41 weeks without any complications were assigned randomly according to computer generated randomization into sequential use ofintracervical Foley followed by vaginal misoprostol after 24 hours (sequential group) or vaginalmisoprostol only. Primary outcome was rate of normal vaginal delivery. Secondary outcomes were induction to delivery interval, maternal and fetal morbidity. Results: Rate of normal vaginal delivery in sequential group (n= 353) and misoprostol alone group(n= 356) was 71.6% and 53.3% (p 〈 0.001) but the time to delivery from induction is statistically more in sequential group (30.3 vs. 11.2 hours, p 〈 0.001). Maternal outcomes like postpartum hemorrhage, hyper-stimulation syndrome, chorioamnitis and neonatal outcomes like low Apgar score, meconium stained baby and stillbirth are similar in both the groups. Conclusions: The priming of cervix using intracervical Foley catheter before giving vaginalmisoprostol was beneficial in increasing the rate of normal vaginal delivery but it increased the induction to delivery interval.

    Abstract: BACKGROUND: Standards for building elements recommend a minimum luminance contrast of 30%. The basis of this value and the metric originally used is not known. OBJECTIVE: To begin to provide an evidence base for the specification of minimum contrast in building elements. METHODS: Subjects with and without a vision impairment were characterized by visual acuity, contrast sensitivity, visual fields and color vision. On an iPad they rated the visibility, as a function of contrast, of simulated door frames, door handles, light switches and stair nosings as “not visible at all”, “poorly visible”, “easily visible” and “extremely easily visible”. RESULTS: The contrasts for each level of visibility were highly correlated with visual acuity and contrast sensitivity. A principal component analysis also verified the importance of visual acuity, contrast sensitivity and visual fields in rating visibility of simulations of building elements. The required contrast for door handles, light switches and stair nosings to attain the same ratings of visibility were very similar but less contrast was required for door frames. CONCLUSIONS: 30% Michelson contrast for building elements renders building elements only poorly visible for those with severe vision impairments. 65% luminance contrast is necessary for all elements to be “easily visible”. Some increase (not a decrease) on the present 30% requirement and encouragement to exceed this requirement would seem appropriate. The use of simulated objects facilitates a systematic examination of the effect of contrast, but the applicability of the results to real-life remains to be demonstrated.

    Abstract: BACKGROUND: Standards writers, national and international, have used different contrast calculations to set requirements in building elements for people with visual impairments. On the other hand, they have typically set a single requirement (30%) for specifying the minimum contrast. The systems are not linearly related and 30% means something rather different in each system. OBJECTIVE: To provide a comparison of the various scales in order to illustrate the differences caused by multiple scales with a single compliance value, recommend a single scale for universal adoption and, if a new measure is problematic for implementation, to recommend the most perceptually uniform of the present methods. METHODS: We use the contrast between combinations of 205 paint colours to illustrate the relationships between the measures. We use an internationally accepted scale, with equal perceptual steps, as a “gold standard” to identify the most perceptually uniform measurement scale in the existing methods. RESULTS: We show that Michelson contrast is the most perceptually uniform of the existing measurement scales. We show the contrasts in the proposed method that equate to the various current requirements. CONCLUSIONS: We propose that CIE Metric Lightness could be used as the contrast measure. Alternatively, Michelson contrast is the most perceptually linear of the current measurement scales.

    Abstract: The maternal mortality ratio is a significant public health indicator that reflects the quality of health care services. The prevalence is still high in developing countries than in the developed countries. This study aimed to determine the MMR and identify the various risk factors and causes of maternal mortality. Methods This is a retrospective study conducted in a tertiary care center in Eastern Nepal from 16 th July 2015 to 15 th July 2020. The maternal mortality ratio was calculated per 100,000 live-births over five year’s study period. The causes of death, delays of maternal mortality and, different sociodemographic profiles were analyzed using descriptive statistics. Results There was a total of 55,667 deliveries conducted during the study period. The calculated maternal mortality ratio is 129.34 per 100,000 live-births in the year 2015 to 2020. The mean age and gestational age of women with maternal deaths were 24.69 ± 5.99 years and 36.15 ± 4.38 weeks of gestation. Obstetric hemorrhage, hypertensive disorder of pregnancy and sepsis were the leading causes of maternal death. The prime contributory factors were delay in seeking health care and reaching health care facility (type I delay:40.9%). Conclusions Despite the availability of comprehensive emergency obstetric care at our center, maternal mortality is still high and almost 75% of deaths were avoidable. The leading contributory factors of maternal mortality are delay in seeking care and delayed referral from other health facilities. The avoidable causes of maternal mortality are preventable through combined safe motherhood strategies, prompt referral, active management of labor and, puerperium.
